A Clinical Properties on Nutrients Intake, anthropometric Measurement and Serum Contents of Diabetic Subjects by Body Fat Distribution in Ansung District (경기도 안성지역 당뇨환자의 체지방분포 형태에 따른 영양소 섭취 실태와 체위 및 혈앵성상의 임상적 특징)

  • This study was carried out to investigate the clinical characteristics of 78 diabetics by body fat distribution pattern. It was based on the survey of nutritional status, anthropometric measurements and serum components of the patients. The results were as follows: the average ages of male and female subjects were 57.1, and that of female subjects was 58.9, respectively. The average diabetic durations of male and female subjects were 4.8 and 5.9 years, respectively. In male, the ratio of upper body obese subject was 62.5% and the lower body obese subjects was 37.5%, while those of female were 69.4% and 30.6%, respectively. In nutrient intake state, there was no significant difference between male and female subjects, but calory intake of upper body obese subjects was difference between male and female subjects, but calory intake of upper body obese subjects was tend to be higher in both male and female subjects. among the three major nutrients/calory ratio, protein/calory ratio was significantly lower in the male upper body obese subjects than in the male lower body obese subjects. Since weight, circumference of arm and waist, waist hip ratio(WHR) of both male and female subjects, body muscle mass(BMM) of male subjects and body mass index (BMI) of female subjects were significantly higher in upper body obese group, upper body obese subjects represented ore fat than lower body obese subjects. In male subjects, hemoglobin(Hb), A/G of upper body obese subjects were lower than the standard value, but there was no significant difference in the serum components between two subjects. In female subjects, Hb and A/G of lower body obese subjects were lower than those of the normal subjects, but Hb and Ht of upper body obese subjects were significantly higher than those of the lower body obese subjects.

The Comparative Analysis of Clinical Characteristics and Surgical Results between the Upper and Lower Lumbar Disc Herniations

  • Objective : There are differences in the clinical characteristics and surgical results between upper (L1-2 and L2-3) and lower (L3-4, L4-5, and L5-S1) lumbar disc herniations. We conducted this study to compare the clinical features and surgical outcomes between the two types of lumbar disc herniations. Methods : We retrospectively reviewed the clinical features of patients who underwent microdiscectomies from 2008 to 2012. We evaluated the clinical characteristics such as age, preoperative autonomic dysfunction, the presence or absence of previous lumbar surgery and fusion required during surgery. Visual Analogue Scale (VAS) scores about back pain and leg pain were evaluated preoperatively and at the final follow-up. Results : Upper lumbar group (n=15) was significantly older than lower lumbar group (n=148). The incidence of autonomic dysfunction was significantly higher in upper lumbar group. The number of patients with a previous lumbar surgery was significantly greater in upper lumbar group. There was no statistical significance for fusion required during surgery between two groups. Both groups showed a significant decrease in the VAS scores of leg pain. VAS scores of back pain were significantly decreased in lower lumbar group. But this was not seen in upper lumbar group. Both groups showed significant improvement of Oswestry Disability Index score. Conclusion : Upper lumbar group had different clinical characteristics from those of lower lumbar group and these include older age, a higher incidence of autonomic dysfunctions and a higher incidence of patients with previous lumbar surgery. There were no significant differences in surgical outcomes, except for back pain, between two groups.

  • The purpose of this study was to collect the information of the straight-wire appliance and to determine the amount of second-order bends in clinical orthodontics. The author analysed the study model of 50 individuals with normal occlusion and results were obtained as follows. 1. The crown angulation was 4 degree in upper central incisor, 7 degree in upper lateral incisor, and 0 degree in lower central incisor and lateral incisor. 2. The crown angulation was 8 degree in upper cuspid and 2 degree in lower cuspid. 3. The crown angulations were 4 degree in upper first bicuspid, upper second bicuspid and lower second bicuspid and 1 degree in lower first bicuspid. 4. The crown angulation was 3 degree in upper first molar, 0 degree in upper second molar, 5 degree in lower first molar and 8 degree in lower second molar. 5. The crown angulations in lower arch were progressively increased from first premolar to second molar. 6. In upper arch, as the crown angulation of one tooth was increased, those of adjacent teeth were increased, too. 7. In the case of lower arch, the crown angulation of cuspid was increased as that of lateral incisor was increased, the crown angulation of second premolar was increased as that of first premolar was increased, and similarity the crown angulation of second molar was increased as that of first molar was increased.

