Most patients with Tourette's disorder experience an uncomfortable sensory phenomenon called the premonitory urge immediately before experiencing tics. It has been suggested that premonitory urges are associated with comorbidities such as obsessive compulsive disorder, anxiety disorders, and attention-deficit/hyperactivity disorder, although these associations have been inconsistent. Most patients experience tics as a result of the premonitory urges, and after the tics occur, most patients report that the premonitory urges are temporarily relieved. As a consequence, several studies have assessed the premonitory urge and its potential therapeutic utility. Based on the concept that the premonitory urge induces tics, behavioral treatments such as Exposure and Response Prevention and Habit Reversal Therapy have been developed. However, it is still unclear whether habituation, the main mechanism of these therapies, is directly related to their effectiveness. Moreover, the observed effects of pharmacological treatments on premonitory urges have been inconsistent.
Objectives: The aim of this systematic review and meta-analysis is to assess and compare the effectiveness of manual therapy in alleviating infant crying, a common symptom of nocturnal crying (NC) and infantile colic (IC). Methods: Total effective rate, crying time and adverse events were used as outcome indicators. To assess the quality, the risk of bias was determined for each study by two authors, using the Cochrane Collaboration's risk of bias tool. RevMan 5.0 was used for data analysis. A total of 98 articles were identified from 6 electronic databases. Results: Among them, twenty-seven studies which included 13 NC and 14 IC were included. Meta-analysis showed favorable effects tuina therapy on total effective rate (TER) of NC (RR: 1.20 [95% CI 1.05 to 1.37], p = 0.007), chiropractic therapy on crying time change of IC (SMD: -0.83 [95% CI -1.61 to -0.06], p = 0.04) and massage on total crying time of IC (SMD: -0.86 [95% CI -1.09 to -0.63], p < 0.00001). This systematic review compares different manual therapies for the treatment of NC and IC. While tuina, chiropractic, and massage show results in alleviating symptoms, the overall evidence remains limited due to the low quality and heterogeneity of the included studies. Conclusion: Therefore, further high-quality research with unified control groups is needed to establish manual therapy as a recommended treatment option for NC and IC. Purpose: To analyze the growth response to growth hormone (GH) therapy in prepubertal patients with Noonan syndrome (NS) harboring different genetic mutations. Methods: Twenty-three patients with prepubertal NS treated at Pusan National University Children's Hospital between March 2009 and July 2017 were enrolled. According to the disease-causing genes identified, the patients with NS were divided into 4 groups. Three groups were positive for mutations of the PTPN11, RAF1, and SOS1 genes. The five genes undetected (FGU) group was negative for PTPN11, RAF1, SOS1, KRAS, and BRAF gene mutations. The influence of genotype was retrospectively analyzed by comparing the growth parameters after GH therapy. Results: The mean chronological age at the start of GH treatment was $5.85{\pm}2.67years$. At the beginning of the GH treatment, the height standard deviation score (SDS), growth velocity (GV), and lower levels of insulin-like growth factor-1 (IGF)-1 levels were not statistically different among the groups. All the 23 NS patients had significantly increased height SDS and serum IGF-1 level during the 3 years of treatment. GV was highest during the first year of treatment. During the 3 years of GH therapy, the PTPN11, RAF1, and SOS1 groups showed less improvement in height SDS, IGF-1 SDS, and GV, and less increase in bone age-to-chronological age ratio than the FGU group. Conclusion: The 3-year GH therapy in the 23 prepubertal patients with NS was effective in improving height SDS, GV, and serum IGF-1 levels. The FGU group showed a better response to recombinant human GH therapy than the PTPN11, RAF1, and SOS1 groups.

