등척성 엉덩관절 모음 및 벌림 시 골반 벨트가 엉덩관절 근육의 근력과 복부 근육 근 활성도에 미치는 영향 (Effects of a Pelvic Belt on Hip Muscle Forces and Abdominal Muscle Activities During Isometric Hip Adduction and Abduction)

  • Background: To improve lumbo-pelvic stability, passive support devices (i.e., a pelvic belt) are recommended clinically. Nevertheless, to understand the influence of passive support on lumbo-pelvic stability, it is necessary to examine the influence of a pelvic belt on the abdominal and hip abductor muscles. Objects: To examine the effects of a pelvic belt on the forces of the hip adductor and abductor muscles and activity of the abdominal muscles during isometric hip adduction and abduction. Methods: This study recruited 14 healthy men. All subjects performed isometric hip adduction and abduction with and without a pelvic belt in a neutral hip position. Load cells, wrapped with a non-elastic belt, were placed above the medial and lateral malleoli of the dominant leg to measure the muscle forces of the hip adductors and abductors, respectively. The forces of the hip adductors and abductors were measured using a load cell during isometric hip adduction and abduction, while the electromyographic activities of the bilateral rectus abdominis, internal oblique, and external oblique muscles were measured. Results: The forces generated by the hip adductors and abductors were significantly greater with the pelvic belt than without (p<.05). No significant differences in abdominal muscle activities between the two conditions were found (p>.05). Conclusion: These findings suggest that use of a pelvic belt could lead to effective strengthening exercise of hip muscles in individuals with sacroiliac joint pain.

The effects of different V-sit positions on abdominal muscle activation

  • Objective: This study aimed to identify the effects of performing shoulder and hip abduction during the V-sit exercise on abdominal muscle activity. Design: Cross-sectional study. Methods: Thirty healthy adults volunteered for this experiment. The participants randomly performed 6 types of V-sit exercises, including V-sit alone (hip 0°, shoulder 0°), V-sit with hip abduction 0° and shoulder abduction 15°, V-sit with hip abduction 0° and shoulder abduction 30°, V-sit with hip abduction 15° and shoulder abduction 0°, V-sit with shoulder and hip abduction 15°, and V-sit with shoulder abduction 30° and hip abduction 15°. EMG data were recorded from the rectus abdominis (RA), external oblique (EO), and internal oblique (IO) muscles of both sides. All abdominal EMG data during the six types of V-sit exercises were measured for 5 seconds, three times, and recorded for the middle 3 seconds excluding the 1 second at the start and end. Results: V-sit with shoulder abduction 30° resulted in significantly greater muscle activity of both RA, EO compared to shoulder abduction 0°, shoulder abduction 15° (p<0.05) and V-sit with shoulder abduction 15° showed significantly greater muscle activation of the RA compared with shoulder abduction 0° (p<0.05). The muscle activity of both EO and IO in the V-sit with hip abduction 15° was significantly greater than hip abduction 0° in all shoulder conditions (p<0.05). Conclusions: Greater angles of shoulder and hip abduction produced more abdominal muscle activity increases during the V-sit exercises. Shoulder abduction affected the RA, EO muscle activation and hip abduction affected the EO, IO muscle activation. This study showed that shoulder and hip abduction during V-sit exercises enabled effective activation of the trunk muscles.

한국 노인의 3D 인체특성 분석을 통한 Hip protector 치수체계 수립 및 입체패턴 설계 (Development of a Sizing System and a Draping Pattern for Hip Protector based on 3D Data Analysis of Korean Older Women)

  • This study aimed to develop an optimal sizing system and a draping pattern for hip protector based on an analysis of anthropometric characteristics of Korean older women. A hip protector is a specialized form of pants or underwear containing pads along the outside of each hip. The 3D body scan data of Korean older women were analyzed to identify their anthropometric characteristics and a four-size system with 93% of population accommodation was developed by clustering analysis based on key dimensions derived from factor analysis. The sizing system consists of small/short, large/short, small/tall, and large/tall. A 3D physical model and hip pads were fabricated; then, a hip protector was draped on the 3D model and hip pads. The sizing system of hip protector was analyzed in terms of size and shape and a draping pattern was compared on back center, back side, front side, front center and pad. Lastly, the pattern deformation and clothing pressure were analyzed using the virtual clothing system CLO. Virtual system have disadvantage of not to suggest the objective value. In the future research the wearing comfort and impact absorption of the hip protector needs to be tested and then a hip protector design will be finalized by considering the hip protector's size, material, comfort testing results, aesthetic appeal, protection effectiveness, and practical utility of everyday use.

