Aortic Root Replacement Using Aortic Homograft in Acute Bacterial Endocarditis One Case Report (심내막염 환자의 동종대동맥편을 이용한 대동맥 근부 치환술 - 1례 보고 -)

  • Aortic valve replacement with aortic allograft has been considered a treatment of choice for aortic valve disease secondary to bacterial endocarditis because of its good homodynamic performance and higher resistance to infection. The aortic root replacement technique might be superior to the subcoronary allograft implantation technique with regard to aortic regurgitation. A 46 yea,rs old male patient had acute aortic regurgitation with progressing heart failure secondary to acute bacterial endocarditis. The patient underwent emergent aortic root replacement using 20 mm aortic allograft. At operation, right coronary cusp perforation and heavy calcification of commissure between right and left coronary cusp were observed. The patient recovered well and postoperative echocardiography demonstrated no aortic regurgitation. Inflammatory signs were subsided after 8 weeks of antibiotics therapy. Medically uncontrolled acute bacterial endocarditis was treated successfully by aortic root replacement using aortic homograft.

Trends of Antifungal Agent Susceptibility of Candida Strains Isolated from Blood Cultures in 2009~2018 (2009~2018년 혈액배양으로부터 분리된 Candida 균종의 항진균제 감수성의 경향)

  • Candida is one of the most common causes of bloodstream infections and a leading cause of morbidity and mortality among hospitalized patients. The purpose of this study was to provide important information for formulating empirical treatment plans for candidemia by investigating the antifungal resistance rate of Candida. Among the Candida strains (973 cases) isolated from blood culture tests at the S hospital in 2009~2018, 4.7% (N=44) comprising the Candida spp. (932 strains) showed resistance to fluconazole. The resistant strains included C. albicans, C. parapsilosis, C. tropicalis, and C. glabrata. In addition Candida spp. (947 strains) showed resistance to amphotericin B (N=6, 0.6%), flucytosine (N=23, 2.4%) and voriconazole (N=24, 3.1%). C. albicans was resistant to fluconazole (N=23, 6.9%) and voriconazole (N=21, 6.0%), The statistical analysis showed that C. albicans and non-albicans Candida species were resistant to fluconazole (P=0.039) and voriconazole (P<0.001). A monitoring system to understand the rate of candidiasis infections in a hospital setting is required. It is also important to make the right choice of the antifungal agent based on drug susceptibility patterns. Therefore, an infection surveillance policy that tracks Candida resistance through regular antifungal susceptibility tests is necessary.

  • Recently, the common grounding systems are adapted in most large structures. Since the ground resistance is insufficient to evaluate the performance of grounding systems, it is needed to measure grounding impedance. Even though the methods of measuring grounding impedance of large grounding systems are presented in IEEE standard 81.2, but they have not been described in detail. In this paper, we present the accurate method of measuring grounding impedance based on the revised fall-of-potential method and measurement errors due to earth mutual resistance and ac mutual coupling depending on locating test electrodes at remote earth were examined for the 15[m]$\times$15[m] grounding grid. As a result, the measurement error due to earth mutual resistance is decreased when the distance to auxiliary electrodes increased. To get rid of measurement errors due to mutual coupling, the potential lead should be installed at a right angle to the current lead. When the angle between the potential and the current leads is an acute angle or an obtuse angle, the mutual couple voltage is positive or negative, respectively. Generally, the measurement errors due to mutual coupling with an obtuse angle route are lower than those with an acute angle route.

  • A new resistant cabbage variety 'CT-171' to Fusarium wilt was bred by crossing $A337MSBC_5$ with resistance to the disease and 397-$G_6$ with good density and color. 'CT-171' was selected after combining ability, seed gathering and regional adaptability test in 2008. For breeding of resistant varieties, we investigated the development of Fusarium wilt on cabbage seedlings inoculated with Fusariumoxysporum f. sp. conglutinans by root dipping inoculation method. As a result, 'CT-171' showed higher resistance to Fusarium wilt than 'Asiaball' used as control. The maturity of new variety was 58 days and was faster than control and well suited for autumn cultivation because of cold resistance. The anthocyanin pigment of plant was not revealed. The weight, height and width of head were 1.5 kg, 14 cm and 15 cm, respectively and the core size was 5.7 cm and stable in various cultivation environments. 'CT-171' which showed good agricultural character and resistance to Fusarium wilt filed for variety protect right in Korea Seed & Variety Service on February 2013. The new variety will be appropriate for export and domestic consumption.

