Long-Term Outcomes in Stage I Lung Cancer After Segmentectomy with a Close Resection Margin

  • Background: In general, a 2-cm surgical margin is recommended for limited resection to obtain equivalent oncologic outcomes to lobectomy for lung cancer. This study aimed to examine the patterns of recurrence and prognostic factors for recurrence in patients with a close parenchymal resection margin. Methods: From January 2009 to April 2017, 156 patients with stage I lung cancer who underwent segmentectomy with a close resection margin (<2 cm) were enrolled. Recurrence-free survival and overall survival were assessed. In addition, predisposing factors for recurrence were evaluated. Results: The mean tumor size was 1.7±0.8 cm and the parenchymal resection margin was 1.1±0.6 cm. Recurrence developed in 17 (10.7%) of the 156 patients, and the 5-year recurrence-free survival rate was 88.9%. Distant metastasis (7.7%) was the predominant recurrence pattern. The isolated local recurrence rate was 1.9%. Multivariate Cox regression analysis revealed that age, tumor size, mediastinal lymph node dissection, postoperative complications, and histologic type were significant predisposing factors for recurrence. However, parenchymal margin distance did not significantly affect the long-term prognosis. Conclusion: Segmentectomy with a close resection margin for early-stage lung cancer in selected patients resulted in acceptable recurrence and survival. However, patients with tumors larger than 2 cm, squamous cell carcinoma histology, and insufficient mediastinal evaluation should be carefully followed up for recurrence.

Development of a Risk Scoring Model to Predict Unexpected Conversion to Thoracotomy during Video-Assisted Thoracoscopic Surgery for Lung Cancer

  • Background: Unexpected conversion to thoracotomy during planned video-assisted thoracoscopic surgery (VATS) can lead to poor outcomes and comparatively high morbidity. This study was conducted to assess preoperative risk factors associated with unexpected thoracotomy conversion and to develop a risk scoring model for preoperative use, aimed at identifying patients with an elevated risk of conversion. Methods: A retrospective analysis was conducted of 1,506 patients who underwent surgical resection for non-small cell lung cancer. To evaluate the risk factors, univariate analysis and logistic regression were performed. A risk scoring model was established to predict unexpected thoracotomy conversion during VATS of the lung, based on preoperative factors. To validate the model, an additional cohort of 878 patients was analyzed. Results: Among the potentially significant clinical variables, male sex, previous ipsilateral lung surgery, preoperative detection of calcified lymph nodes, and clinical T stage were identified as independent risk factors for unplanned conversion to thoracotomy. A 6-point risk scoring model was developed to predict conversion based on the assessed risk, with patients categorized into 4 groups. The results indicated an area under the receiver operating characteristic curve of 0.747, with a sensitivity of 80.5%, specificity of 56.4%, positive predictive value of 1.8%, and negative predictive value of 91.0%. When applied to the validation cohort, the model exhibited good predictive accuracy. Conclusion: We successfully developed and validated a risk scoring model for preoperative use that can predict the likelihood of unplanned conversion to thoracotomy during VATS of the lung.

Early Clinical Outcomes of Thoracoscopic Major Pulmonary Resection and Thymectomy Using Novel Articulating Endoscopic Forceps

  • Background: Video-assisted thoracoscopic surgery (VATS) is recognized as a safe and effective treatment modality for early-stage lung cancer and anterior mediastinal masses. Recently, novel articulating instruments have been developed and introduced to endoscopic surgery. Here, we share our early experiences with VATS major pulmonary resection and thymectomy performed using ArtiSential articulating instruments. Methods: At the Seoul Metropolitan Government-Seoul National University Boramae Medical Center, 500 patients underwent VATS pulmonary resection between July 2020 and April 2023, while 43 patients underwent VATS thymectomy between January 2020 and April 2023. After exclusion, 224 patients were enrolled for VATS major pulmonary resection, and 38 were enrolled for VATS thymectomy. ArtiSential forceps were utilized in 35 of the 224 patients undergoing pulmonary resection and in 12 of the 38 individuals undergoing thymectomy. Early clinical outcomes were retrospectively analyzed. Results: No significant differences were observed in sex, age, surgical approach, operation time, histological diagnosis, or additional procedures between the patients who underwent surgery using novel articulating instruments and the group treated with conventional endoscopic instruments for both VATS major pulmonary resection and thymectomy. However, the use of the novel articulating endoscopic forceps was associated with a significantly larger number of dissected lymph nodes (p=0.028) and lower estimated blood loss (p=0.009) in VATS major pulmonary resection. Conclusion: Major pulmonary resection and thymectomy via VATS using ArtiSential forceps were found to be safe and effective, with early clinical outcomes comparable to established methods. Further research into long-term clinical outcomes and cost-effectiveness is warranted.

