Treatment of Hip Microinstability with Arthroscopic Capsular Plication: A Retrospective Case Series

  • Purpose: Hip microinstability is defined as hip pain with a snapping and/or blocking sensation accompanied by fine anatomical anomalies. Arthroscopic capsular plication has been proposed as a treatment modality for patients without major anatomic anomalies and after failure of properly administered conservative treatment. The purpose of this study was to determine the efficacy of this procedure and to evaluate potential predictors of poor outcome. Materials and Methods: A review of 26 capsular plications in 25 patients was conducted. The mean postoperative follow-up period for the remaining patients was 29 months. Analysis of data included demographic, radiological, and interventional data. Calculation of pre- and postoperative WOMAC (Western Ontario and McMaster Universities Osteoarthritis) index was performed. Pre- and postoperative sports activities and satisfaction were also documented. A P<0.05 was considered significant. Results: No major complications were identified in this series. The mean pre- and postoperative WOMAC scores were 62.6 and 24.2, respectively. The WOMAC index showed statistically significant postoperative improvement (P=0.0009). The mean satisfaction rate was 7.7/10. Four patients with persistent pain underwent a periacetabular osteotomy. A lateral center edge angle ≤21° was detected in all hips at presentation. We were not able to demonstrate any difference in postoperative evolution with regard to the presence of hip dysplasia (P>0.05), probably because the sample size was too small. Conclusion: Capsular plication can result in significant clinical and functional improvement in carefully selected cases of hip microinstability.

고관절 전치환술 후 한방 재활치료에 대한 증례 보고 (A Rehabilitation for Total Hip Replacement in Korean Medicine: A Report of Three Cases)

  • The objective of this study was to propose a rehabilitation protocol for total hip replacement in Korean medicine and to report its effectiveness. Three patients who had undergone total hip replacement were treated using the rehabilitation protocol in Korean medicine. We evaluated the outcome evaluating physical examination findings of the hip joint, numeric rating scale, pain disability index, Harris hip score, and walking state. After the treatment, we observed that hip movement and muscular strength improved. Moreover numeric rating scale, pain disability index, Harris hip score, and walking state of patients improved. A rehabilitation in Korean medicine can be effectively used for total hip replacement patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean medicine.

클램 운동 시 운동범위에 따른 엉덩관절 벌림근의 활성도와 사용 비교 (Comparison of Activity and Use of Hip Abductor Muscle Group According to Range of Motion during the Clam Exercise)

  • Purpose: The purpose of this study is to compare the muscle activity of the hip abductors and the ratio between the hip abductor muscle group according to the range of hip abduction during the clam exercise. Methods: This study was conducted on 18 healthy men in their aged 20 to 29 who had not been diagnosed with spine-related diseases. The subjects performed a clam exercise without rotation of the pelvis in a state of 60° hip flexion and 90° knee joint flexion in the side-lying position. Using Myomotion equipment and EMG, the muscle activity of the hip abductor muscles and the activity ratio between the hip abductor muscle group were measured during the clam exercise by dividing the range into initial, mid-range, and terminal sections. Repeated measures analysis of variants was employed to compare the activity and use of hip abductor muscles according to range of motion during the clam exercise. Results: Gluteus medius muscle activation was significantly increased in the comparison of muscle activity in the initial, mid-range, and terminal sections of hip abduction. Tensor fasciae latae muscle activation was significantly increased in the comparison of muscle activity in all range of motion sections as well. The gluteus medius-tensor fasciae latae muscle activation ratio was significantly increased in the terminal section compared to the initial section. Conclusion: The gluteus medius and tensor fasciae latae had higher muscle activities as they approached the terminal section during the clam exercise, and the hip abduction activity ratio of the gluteus medius and tensor fasciae latae was higher as the range of motion approached the terminal section.

거들 착용에 따른 의복압과 신체치수 변화 및 거들 사이즈 개선 방안 (Distribution of garment pressure and body measurements after wearing a girdle, and suggestions for a girdle sizing system)

  • A girdle is a body-shaping article of clothing. The garment pressure of the girdle is considered an indispensable factor. The purpose of this study was to identify changes in body size and distribution of garment pressure after donning the girdle. The changes of body size and garment pressure were analyzed by body types. Korean women (n=19) in their 20s participated in the experiment. Their body types were classified according to four factors: the index value(hip girth-waist girth), and waist, hip, and thigh girths. The garment pressure was measured at 12 points. The results of this study showed that the hip and thigh girths were reduced mostly after donning the girdle. These values were 2.0~2.8cm and 1.7~2.3cm, respectively. The garment pressure was high at the waist band, the hip joint, and the gluteal furrow region at the back. The subjects whose waists, thighs, or hips were well developed showed great garment pressure in the hip area, but their hip girth decreased very little. The subjects with less developed or slim thighs or hips showed a slimming effect, with moderate pressure in the hip and thigh regions. These results show that the hip and thigh can be slimmed with moderate pressure by donning a thigh-length girdle. High garment pressure is not necessary for the girdle's body shaping effect. The hip area is hard to get body slimming effect with high garment pressure. The current girdle sizing system needs to be revised in order to lower garment pressure in the hip region. The researchers suggest using 3cm size intervals rather than 6cm size intervals for hip girth.