고관절 내외회전 가동범위 검사에 대한 범용플라스틱 측각기의 동시타당도와 임상적 유용성 (Concurrent Validity and Clinical Usefulness of Universal Plastic Goniometer for Hip Internal and External Rotation Range Measurement)

  • PURPOSE: The aim of this study was to evaluate the concurrent validity and clinical usefulness of the universal plastic goniometer to measure the range of motion of the internal and external rotation of the hip joint using the three dimensional motion analysis which can analyze the joints and segment movements in the most objective and quantitative method. METHODS: Clinical and kinematic data were collected from thirty individuals using a universal plastic goniometer and a ten camera motion analysis system. Passive hip rotation range was obtained three trials for left and right hip joints using two measure methods simultaneously. RESULTS: There were significant differences between all matching measures of the two measures of internal and external rotation of the hip joint (p<.05). The relationship between the two tests for all measurements of the internal and external rotation of the hip was statistically significant with correlation coefficient form r=.87 to .96. (p<.01). CONCLUSION: Clinical measurement of the internal and external rotation of the hip using a universal plastic goniometer is effective to assess the hip condition. However, application of universal plastic goniometer requires careful attention in more accurate evaluation and research verification of the internal and external rotation of hip joint.

한국 유의 일본 전파와 변천에 관한 연구 (A Study on the Diffusion and Change of the Korean Hip-length Jacket)

  • The hip-length jacket(유) worn by the Korean from the ancient times is tole prototype of the contemporary jacket. Hip-length jacket was transferred to Japan and worn by all the people beyond the class. From 12C, the King and the high class people wore the clothing influenced by Tang(唐) country but the common people continuously wore the hip-length jacket. Currently the hip-length jacket is worn by the merchant and is used as the clothing of the celebration. The hip-length jacket has not been much changed in terms of the form.

3차원 보행분석을 통한 무릎 모음 모멘트와 고관절 내외회전의 임상적 특성 (Clinical Characteristics of Hip Joint Rotations and Knee Adduction Moment through 3D Gait Analysis)

  • Purpose : The purpose of this study was to verify the relationships among the knee adduction moment, hip rotation range, strength of hip rotators, and Foot Posture Index of healthy young adults. Method : Thirty-two healthy adults(24 male, 8 females) participated in this study. Subjects performed 5 walking trials to evaluate the knee adduction moments using a three-dimensional motion analysis system. Hip rotation ranges and hip rotator strengths were measured using a standard goniometer and a handheld dynamometer, respectively. The mean of three trials of clinical tests was used for data analysis. Results : The first peak knee adduction moment was significantly correlated with the hip rotation ranges and hip rotator strengths (P<.05). The second peak knee adduction moment was showed significant correlations with hip external rotation and rotation ratio. There were no correlations between Foot Posture Index and all knee adduction moments (P>.05). Conclusion : This study suggests that imbalances of the range of motion and strength of the internal and external rotation of the hip joint can affect knee adduction moments. The impact may exacerbate musculoskeletal disorders such as osteoarthritis of the knee. Therefore, further studies should be conducted to evaluate the effects of clinical interventions to correct these imbalances on the reduction of the knee adduction moments in patients with knee osteoarthritis.

Effect of Hip Flexion and Internal Rotation on the Hip Abductor Muscle Activity During Side-Lying Hip Abduction in Subjects With Gluteus Medius Weakness

  • Background: Many previous studies recommended the side-lying hip abduction (SHA) exercise for targeting the gluteus medius (Gmed) and gluteus maximus (Gmax) muscle activity while the decreasing tensor fasciae latae (TFL) activation. Mischoice of hip position and angle in SHA may increase the risk of lower extremity injuries and undesirable muscle activation. However, information is limited on the effect of composite hip flexion angles and hip rotation on the gluteal muscle activity during SHA. Objects: This study aimed to compare muscle activity (Gmed, TFL, and Gmax) and activity ratios (Gmed/TFL, Gmax/TFL, and Gmed/Gmax) using surface electromyography (EMG) during SHA exercise at three different hip flexion angles either with or without internal rotation (IR) in subjects with Gmed weakness. We hypothesized that applying hip flexion and IR during SHA would increase gluteal muscle activity and decrease TFL activity. Methods: Muscle activity and activity ratios in 20 volunteers with Gmed weakness during 6 different SHA were investigated with surface EMG. One-way repeated-measures analysis of variance was used to determine the statistical significance. Results: Significant differences were found among the six different exercises for Gmed ($F_{2,41}=11.817$, p<.001) and Gmax ($F_{3,52}=5.513$, p=.003) muscle activity, and Gmed/TFL ($F_{3,54}=8.735$, p<.001) and Gmax/TFL ($F_{2,37}=4.019$, p=.028) activity ratios. Conclusion: Applying hip flexion is an effective method for increasing gluteal activity, and it elicits great Gmed/TFL and Gmax/TFL activity ratios during SHA in subjects with Gmed weakness.

