경쟁과 비경쟁 상황에서 공감의 신경학적 기제 (Neural Bases of Empathy in Competitive vs. non-Competitive situation)

  • 본 연구에서는 fMRI 기법을 이용하여 경쟁 상황 노출 여부에 따라 개인이 타인의 상황을 이해하고 인지적 공감을 유발하는 신경학적 기제의 차이를 조사하였다. 공감은 타인에 대한 이해를 바탕으로 적절한 반응을 보이는 심리적 반응이며, 인간의 생존에 있어서 적응적 요소로 알려져 있다. 연구의 목적은 Preston과 de Waal(2002)에 기초하여 거의 자동적인 반응인 공감이 타인을 이겨야 하고 본인의 수행에 따라 보상이 달라지는 경쟁 상황에서도 1) 타인이 처한 상황을 인지할 수 있는지, 2) 인지한다면 인지적 공감이 유발되는지를 확인해 보는 것이다. 대학생 21명(남:여=9:12)을 대상으로, 일상생활에서 경험할 수 있는 6가지 정서가 제시된 그림자극을 보고 '나'의 관점에서 유발되는 정서가 긍정정서인지 부정정서인지 평정하는 과제 수행동안 뇌 활성화를 촬영하였다. 경쟁 유무에 따라 공감과정에서 두뇌 활동의 차이를 비교하기 위해 실험보조자 1인이 경쟁자 역할을 수행하고, 수행 결과의 정확도와 반응 시간을 기준으로 피험자가 경쟁자(실험보조자)를 이길 경우 보상(돈)을 제공하는 조건으로 경쟁심을 유발하여 경쟁 조건을 조작하였다. 연구결과, 피험자들의 판단 반응시간은 비경쟁 상황보다 경쟁에 노출되었을 때 통계적으로 유의하게 빠름을 확인하였고, 경쟁 상황에서는 전대상회(ACC, BA 32), 내측전두엽(mPFC, BA 6), 측두두정연합부의 한 부분인 연상회(SMG, BA 40), 시상(thalamus), 중전두엽(middle frontal gyrus, BA 10) 등이 활성화 된 반면, 비경쟁 상황에서는 인접대상회(paracingulate, BA 32), 측두엽극(temporal pole, BA 38), 복내측전전두엽(vmPFC, BA 11), 상후두엽/쐐기소엽(superior occipital gyrus/cuneus, BA 19) 등의 영역에서 활성화가 관찰되었다. 또한 경쟁상황에서 개인의 공감 수준에 따라 두뇌 영역과의 상관을 알아보기 위해 공감에 대한 자기 보고식 반응 점수를 공변량으로 이용하여 중다회귀분석을 실시한 결과, 경쟁 상황에서는 우측 연상회(rSMG, BA 40), 하전두엽(inferior frontal gyrus, BA 47/45), 내측전두엽(mPFC, BA 10/6), 시상(thalamus), 피각(putamen), 방추형회(fusiform gyrus, BA 20/21)영역과의 상관이 높은 반면, 비경쟁 상황에서는 공감 수준이 높을수록 측두두정연합부/상측두엽(temporoparietal junction/superior temporal gyrus) 영역이 높은 상관을 보였다. 이 연구의 결과를 통해 경쟁 노출 여부에 따라서 타인에 대한 공감적 인지과정에 서로 다른 두뇌영역이 사용되며, 특히 개인의 공감 수준이 높을수록 경쟁 상황에서도 타인의 상황에 대해 이해하는 인지적 공감과 관련한 두뇌 영역이 높은 상관을 보여 개인차에 따라 경쟁 지각이 다르게 반응함을 알 수 있었다.

목동맥 스텐트 삽입술 후 시상을 포함한 후대뇌동맥 영역에 발생한 뇌출혈 : 증례보고 (Posterior Cerebral Artery Territorial Hemorrhage Including Thalamus After Carotid Artery Stenting : A Case Report)

  • 목동맥 스텐트 삽입술은 목동맥 내막절제술에 적합하지 않은 환자에게 시행할 수 있는 목동맥 협착증의 대체 치료법으로 알려져 있다. 목동맥 내막절제술, 혈관성형술 또는 스텐트 삽입술 후에 드문 부작용으로 뇌내출혈이 발생할 수 있고, 이러한 출혈이 발생하는 원인은 대부분의 경우 재관류 손상과 관련이 있는 것으로 추정하고 있다. 이전의 연구에서는 내막절제술과 비교하여 목동맥 스텐트 삽입술 후 뇌내출혈의 빈도가 더 높다고 보고한 바 있다. 본 연구에서는 뇌경색으로 내원한 80세 남자환자를 대상으로 증례보고를 통해 동일 분야 연구에 활용하고자 자료 분석을 하였다. 80세 남자가 갑자기 발생한 오른 팔의 근력저하를 주소로 방문하였다. 왼쪽 속목동맥의 90% 협착이 발견되어 목동맥 스텐트 삽입술을 시행하였고, 시술 후 실시한 뇌 CT에서 시상을 포함하는 후대뇌동맥 영역의 뇌출혈이 뇌실내출혈까지 진행되어 있음을 관찰하였다. 이러한 출혈은 스텐트 삽입술이 시행된 동맥에서 공급될 가능성이 낮은 혈관 영역에서 발생했기 때문에, 이 경우에 내막절제술 시행 후 과다혈류에 의한 출혈과 다른 양상을 확인하였다.