Body Surface Changes at Armhole Area for the Pattern of Armhole Line (진동둘레선 설계를 위한 진동체표변화에 관한 연구)

  • This study was done to provide the characteristics of body surface changes at armhole area. Experiments were carried out at upper arm and upper body inchuding 67 items, 74 segments by the stanard posture and arm movements. The subjects were 15 females of twenty aged. The major conclusions of this study are; 1. On the circumference items of upper arm, armhole circumference was decreased by all arm movement, especially high contracted on front armhole line. The most contracted segments of armhole circumference were from the shonlder point to front and back interscye breadth point. Axillary circumference was increased 5 cm to the utmost, so the function of sleeve pattern would be decided by axillary circumference. 2. The lengths of upper arm were decreased near center line, sleeve cap length was contracted 3∼4 cm. Posterior armpit point area was increased both length and breadth. According to the rates of expansion and contraction, the diagram of expansion and contraction of upper arm was suggested. 3. On upper body, back side chest breadth was increased exceedingly and shoulder length was decreased most. It was apparent that surface changes of upper body were greatly larger as far away from center front and center back line. The rates of expansion and contraction of upper body were also represented as a diagram of surface changes.

Representative Male Upper Body types of the 20s by the Combination of Direct and Indirect Measurement Values (직접측정치(直接測定値)와 간접측정치(間接測定値)의 결합(調合)에 의한 20대(代) 남성(男性) 상방신(上半身) 대표체형(代表體型) 연구( 硏究))

  • This study is human body measurement of 200 adult males in their 20s by both direct and indirect methods in order to reveal the representative male upper body types. Composition factors of body types were classified while the combination of direct-indirect measurement values was chosen. The following are the findings: 1) The analysis of direct-indirect measurement statistics showed the following results: 173.80cm (height), 69.87kg (weight), 95.58cm (chest girth), $24.67^{\circ}$ (right shoulder angle), and $9.34^{\circ}$ (shoulder width angle). 2) The factor analysis of the body types by direct measurement produced 5 factors: 1 (front length of upper body), 2 (front length of upper body), 3 (back length of upper body), 4 (circumference of upper body), and 5 (shoulders length). These factors accounted for 90.08%. Also, the cluster analysis of factor scores led to 3 types: 1 (33%, short, comparatively wide shoulders and full in the hips), 2 (25.1%, well-developed upper body in tall, inversed triangle), 3 (41%, average height, short upper body). 3) The body-type factor analysis by indirect measurement resulted in 6 factors with the explanation of 83.24%: 1 (rear upper bady thickness), 2 (front upper body width), 3 (front chest thickness), 4 (left-right shoulder angle), 5 (front width of protrusion distance in chest and shoulders), and 6 (neck's front-rear side angle).In addition, the cluster analysis of factor scores brought about 4 types: 1 (15%, well-developed front chest, beardless waist), 2 (23.5%, flat chest, with shoulder, drooping shoulders, strait neck), 3 (39%, with shoulder, curved back), and 4 (22.5%, narrow and thin, curved waist). 4) Among the 118 subjects (75%), 58 subjects commonly appearing in indirect measurement values group and direct measurement values group were chosen the representative body type group according to asterisk indexes. They had the highest frequency in direct type 3 and indirect type 3, whose combination represented the physical characteristics of the representative body types.

  • Purpose: The current study (case report) seeks to examine the effects of a proprioceptive neuromuscular facilitation (PNF) exercise program on the upper extremity function and activities of daily living (ADLs) in a stroke patient. Methods: The subject was a 42 year-old woman diagnosed with right-sided hemiplegia due to stroke. Exercise therapy was provided for 50 minutes per day (5-minute warm-up, 40-minute exercise, 5-minute wrap-up) three times per week for a four-week period. The manual function test (MFT) was used to evaluate upper extremity functions. The modified Barthel index (MBI) was utilized to measure ADL. Results: After intervention using various PNF program, the MFT score increased from 23 to 26 and the MBI score increased from 62 to 66. Conclusion: After the application of the PNF program, the MFT and MBI scores increased. As compensative movement of the upper extremities declined, the movement of the upper extremities became faster and softer.

