Measurement of Cerebral Blood Volume and Relative Perfusion Rate Mapping Using MR Imaging in Cats

  • To measure regional cerebral blood volume (rCBV) with perfusion MR imaging of cerebral fat embolism by neutral fat and free fatty acids in cats. Triolein (group 1, n=15), oleic acid (group 2, n=9) and linoleic acid (group 3, n=11) were infused into unilateral internal carotid artery using microcatheter through the transfemoral approach. PVA particle was used as a non-fat embolic material in a control group (group 4, n=9). Perfusion-weighted MR image was obtained at 30 minutes and 2 hours postembolization, based on T2-and diffusion-weighted images. The data of lesion and contralateral normal area were transferred to personal computer, time-to-signal intensity curve was drawn and trans for used to △R2/sup */ curve in regular order. The process in the personal computer was done by using the author's developmental image processing program and interactive data language (IDL) softwares. Statistical significance was approved by paired t-test and ANOVA. rCBV of the lesion was decreased comparing to the normal area in all groups. The ratios of rCBV were as follows (group No, at 30 minutes, at 2 hours); group 1,32%, 51%; group 2, 30%, 44%; group 3, 39%, 61%; group 4, 21%, 36%. rCBVs of 2 hours was significantly increased compared to those of 30 minutes in all groups (P<0.005). rCBV was decreased at 30 minutes in cerebral fat embolism and recovered a little, but significantly at 2 hours. Perfusion-weighted images was useful method in offering hemodynamic information in cerebral fat embolism.

  • Purpose : To reveal clinical usefulness of functional MRI (fMRI) using sensorymotor and language stimuli for demonstrating anatomic relationship between sensorimotor or language cortices and lesions in the planning of brain tumor surgery. Materials and Methods : This study included 12 right-handed patients with brain tumors in or around sensorimotor or language cortices. Eleven patients were evaluated with primary motor and sensory stimuli. Of these patients, six patients were also evaluated with language stimuli. One patient was evaluated with language stimuli only. For fMR imaging, a 1.5T scanner was used and the EPI BOLD technique was employed. For postprocessing image, the SPM99 program and a program made by our department was utilized. We evaluated whether sensorimotor and language stimuli activate sensorimotor and language cortices. And also, clinical efficacy of revealing anatomic relationship between cerebral cortices and lesions for planning neurosurgical operation were evaluated. Finally, we compared post-operative neurologic function with pre-operative neurologic function in same patients. Results : The fMRI examination was successful in identifying the functional cortices and depicting anatomic relationship between functional cortices and lesions in all patients. In nine patients of 11 patients with identified sensorimotor cortices, postoperative grade of manual motor test was not changed, compared with preoperative grade. Whereas postoperative improved than preoperative grade in one patient of remaining two patients, postoperative aggravated than preoperative grade in the other. This result was due to atherosclerotic lacunar infarction, regardless of tumor resection. Postoperative deficit of language function was not found in seven patients with identified language cortices. Conclusion : fMRI could be a helpful method for determining the best approach to neurosurgical treatment in patients with brain tumors in or around sensorimotor or language cortices.

  • For the strategic management of the small and/or mid sized clinics, the introduction of the clinic information system is necessary. Currently there are not many information systems for the clinics, yet. In this study, we have developed a clinic information system (CLINSYS) as a guiding prototype and suggested the development directions of the total integrated clinic decision support system. For the better user interface and easy communication. the object-oriented language Macintosh Hyper-Card and Excel are used as the developing tools. And as a part of the integrated decision system, an expert system to consult about the urine test and the blood test interpretation has been developed using the Macintosh spreadsheet Excel.

Medieval Female Mystics and the Divine Motherhood (여성의 몸·여성의 주체성 -중세여성 명상가와 여성으로서의 예수)

