  • Objectives: After the discontinuance of the Women's Health Initiative (WHI)clinical trial in the U.S. in July 2002, I thought that the domestic medical field would come to a crisis in hormonal treatment for perimenopausal and postmenopausal women. However, the domestic condition of HRT has barely changed. This study was designed to investigate world research trends in HRT and bring forward the response of the Traditional Korean Medicine (TKM) Research Group. Methods: I investigated recent domestic ;md world research trends about HRT and climacteric syndrome. Based on this analysis, I predicted western medical research trends and direction for HRT and medical care of perimenopausal and postmenopausal women. Results and Conclusions: I propose that we (TKM Research Group) have to explain the side effects of HRT for postmenopausal women and try to suggest complementary therapy for it. For that aim, the Association of Korean Medicine's effort is very important, as well as personal practitioner's actions.

Natural Standard Methodology를 이용한 녹즙재료의 항암효과에 대한 근거 및 권고수준에 대한 연구 (Study for Level of Evidence and Recommendation of Anticancer Effect for the Materials of Green Vegetable Juice by Using Natural Standard Methodology)

  • Objective : We designed this study to analyse the anti-cancer effects of materials of green vegetable juice by using the natural standard methodology and apply the results to new studying and evaluating methods. Methods : We investigated the main stream of complementary and alternative medicine (CAM), the methodology of natural standard, guideline in CAM, and concrete 15 green vegetable juice in cancer therapy. Results : There are 15 graded materials of green vegetable juice that associated with cancer in natural standard contents. Most of them get grade C, unclear or conflicting scientific evidence. Conclusion : Natural Standard aims to provide high-quality, reliable information about CAM therapies to clinicians, patients, and healthcare institutions. We have to acquire more reliable evidence in future.

  • At here, we investigated the historical background, current stage, and concrete therapies of alternatives in cancer therapy. We obtained the conclusion below. 1. Alternative medicine(AM) is a unothodoxial medical physiology, medical practice or intervension that overcome the side effects, faults, limits of coventional medicine. 2. Recentely, as emerging New-age movement, collapse of Decarte's mecanical cosmology, and emphasis on patient rights, the interest of AM is increased, and foreign & domestic use rates of AM are risen. 3. After coventional medicine in cancer therapies that depends on operations, radiations, and chemotherapy is bounded, Alternatives in cancer therapy are rapidly developed, and the majority of users are often considered satisfactory. 4. Alternatives in cancer therapy are classified six boundaries; Diet and Nutrition, Mind-body Technique, Integrated System, Pharmacologic and Biologic treatments, Immuno Enhanced Therapy, Natural and herbal medicine. 5. Looking at contents of Alternatives in cancer therapy, we get to know most of them are similar to the treatment of oriental medicine in holistic method, therapy, philosophy. If we synthesize the alternative medicine's treatment mentioned above and investigate it with oriental medical viewpoint, that may help us approach the conquest of cancer and improve the rate of cure. 6. It is certain that foreign AM will be imported reversely, and we must prepare for rebuilding of Korean traditional AM systemically, investing for activity positively, making national policy for medical system.

The use of complementary and alternative medicine in children with common neurologic diseases

  • Complementary and alternative medicine (CAM) is a phrase used to describe additional health care methods such as mind/body practices and natural products not regarded as treatments by conventional medicine. The use of CAM in children with common neurologic diseases is more frequent than its use in healthy children (24%-78% vs. 12%). However, less than half of patients report such use to their physicians. The preferred modalities of CAM vary in different countries due to their different cultures and traditions. The most common factor significantly associated with the use of CAM is parental CAM use in most studies. The frequency of the use of CAM in children and adults with neurologic diseases is similar, and both rates are higher than the rates in those without these conditions. The preferred modalities of CAM in adults are diverse, and megavitamins and mind/body therapy (prayer and chiropractic care) are included. The most common factor significantly associated with the use of CAM in adults with neurologic diseases is high educational level. Physicians need to be concerned with patients' use of CAM and provide correct information about CAM so that patients may make the right decisions. Further study is needed to determine the evidence-based efficacy of CAM use in children with common neurologic diseases.

