A Study on Shape Design of Cylindrical Cam with Rotating Roller Follower in Roller-Gear-Cam Mechanism (롤러기어캠 기구를 위한 회전운동형 롤러 종동절을 가진 원통 캠의 형상 설계에 관한 연구)

  • When a mechanism transfers a motion to an intersected shaft, a cylindrical cam mechanism may be the best choice among the mechanisms. The cylindrical cam with a roller follower provides to transfer the motions to the intersect shafts simply without other connecting equipments of the intersect shafts. Typical example may be a roller-gear-cam mechanism. But the shape of the cam must be exactly defined in order to satisfy the conditions for the prescribed motion of the follower. This paper proposes a new method for the shape design of the cylindrical cams and also a CAD program is developed by using the proposed method. The relative velocity method calculates the relative velocity of the follower versus the cam at a center of roller, and then determines a contact point by using the geometric relationships and the kinematic constraints. The constraint used in the relative velocity method is that the relative velocity must be parallel to a common tangent line at the contact point of two independent bodies, i. e. the cam and the follower. Then, the shape of the cam is defined by the coordinate transformation of the trace of the contact points. Finally, this paper presents an example in order to prove the accuracy of the proposed methods in this paper and the application of the CAD program"CamDesign".

Korean Medicine, Complementary and Alternative Medicines (CAM) Therapy after Gastrectomy of Gastric Cancer in a Hospital (위암 환자의 위 절제수술 후 한의학과 대체요법 사용실태 - 지방소재 일개병원의 경험 -)

  • Objectives : Cancer patients who are afraid of unpleasant therapy and doubt a perfect cure often seek out traditional Korean medicine or many other kinds of complementary and alterative medicine (CAM) instead. This study was carried out in order to learn the prevalence and kinds of the traditional Korean medicine and CAM to evaluate awareness of and attitude toward it. Methods : 213 patients with gastric cancer who had undergone surgery in the department of gastrointestinal surgery and visited for routine follow-up from November to December of 2004 were included. A questionnaire survey was done by trained interviewers for CAM and demographic information. Results : 96.7% of patients had experience with CAM. The kinds and frequencies of CAM were diet and nutrition 21.0%, folk remedy 21.0%, pharmacological treatment 13.5%, oriental medicine 5.3% and mind-body intervention 1.4%. 56.8% of patients had experienced 2-6 kinds of CAM, 37.4% more than 7. Among diet and nutrition, ginseng (62.9%) was highest, followed by ox leg bone soup (50.8%) and pumpkin (31.5%). Among the folk remedies, elm tree was highest at 46.5%, followed by phellinus linteus (Sang-Whang) at 26.8% and ganoderma lucidum (Young-Gee) 20.7%.satisfaction with CAM was relatively low (43.2%), many patients (78.2%) replied that they would use it continuously. Conclusion : Most patients had experience with many kinds of CAM, but didn't discuss it with medical doctors. If scientific study proves whether or not each kind of CAM is actually effective ondisease, it will help to improve the health of patients and prevent the inappropriate usage and cost.

A study on the Flexural strength of zirconia CAD/CAM block (지르코니아 CAD/CAM block의 강도에 관한 연구)

  • Recently, zirconia is widely used in the field of dental ceramics thanks to the proliferation of CAD/CAM systems. Accordingly, different types of zirconia block are being solid in the market. However, there are no precise, objective standards for properties of zirconia. This study concerns the flexural strength of zirconia ceramic for CAD/CAM block. The test specimens used for this study were ZirBlank(Acucera), ZirBlank shade(Acucera), VITA, Cercon(Densply) and Cerasys. The test results suggest that ZirBlank shade block shows the highest flexural strength and density among the zirconia blocks tested. Its flexural strength was $971{\pm}58MPa$ and its density was 99.89%. On the other hand, Cerasys block shows the lowest flexural strength of $576{\pm}36MPa$ and the lowest density of 94.85%. Given all, the density of the specimens is found to be directly proportional to strength and inversely to grain size.

