Clinical Analysis of Bileaflet Mechanical Valve Replacement (Bileaflet Mechanical Valve의 임상적 고찰)

  • Experience with bileaflet mechanical valve replacement at the Inha Hospital in 192 patients, operated on from June 1986 until April 1993. Two hundred fourty-one prostheses [51 Duromedics, 79 St.Jude Medical, and 111 CafboMedics]were implanted during the total 195 operations. Mitral valve replacment[MVR]was done in 113 cases, aortic valve replacement[AVR]in 34, tricuspid valve replacement[TVR]in 2, and double valve replacement[DVR]in 46 cases.Of the total patients, 63.0% were women and 37.0% were men. The mean age of the patients was 40.8 years, ranged from 14 to 67years. Overall early mortality was 9.2\ulcorner%[18 out of 195]; 9.7%[11 out of 113]for MVR, 14,7% [5 out of 34]for AVR, and 4.3%[2 out of 46]for DVR. All of the operative survors were followed over a period of one to 83 months with a mean of 37 months, for total 543 patient-years. So far, eleven patients[6.7% of the long-term survivors]were lost to follov-up after a mean postoperative follow-up of 22.8 months. There were nine late deaths; three deaths due to prostetic valve endocarditis, two due to persistent heart failure, one due to cerebral hemorrhage, one due to aortic dissection after Bentall oreration, and two sudden deaths. Actuarial survival rate at 6.9 years was 94.8%, There were seventeen valve-related complications; three prosthetic valve thromboses, three thrombembolisms, three instances of prosthetic valve endocarditis, two paravalvular leakages, and six hemorrhagic complications related to anticoagulation. The actuarial rate of freedom from all valve-related complications at 6.9years was 91.3%. There were significant decreases in the heart size postoperatively that can be demonstrated by comparison of cardio-thoracic ratios on simple chest X-ray and left ventricle dimensions on echocardiography. We conclude that this midterm follow-up shows good results in terms of hemodynamics and durability although further long-term evaluations are mandatory.

Thermodynamic Modeling of Long-Term Phase Development of Slag Cement in Seawater (해수에 노출된 슬래그 시멘트의 장기 상변이 열역학 모델링)

  • Known to improve resistance to chloride ingress, blast furnace slag is a widely used supplementary cementitious material. However, a detailed characterization of cements blended with slag exposed to seawater remains unavailable. This study employs thermodynamic modeling as a toolkit for assessing the long-term phase evolution of slag cement in seawater. The modeling result shows that slag incorporation leads to the formation of phases that are less prone to structural alteration in seawater. Formation of more ettringite is expected to induce expansion in both plain and blended cements, while brucite is unstable in the blended systems. Despite this, the porosity is expected to increase in the blended cements, and aluminate hydrates with a higher chloride binding capacity are more abundant in the blended cements. The results suggest that the use of slag in concrete improves the durability performance of concrete in marine environments.

  • Solid oxide fuel cells(SOFCs) directly convert the fuel gases to electric energy through electrochemical reactions. The advantage of SOFCs is that they easily operate with diversified fuels such as natural gases owing to their high temperature operation. However, high temperature operation also incurs the challenge in enhancing long term reliability and durability of SOFCs. The most commonly used anode material is Ni/YSZ. This has, however, some drawbacks in terms of long-term reliability at high temperatures, hydrocarbon fuel usages, and so on, therefore the need to develop the new anode materials increases. This article summarizes the trend of the novel anode materials development of SOFCs.

Pilot Test with Pervaporation Seperation of Aqueous IPA Using a Composite PEI/PDMS Membrane Module (IPA/물 혼합액의 PEI/PDMS 복합막 모듈을 이용한 투과증발 파일롯 분리특성)

  • To determine the pervaporation separation characteristics of IPA/water mixtures, PEI/PDMS hollow fiber membrane module commercialized by Airrane Co. was subjected to both lab and pilot tests. The flux of $0.52kg/m^2h$ and IPA concentration of 68.5% at $25^{\circ}C$ were obtained whereas the $1.368kg/m^2h$ and 61.2% were measured at $55^{\circ}C$. In order to realized the durability of the module, the long-term test (at $50^{\circ}C$) of 100 days has been conducted and as a result, the flux $1.03{\sim}1.15kg/m^2h$ and IPA concentration 61.8~62.5% were maintained with the initial values.

