냉수 가글링이 정형외과 수술환자의 갈증, 구강상태, 인후통에 미치는 효과 (The Effects of Cold Water Gargling on Thirst, Oral Cavity Condition, and Sore Throat in Orthopedics Surgery Patients)

  • Purpose: A purpose of this study was to identify the effects of cold water gargling on thirst, oral cavity condition and sore throat in orthopedics surgery patients who underwent general anesthesia. Methods: Pretest-posttest non-synchronized design with a nonequivalent control group was used. A total of 52 participants were conveniently selected and assigned to each group. The experimental group gargled hourly with cold water for 8 hours after operation. The control group received wet gauze as needed. We compared the thirst, oral cavity condition and sore throat at 0, 2, 4 and 8 hours after operation. Results: The score of thirst and oral cavity condition was lower in the experimental group than in the control group. However, there was no significant differences in the levels of sore throat between the experimental and control group. Conclusion: These findings suggest that cold water gargling can be a useful nursing intervention for alleviating thirst and improving oral condition for patients with orthopedic surgery after general anesthesia.

  • Purpose: This study was aimed to compare the effect between wet gauze with cold normal saline and wet gauze with cold water on thirst, oral cavity condition, and saliva pH among postoperative patients. Methods: This study was a quasi-experimental study with 56 participants. The experimental group received wet gauze with cold normal saline, and control group received wet gauze with cold water for three times at 15 minute intervals. Data were collected using visual analog scale for thirst, oral assessment guide for oral cavity condition and acidity tape for saliva pH. Measurement were made before applying the gauze and at 15min, 30min, and 45min after applying gauze. Data were analyzed using repeated measured ANOVA. Results: Thirst, oral cavity condition, and saliva pH improved in both groups with increasing number of gauze application. Thirst for the experimental group improved more than for the control group (F=4.29, p=.009), oral cavity condition except saliva, and saliva pH were not significantly different between the groups. Conclusion: This study results indicated that nurses can apply wet gauze with cold normal saline to reduce thirst and saliva acidity, and to improve the oral cavity condition for postoperative patients.

  • Purpose: This study compared the effects of three oral care methods on thirst and oral status in patients after general anesthesia surgery. Methods: Sixty five surgical patients were prospectively randomized into three groups. Each group received one of three oral care methods: wet water gauze, frozen water gauze, or frozen normal saline gauze. The outcomes of thirst and oral status were assessed three times by trained investigators blinded to the oral care methods. Assessment times were right after a participant's arrival to his/her room, and at 30 and 60 minutes after the pre-test. Results: The levels of thirst and oral status were significantly improved as time passed in all three oral care methods. While the level of thirst was not significantly different between the three groups, the level of oral status was significantly better with wet water gauze than frozen gauze groups, soaked in either water or normal saline. Conclusion: We recommend nurses choose one of three oral care methods based on patients' preferences since all three oral care methods showed similar effects in improving postoperative patients' levels of thirst and oral status.

  • Purpose: This study was aimed to compare the effects of lemon ice and water ice on decreasing thirst of the patients with nasal surgery. Methods: A nonequivalent control group non-synchronized design was used for the study. The level of thirst and oral assessment were measured. A total of 60 subjects was recruited; 30 subjects for the experimental group in which lemon ice was provided and the other 30 subjects were in the control group in which on water ice was administered. Results: The scores of thirst were decreased in both lemon ice and water ice group. But it appeared that the score of thirst in the experimental group is significantly lower than that of the control group. Moreover, the score of the assessment of the patient's oral cavity was also significantly improved in the experimental group than that of the control group. Conclusion: The lemon ice seems an effective and easy-to-apply intervention in reducing thirst and mouth dryness over water ice in nursing practice.