  • Meditation on Christ's body is peculiar to late medieval female mysticism. The somatic meditation on Christ basically derives from the Incarnation, but the female mystics focused more on the Passion and the Eucharist, i.e., Christ's bleeding and feeding. Then, female body structure and the gender role of nurturing were combined to make facile her imitatio Christi, because the female body was aptly identified with Christ's body. The blood flowing in the side of Christ was often in medieval graphics and texts identified with a mother's milk for a baby to suck. Wound and food, suffering and nourishing, were inseparable in Christ's and the female mystics' body. Thus, in late medieval female mystical practice, it is important to note, first, female mystics' bodily pain was not to be cured but endured; second, that not only did a female mystic eat Christ's body, but her own body was to be "eaten" by poor neighbors, just as Christ gave his own body to be eaten by believers. As Christ's body is punctured, so does the female body have open holes, and as Christ is food, so is the female body. This female meditation on Christ's body developed the notion of "divine motherhood" to be accepted and enjoyed quite literally by the female mystics in late medieval times. Yet, in a sense, the female mystics' meditating on Christ's feminine function of nourishing can be considered as their accepting and interiorizing the socially constructed female gender role and thus lacking in subversive power. Nevertheless, this meditative practice at least functioned to redeem the female body which had typically been labelled inferior and even dirty. Through Christ's feminized body, the female mystics rehabilitated their bodily dimension, presenting it to be shared by male believers. Capitalizing on the gender stereotype of womanhood itself, they converted female weakness to power.

The Layer of Emotion that Makes up the Poem "Falling Flowers(落花) " by Cho Ji-Hoon

  • This study of Cho Ji - Hoon's Poem "Falling Flowers" was attempted to find the mechanism of poetic healing and utilize it for literary therapy. In this study, I examined how Cho Ji-Hoon's poem "Falling Flowers" encoded crying. Especially, we focused on the organic relationship of each layer represented by poem and put emotional codes on the layer of functor and argument. The results are as follow. It represents the Separation Layer of 1-3strophes, 4-6strophes constitute the Time Layer, and 7-9strophes the Sadness Layer. This poem proceeds the encoding of the sentence in which the crying of cuckoo in the 1-3strophes transforms into the crying of the poetic narrator in the last 9strophe. The relation of emotional layers in this poem is in the same function relations as "(1-3strophes) ${\subset}$ (4-6strophes) ${\subset}$ (7-9strophes)". Since these functional relations consist of the encoding of sadness, encrypts emotion signals of sadness as "U+U+U" becomes "UUU". 1-3strophes' U is the cry of the cuckoo, and U of the 4-6strophes is blood cry. Therefore, "UUU" is the blood cry of poetic narrator. This Cho Ji-Hoon's poem has a Han(恨) at its base. So, as Cho Ji-Hoon's poem "Falling Flowers" is uttered, the poetic mechanism of U, the code of sadness, is amplified. Then we get caught up in the emotions we want to cry. The poetic catharsis of "crying" is providing the effect of literary therapy. In the future, it will be possible to develop a more effective literary therapy technique by developing a literary therapy program like this poetic structure.

Ginseng for Reducing the Blood Pressure in Patients with Hypertension: A Systematic Review and Meta-Analysis

  • Ginseng is one of the most-widely used herbal remedies. This systematic review evaluates the current evidence for its use in the reducing blood pressure (BP) in patients with hypertension. Systematic searches of 12 electronic databases were conducted without language restrictions. All randomized clinical trials (RCTs) of ginseng as a treatment for hypertension were candidates for inclusion. Methodological quality was assessed using the Cochrane risk of bias. Five RCTs met the inclusion criteria. The risk of bias was low in most of the trials. Four of the included RCTs compared the effectiveness of ginseng to placebo. The meta-analysis of these data failed to show a statistically significant acute effect on systolic BP (SBP) or diastolic BP (DBP). However, subgroup analyses showed beneficial effects of Korean red ginseng (KRG) on both SBP (n=54, mean difference [MD], -6.52; 95% confidence interval [CI], -9.99 to -3.04; p=0.0002) and DBP (n=54, MD, -5.21; 95% CI, -7.90 to -2.51; p=0.0001). Two RCTs tested the long-term effects of ginseng for BP for 24hours. One of these trials failed to show any benefits of KRG compared to no treatment, and the other failed to show superior effects of North American ginseng compared to placebo. Adverse events with ginseng were none in one trial or not assessed. Collectively, these RCTs provide limited evidence for the acute effectiveness of KRG in the treatment of high BP. The total number of RCTs included in the analysis and the total sample size were insufficient to draw definitive conclusions. More rigorous studies are warranted.