Streptozotocin Diabetes Attenuates the Effects of Nondepolarizing Neuromuscular Relaxants on Rat Muscles

  • The hypothesis of this study was that diabetes-induced desensitization of rat soleus (SOL) and extensor digitorum longus (EDL) to non-depolarizing muscle relaxants (NDMRs) depends on the stage of diabetes and on the kind of NDMRs. We tested the different magnitude of resistance to vecuronium, cisatracurium, and rocuronium at different stages of streptozotocin (STZ)-induced diabetes by the EDL sciatic nerve-muscle preparations, and the SOL sciatic nerve-muscle preparations from rats after 4 and 16 weeks of STZ treatment. The concentration-twitch tension curves were significantly shifted from those of the control group to the right in the diabetic groups. Concentration giving 50% of maximal inhibition ($IC_{50}$) was larger in the diabetic groups for all the NDMRs. For rocuronium and cisatracurium in both SOL and EDL, $IC_{50}$ was significantly larger in diabetic 16 weeks group than those in the diabetic 4 weeks group. For SOL/EDL, the $IC_{50}$ ratios were significantly largest in the diabetic 16 weeks group, second largest in the diabetic 4 weeks group, and smallest for the control group. Diabetes-induced desensitization to NDMRs depended on the stage of diabetes and on the different kind of muscles observed while was independent on different kind of NDMRs. The resistance to NDMRs was stronger in the later stage of diabetes (16 versus 4 weeks after STZ treatment). Additionally, when monitoring in SOL, diabetes attenuated the actions of neuromuscular blockade more intensely than that in EDL. Nonetheless, the hyposensitivity to NDMRs in diabetes was not relevant for the kind of NDMRs.

Pipe Stiffness Prediction of GRP Flexible Pipe (GRP 연성관의 관강성 예측)

  • In this paper, we present the load-deflection behavior of GRP pipes. GRP buried pipes are widely used in construction in the advantage of their superior mechanical and physical characteristics such as high chemical resistance, high corrosion resistance, right weight, smooth surface of the pipe, and cost effectiveness from soil-structure interaction. To design flexible pipes to be buried underground, it should be based on the ASTM D2412(2010). When applying ASTM D 2412(2010) to the design, pipe stiffness(PS) must be predetermined by the parallel-plate test which requires tedious and laborious working process. To overcome such problems, the finite element simulations for finding the load-deflection behavior of the GRP flexible pipes is installed at UTM testing machine. In the finite element simulations, basic data, such as the modulus of elasticity of the material and cross-sectional dimension, is used. From the investigation, we found that the difference between experimental result and analytical prediction is less than 15% when the pipe deflected 3% and 5% of its vertical diameter although the pipe material is not uniform across the cross-section.

Assessment of Recovery of Chloride Penetration Resistance of Self-healing Cement Mortars Containing Layered Double Hydroxide (이중층수산화물을 혼입한 자기치유 시멘트 모르타르의 염화물 침투 저항성 회복 평가)

  • This study investigates the effect of layered double hydroxide (LDH) on the healing performance of self-healing concrete by assessing the chloride penetration resistance of self-healing cement mortars using electrical chloride ion migration-diffusion test. Test results show that both mortars containing healing materials only and mortars containing healing materials and Ca-Al LDH together mostly had higher migration-diffusion coefficients right after cracking, but the migration-diffusion coefficients decreased more than that of OPC with increasing healing ages, and thus, they yielded higher healing capacities than OPC. Also, mortars containing Ca-Al LDH together with healing materials showed higher reduction of their migration-diffusion coefficients, and thus, higher healing capacities than mortars containing healing materials only. This suggests that as the self-healing product increases on the crack surface, the binding of chloride ions by LDH inside the crack increases.