Efficacy of Single-Port Video-Assisted Thoracoscopic Surgery Lobectomy Compared with Triple-Port VATS by Propensity Score Matching

  • Background: In recent years, single-port video-assisted thoracoscopic surgery (VATS) for lobectomy in non-small cell lung cancer (NSCLC) patients has become increasingly common. The objective of this study was to compare the feasibility and safety of single-port and triple-port VATS lobectomy. Methods: A total of 73 patients with NSCLC who underwent VATS lobectomy from December 2011 to August 2016 were retrospectively reviewed, including 47 in the triple-port group and 26 in the single-port group. Statistical analysis was performed after propensity score matching. Patients were matched on a 1-to-1 basis. Results: Operative time and intraoperative blood loss in the triple-port group and the single-port group were similar ($189.4{\pm}50.8minutes$ vs. $205.4{\pm}50.6minutes$, p=0.259; $286.5{\pm}531.0mL$ vs. $314.6{\pm}513.1mL$, p=0.813). There were no cases of morbidity or mortality. No significant differences in complications or the total number of dissected lymph nodes were found between the 2 groups. In the single-port group, more mediastinal lymph nodes were dissected than in the triple-port group ($1.7{\pm}0.6$ vs. $1.2{\pm}0.5$, p=0.011). Both groups had 1 patient with bronchopleural fistula. Chest tube duration and postoperative hospital stay were shorter in the single-port group than in the triple-port group ($8.7{\pm}5.1days$ vs. $6.2{\pm}6.6days$, p=0.130; $11.7{\pm}6.1days$ vs. $9.5{\pm}6.4days$, p=0.226). However, the differences were not statistically significant. In the single-port group, the rate of conversion to multi-port VATS lobectomy was 11.5% (3 of 26). The rates of conversion to open thoracotomy in the triple-port and single-port groups were 7.7% and 3.8%, respectively (p=1.000). Conclusion: In comparison with the triple-port group, single-port VATS lobectomy showed similar results in safety and efficacy, indicating that single-port VATS lobectomy is a feasible and safe option for lung cancer patients.

양전자단층촬영/전산화단층촬영(integrated PET/CT)을 이용한 비소세포폐암의 림프절 병기판정 (Accuracy of Nodal Staging with Integrated PET/CT Scanning in Non-small Cell Lung Cancer)

  • 배경: 최근 양전자단층촬영/전산화단층촬영(PET/CT) 검사가 점차 보편화 되어가고 있다. 림프절 병기 진단에 있어서 CT와 PET/CT의 유용성 및 실효성에 대해 알아보고자 하였다. 대상 및 방법: 2006년 1월부터 2009년 8월까지 본원에서 악성종양으로 폐절제술을 받은 110명의 환자 중, 술 전 본원에서 조영증강 흉부전산화단층촬영 (CT)과 전신 양전자단층촬영/전산화단층촬영 결합영상(PET/CT fusion imaging, integrated PET/CT) 검사를 모두 시행 받고, 술 후 원발성 비소세포폐암을 진단 받은 48명의 환자들을 대상으로 하였다. 림프절을 superior mediastinal nodes, aortic nodes, inferior mediastinal nodes, 그리고 Nl nodes의 4군으로 분류하여 CT와 PET/CT의 진단력에 대한 통계분석을 실시하였다. 결과: 4개의 군 가운데, inferior mediastinal nodes를 제외한 나머지 군들은 CT보다 PET/CT에서 민감도가 향상되는 소견을 보였다. 하지만 두 검사의 진단력 차이를 각각의 군에 대해 McNemar's test를 사용하여 분석한 결과 통계적으로 유의한 차이는 보이지 않았다(p-values; superior mediastinal nodes=0.109, aortic nodes=1.000, inferior mediastinal nodes=0.625, Nl nodes=0.424). 결론: 본 연구에서 종격동 림프절에 대한 PET/CT의 진단력은 조영증강 CT와 비슷한 정도로 나타났다. 현재 PET/CT는 CT와 상호보완적인 수단으로써 사용되어야 할 것으로 사료되며, 앞으로 촬영 기술 및 판독 수준이 더욱 발전하고 결과들이 많이 축적되었을 때에는 보다 좋은 성적을 보일 것으로 생각된다.