Stress distribution in implant abutment components made of titanium alloy, zirconia, and polyetheretherketone: a comparative study using finite element analysis (티타늄 합금, 지르코니아, 폴리에테르에테르케톤 지대주 재질에 따른 임플란트 구성요소의 응력분포: 유한 요소 분석을 통한 비교 연구)

  • Purpose: This study aimed to analyze the stress distribution and deformation in implant abutments made from titanium (Ti-6Al-4V), zirconia, and polyetheretherketone (PEEK), including their screws and fixtures, under various loading conditions using finite element analysis (FEA). Methods: Three-dimensional models of the mandible with implant abutments were created using Siemens NX software (NX10.0.0.24, Siemens). FEA was conducted using Abaqus to simulate occlusal loads and assess stress distribution and deformation. Material properties such as Young's modulus and Poisson's ratio were assigned to each component based on literature and experimental data. Results: The FEA results revealed distinct stress distribution patterns among the materials. Titanium alloy abutments exhibited the highest stress resistance and the most uniform stress distribution, making them highly suitable for long-term stability. Zirconia abutments showed strong mechanical properties with higher stress concentration, indicating potential vulnerability to fracture despite their aesthetic advantages. PEEK abutments demonstrated the least stress resistance and higher deformation compared to other abutment materials, but offered superior shock absorption, though they posed a higher risk of mechanical failure under high load conditions. Conclusion: The study emphasizes the importance of selecting appropriate materials for dental implants. Titanium offers durability and uniform stress distribution, making it highly suitable for long-term stability. Zirconia provides aesthetic benefits but has a higher risk of fracture compared to titanium. PEEK excels in shock absorption but has a higher risk of mechanical failure compared to both titanium and zirconia. These insights can guide improved implant designs and material choices for various clinical needs.

Long-term Repeated-Batch Operation of Immobilized Escherichia coli Cells to Synthesize Galactooligosaccharide

  • In this study, we investigated whether galactooligosaccharide (GOS) can be stably and steadily synthesized using immobilized ${\beta}$-galactosidase (${\beta}$-gal) inclusion body (IB)-containing E. coli cells during long-term repeated-batch operation. To improve the operational stability of this enzyme reactor system, immobilized E. coli cells were crosslinked with glutaraldehyde (GA) after immobilization of the E. coli. When we treated with 2% GA for E. coli crosslinking, GOS production continued to an elapsed time of 576 h, in which seven batch runs were operated consecutively. GOS production ranged from 51.6 to 78.5 g/l ($71.2{\pm}10.5$ g/l, n = 7) during those batch operations. In contrast, when we crosslinked E. coli with 4% GA, GOS production ranged from 31.5 to 64.0 g/l ($52.3{\pm}10.8$, n = 4), and only four consecutive batch runs were operated. Although we did not use an industrial ${\beta}$-gal for GOS production, in which a thermophile is used routinely, this represents the longest operation time for GOS production using E. coli ${\beta}$-gal. Improved stability and durability of the cell immobilization system were achieved using the crosslinking protocol. This strategy could be directly applied to other microbial enzyme reactor systems using cell immobilization to extend the operation time and/or improve the reactor system stability.

Long-Term Efficacy and Safety of Golimumab for Ulcerative Colitis in a Pediatric Inflammatory Bowel Disease Center in Japan

  • Purpose: Golimumab (GLM) is an anti-tumor necrosis factor (TNF)-α antibody preparation known to be less immunogenic than infliximab (IFX) or adalimumab. Few reports on GLM in pediatric patients with ulcerative colitis (UC) are available. This study aimed to review the long-term durability and safety of GLM in a pediatric center. Methods: The medical records of 17 pediatric patients (eight boys and nine girls) who received GLM at the National Center for Child Health and Development were retrospectively reviewed. Results: The median age at GLM initiation was 13.9 (interquartile range 12.0-16.3) years. Fourteen patients had pancolitis, and 11 had severe disease (pediatric ulcerative colitis activity index ≥65). Ten patients were biologic-naive, and 50% achieved corticosteroid-free remission at week 54. Two patients discontinued prior anti-TNF-α agents because of adverse events during remission. Both showed responses to GLM without unfavorable events through week 54. However, the efficacy of GLM in patients who showed primary nonresponse or loss of response to IFX was limited. Four of the five patients showed non-response at week 54. Patients with severe disease had significantly lower corticosteroid-free remission rate at week 54 than those without severe disease. No severe adverse events were observed during the study period. Conclusion: GLM appears to be safe and useful for pediatric patients with UC. Patients with mild to moderate disease who responded to but had some adverse events with prior biologics may be good candidates for GLM. Its safety and low immunogenicity profile serve as favorable options for selected children with UC.