  • Purpose: This study aimed to compare the effects of oral swab and non-swab using cold water gauze on patients' thirst and oral status following nasal surgery under general anesthesia. Methods: A non-equivalent control group was applied. Participants were divided into a control group (n=30) that was treated by cared for with cold water gauze application without oral swab and an experimental group (n=29) that was treated cared for with oral swab using cold water gauze after nasal surgery at the G University Hospital in Korea. Data were collected from May 14, 2020, to April 30, 2021, and analyzed using descriptive statistics, a chi-squared test, independent t-test, Wilcoxon signed-ranks test, and Mann-Whitney test. Results: The results showed no significant differences between the two groups in thirst (U=-0.04, p=.693) and overall oral condition (U=-0.34, p=.813) after the intervention. However, participants' thirst and oral condition were significantly improved in each group after intervention. Conclusions: It was confirmed that both oral care methods reduced thirst and improved oral condition after nasal surgery. These findings indicate the need for intervention for patients' thirst and oral condition after nasal surgery. Furthermore, they show that these oral care protocols can be used as a safe and effective nursing intervention for patients who undergo nasal surgery under general anesthesia.

  • Purpose: The main purpose of this study was to effect oral care methods between essential oil gargling and purified water gargling for postoperative patients who had general surgery or orthopedic surgery. Methods: The postoperative patients were assigned to one of two groups. One group gargled with essential oil and the other with purified water. All group gargled three times interval 2 hours. Each patients thirst, oral condition and halitosis were assessed four times. Results: After oral care was provided once, there were significant differences in thirst level between two groups. when oral care was provided once and three times, there were significant differences in oral condition between two groups. but there were no significant differences in halitosis between two groups. Conclusion: The results show that essential oil gargling is a more effective intervention than purified water gargling for post operative patients oral care in reducing the thirst level and improving the condition of the oral cavity.

  • Purpose: The aim of this study was to investigate the effect of hominis placenta herbal aupuncture therapy on the postpartum women's heat feeling, sweat and thirst. Methods: The subjects were 25 women who admitted for postpartum treatment in Hospital of Woosuk University from 15th November 2008 to 5th February 2010. The control group(normal saline group, NSG) were 12 women and hominis placenta herbal acupunture treatment group(HPAG) were 13 women. We injected at Kwanwon(CV4), Sinsu(BL23). We checked VAS(heat feeling, sweat and thirst), vital sign, wrong response. We tested CBC, 7-zone-diagnostic system, HRV before and after clinical study. Results: VAS of thirst was significantly decresed in the HPAG than NSG. Ln(LF) of HPAG was lower than NSG after clinical study. Conclusion: The results suggest that hominis placenta herbal aupuncture therapy may be useful for the postpartum women.

  • Purpose: The purpose of this paper was to identify blood pressure, interdialytic weight gain, thirst and intradialytic discomfort in subjects after applying individual low-sodium dialysis fluid (1,2,3 mEq/L) to hemodialysis patients for 12 weeks. Methods: This study was a non-equivalent pre-post design. For 12 weeks, dialysate concentration was maintained at 1 mEq/L or 2 mEq/L or 3 mEq/L based on average sodium concentration of each individual, and the difference was compared after applying individually. Results: Change in blood pressure significantly decreased in the group where in pre-hemodialysis systolic pressure decreased the gradient of sodium concentration in serum sodium and dialysis solution by 2mEq/L. Interdialytic weight gain, and thirst showed significant decrease in all three groups. But in all three groups, intradialytic discomfort among dialysis showed no significant changes. Conclusion: Although application of low sodium dialysis fluid showed no change in intradialytic discomfort, lowered blood pressure, thirst, and interdialytic weight gain, which could be used for individual showing increased interdialytic weight gain and increased blood pressure. There is need for continued study on this.