Systematic Review of Korean Medicine-related Study on Diagnostic Tools and Pattern Identification registered of Dysmenorrhea in the Korean Journal (국내 전자저널에 수록된 월경통 평가지표 및 변증에 대한 한의학적 임상연구 고찰)

  • The purpose of this review was to survey the Korean Medicine related papers about women with primary dysmenorrhea in order to develop the clinical protocol of the diagnostic medical device. We searched the literature from 2000 through April 2015 using 5 online databases including Oriental Medicine Advanced Searching Integrated Sysptem (OASIS), Research Information Sharing Service (RISS), DataBase Periodical Information Academic (DBpia) and Korean Medical Database (KMBase). We selected papers to meet the following inclusion criteria: the papers involved dysmenorrhea (excluding secondary dysmenorrhea), published papers (excluding textbook, educational materials, conferences, etc.) and the papers matched search keywords or scope, but excluded papers to meet the following exclusion criteria: the duplicative papers, get out of the keywords and scope and not in english or korean language. Finally we found 17 papers and classified the papers according to the three search purposes which were diagnostic tools for evaluating the menstrual pain, dysmenorrhea' pattern identification and menstrual phase. Out of the 16 studies, 4 studies were focused on the diagnostic tools including Visual Analogue Scale (VAS), Measurement of Menstrual Pain (MMP) and etc. Other 5 studies were aimed at menstrual phase, and the other 7 studies were studied for pattern identification. The VAS has been widely used in research and in clinical practice for the detection of the menstrual pain. Treatments for patients with primary dysmenorrhea can be prescribed in consideration of their patterns of sasang constitution or body constitution as following: Qi stagnation-Blood deficiency, cold dampness, Qi deficiency-blood deficiency and liver-kidney deficiency etc. This results of research will be used as a useful material during plan a clinical study of primary dysmenorrhea and acquisition of good clinical data.

  • Laboratory information system (LIS) is a key tool to manage laboratory data in clinical pathology. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16 MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with two complete blood count analyzers (Sysmex WE-8000 and Coulter STKS) for unidirectional data transmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology.

Laparoscopic Versus Open Surgery for Rectal Cancer: A Systematic Review and Meta-analysis of Randomized Controlled Trials

  • Background and Aim: Laparoscopic and open rectum surgery for rectal cancer remains controversial. This systematic review compared the short-term and long-term efficiency and complications associated with laparoscopic and open resection for rectal cancer. Materials and Methods: We searched PubMed, Embase, Cochrane Library, ISI Web of Knowledge and the China Biology Medicine Database to identify potential randomized controlled trials from their inception to March 31, 2014 without language restriction. Additional articles were identified from searching bibliographies of retrieved articles. Two reviewers independently assessed the full-text articles according to the pre-specified inclusion and exclusion criteria as well as the methodological quality of included trials. The meta-analysis was performed using RevMan 5.2. Results: A total of 16 randomized controlled trials involving 3,045 participants (laparoscopic group, 1,804 cases; open group, 1,241 cases) were reviewed. Laparoscopic surgery was associated with significantly lower intraoperative blood loss, earlier return of bowel movement and reduced length of hospital stay as compared to open surgery, although with increased operative time. It also showed an obvious advantage for minimizing late complications of adhesion-related bowel obstruction. Importantly, there were no significant differences in other postoperative complications, oncological clearance, 3-year and 5-year or 10 year recurrence and survival rates between two procedures. Conclusions: On the basis of this meta-analysis we conclude that laparoscopic surgery has advantages of earlier postoperative recovery, less blood loss and lower rates of adhesion-related bowel obstruction. In addition, oncological outcome is comparable after laparoscopic and open resection for rectal cancer.

  • The higher cognitive functions of the human brain including teaming are hypothesized to be selectively distributed across large-scale neural networks interconnected to the cortical and subcortical areas. Recently, advances in functional imaging have made it possible to visualize the brain areas activated by certain cognitive activities in vivo. Neural substrates for teaming and motivation have also begun to be revealed. Functional magnetic resonance imaging (fMRI) provides a non-invasive indirect mapping of cerebral activity, based on the blood- oxygen level dependent (BOLD) contrast which is based on the localized hemodynamic changes following neural activities in certain areas of the brain. The fMRI method is now becoming an essential tool used to define the neuro-functional mechanisms of higher brain functions such as memory, language, attention, learning, plasticity and emotion. Further research in the field of education will accelerate the verification of the effects on loaming or help in the selection of model teaching strategies. Thus, the purpose of this study was to review brain study methods using fMRI in science education. In conclusion, a number of possible strategies using fMRI for the study of elementary science education were suggested.