A Long Term Characteristics of Hydraulic Conductivity and Tensile Strength of Natural Fiber Drain with respect to Installation Conditions (천연섬유배수재의 타설 조건에 따른 장기 투수계수 및 인장강도의 특성)

  • Long term changes of hydraulic properties and tensile strength of natural fiber drain (NFD) are analyzed and compared with those of intact NFD's. NFD was buried in distilled water, two types of seawater and clay soils obtained in southern and western parts of Korea, Kwang-Yang and Si-hwa. Specimens are taken out in 0, 3, 9 and 18 month intervals, and durability tests of the NFD are performed. Hydraulic conductivity of the NFD samples decreased compared with that of intact NFD samples, because clay particles easily passed to coarsened mesh of filters and clogged the porous stone below and reduced hydraulic conductivity. Tensile strength of drains from the soil bucket is reduced larger than those in the seawater and the distilled water. Strength reduction was higher in summer than winter.

Effects of Partial Substitution of CeO2 with M2O3 (M = Yb, Gd, Sm) on Electrical Degradation of Sc2O3 and CeO2 Co-doped ZrO2

  • Scandia-stabilized zirconia co-doped with $CeO_2$ is a promising electrolyte for intermediate temperature SOFC, but still shows rapid degradation during a long-term operation. In this study, $CeO_2$ (1 mol%) as a stabilizer is partially substituted with lanthanum oxides ($M_2O_3$, M=Yb, Gd, Sm) to stabilize a cubic phase and thus durability in reducing atmosphere. 0.5M0.5Ce10ScSZ electrolytes were prepared by solid state reaction and sintered at $1450^{\circ}C$ for 10 h to produce dense ceramic specimens. With addition of the lanthanum oxide, 0.5M0.5Ce10ScSZ showed lower degradation rates than 1Ce10ScSZ. Since $Gd_2O_3$ showed the highest ionic conductivity among the co-dopants, an electrolyte-supported cell with 0.5Gd0.5Ce10ScSZ was prepared to compare its long-term performance with that of 1Ce10ScSZ-based cell. Maximum power density of 0.5Gd0.5Ce10ScSZ-based cell was degraded by about 2.3% after 250 h, which was much lower than 1Ce10ScSZ-based cell (4.2%).

Freezing and Thawing Resistance and fundamental Properties of Antiwashout Underwater Concrete Containing Mineral Admixtures (광물질혼화재 혼합 수중불분리성 콘크리트의 물성 및 동결융해 저항성)

  • Today the application of antiwashout underwater concrete to the construction sites is increasing steadily, while its reliability is in issue. Particularly, antiwashout underwater concrete is known to have very weak durability on frost attack, and hence Japan society of civil engineers(JSCE) regulated that not to use of antiwashout underwater concrete where the freezing and thawing is suspected. This study aims the improvement of the freezing and thawing resistance for antiwashout underwater concrete. From the results of fundamental test, FA20 and SG50 showed good performance in fluidity and long term compressive strength than control concrete. Meanwhile, MK10 marked the highest compressive strength through the whole curing age but a defect on fluidity was discovered. The results from the repeated freezing and thawing test show that the large volumes of air entrapped by cellulose based antiwashout underwater admixture gave bad effects to frost durability and hence not much benefits were confirmed from the use of mineral admixtures. However there were some increasing effects on frost durability of MK10 and SG50 by securing $6{\pm}0.5\%$ of entraining air. In the meantime, there was a increasing tendency of frost durability by increasing blame's fineness of ground granulated blast furnace slag.