Review of the Use of Korean Medicine or Complementary and Alternative Medicine among Children with Cerebral palsy (뇌성마비 아동의 한방 및 보완대체의료 이용 실태)

  • Objective : The aim of this study was to review the use of Korean medicine (KM) or Complementary and Alternative Medicine (CAM) therapies among children with cerebral palsy (CP). Method : Literature searches were performed using Pudmed, CNKI, J-STAGE and four Korean databases. We investigated the frequency, types, predictors of the use of KM or CAM therapies used in children with CP. Results : Thirteen survey studies including use of KM or CAM among children with CP were selected - six Korean studies and seven overseas studies. The utilization rate of CAM among children with CP varied 26.8~56.0% according to the studies. CP children used KM or CAM more than other children with chronic disease. Acupuncture and Korean herbal medicine was the major treatment used in Korea, however physical manipulations such as massage, osteopathy and chiropractic were in high demand in western country. Disease severity of CP, use of CAM in the past by the parent or family member and high education of parent are the factors that significantly affect CAM utilization. Conclusion : Further studies on effectiveness and safety of KM therapies combined with conventional rehabilitation treatment would be required to expand the implementation of KM treatment for CP children.

Comparison of Health Promoting Behavior in Rural Elderly by Complementary Alternative Medicine Utilization (농촌노인의 보완대체요법 사용에 따른 건강증진행위 비교)

  • Purpose: To compare the health promoting behavior in rural elderly people by complementary alternative medicine(CAM) utilization. Methods: The data were collected from March 06th to May 26th, 2006. The participants were 207 rural elderly persons in Korea. Data were collected using structured questionnaires and analyzed with the SAS win 8.0 program. Results: The rate of using CAM was 60.9%. Non-religious, perceived poor health and high concerns about health group used it more. The most common type was oriental medicine(35.7%), the most common motive was body protection(21.2%) and major source of information was other patients(35.9%). Almost all the subjects(90%) were satisfied with using CAM. The average score for the health promoting behavior was $2.67{\pm}.32$(range 1.65-3.71). The average scores for each subscale were, personal communication 2.98, self-actualization 2.79, nutrition 2.78, health responsibility 2.58, exercise 2.40, stress management 2.38. Significant differences in health promoting behavior were found according to CAM utilization: personal communication, self actualization. Conclusion: There were differences health promoting behavior between the elderly CAM utilization: Systemic guidelines of CAM use are needed.

Predictors of Complementary and Alternative Medicine Use by Breast Cancer Patients in Bandung, Indonesia

  • Background: The study aimed to assess complementary and alternative medicine (CAM) use and their associated factors with breast cancer patients in Bandung, Indonesia. Materials and Methods: In total, 330 breast cancer patients were administered questionnaires on their CAM use and CAM predictive factors including socio-demographic parameters, clinical data and quality of life, trust in physicians, trust in hospitals, satisfaction and informational needs. Data were analyzed using univariate analysis and multivariate log regreesion analysis. Results: Overall 33.3% of patients reported use of CAM. Lower income, lower education, presence of metastasis, prolonged diagnosis, less trust in physician were found to be highly associated with CAM use. Conclusions: CAM use by breast cancer patients can be interpreted as an attempt to explore all possible options, an expression of an active coping style, or expression of unmet needs in the cancer care continuum. Physicians need to openly discuss the use of CAM with their patients and identify whether they have other unmet supportive needs.

  • The integrated management system IMMC system) is developed for machining, measuring and checking by using IBM-PC under Borland C $\^$++/ environment. MMC system consists of following systems : DNC system, MascMC system (for measuring and checking) and MascCAM system : DNC system, MascNC system (for measuring and checking) and MascCAM system (for CAM). And some other modules support MMC system for practical usages. A vertical machining center equipped with FANUC 0MC is used for experiments. Feasibility of the system is confirmed by a large amount of experiments.

