The Effect of Low Molecular Weight Heparin on Groin Flap Transplantation in Rat (저분자량 헤파린이 쥐 서혜부 유리피판 이식술에 미치는 영향)

  • Free tissue transplantation is commonly performed with the brilliant achievement in microsurgery and anticoagulants and antithrombotic agents have been prescribed in the procedures. However, there is no clean-cut indication as to which agents would be more effective in every steps and final consequences. Low molecular weight heparins inhibiting coagulation in plateletrich plasma and acting on the vascular endothelium have antithrombotic and fibrinolysis action. The experiment with rat groin free flap transplantation after 6-hour ischemia and injection of the low molecular weight heparin was performed and the results between the injection and non-injection group were analysed as follows, 1. Both of the 24-hour groups, vessel patency was not proportional to color change of the groin flap. 2. On the second day after anastomois, heparin-injection group showed intact intima, patent lumen without thrombus, and mild granulomatous inflammation around the suture material and control group with doubtful patency revealed intimal loss and thrombus formation. 3. On the 5th, 7th, and 9th postoperative day, heparin group was patent in anastomosis and showed acute inflammatory cells. 4. The 7th-week period, heparin-injection group showed intact flap color, patent lumen with intact intima and persistent foreign body granuloma.

Effects of Shiitake Mushroom on Anti-platelet Aggregation and Anti-thrombotic (표고버섯 추출물의 항혈소판 응집 및 항혈전 효과)

  • In in vitro study, the anti-platelet aggregation effect of Shiitake mushroom extract was examined by measuring the collagen induced platelet aggregation and the DPPH radical scavenging. In in vitro study, anti-thrombotic effect of Shiitake mushroom extract was examined using the carotid artery thrombosis rat model. Carotid artery thrombosis rat model was made by 35% $FeCl_3$ treatment. After that, we investigate thrombus weight and blood flow. In platelet aggregation test, the extract significantly inhibited platelet aggregation in a concentration dependent manner(p<.001). Also, extract increased DPPH radical scavenging activity in a concentration dependent manner. Extract significantly inhibited thrombus weight to compare with control group. And blood passage time were shorter in the Shiitake mushroom extract supplemented groups than in control group. These results provide experimental evidence that Shiitake mushroom can be used to prevent platelet aggregation and thrombosis, then could apply the clinical diseases such as cardiovascular disease, and so on.

Pulmonary thromboembolism combined with intracardiac thrombus occurred during the steroid reduction in nephrotic syndrome patient (미세변화신증에서 스테로이드 감량 중 발생한 심장내 혈전을 동반한 폐색전증)

  • Nephrotic syndrome is associated with a hypercoagulable state, which results in thromboembolism as one of its main complications. Various pathogenetic factors that cause the hypercoagulable state in nephrotic syndrome have been recognized. We report on a 19-year-old female with a minimal-change disease who developed pulmonary thromboembolism combined with intracardiac thrombus while on tapering steroid. Our patient showed hypoalbuminemia with an episode of shock, and was successfully treated with thrombolysis and anticoagulation therapy.

  • A 39-year-old man presented with cough, chest discomfort, and weight loss. On the basis of the patient history and laboratory findings, he was diagnosed with the hypereosinophilic syndrome. Transthoracic echocardiography revealed a large thrombus in the left ventricle. Medical treatment with anticoagulation and immunosuppression was commenced immediately. Fourteen days after the initial diagnosis, the patient presented with acute pain in his right leg. Computed tomographic angiogram showed embolic occlusion of the infrarenal abdominal aorta and bilateral iliac (including common, external, and internal iliac) arteries. Emergent thromboembolectomy and left ventricular thrombectomy were performed. The postoperative course was uneventful, and the patient has undergone follow-up for 2 months without any evidence of recurrence of thromboembolism.

A Numerical Analysis on the Curved Bileaflet Mechanical Heart Valve (MHV): Leaflet Motion and Blood Flow in an Elastic Blood Vessel

  • In blood flow passing through the mechanical heart valve (MHV) and elastic blood vessel, hemolysis and platelet activation causing thrombus formation can be seen owing to the shear stress in the blood. Also, fracture and deformation of leaflets can be observed depending on the shape and material properties of the leaflets which is opened and closed in a cycle. Hence, comprehensive study is needed on the hemodynamics which is associated with the motion of leaflet and elastic blood vessel in terms of fluid-structure interaction. In this paper, a numerical analysis has been performed for a three-dimensional pulsatile blood flow associated with the elastic blood vessel and curved bileaflet for multiple cycles in light of fluid-structure interaction. From this analysis fluttering phenomenon and rebound of the leaflet have been observed and recirculation and regurgitation have been found in the flow fields of the blood. Also, the pressure distribution and the radial displacement of the elastic blood vessel have been obtained. The motion of the leaflet and flow fields of the blood have shown similar tendency compared with the previous experiments carried out in other studies. The present study can contribute to the design methodology for the curved bileaflet mechanical heart valve. Furthermore, the proposed fluid-structure interaction method will be effectively used in various fields where the interaction between fluid flow and structure are involved.

