An Analysis of Age Estimation Cases in Korea from the View of Social Aspects (사회적 측면에서 본 한국 연령 감정 대상자 사례의 분석)

  • Age estimations by forensic odontology play a great role in personal identification. The purpose of this study was to analyze the characteristics of age estimation cases in Korea. Surveying clients who requested age estimation at the Department of Oral Medicine, Kyungpook National University Dental Hospital from March 2012 to March 2013. The results were as follows: 1. On gender distribution, females(57.14%) were more than males(42.86%). The elderly with 50's and over 60's were majorities(89.28%) of clients, and no clients were below 40's. Most of clients were equal to and lower than elementary school graduate(69.64%). 2. The most frequent reason for age discrepancy between registered age and alleged age was mistakes by family or relatives(80.36%). The purposes of age estimation were welfare(62.50%), social relationship problem(12.50%), to find right age(10.71%), and occupation(8.93%). 3. In order of route to visit at Department of Oral Medicine, they were via government office(48.21%), acquaintances(21.43%), mass media(14.29%), and clinic(10.71%). Clients had high degree of comprehension on age estimation with forensic odontology (scored 7.03 out of 10). The 2/3 of clients were satisfied with present fee for age estimation. 4. The percentage on the proximity of estimated age to alleged age was 69.81%. 4(11.43%) clients were approved on age correction by court.

Antioxidative Capacity and Quality Characteristics of Yanggaeng using Fermented Red Ginseng for the Elderly (발효홍삼 농축액을 이용한 노인용 기능성 간식(양갱)의 항산화성 및 품질특성)

  • In this study red ginseng was extracted with ethanol and then fermented by yeasts including Lactobacillus casei and Bifidobacterium longum. Fermented red ginseng extracts(FRGE) were found to be more effective antioxidants in vitro with regards to 1,1-diphenyl-2-picrylhydrazyl(DPPH) radical scavenging activity than red ginseng extracts(RGE). In FRGE, the contents of ginsenosides $Rb_1$, $-Rb_2$ and -Rc were much lower than in RGE, however, the contents of ginsenosides 20(S)$-Rg_3$, 20(R)$-Rg_3$ and compound K were higher than RGE. FRGE was added to Yanggaeng(0, 5, 10, 15, 20%), and physicochemical and sensory evaluations of the Yanggaeng were conducted. The L and b values of Yanggaeng with added FRGE were decreased by increasing the ratio of FRGE, while the a value was increased. Sensory evaluations for, taste, color, flavor, texture and overall acceptability of Yanggaeng with addition of FRGE (10%) were applicable for improving the Yanggaeng product.

Characteristics of Pain Threshold and Pain Experience in Elderly Patients with Dementia (노인 치매 환자의 통증 역치 및 통증 경험의 특성)

  • Objectives: We compared the characteristics of the pain threshold and pain experience between demented group and non-demented group. Methods: This study was part of Gangwon projects for early detection of dementia in 2010. We recruited 8302 local resident ages over 65 years old. Of theses, 1259 people who scored low MMSE were selected and 365 of them completed CERAD-K(Consortium to Establish a Registry for Alzheimer's disease). Finally, 90 in non-demented group and 57 in demented group(mild to moderate Alzheimer's disease) were analyzed. Pain threshold was experimentally measured by pressure algometer and we investigated the pain experience, by Brief pain inventory (BPI), a self-report test. Results: In the demographic characteristics, there are more female, higher ages, lower education in the demented group. There was no significant difference between the two groups in the pain threshold. On the BPI results, 'shoulder pain', 'the number of pain' and 'interference of working' were significantly more prevalent in non-demented group. However, there are no significant differences between the groups in the 'pain severity', 'prevalence of pain' and 'pain treatment'. Conclusions: Demented group report less pain experience but, still perceived pain. It support previous studies that patient with dementia have increased pain tolerance but preserved pain threshold. Thus, active pain assessment and treatment for patients with dementia is needed.