Surgical Outcomes Associated with Operable Gastric Cancer in a Tertiary Care Indian Hospital

  • Purpose: Data on operable gastric cancer from India is sparse. The purpose of this study was to investigate the clinical details, histopathological demographics, and 5-year overall survival (OS) and disease free survival (DFS) associated with operable, non-metastatic gastric cancer in a dedicated upper gastrointestinal (GI) surgical unit in India. Materials and Methods: Data for patients diagnosed with operable gastric cancer between January 2006 and December 2014 were retrospectively analyzed. Data were collected from electronic hospital records in addition to mail and telephonic interviews when possible. Results: A total of 427 patients were included. The tumor was located in the pyloro-antral region in 263 patients (61.7%). Subtotal gastrectomy was performed in 291 patients and total gastrectomy in 136 patients. Tumor stage classification revealed 43 patients (10.0%) with stage I, 40 patients (9.4%) with stage IIA, 59 patients (13.9%) with stage IIB, 76 patients (17.8%) with stage IIIA, 96 patients (22.5%) with stage IIIB, and 113 patients (26.4%) with stage IIIC disease. Follow-up data were available for 71.6% of the patients with a mean duration of 32.4 months. Five-year DFS and OS were 39% and 59%, respectively. Conclusions: Despite presenting at an advanced stage, the 5-year DFS and OS of patients with operable gastric cancer treated at a dedicated upper GI unit of a tertiary care center in India was good.

Relationship between occupational stress and musculoskeletal symptoms of upper extremities among dental hygienists (치과위생사의 직무스트레스와 상지 근골격계 증상의 관련성)

  • Objectives : This study was purposed to estimate prevalence of upper extremity musculoskeletal symptoms, level of occupational stress and their relationship among dental hygienists. Methods : This questionnaire study was performed during May to October 2010 with 317 dental hygienists worked in Busan and Gyeongsangnam-do recruited as study population. The occupational stress and upper extremity musculoskeletal symptoms were accessed using questionnaire of the Korean Occupational Stress Scale and musculoskeletal symptoms from the Korea Occupational Safety Health Agency. Data analysis was performed with the descriptive analysis, chi-square test, and multiple logistic regression analysis using SPSS(ver 19.0K)program. Results : The prevalence of upper extremity musculoskeletal symptoms and the level of occupational stress were 79.2% and $52.67{\pm}4.86$ in dental hygienists. By multiple logistic regression analysis, job insecurity(OR=2.86, 95% CI: 1.38-5.94 in stress high risk group; reference-stress low risk group) and organizational system(OR=3.32, 95% CI: 1.55-7.08 in stress high risk group; reference-stress low risk group) were associated with upper extremity musculoskeletal symptoms in dental hygienists. Conclusions : The overall prevalence of upper extremity musculoskeletal symptoms and level of occupational stress in dental hygienists were relatively high and some sub-scales of occupational stress were related with upper extremity musculoskeletal symptoms. Not only previously reported risk factors of musculoskeletal symptoms but also occupational stress should consider to prevent and improve upper extremity musculoskeletal symptoms in dental hygienists.

  • Obstructive sleep apnea (OSA) is a common disorder characterized by recurrent cessation of breathing due to complete or partial upper airway occlusion during sleep. The incompetent tone of palatal, pharngeal, and glossal muscles which fail to maintain airway patency during sleep causes narrowing of the airway dimension and increased resistance of breathing. The identification of the sites of upper airway obstruction in patients with OSA is important in understanding the pathogenesis and deciding the treatment modality of snoring and/or OSA. Various upper airway imaging modalities have been used to assess upper airway size and precise localization of the sites of upper airway obstruction during sleep. Dynamic imaging modalities enabled assessment of dimensional changes in the upper airway during respiration and sleep. This article focused on reviews of various upper airway imaging modalities, especially dynamic upper airway imaging studies providing important information on the pathogenesis of OSA.

  • Development cases of space launch vehicle upper stage engine were studied. HM-7, Vinci, LE-5, RL10 engines are representative upper stage engines of Europe, Japan, and United States. It was realized that upper stage engines were developed with more than two engine test facilities and the development period was 5 to 8 years accompanied with 10~11 engines.