인터넷시대에서의 생리통과 대체요법이용과의 관련성 (The Relationship between Dysmenorrhea and Alternative Medicine among the Internet Age)

  • 본 연구는 인터넷시대에서 월경통 대상자의 보완대체요법에 대한 만족도를 파악하고자 하였다. 연구대상자는 광주시의 5개 보건소 방문자와 A 교회 여성 교인 중 18세 이상 55세 이하의 생리를 하고 있는 200명의 여성이며, 자료수집기간은 2010년 9월부터 2010년 10월 10일까지였다. 연구도구로는 안승덕(2003)의 설문지에서 사용한 문항을 기초로 한 설문지를 사용하였다. 자료분석은 SPSS 17.0을 이용, 빈도분석, 교차분석, 카이제곱검정을 하였고, 연구결과 월경통환자의 보완대체요법 이용률은 43.4%, 이용장소는 한의원이나 한약방이 56.3%, 대체요법의 종류도 한의원의 한약, 침술, 뜸, 부항(42.5%)의 비율, 이용시 지출비용(1년기준)은 50만원 미만 56.3%, 이용후의 만족도는 만족한다 51%의 분포도를 나타냈다. 결론적으로 사회활동이 많은 21세기에서 월경통 환자의 보완대체요법 이용률이 높아지고, 본인스스로 혹은 주위의 권유로 대체요법을 이용하며, 신뢰할 수 있는 의료기관인 한의원과 전통적 민간요법에 의존을 하는 것으로 나타났고, 보완대체요법에 대한 보험 혜택이 이루어지도록 연구와 협조가 필요할 것으로 사료된다.

  • In this paper, we checked health problems of outpatients who suffered from cancer, apoplexy, and arthritis. After accessing each patients condition, in order to alleviate their sufferings, we introduced several methods of aroma therapy as a holistic approach to these health problems. Aromatherapy, which can be defined as a nature-friendly complementary / alternative method, has its great merit in its easy way of treatment by the family member of patient or patient himself. Recently, it was scientifically proved that aroma therapy has various curative effects. The easiness of applying aroma therapy is full of suggestions concerned with the future of nursing science. Recently, in the domain of home care nursing, there happened a fundamental change in its structure: a change from the supplier/professional-centered to the consumer/patient-centered one. With this change, home care nursing as a cherished desire of nursing science obtained its legal ground in the established medical programs and, in the same context, patients came to have the chance to receive demanded medical services in their home without going through complicated hospital admission procedures. Considering the future status of home care nursing as a major contributor in the consumer-centered structure of medical health service, aroma therapy as a complementary/alternative method is expected to contribute not only to establishing more effective structure of health service supply but also to resolving chronic health problem of outpatients.

두경부암 환자의 대체요법 이용 실태 (Complementary and Alternative Medicine in the Patients with Head and Neck Cancer)

  • Background and Objectives: The popularity of complementary and alternative medicine(CAM) has grown at an astonishing rate in recent years. Because patients are interested in CAM and want to use it, physicians also should be concerned about CAM and know about the current situation of its usage. In this study, we tried to determine the prevalence of CAM use in the patients with head and neck cancer and correlate with demographics and tumor characteristics. Materials and Methods: A total of 87 patients with head and neck cancer previously treated with surgery at Seoul National University Hospital from March through June, 2004 were enrolled in this study. The patients were completed, questionnaire about the use of CAM and demographic information. Tumor characteristics were obtained from the patient's medical records. Results: Of the 87 patients, 33 patients(37.9%) use at least one CAM. There was no statistically significant correlation between patient's data and prevalence of CAM use. Of those patients using CAM, 16(48.5%) of 33 were using 2 or more therapies. The majority of agents used by patients in this study are mushrooms such as Phellinus and Reishi, followed by dietary therapy. A majority(60.6%) of patients were introduced to alternative medicine by friends or family members. Conclusion: Considerable cancer patients use CAM. Head and neck oncologists should inform themselves about CAM to counsel and educate the patients. Further scientific investigations on promising CAM agents may help advance cancer treatment.