The Effect of PM10 on Respiratory-related Admission in Seoul (서울지역의 미세먼지가 호흡기계 질환으로 인한 병원입원에 미치는 영향)

  • This study was performed to examine the effect of particulate matter less than 10 ${\mu}m$ in diameter($PM_{10}$) on respiratory-related admission in Seoul, 1999. Daily counts of respiratory-related admission were analyzed by generalized additive model with adjustment for effects of air temperature, humidity, and day of the week as confounders in a nonparametric approach. The results follow associations between $PM_{10}$ and asthma, acute upper respiratory disease, acute lower respiratory disease, pneumonia, and chronic respiratory disease. The relative risks were 1.30(95% CI=1.14$\sim$1.50) for pneumonia, 1.18(95% CI=1.01$\sim$1.37) for acute lower respiratory disease in less than 15 years, respectively. The relative risks were 1.85(95% CI=1.22$\sim$2.81) for acute lower respiratory disease, 1.28(95% CI=1.04$\sim$1.57) for asthma, 1.25(95% CI=1.01$\sim$1.54) for pneumonia and 1.19(95% CI=1.01$\sim$1.41) for acute upper respiratory disease in 15 to 64 years, respectively The relative risks were 1.54(95% CI=1.15$\sim$2.08) for asthma, 1.38(95% CI=1.06$\sim$l.80) for chronic respiratory disease in more than 65 years, respectively. The study showed that $PM_{10}$ was considerably affects daily counts of respiratory-related admission in Seoul, 1999 Statistically significant associations were mostly found in the adult group like If to 64 years. The highly relative risks come out in the elderly.

The Effects of Married Immigrant Women's Parenting Experiences on their Mental Health (결혼이주여성의 자녀양육 경험이 정신건강에 미치는 영향)

  • The purpose of this study is to examine the relationships between parenting experiences of married immigrant women and their mental health including self-esteem and depression. Data came from 471 married immigrant women with a child who participated in the home visiting program for children of multicultural families provided by Multicultural Family Support Centers. The results indicated that, the mothers' self-esteem was related to their perceptions of parenting, parenting stress, marital satisfaction, and social support. In addition, the mothers' depression was associated with their proficiency in Korean, household income, perception of parenting, parenting stress, knowledge on child development, marital satisfaction, and social support. These findings suggest that the parenting experiences of married immigrant women are closely related to their mental health. Parenting education programs that increase parenting efficacy and reduce parenting stress would be helpful to improve the mental health of married immigrant women.

Species and Distribution of Native Wild Mushrooms Traditionally using in Jeju Island (전통적으로 이용되는 제주 야생버섯의 종류와 분포)

  • The knowledge of native wild mushrooms traditionally using in Jeju Island, that have specific native environment and various biodiversity, was investigated. From Jeju and Seogwipo cities including 7 provinces 4 locals, so total 50 villages, data was collected in the entire area and voice of the 93 elderly people between the age of fifty and ninety were recorded. As a result, total 23 native wild mushrooms were used in which 2 species were poisonous mushrooms, 7 species were medicinal mushrooms, 2 species was used for folk religion, 2 species were not used for living but for attention and 12 species were edible mushrooms. Also, a total of 267 cases of traditional knowledge was collected, in which 197 cases were about edible mushrooms for 12 species, 43 cases were about poisonous mushrooms for 2 species, 16 cases were about medicinal mushrooms for 7 species. Interestingly, the fortune for agriculture was told depending on spore mass release of Cyathus stercoreus which grows in animal feces and compost. It is considered as the distinctive traditional knowledge of Jeju Island.

Study on Frailty Profiles and Associated Factors in Later Adulthood (노년기 허약 유형과 영향요인에 관한 연구)

  • The purpose of this study was to identify frailty profiles based on physical, psychological, and social domains of functioning and to examine the associated factors showing the differences among frailty profiles. Respondents were 70 years and older(n=403) and latent class analysis was applied to determine the optimal subgroups based on Tilberg Frailty Indicators which comprised of three domains(the physical, psychological, and social domain). Also, we performed multinominal logistic regression analysis to find out factors making differences among frailty profiles. Latent class analysis(LCA) identified three distinct types: multi-frail type(27.0%), psychologically frail type(26.8%), inadequate support type(46.2%). All three types had common difficulties in dealing with daily life problems and did not receive enough help with theses difficulties. Based on the results of the LCA three-class models, people in multi-frail type accumulated problems in physical and psychological domains and had partially social domain. On the other hands, psychologically frail type showed a relatively high anxiety disorder and depression. Lastly, people in inadequate support type reported the lack of helps, but they were relatively healthy. Comparing these groups with inadequate support type, people with multi-frail had lower educational level, poor nutritional management status and were less likely to participate in labor market. People in psychologically frail type were more likely to be male, to live in big cities rather than middle and small cities, and less likely to smoke. Based on these results, our results showed the multifaceted concept of frailty among Korean elderly people and we suggested several implications for preventing frail process.