맞춤형 인공관절 설계를 위한 인체 고관절의 3차원 형상 정보 추출 (Extracting 3D Geometry Parameters of Hip Joint for Designing a Custom-Made Hip Implant)

  • Total Hip Replacement(THR) is a surgical procedure that replaces a diseased hip joint with a prosthesis. A plastic or metal cup forms the socket, and the head of the femur is replaced by a metal ball on a stem placed inside the femur. Due to the various types and shapes of human hip joint of every individual, a selected commercial implant sometimes may not be the best-fit to a patient, or it cannot be applied because of its discrepancy. Hence extracting geometry parameters of hip joint is one of the most crucial processes in designing custom-made implants. This paper describes the framework of a methodology to extract the geometric parameters of the hip joint. The parameters include anatomical axis, femoral head, head offset length, femoral neck, neck shaft angle, anteversion, acetabulum, and canal flare index. The proposed system automatically recommends the size and shape of a custom-made hip implant with respect to the patient's individual anatomy from 3D models of hip structures. The proposed procedure creating these custom-made implants with some typical examples is precisely presented and discussed in this paper.

진도개의 고관절이형성에 대한 방사선학적 연구 (Radiographic Evaluation of Coxofemoral Joints in the Jindo (Canis familiaris))

  • The pelvic radiographs of 47 clinically normal Jindoes (26 dogs and 21 bitches) were investigated to evaluate the coxofemoral joints, prospectively. The hip joints were graded according to the Orthopedic Foundation for Animals criteria of excellent, good, fair, borderline, mild canine hip dysplasia (CHD), moderate CHD, severe CHD. Of the 47 Jindoes evaluated, 8 (17%) were graded as dysplastic, 3 (6.4%) were mildly dysplastic, 3 (6.4%) were moderately dysplastic, and 2 (4.3%) were severly dysplastic. Of the 39 Jindoes graded as normal, 16 (34%) were classified as having excellent hip joint phenotype, 15 (31.9%) were classified as having fair, and 8 (17%) were classified as good. Each hip joint was scored based on the British Veterinary Association/The Kennel Club hip scoring system. The normal hip joint (excellent, fair, good) received score less than 5, while dysplastic hip joint (mild, moderate, severe CHD) received score form 5 to 29. Norberg angle (r=0.76) and femoral angle of inclination (r=0.6) were in inverse proportion to grade of the hip joint (P<0.01). The width of medial joint space was in proportion to grade of hip joint (r=0.7, P<0.01).

슬개대퇴통증 증후군 환자에게 적용한 고관절 견인과 근력 강화 운동이 무릎 통증, 관절가동범위, 하지기능에 미치는 영향-사례 연구 (A Case Study of a Female Patient with Patellofemoral Pain Syndrome for Effect of Hip Joint Traction and Hip Posterolateral Muscles Strengthening on Knee Pain, Range of Motion, and Lower Extremity Function Scale)

  • Background: The case study examined the effect of a hip joint traction and hip posterolateral muscles strengthening on knee pain, range of motion, and lower extremity function scale of patients with patellofemoral pain syndrome (PFPS). Although PFPS has previously been attributed to quadriceps dysfunction, more recent research has linked this condition to impairment of the hip musculature and kinematic. Methods: Subject is a 27-years-old female with PFPS. Performed hip joint traction with belt and posterolateral muscles(hip abductors, external rotators) strengthening for 4 weeks, 3 times a week, once a day. Before and after the therapy, measurements were made on the visual analog scale (VAS) and of the ROM, and a lower extremity functional scale (LEFS) was conducted. Results: The results showed positive changes in VAS and range of motion and lower extremity functional scale. First VAS of knee changed from 6 to 2. Second hip joint range of motion showed that internal rotation recored from $53^{\circ}$ to $58^{\circ}$ and external rotation recorded from $32^{\circ}$ to $37^{\circ}$. Third The lower extremity functional scale showed before therapy of 44; after therapy, 63. Conclusion: The hip joint traction and hip posterolateral muscles strengthening was effective in alleviating knee pain, increasing ROM and Lower extremity functional scale of the PFPS patients.