Purpose : The purpose of this experiment is to find out the effectiveness which exert influence on cognitive skills by using the Driver Rehabilitation program for senior citizens who are over 65 years old and live in Busan. Method : From July first, 2014 to August 28th, 2014, we researched the 60 elderly people who are over 65 and go to community relief center which is in Busan. - 30 are experimental group and the other is control group. In the experimental group, we used Nintendo wii's driving simulation program and RC Car driving program in the model road. For estimation, we used MVPT-3(Motor-Free Visual Perception Test-3), Trail Making Test - 1, Trail making Test - 2 and LOTCA(Loewenstein Occupational Therapy Cognitive Assessment). Result : Nintendo wii's driving simulation program and RC Car driving program in model road results efficient visual perception ability. This programs results effectively in visual perception ability and space perception ability. This programs results effectively in motor apraxia ability. This programs results effectively in control ability for visual perception. This programs results effectively in thinking operation. Conclusion : Nintendo Wii's driving simulation program and RC Car driving program in model road positively influence improving for visual perceptual ability and cognitive function of elderly people. Also it is considered as being more efficient for improving visual perceptual ability and cognitive function to implement basic rehabilitation training with driving rehabilitation program than basic training itself.

Objectives: This study conducted a retrospective review to compare the effectiveness of herbal medicine, while confirming the effectiveness of Korean medicine (KM) in traffic accidents (TA). Methods: We investigated the medical records of TA patients who were hospitalized at Muuido Korean medicine hospital from January 2016 to June 2018 retrospectively. Medical records included general characteristics (Gender, age), TA & treatment-related information (Collision type, chief complaint, hospitalization period, and herbal medicine prescription), and clinical scales (Five-scale improvement, numerical rating scale (NRS)). Statistical analysis was performed for data distribution and effectiveness comparison. Results: 755 TA patients showed 0.78:1 gender ratio. 20-50s years old, rear collision, pain in the cervical and lumbar spine, and hospitalization period within 14 days were the majority. KM improved 83.18% of 755 TA patients' symptoms and ROM over 'improvement' level, which indicated 30-70% improvement. With common KM treatments including acupuncture, cupping, heat therapy, and manipulation therapy, TA patients received various herbal medicines and herbal medicine for breaking severe blood stasis (Tongdo-san) showed improvement in both five-scale improvement and NRS change at the same time. Conclusions: We could confirm the effectiveness of KM, particularly herbal medicine in TA. It would be necessary to develop new herbal medicines such as Tongdo-san to treat TA-related symptoms.
Purpose: This study compared the effectiveness of sling exercise and McKenzie exercise in patients with acute cervical whiplash associated disorder (WAD) caused by rear-end collision. Methods: Thirty WAD patients were randomly assigned to one of three groups; a sling exercise (SE) group, a McKenzie exercise (McE) group, and a control group. Members of the SE and McE exercised three times a week for four weeks under the researcher's guidance. Three groups performed TENS treatment three times a week for four weeks. Diagnostic radiological equipment was used to measure whiplash injury. Visual analog scale (VAS), neck disability index (NDI), and range of motion (ROM) were used in this study. Results: The three groups showed a significant pre-post treatment difference in measures of VAS and NDI (p<0.05). The SE group showed a significant pre-post treatment difference in measures of flexion and extension changes compared to the McE and control group (p<0.05). The SE group also showed significantly greater improvement in the VAS, NDI, and ROM changes than the control group (p<0.05). Conclusion: These findings indicate that sling exercise is a stronger and more aggressive intervention for treatment of acute WAD patients.
The purpose of this research was to evaluate the effectiveness of the Counseling-Utilized Parent Education for Mothers (CUPEM). CUPEM was developed based on Developmentally Appropriate Practices, Solution-Focused Therapy, Satir's Experiential Family Therapy, and Cognitive-Behavioral Therapy. The subjects were 51 mothers of 4 and 5 year old children and randomly assigned either to the intervention group (n=26) or the comparison group (n=25). Mothers in the intervention group had 10-2 hour intervention sessions while those in the comparison group received the intervention after this study was completed. Both groups received pre-and post-test evaluation, and a follow-up test. The data were analyzed by t-test and paired t-test. The results showed that participanting mothers' parenting stress was lessened while sense of parenting competence and positive maternal behaviors during mother-child interaction were increased significantly. Second, these mothers' self-confidence and maternal attitude toward childrearing were improved. Third, this effect lasted for at least one month after the termination of this program.