Unmet Care Needs Among Community-dwelling Middle-aged and Older People in Korea (지역사회 거주 중고령자의 미충족 돌봄요구와 관련요인)

  • This study examined the prevalence of and factors associated with unmet care need among community-dwelling middle-aged and older people in Korea. Data were from the 2006 Korean Longitudinal Study on Ageing (KLoSA), a national survey of 10,254 non-institutionalized adults aged 45 or older. Having unmet care needs was defined as needing personal assistance with activities of daily living (ADL) or instrumental activities of daily living (IADL) but having no available helper. Weighted logistic regressions were fitted to examine factors associated with unmet care needs. Overall, 7.3%, 14.5%, and 41.8% of subjects among the middle-aged, younger old, and older old, respectively, reported care needs of these, 34%, 33%,and 24% had unmet needs. Factors associated with unmet needs differed among the three groups: Education and income level were negatively associated with unmet needs among the middle-aged, but living alone was the only factor positively associated with unmet needs in both the younger and older old. The prevalence of and factors associated with unmet care needs differ by life-stage. Needed are home- and community-based care and services to meet the need for personal assistance among the elderly living alone in a community.

A General Hospital-Based Model for Early Detection of Depression in the Geriatric Patients with Chronic Medical Diseases (만성적인 신체질환이 있는 노인 환자의 우울증 조기발견을 위한 병원기반 모델)

  • The geriatric patients with chronic physical diseases are frequently associated with the continuous clusters of depression including nonpathological sadness, subsyndromal depression, minor depressive disorder, and major depressive disorder. Because of the complex and reciprocal relationships among depression, elderly, and chronic physical diseases, screening approaches with specific nosological methods should be needed in the realm of early detection of depression. Cognitive decline is frequently manifested in geriatric depression with medical or neurological diseases. Also, somatic symptoms of depression or emotional symptoms of physical diseases can play a role as a hampering factor in the early detection of depression. Furthermore, after-care has been regarded as an essential factor of depression screening in the geriatric patients with chronic physical diseases. We reviewed the most popular examples of integrated medicine for depression in primary care. Thus, we propose a general hospital-based model for early detection of depression which includes favorable response loop between screening and therapeutic intervention. Our model can be a basis for evidence-based detection and after-care for depression in the geriatric patients with chronic medical diseases.

Tardive Dyskinesia and Tardive Dystonia with Second-Generation Antipsychotics in Bipolar Disorder Patients Unexposed to First-Generation Antipsychotics (양극성 장애 환자에서 비정형 항정신병약물 치료 후에 나타나는 지연성 운동장애와 지연성 근긴장이상의 유병률 및 위험요인)

  • Objectives Second-generation antipsychotics (SGAs) are frequently used in the treatment of bipolar disorder. However, there is still no consensus on their risk of tardive movement syndromes especially for first-generation antipsychotics (FGAs)-naïve patients. This study aimed to investigate the prevalence and associated factors of SGAs-related tardive dyskinesia and tardive dystonia in patients with bipolar disorder, in a naturalistic out-patient clinical setting. Methods The authors assessed 78 non-elderly patients with bipolar (n = 71) or schizoaffective disorder (n = 7) who received SGAs with a combined use of mood stabilizers for more than three months without previous exposure to FGAs. Multiple direct assessments were performed and hospital records longer than one recent year describing any observed tardive movement symptoms were also reviewed. Results The prevalence rates of tardive dyskinesia and tardive dystonia were 7.7% and 6.4%, respectively. These patients were being treated with ziprasidone, risperidone, olanzapine, quetiapine, or paliperidone at the time of the onset of the movement symptoms. Tardive dyskinesia was mostly observed in the orolingual area, and tardive dystonia was most frequently detected in oromandibular area. A past history of acute dystonia was significantly associated with presence of both tardive movement syndromes. Conclusions Our findings suggest that SGAs-related tardive movement syndromes occur in a substantial portion of bipolar disorder patients. Acute dystonia, a reported risk factor of tardive movement syndromes in the era of FGAs is confirmed as a risk factor of both tardive dyskinesia and tardive dystonia that were induced-by SGAs.