Mechanisms Controlling Feed Intake in Large-type Goats Fed on Dry Forage

  • An intracerebroventricular (ICV) infusion of somatostatin 1-28 (SRIF) was used as a thirst-controlling peptide antagonist to investigate whether or not thirst-controlling peptides are involved in the significant decrease in feed intake during the initial stages of feeding large-type goats on dry forage. A continuous ICV infusion of SRIF was conducted at a small dose of $4{\mu}g$ ml/h for 27 h from day 1 to day 2. Goats (n = 5) were fed roughly crushed alfalfa hay cubes for 2 h twice daily and water was given ad libitum. Feed intake was measured during ICV infusion of artificial cerebrospinal fluid (ACSF) and SRIF. The feed intake during SRIF infusion increased significantly compared to that during ACSF infusion. In comparison to the ACSF treatment, plasma osmolality during the SRIF treatment significantly decreased during the first half of the 2 h feeding period. The factor causing the decrease in plasma osmolality during the ICV infusion of SRIF was a decrease in plasma Na, K, Cl, and Mg concentrations. In comparison to the ACSF infusion treatment, parotid saliva secretion volumes during the 2 h feeding period in the SRIF infusion treatment were significantly larger. While there was no significant difference in cumulative water intake (thirst levels) between the SRIF and the ACSF treatments upon conclusion of the 2 h feeding period, based on the plasma osmolality results it is thought that thirst level increases brought about by alfalfa hay cube feeding in the first half of the feeding period were reduced. It is thought that the somatostatin-induced increases in feed intake during the 2 h feeding period in the present experiment were caused by decreases in plasma osmolality brought about by the somatostatin infusion. As a result, it is suggested that the significant decrease in feed intake during the initial stages of feeding in large-type goats given roughly crushed alfalfa hay cubes, was due to the actions of thirst-controlling peptides.

VENGEANCE, VIOLENCE, VAMPIRES: Dark Humour in the Films of Park Chan-wook

  • This essay places the South Korean film Thirst (2009) within Park Chan-wook's oeuvre as a filmmaker notorious for graphic depictions of violence and revenge. Park's use of dark humour in his films, which is emphasized in Thirst perhaps more than ever, allows for a more self-aware depiction of violence, where both the viewer and the protagonist are awakened to the futility of revenge. This ultimately paints his characters as fascinatingly crazy - simultaneously heroes, villains, and victims. Film theorist Wes D. Gehring's three themes of dark humour ('man as beast,' 'the absurdity of the world,' and 'the omnipresence of death') become most obvious in Park's most recent film, which pays closer attention to character development through narrative detail. Rather than portraying the characters as sentimental, dark humour depicts their misfortunes in an alternative way, allowing for consideration of such taboo subjects as religion, adultery, and death/suicide. These issues are further tackled through Thirst's portrayal of its vampire protagonist, which ultimately de-mystifies the traditional vampire figure. While this character has more often been associated with romance, exoticism and the mystical powers of the supernatural, Thirst takes relatively little from the demons of Nosferatu (Murnau, 1922) and various other Dracula adaptations, nor the romantic figures of Interview with the Vampire (Jordan, 1994), and Twilight (Hardwicke, 2008). Instead, it is part of a much smaller group of contemporary vampire films, which are rather informed by a postmodern reconfiguration of the monster. Thus, this paper examines Thirst as an important contribution to the global and hybrid nature of those films in which postmodern vampires are sympathetic and de-mystified, exhibiting symptoms stemming from a natural illness or misfortune. Park's undertaking of a vampire film allows for a complex balance between narrative and visuals through his focus on the Western implications of this myth within Korean cinema. This combination of international references and traditional Korean culture marks it as highly conscious of New Korean Cinema's focus on globalization. With Thirst, Park successfully unites familiar images of the vampire hunting and feeding, with more stylistically distinct, grotesque images of violence and revenge. In this sense, dark humour highlights the less charming aspects of the vampire struggling to survive, most effective in scenes depicting the protagonist feeding from his friend's IV in the hospital, and sitting in the sunlight, slowly turning to ash, in the final minutes of the film. The international appeal of Park's style, combining conventions of the horror/thriller genre with his own mixture of dark humour and non-linear narrative, is epitomized in Thirst, which underscores South Korea's growing global interest with its overt international framework. Furthermore, he portrayal of the vampire as a sympathetic figure allows for a shift away from the conventional focus on myth and the exotic, toward a renewed construction of the vampire in terms of its contribution to generic hybridization and cultural adaptation.