The demand for orthodontic clear aligner therapy (CAT) has increased significantly over the last decade, offering advantages over the fixed appliances (FA) including enhanced aesthetics, better hygiene and comfort, along with minimal restrictions on the patient's diet. Moreover, a marked improvement in the efficacy of tooth movement using aligners has been documented. On the contrary, there have been known limitations of CAT including the compliances issues and the apparent lack of efficacy for certain types of tooth movement such as closure of extraction space compared to FA. Thus, evidence-based evaluation of the accuracy of prediction of tooth movement with clear aligners and their ability to effectively perform major tooth and root movements compared to FA are crucial. Although several systematic reviews have investigated various aspects of the effectiveness of CAT, we are yet to obtain a rather conclusive answer to this question. The current review attempted to summarize the evidence-based findings of most systematic reviews about CAT available to date. Major issues regarding the predictability of tooth movement, the role of attachments and auxiliaries in improving the effectiveness of CAT, and the treatment outcomes in comparison to FA were investigated. Clinical recommendation have been also elaborated based on the interpretation of the findings of all systematic reviews included in this study.

Objective : The purpose of this study was to identify the effect and difference of abdominal muscle strengthening exercise, back muscle stretching and multi-training on the lumbar flexibility of 20s adults. Method : The abdominal muscle strengthening exercise, back muscle stretching and multi-training were conducted 9 times targeting 21 subjects who attended K University from 2013 May 29 to June 14. Sit and Reach Tests were conducted 2 times(before and after exercise program) for flexibility test and measured data were processed with SPSS program WIN 12.0K. By the Wilcoxon signed rank test, the effectiveness of exercises are verified. By the Kruskal-Wallis test and Mann-Whitney test, the difference of effectiveness among the exercise groups are verified. Result : The results of this study were summerized below 1. Before and after exercise, in abdominal muscle strengthening exercise group, back muscle stretching group and multi-training group are showed statistically significant differences(p<.05), 2. There were statistically significant difference in the improvement of the flexibility between each group(p>.05). Conclusion : These data suggests that all of the 3 exercise programs are brought positive influences on the improvement of flexibility, and abdominal muscle strengthening exercises and flexibility multi-training are effective on the flexibility more than, back muscle stretching in 20s adults.

Purpose: The purpose of this study was to investigate the effect of exercise therapy and relaxation therapy on the level of ACTH and cortisol in workers and to determine, from this information, the effectiveness of applied exercise therapy and relaxation therapy as a nursing intervention method for workers experiencing stress. Method: The subjects were divided into an exercise therapy group. a relaxation therapy group, and a control group. The research design was a nonequivalent control group pretest-posttest design (exercise therapy : n=12. relaxation therapy: n=12, control group, the group without exercise or relaxation therapy: n= 12). The subjects in the exercise therapy group were given a particular intensity for each kp over 30 min, bicycle ergometer which uses the LX PE training system before and after four weeks of training. The subjects did 60-70% exercise intensity exercise therapy for eight weeks The relaxation therapy subjects were trained using a modified Jacobson's relaxation technique for eight weeks. The exercise and relaxation therapy groups were trained at the following intensity for eight weeks (3 times/week, 30 min/day). Changes in the levels of ACTH and cortisol were identified. Two-way ANOVA and multiple range test (SNK: Student Newman Keul) were used to examine the effects of exercise therapy and relaxation therapy at four and eight weeks. Result: 1. Changes in the level of ACTH in the relaxation therapy group were statistically significant at .05 level, and were better than that of the exercise therapy group after eight weeks. But there was no significant difference between the exercise therapy group and the control group. 2. Change in the level of cortisol in the relaxation therapy group were statistically significant at .05 level as compared to the exercise therapy group and the control group after four weeks, but not after eight weeks. Also, the relaxation therapy was statistically significant at .05 level after four weeks compared to before treatment, but there was a tendency toward increased cortisol after eight weeks. Conclusion: Relaxation therapy was found to be more effective in stress treatment than exercise therapy and as such is a desirable nursing intervention method for stress hormone reduction in workers.