심부 안정성이 부족한 대상자에게 외부적 지지가 엉덩관절 벌림근의 근력과 근활성도에 미치는 영향 (The Effectiveness of the External Support on the Strength and Muscle Activity of Hip Abductor in Subject Without Core Stability)

  • Background: Gluteus medius muscle is important for the stability of hip joint. The sufficient core stability can be contributed to the performance of gluteus medius muscle in standing position. In addition, the external support may affect core stability in standing position. Objects: The purpose of this study was to investigated the effectiveness of the external support on the strength and muscle activity of hip abductor muscle during hip abduction in standing position in subjects without core stability. Methods: Fifteen subjects participated in this study. The subjects were evaluated by using the double-leg lowering with bent knees to measure the core stability. The strength and muscle activity of hip abductors was measured in standing position with the condition with and without external support using the tensiometer sensor and the surface electromyography. The paired t-test was used to investigate the difference between hip abductor strength and activity according to external support. The level of statistical significance was set at α = 0.05. Results: The hip abductor strength and muscle activity of gluteus medius muscle with external support were significantly greater than those without external support during hip abduction in standing position (p < 0.05). Conclusion: During hip abduction in standing position, the external support may be contributed to the improvement of the hip abductor strength and muscle activity of gluteus medius especially in the subjects without core stability.

고관절 운동 프로그램이 만성 요통이 있는 아마추어 골퍼의 통증과 골프수행 수준에 미치는 즉각적인 효과 (Immediate Effect of Hip Joint Exercise Program on Low Back Pain and Performance Level in Amateur Golfers with Chronic Low Back Pain)

  • PURPOSE: This study investigated the influence of a specific hip joint exercise program on the characteristics of chronic low back pain (LBP), hip joint range of motion (ROM), and performance level in amateur golfers with LBP. METHODS: The study included 28 adult male amateur golfers with LBP and reduced internal rotation (IR) of the lead side hip joint. Subjects were randomly assigned to an experimental (EG) (n=14) or control group (CG) (n=14). The Numerical Rating Scale score for pain, ROM of the hip joint and trunk rotation, performance parameters for golfing were evaluated. A specific exercise program designed to improve IR of the lead side hip joint was performed by the EG, but not the CG. Post-intervention evaluation was performed and the collected data were analyzed. RESULTS: After the intervention, the severity of LBP was significantly reduced (p<.05) and the IR ROM of the lead side hip joint was significantly increased (p<.01) in the EG. Moreover, the trunk rotation range was significantly increased in the EG (p<.01). The drive distance was significantly difference between the EG and CG (p<.05). CONCLUSION: A specific hip joint exercise program can help to reduce pain level and improve performance level in amateur golfers with LBP and increased IR ROM of the lead side hip joint. A significant increase in ROM of the lead side hip joint reduce LBP and was related to driving distance in the EG.

Comparison of trunk muscle activity according to hip abduction angle during plank exercise

  • Objective: The purpose of this study was to investigate the conditions for the application of various plank exercises to people who require trunk stabilization by comparing trunk muscle activity according to the degree of hip abduction in the plank exercise. Design: Cross-sectional study. Methods: Twenty healthy participants voluntarily participated in this study and the plank exercise was performed under 5 conditions (two-legged support plank [TSP] with hip abduction, TSP with hip abduction of 15 degrees, TSP with hip abduction of 30, one-legged support plank [OSP] with hip abduction of 15 degrees, OSP with hip abduction of 30 degrees). In order to measure the trunk muscle activity according to the 5 conditions, surface electromyography was used. The electrical activities of the rectus abdominis (RA), external oblique (EO), and internal oblique (IO) muscles were measured during the 5 plank exercises. Subjects practiced each of the 5 conditions three times in random order and the average values were obtained. Results: In the OSP condition with 15/30 degrees of hip abduction, activities of the RA, EO, and IO were significantly greater than during the TSP (p<0.05). In the OSP with 30 degrees of hip abduction condition, activities of the left EO, IO were significantly greater than other plank exercise conditions (p<0.05). Conclusions: The plank exercise with hip abduction of 30 degrees and the OSP exercise can be suggested as an effective method to enhance the activity of the trunk oblique muscles.