  • The two major problems related to the blood flow in a floating type polymer valve are thrombus formation and hemolysis. It is well known that the shear stress in the fluid and flow separation around the valve are blamed for such disastrous phenomena. In this viewpoint, through study of the flow field around the valve is imperative to improve design of the valve. The aim of this study is to investigate the fluid flow around a floating type polymer valve. The numerical method employed in this study is the finite element software called ADINA. Incompressible viscous flow is assumed for blood using the assumption of Newtonian fluid. In this study, two prominent features of the axisymmetric flow around the floating type polymer valve are observed: jet-like flows observed near the gap between the conduit and the valve, and recirculating flow downstream of the valve. We also provided a detailed description of shear stress field according to the variation of flow conditions. The shear stress in fluid has its maximum value near the gap between the valve and the conduit.

Successful Arterial Thromboembolism Therapy in a Cat with Recombinant Tissue Plasminogen Activator Using an Accelerated Dosing Protocol

  • An 8-year old female Korean Short Hair cat with a history of paralysis of both hind limbs less than 1 hour before admission was referred. On physical examination, the left hind limb was cold and there was no pulsation or mobility. On abdominal ultrasound examination, a thrombus 8 mm in length was found at the aortic bifurcation. The patient was diagnosed with hypertrophic cardiomyopathy (HCM) and cardiogenic pulmonary edema through radiologic evaluation and echocardiography. A tissue plasminogen activator (tPA) was applied intravenously using an accelerated dosing protocol (1 mg administered intravenously [IV] bolus, 2.5 mg IV over 30 min, 1.5 mg IV over 1 h) to treat the feline arterial thromboembolism. Within 12 h after administration of tPA, pulsation and mobility of both hind limbs were normal, without any noticeable complications. Clopidogrel was prescribed to prevent additional thrombus formation, and pimobendan, benazepril, and furosemide were prescribed for administration at home. The patient was discharged and survived 377 days.

A fatal case of acute pulmonary embolism caused by right ventricular masses of acute lymphoblastic lymphoma-leukemia in a 13 year old girl

  • We report a case of a 13-year-old girl with acute lymphoblastic lymphoma- leukemia, who presented with a cardiac metastasis in the right ventricle, resulting in a pulmonary embolism. At the time of her leukemia diagnosis, a cardiac mass was incidentally found. The differential diagnosis for this unusual cardiac mass included cardiac tumor, metastasis, vegetation, and thrombus. Empirical treatment was initiated, including anticoagulation and antibiotics. She underwent plasmapheresis and was administered oral prednisolone for her leukemia. Five days later, she experienced sudden hemodynamic collapse and required extracorporeal membrane oxygenation insertion and emergency surgery. These interventions proved futile, and the patient died. Pathology revealed that the cardiac mass comprised an aggregation of small, round, necrotic cells consistent with leukemia. This is the first known case of acute lymphoblastic leukemia presenting as a right ventricular mass, with consequent fatal acute pulmonary embolism. A cardiac mass in a child with acute leukemia merits investigation to rule out every possible etiology, including vegetation, thrombus, and even a mass of leukemic cells, which could result in the fatal complication of pulmonary embolism.

  • In this experiment, artificial blood vessel was fabricated from electro-spun PU/PCL. To assist with endothelial growth, PU/PCL surface was coated with the RGD peptide. To prevent a clot of blood, anti-thrombus agent was loaded to the fibrous mat and values were reflected through FT-IR data. In vitro study, SEM and MTT data showed that the component was of excellent biocompatibility and cell proliferation. In in vivo study, the artificial blood vessel was implanted in a dog's artery. The results of the CT scan, ultrasound and H&E staining showed that artificial blood vessel was excellent for artery replacement applications.

The Changes of Blood Biochemical Values during Warfarin Therapy in Calves (송아지에 warfarin 투여 시에 혈액 생화학치의 변화)

  • We executed this experiment to observe side effects of warfarin, the anticoagulant that is used for preventing thrombus in cardiovascular surgery for calves. The 6 calves(70-130 kg) were used in this experiment regardless of sexes. We administered warfarin at 0.07 mg/kg daily for 25 days. Blood was collected before warfarin administration, every five days for 30 days. PCV, RBC, WBC, fibrinogen, total protein and platelet as blood test, prothrombin time (PT) as blood coagulation test, and AST, SDH, total bilirubin, BUN and creatinine as serum biochemical tests were measured. As the result of the experiment, PT has been gradually increased after warfarin administration. It has been gradually increased and remains within the therapeutic range from the third day to the 28th day. PCV and RBC were decreased significantly from the value before the administration (p < 0.05). In the serum biochemical test, SDH shows significant increase comparing the value before the administration (p < 0.05). AST and total bilirubin were increased gradually from the value before the administration. Considering the result of the experiment, to give wafarin to prevent thrombus in cardiovascular surgery, we can get anticoagulation effect with minimal administration(0.07 mg/kg, PO) from the third day of the administration. However because of the decreased PCV and RBC, it may cause anemia. It may cause damage to liver based on the result of serum biochemical test.