Characterization of Acryl Polymer Concretes for Ultra Thin Overlays (초박층 덧씌우기용 아크릴 폴리머 콘크리트의 특성 연구)

  • This study is performed to evaluate the physical and mechanical characteristics of an acryl polymer concrete that is developed as an overlay material for cement concrete slabs and pavements. Various laboratory tests including viscosity, flow, compressive strength, flexural strength, tensile strength, linear shrinkage, thermal expansion and thermal compatibility tests are performed. It is observed from the laboratory tests that the acryl polymer concrete developed in this study satisfies all the requirements suggested by ACI guideline. In addition to the laboratory tests, an accelerated performance testing (APT) is conducted to validate the performance of the acryl polymer concrete. During the APT, no significant distresses are observed until 15,903,939 cycles of equivalent single axle loading is applied. Finally, a 10mm thick overlay with the acryl polymer concrete is applied on top of an old deteriorated concrete pavement to evaluate field performance. Right after the field construction, skid resistance, noise and roughness are measured. The skid resistance and noise level have been significantly improved while the roughness is increased. Periodic investigation for the field study section will be conducted to evaluate the long-term performance.

Preparation of Nanocolumnar In2O3 Thin Films for Highly Sensitive Acetone Gas Sensor

  • Well-ordered nanocolumnar indium oxide ($In_2O_3$) thin films have been successfully fabricated by glancing angle deposition (GAD) using an e-beam evaporator. Nanocolumnar structures have a porous and large surface area with a narrow neck between nanocolumns, which allows them to detect minute amounts of gases. The nanocolumnar $In_2O_3$ thin films were fabricated by the GAD process at five different positions, viz. top, bottom, center, left, and right in a four inch substrate holder. There was a divergence in the thickness and the base resistance of each sensor. However, all the sensors exhibited extremely high sensitivity that was greater than $10^3$ times the change in electrical resistance after being exposed to 50 ppm of acetone gas at $300^{\circ}C$. Furthermore, the nanocolumnar $In_2O_3$ sensors displayed an extremely low detection limit (1.2 ppb) in dry atmosphere as well as in high humidity (80%). We demonstrated that the GAD nanocolumnar $In_2O_3$ sensors have an enormous potential for many applications owing to their particularly simple and reliable fabrication process.

Multi-Parameter Approach for Evaluation of Genomic Instability in the Polycystic Ovary Syndrome

  • Background: The polycystic ovary syndrome (PCOS), characterized by hyperandrogenism and chronic anovulation, is a common endocrine disorder in women. PCOS, which is associated with polycystic ovaries, hirsutism, obesity and insulin resistance, is a leading cause of female infertility. In this condition there is an imbalance in female sex hormones. All the sequelae symptoms of PCOS gradually lead to cancer in the course of time. It is heterogeneous disorder of unknown etiology so it is essential to find the exact cause. Materials and Methods: In this study both invasive and non-invasive techniques were employed to establish the etiology. Diagnosis was based on Rotterdam criteria (hyperandrogenism, ovulatory dysfunction, PCOM) and multiparameters using buccal samples and dermatoglypic analysis and cytogenetic study for 10 cases and four age and sex matched controls. Results: In clinical analysis we have observed the mean value of total testosterone level was 23.6nmol/L, total hirsutism score was from 12-24, facial acne was found in in 70% patients with 7-12 subcapsular follicular cysts, each measuring 2-8 mm in diameter. In dermatoglypic analysis we observed increases in mean value ($45.9^{\circ}$) of ATD angle when compared with control group and also found increased frequency (38%) of Ulnar loops on both fingers (UU), (18%) whorls on the right finger and Ulnar loop on left finger (WU) and (16%) arches on right and left fingers (AA) were observed in PCOS patients when compared with control subjects. Features which could be applied as markers for PCOS patients are the presence of Ulnar loops in middle and little fingers of right and left hand. The buccal micronucleus cytome assay in exfoliated buccal cells, we found decrease in frequency of micronuclei and significant increases in frequency of karyolysed nuclei in polycystic ovarian syndrome patients. Chromosome aberration analysis revealed a significant increase in frequency of chromosome aberrations (CAs) in PCOS patients when compared with controls. Conclusions: From this present work it can be concluded that non-invasive technique like dermatoglypics analysis and buccal micronucleus cytome assays with exfoliated buccal cell can also be effective biomarkers for PCOS, along with increased CAs in lymphocytes as a sign of genetic instability. There is a hypothesis that micronuclei and chromosomal aberrations could have a predictive value for cancer. From this present work it can be concluded to some extent that non-invasive technique like dermatoglypics and buccal cell analysis can also be effective for diagnosis.
