우심증에서 부분 폐정맥 이상환류를 동반한 심방중격결손 -1례 보고- (Partial Anomalous Pulmonary Venous Drainage Associated with Septal Defect in Dextrocardia)

  • Partial anomalous pulmonary drainage is frequently accompanied by an atrial septal defect. The anomaly is entirely compatible with life and the clinical findings are dependent primarily on the proportion of the pulmonary venous return to the right atrium. The anomaly is frequently missed even with complete diagnostic study. In this report we are presenting a case of anomalous pulmonary venous drainage combined with atrial septal defect and dextrocardia and the treatment of this condition.

  • Recently, cardiac injury due to blunt thoracic trauma appears to be increasing in frequency. The rising incidence of this mishap may relate to the absolute increase in automobile accidents as well as to more universal recognition that cardiac damage may have been sustained. We have experienced a rare case of ventricular septal defect caused by non-penetrating thoracic trauma. Of further interest is the history of chest trauma, clearly resulting in rupture of the chordae tendineae of the tricuspid valve successfully treated by operation-re-placement with two, 6 - 0, double-armed, expanded polytetrafluoroethylene sutures-2 months later. The unique combination of ventricular septal defect and rupture of the chordae tendineae of the tricuspid valve secondary to non-penetrating thoracic trauma is presented below to emphasize another variety of cardiac injury.

  • We experienced 1 year old female patient with partial anomalous pulmonary venous return of the right lung into the inferior vena cava with atrial septal defect. Total anomalous venous drainage from the right lung is an uncommon form of congenital defect, and it may or may not be associated with an atrial septal defect. One patient having this venous anomaly is presented with a detailed description of the surgical reconstruction used. The postoperative result has been satisfactory.

  • A case of ventricular septal defect associated with aorticopulmonary septal defect, left superior vena cava, and pulmonary hypertension in 6 years old boy is presented. Teflon patch closure of ventricular septal defect through transtricuspital procedure and division and suture closure of aorticopulmonary window performed under cardiopulmonary bypass with cardioplegia. The postoperative course was uneventful and discharged with good general condition.

  • 부분 심내막상 결손증은 일차공 결손과 승모판 전엽에 열공을 동반하는 질환으로 완전형 심내막상 결손증에 비해 임상 증상이 심하지 않으나, 승모판 부전이 진행할수록 심비대, 심부전, 폐동맥 고혈압 등이 더욱 쉽게 발현되어 그 예후가 좋지 않고, 따라서 고령에서는 보기 힘든 질환이다. 본원으로 30년 전부터 증상이 있던 67세 부분 심내막상 결손증 환자가 내원하였다. 심도자 검사상 폐동맥압이 45/22 mmHg, 평균 압력은 32 mmHg로 약간의 폐동맥 고혈압이 있었고, 수술 소견상 $3.5{\times}2.5\;cm$ 크기의 일차공 결손과 심한 석회화를 동반한 승모판 전엽의 열공이 보여 일차공 결손 첨포 봉합과 승모판막 치환술을 시행하였다. 수술 후 11일째부터 심한 방설 차단이 발생하여, 영구 심박동기를 삽입하였고, 수술 27일 후 퇴원하였다. 저자들은 임상적으로 보기 드문 고령에서의 부분 심내막상 결손증을 수술적으로 교정하였기에 문헌 고찰과 함께 보고하는 바이다.

  • 2005년 12월부터 본원에서는 내시경 수술 보조 로봇(AESOP2000)을 이용하여 승모판막질환, 삼첨판막질환, 심방중격결손, 심방세동이 있는 경우 선택적으로 최소침습적 수술을 시행하고 있다. 이에 대한 경험이 쌓이면서 보다 더 나은 수술 시야와 기구조작의 숙련성을 얻을 수 있었고, 최근 성인에서 막상주위 심실중격결손을 성공적으로 교정하였기에 이를 보고하는 바이다.

  • 심근경색 후 심실중격결손은 흔히 심인성 쇼크를 일으키며 심근경색 후 초기에 수술적 치료를 요하는 질환이다. 비록 지난 30년간 이 질환의 사망률은 감소하였으나 아직도 상대적으로 높은 편이다. 본 증례에서는 전반적인 심근허혈을 예방하기 위하여 심근경색 후 심실중격결손을 심박동 상태에서 교정하는 수술수기를 사용하였다. 이러한 접근방법은 대동맥차단이 필요없으며 우수한 심근보호를 제공한다.

  • Here, we report the rare case of a 13-year-old girl with a congenital diaphragmatic hernia (also known as Bochdalek hernia), which was revealed to be an extralobar pulmonary sequestration that was treated using laparoscopic and video-assisted thoracic surgery sequestrectomy and repair of the diaphragm defect after detection of a supradiaphragmatic mass connected with the retroperitoneum. The patient showed no postoperative complications at a 1-month follow-up examination.

Surgical Outcomes of Congenital Atrial Septal Defect Using da VinciTM Surgical Robot System

  • Background: Minimally invasive cardiac surgery has emerged as an alternative to conventional open surgery. This report reviews our experience with atrial septal defect using the da VinciTM surgical robot system. Materials and Methods: This retrospective study included 50 consecutive patients who underwent atrial septal defect repair using the da VinciTM surgical robot system between October 2007 and May 2011. Among these, 13 patients (26%) were approached through a totally endoscopic approach and the others by mini-thoracotomy. Nineteen patients had concomitant procedures including tricuspid annuloplasty (n=10), mitral valvuloplasty (n=9), and maze procedure (n=4). The mean follow-up duration was $16.9{\pm}10.4$ months. Results: No remnant interatrial shunt was detected by intraoperative or postoperative echocardiography. The atrial septal defects were mainly repaired by Gore-Tex patch closure (80%). There was no operative mortality or serious surgical complications. The aortic cross clamping time and cardiopulmonary bypass time were $74.1{\pm}32.2$ and $157.6{\pm}49.7$ minutes, respectively. The postoperative hospital stay was $5.5{\pm}3.3$ days. Conclusion: The atrial septal defect repair with concomitant procedures like mitral valve repair or tricuspid valve repair using the da VinciTM system is a feasible method. In addition, in selected patients, complete port access can be helpful for better cosmetic results and less musculoskeletal injury.

  • Patients with an atrial septal defect (ASD) and severe pulmonary arterial hypertension (PAH) are considered ineligible for defect closure surgery because of the risk of right ventricular decompensation and death after the operation. We report the case of a patient with large ASD and severe PAH who was able to undergo defect closure surgery successfully following long-term use of combined oral sildenafil and beraprost.