Isolated Cervical Lymph Node Sarcoidosis Presenting in an Asymptomatic Neck Mass: A Case Report

  • Sarcoidosis, a systemic granulomatous disease of unknown etiology. The presentation of sarcoidal granuloma in neck nodes without typical manifestations of systemic sarcoidosis is difficult to diagnose. We describe the case of a 37-year-old woman with an increasing mass on the right side of neck. The excisional biopsy from the neck mass showed noncaseating epithelioid cell granuloma of the lymph nodes. No evidence of mycobacterial or fungal infection was noted. Thoracic evaluations did not show enlargement of mediastinal lymph nodes or parenchymal abnormalities. Immunohistochemistry showed abundant expression of tumor necrosis factor-${\alpha}$ in the granuloma. However, transforming growth factor-${\beta}$ was not expressed, although interleukin-$1{\beta}$ was focally expressed. These immunohistochemical findings supported characterization of the granuloma and the diagnosis of sarcoidosis. Sarcoidosis can present with cervical lymph node enlargement without mediastinal or lung abnormality. Immunohistochemistry may support the diagnosis of sarcoidosis and characterization of granuloma.

폐에 발생한 암육종 - 치험 2례 - (Sarcomatoid Carcinoma of the Lung - Two cases report -)

  • The pulmonary sarcomatoid carcinoma is a rare malignant tumor, which is composed of an admixture of carcinomatous and sarcomatous components, and accounts for 0.3% of all pulmonary neoplasms. Clinicopathological features are often related to anatomical location: central endobronchial type and peripheral parenchymal type. Noninvasive diagnostic test had a low yield in detection sarcomatoid carcinoma. Metastasis to the regional lymph nodes and to distant organ is common. The prognosis is poor and the median survival is about 15 months. We report two cases of pulmonary sarcomatoid carcinoma with review of literatures.

종격동 임파관종;1례 보고 (Mediastinal lymphangioma - 1 Case -)

  • Lymphangioma, or cystic hygroma, a malformation resulting from the sequestration of lymph vessels, is owing to a failure of the primitive jugular lymphatic sacs to join with the venous system. They are rare benign lesions found more frequently in children than in adults. We experienced a case of huge mediastinal lymphangioma in 37-year-old female patient. It was associated with a cervical prolongation, and caused deviation and compression of the trachea and the heart. The cervicomediastinal lymphangioma was completely resected by means of median sternotomy, and the post-operative course was unevenful.

폐의 암육종 치험2례 (Pulmonary Carcinosarcoma; Two Cases Report)

  • Carcinosarcoma of the lung is an admixture of cardinomatous and sarcomatous lesion and accounts for 0.3% of all pulmonary neoplasm. Clinicopathological features are often related to anatomical location : central endobronchial type and peripheral parenchymal type. Noninvasive diagnostic tests had a low yield in detection carcinosarcoma. Metastasis to regional lymph nodes and to distant organ is common. The prognosis is poor and the median survival is less than one year.Two cases of carcinosarcoma patients are reported here - one case is composed of undifferent cell carcinoma and spindle cell sarcoma in the male of 72 year old and the other case squamous cell carcinoma and spindle cell sarcoma in the male of 65 year old.