Challenges and opportunities in integrating complementary and alternative medicine into mainstream of the Malaysian healthcare system

  • Complementary and alternative medicine (CAM) practice is still popular among the Malaysian population nowadays although western or allopathic medicine is the first line of treatment. Dissatisfaction with health services and therapeutic effects of western medicine or preference for holistic, integrative approach in treatment are common reasons favouring the increasing popularity of CAM practices. The efforts toward integration of CAM and western medicine in Malaysia were rather slow and in a piece-meal fashion. Strategic efforts in strengthening government and self-regulation among practitioners, formalizing education, promoting research, and cultivating national and international networks are necessary to achieve an integrative system. Regulations to restrict the practice and sale of CAM products to licensed practitioners, strict and mandatory registration of the practitioners, inclusion of CAM in essential medicines list, and pricing regulations must be comprehensively discussed. Development of curriculum, offers of scholarship and incentives, promotion of courses and seminars for professionals is necessary to increase the numbers of CAM experts. Malaysia should follow the efforts of other countries on the production and documentation of local CAM data, allocation of funding, and establishment of research centres to assess the efficacy of potentially useful local products. Local and international collaboration in research and continuous education is important for exchange of knowledge and skills. In conclusion more coordinated efforts in regulation of CAM practice and products, formalizing CAM training and education would significantly move the process forward and allow the public to enjoy more health benefits from CAM practice in Malaysia.

L1 Cell Adhesion Molecule Suppresses Macrophage-mediated Inflammatory Responses (L1 Cell Adhesion Molecule에 의한 대식세포 매개 염증반응의 억제 기전 분석)

  • L1 cell adhesion molecule (L1CAM) is a cell surface molecule to initiate a variety of cellular responses through interacting with other cell adhesion molecules in a homophilic or heterophilic manner. Although its expression was found to be upregulated in some tumor cells, including cholangiocarcinomas, and ovarian cancers, and many studies have investigated the role of L1CAM in these cancers, its role in inflammatory responses has been poorly understood. In this study, we explored the role of L1CAM in macrophage-mediated inflammatory responses. L1CAM significantly suppressed the production of nitric oxide (NO), but induced cell proliferation in RAW264.7 cells. L1CAM expression was detectable, but its expression was markedly decreased by lipopolysaccharide (LPS) in RAW264.7 cells. In addition, the expression of pro-inflammatory genes, such as tumor necrosis factor (TNF)-${\alpha}$, cyclooxygenase (COX)-2, and inducible nitric oxide synthase (iNOS) induced by LPS was dramatically suppressed by L1CAM in RAW264.7 cells. L1CAM inhibited the transcriptional activities of NF-${\kappa}B$ and AP-1 while its cytoplasmic domain deletion form, $L1{\Delta}CD$ did not suppressed their activities in RAW264.7 cells. Moreover, L1CAM suppressed nuclear translocation of p65 and p50 as well as c-Jun, c-Fos and p-ATF2 which are transcription factors of NF-${\kappa}B$ and AP-1, respectively. In conclusion, L1CAM suppressed inflammatory responses in macrophages through inhibiting NF-${\kappa}B$ and AP-1 pathways.

  • The CAD/CAM system for the manufacturing automation is the newest technology in mechanical engineering area and becomes the important research subject nowadays. Most of all hardwares and softwares for the CAD/CAM system used in the our manufacturing companies such as automobile company are developed by the foreign country and the purchasing price of them is very expensive but their applicability to a certain area is very limited. This study was conducted to develope a CAD/CAM system for the design and the automatic manufacturing of the iron pattern shaped with 3 - dimensional free curved surface, and to test its applicability to the design and the manufacturing of the rotary blade. The results obtained from the study are as follow; 1. The CAD system which can process graphic procedures from the free curved surface shaped data was developed with personal computer. 2. The CAM main program was developed. This main program could produce CL data from CAD data file by checking the tool interference according to the cutting mode. 3. The sub. program which can simulate the tool trace from the CL data was developed. 4. The post processor for the Deckel FP2NC NC milling machine from CL data file was developed and the sub program could transmit NC program through modem to NC milling machine was developed. 5. The developed CAM system seemed to be applicable to any other system. Because the measuring results of the cross sectional thickness of the plastic model from the manufacturing iron pattern by the system showed that this system could properly check the tool interference. 6. In took 75~90 hours to manufacture two iron patterns of rotary blade. For the sake of convenience in applying to the other systems, this system was developed in BASIC and FORTRAN computer language and minimum portion of machine language as possible.