우리나라 소화기암 환자들의 대체의료이용에 관한 연구 (Use of Complementary and Alternative Medicine among Cancer Patients in Korea)

  • Complementary and alternative medicine (CAM) has gained in popularity among cancer patients in recent years. The use of CAM in cancer patients is common with about one third of patients using some form of CAM in Western countries. The purpose of this study was to analyze the use of CAM and determine what factors affect to use CAM among cancer patients to provide CAM therapy information and assist therapy selection among various CAM therapies to cancer patients. The design of the study was descriptive cross-sectional, and data were collected using a 16-item questionnaire. This study was conducted in subjects with confirmed diagnosis of stomach, colon, liver, or pancreas cancer, in both out-patients clinics and inpatients setting in a tertiary hospital in Seoul Korea. As a result, among the participants, past or current CAM use was reported by 75%, which shows a statistically significant difference in income groups(P<0.05), but no difference in age and religion groups. The most common therapies use by cancer patients included traditional Korean medicine (32.1%), folk remedies (26.6%), exercise (14%), dietary supplements (11.6%), physical therapy (9.9%), diet therapy (5%), and meditation (4%). 77.8% of patients show satisfaction and 64.4% shows perceived effectiveness of CAM. Male patients with higher income, and previous treatment were more likely to use CAM. The main benefits from CAM reported by cancer patients were psychological improvement and symptom improvement. Of the cancer patients used CAM, 30.9% were dissatisfied, 25.8% did not have benefits from the use, and 7.6% experience side effects. Cancer patients who prefer CAM (more than 3 kinds) used it to cure cancer, on the contrary, the one who do not prefer CAM used to improve symptoms and psychological stability. The main sources of information about CAM were family and friends(54.4%), and media(24.5%), doctor and nurse(18.3%), and religion group(2.6%). Findings suggest that due to the relatively high use of CAM among cancer patients in Korea, this topic should be taken into account in the development of a holistic approach to cancer patients and efficient cancer patients management system and proactive and consistent management of CAM is necessary in the health care system in Korea.

세계 암 보완대체의학의 현황에 대한 연구 (Review for the Current Status of Cancer Complementary and Alternative Medicine in the World)

  • Objectives: Many countries have been using Complementary and alternative medicine (CAM) for treating cancer and it has been given more weight than before. This study aimed to look into its current status and perspective way to suggest the way which Korean oriental medicine goes to. Methods: We searched internet web sites that are relative with CAM in oncology parts and made lists for this study. We downloaded the essays in the form of electronic files in that web sites and copied the originals consulting to each libraries and relative institution. Results: North America was processing the standardization of CAM based evidences and China was making their traditional medicine representative medicine of CAM. India was developing their traditional medicine because of their domestic need and its results were recognized by National Cancer Institute (NCI). CAM was regulated and studied legally in Europe. Japan was using mushroom for immune therapy clinically. Mexico had a large scale of medical valley in Tijuana area and made it international. Conclusions: The efforts to develop and study CAM for treating cancer are increasing in the world. We need to know the current status of CAM in the world, and make our way that Korean oriental medicine goes to.

국내 통합의학 저널의 연구 동향에 대한 계량서지학적 분석 : Integrative Medicine Research를 중심으로 (A Bibliometric Analysis of Research Trends in Domestic Integrative Medicine Journals : Focused on Integrative Medicine Research)

  • Purpose : This study aimed to analyze research trends in the field of integrative medicine through a bibliometric analysis of articles published in Integrative Medicine Research (IMR) journal from 2017 to 2022. Methods : Articles published in IMR journal between 2017 and 2022 were searched using the Web of Science database on August 22, 2023. The analysis was performed using the Bibliometrix and Biblioshiny tools in R (version 4.3.1) and VOSviewer (version 1.6.19). Results : The key findings were as follows: average citations per article (9.41), total authors (1,142), single-authored articles (12), average articles per author (0.27), average co-authors per article (5.27), and rate of international co-authorships (15.69 %). The most-cited article was on the cryopreservation of cells or tissues and their clinical applications. The top keyword analysis by author keywords showed that "acupuncture" was the most frequently used keyword (33 times). Co-occurrence network analysis showed 85 high-frequency keywords that appeared five or more times, and the top five keywords by total link strength were "acupuncture," "herbal medicine," "prevalence," "alternative medicine," and "complementary." The study found that, contrary to the trend in complementary and alternative medicine research in Korea, the IMR journal actively conducts intervention studies to provide clinical evidence. Conclusion : In the IMR journal, "acupuncture" was the most frequent of author keywords. The analysis of keyword trend topics over time showed that the keyword "systematic review" continued to appear from 2020 to 2022, and the keyword "clinical practice guideline" appeared for the first time in 2021. In particular, the co-occurrence network analysis highlighted keywords related to intervention research, in contrast to domestic research trends. While this study analyzed only one journal, future studies expanding the category of integrative medicine and increasing the number of journals analyzed may provide further insights.