만성 요통 환자에게 수축 이완과 등장성 수축 결합이 엉덩관절과 허리골반 움직임에 미치는 영향 (The Effects of Contract Relaxation and a Combination of Isotonics on the Hip and Lumbopelvic Motions in Patients with Chronic Low Back Pain)

  • Purpose: The purpose of this study was to determine the effects of contract relaxation and a combination of isotonics in proprioceptive neuromucular facilitation (PNF) on hip and lumbopelvic motions of male patients with chronic low back pain. Methods: As the subjects of this study, a total of 45 male patients with chronic low back pain were divided into groups: 15 who underwent contract relaxation (CR) of the PNF of their hip joints, 15 who underwent a combination of isotonics (CI) of the PNF, and 15 who underwent both techniques. A device for analyzing three-dimensional motion was used to measure hip medial rotation angles, lumbopelvic rotation angles, and hip medial rotation angles at the start of lumbopelvic rotation during hip medial rotation. A two-way repeated measures ANOVA was used to compare the average values. Results: There were interactions in the hip medial rotation angles, lumbopelvic rotation angles and hip medial rotation angles at the start of lumbopelvic rotation based on the methods and periods of exercise (p < 0.05). The CR and the CR+CI groups displayed more increased hip medial angles when compared to the CI group. The CR+CI group had more decreased lumbopelvic rotation angles when compared to the CR and the CI groups. The CR+CI group had more increased hip medial rotation angles at the start of lumbopelvic rotation when compared to the CR and the CI groups. Conclusion: It can be concluded that the combination of stretching and stability exercises was effective in male patients with chronic low back pain who showed limited hip medial rotation.

Comparison of Knee Extensor and Hip Extensor Strength According to Wall Squat Performance

  • Background: The wall squat is considered an effective exercise because it can reduce the knee load and prevent excessive lumbar movement. However, the relationship between wall squat performance and strength of knee extensors and hip extensors remained unclear. Objects: The purpose of this study was to compare the strengths of the knee extensors and hip extensors between groups with low and high wall squat performance. Method: Nineteen males (low performance group: 9 subjects, high performance group: 10 subjects) participated in this study and performed wall squats. The subjects who were performing less than 30% of the average wall squat count were classified into the low wall squat performance group (less than or equal to 4 times) and the subjects who performed more than 30% of the average wall squat count were classified into the high wall squat performance group (greater than or equal to 8 times). Knee extensor and hip extensor strength were measured with a strength measurement system. An independent t-test was used to compare the strengths of the knee extensors and hip extensors between the groups with low and high wall squat performance. Results: The ratios of knee extensor and hip extensor strength to bodyweight were greater in the high wall squat performance group than in the low wall squat performance group (knee extensors: p<.001; hip extensors: p=.03). In the high- and low-performance groups, the ratios of knee extensor strength to bodyweight were $42.74{\pm}5.72$ and $30.76{\pm}8.54$, respectively, and the ratios of hip extensor strength to bodyweight were $31.95{\pm}10.61$ and $20.66{\pm}11.25$, respectively. Conclusion: Our findings suggest that knee extensor and hip extensor strength are needed for high wall squat performance. Thus, exercise to increase the knee and hip extensors strength can be recommended to improve squat performance.

Effect of Hip Adduction Position on the Vastus Medialis Oblique and Vastus Lateralis During Closed Kinetic Chain Exercise in Sitting Posture

  • Background: Several studies have discussed diverse exercise methods considered to be useful for the selective contraction of the vastus medialis oblique (VMO) muscle for the treatment of patellofemoral pain syndrome. Some studies have reported that exercise methods, including hip adduction, in closed kinetic chain exercises are more effective in terms of the muscle activation of the VMO and the timing of the muscle's initial contraction. We focused on isometric contraction during a closed kinetic chain exercise with hip adduction. Objects: The purpose of this study was to examine muscle activation in the VMO and the vastus lateralis (VL) and the onset time difference between their initial contractions via closed kinetic chain isometric quadriceps femoris exercises including hip adduction. Methods: In total, 36 healthy subjects adopted two hip positions during isometric contraction of the quadriceps femoris in a closed kinetic chain exercise (hip neutral and hip adduction position). Statistical analyses were conducted using a paired t-test (${\alpha}=.05$). Results: Isometric contraction of the quadriceps femoris in a closed kinetic chain exercise caused a greater increase in VMO muscle activity in the hip adduction position [$52.68{\pm}22.21$ percentage of maximal voluntary isometric contraction (%MVIC)]than the hip neutral position ($43.43{\pm}19.85%MVIC$). The onset time difference (VL-VMO) decreased more in the hip adduction position ($-82.14{\pm}34.2ms$) than the hip neutral position ($73.94{\pm}2.94ms$). Conclusion: We recommend this exercise as a clinically useful therapeutic method for patients with patellofemoral pain syndrome due to weakening of the VMO muscle and lateral inclination of the patella.