Metabolic Changes in Patients with Parkinson's Disease after Stereotactic Neurosurgery by Follow-up 1H MR Spectroscopy

  • Authors investigated neuronal changes of local cellular metabolism in the cerebral lesions of Parkinsonian symptomatic side between before and after stereotactic neurosurgery by follow-up 1H magnetic resonance spectroscopy (MRS). Patients with Parkinson's disease (PD) (n = 15) and age-matched normal controls (n = 15) underwen MRS examinations using a stimulated echo acquisition mode (STEAM) pulse sequence that provided 2${\times}$2${\times}$2 ㎤ (8ml) volume of interest in the regions of substantia nigra, thalamus, and lentiform nucleus. Spectral parameters were 20 ms TE, 2000 ms TR, 128 averages,2500 Hz spectral width, and 2048 data points. Raw data were processed by the SAGE data analysis package (GE Medical Systems). Peak areas of N-acetylaspartate (NAA), creatine (Cr), choline-containing compounds (Cho), inositols (Ins), and the sum (Glx) of glutamate and GABA were calculated by means of fitting the spectrum to a summation of Lorentzian curves using Marquardt algorithm. After blindly processed, we evaluated neuronal alterations of observable metabolite ratios between before and after stereotactic neurosurgery using Pearson product-moment analysis (SPSS, Ver. 6.0). A significant reduction of NAA/Cho ratio was observed in the cerebral lesion in substantia nigra of PD patient related to the symptomatic side after neurosurgery (P : 0.03). In thalamus, NAA/Cho ratio was also significantly decreased in the cerebral lesion including the electrode-surgical region (P : 0.03). A significant reduction of NAA/Cho ratio in lentiform nucleus was not oberved, but tended toward significant reduction after neurosurgery (P = 0.08). In particular, remarkable lactate signal was noted from the surgical thalamic lesions of 6 among 8 patients and internal segments of globus pallidus of 6 among 7 patients, respectively. Significant metabolic alterations of NAA/Cho ratio might reflect functional changes of neuropathological processes in the lesion of substantia nigra, thalamus, and lentiform nucleus, and could be a valuable finding fur evaluation of Parkinson's disease after neurosurgery. Increase of lactate signals, being remarkable in surgical lesions, could be consistent with a common consequence of neurosurgical necrosis. Thus, IH MRS could be a useful modality to evaluate the diagnostic and prognostic implications fur Parkinsons disease after functional neurosurgery.

  • Memory disorder is the most consistent neuropsychological finding in schizophrenia and seems to be a stable trait in it. It is suggested that memory dysfunction found in patients with schizophrenia is primary to biological abnormalities, not secondary to attention deficits they have. Although temporal lobe structures including hippocampus and thalamus have traditionally been thought to be implicated regions for memory disorder in schizophrenia, recent studies indicate the possibility of abnormalities in the frontal lobe and the neural circuits between brain regions. Advanced research methods such as functional imaging technique are expected to produce more detailed informations about memory function in schizophrenia.

  • Ganglioglioma is an infrequent tumor of the central nervous system (CNS); mostly supratentorial region. But, they can occur anywhere in the central nervous system such as brainstem, cerebellopontine angle (CPA), thalamus, optic nerve and spinal cord. Although it occurs rarely, ganglioglioma should be included in the differential diagnosis of a posterior fossa mass because early recognition is important for treatment and patient counseling.

  • 본 연구는 운동 표상에 관한 뇌 메커니즘의 이해를 위해 fMRI를 이용한 측정 시스템을 개발하였다. 개발된 시스템의 검증을 위해 예비 실험을 시행하였다. fMRI task는 visual feedback을 제공하는 task와 차단하는 task 두 가지의 task로 구성되었다. 실험 결과 visual feedback이 제공되었을 때는 left superior orbital gyrus, bilateral precentral gyrus, left superior occipital gyrus, left supplementary motor area, right thalamus, right postcentral gyrus 그리고 right superior parietal lobule 같은 영역들이 활성화 되었다. Visual feedback이 차단되었을 때는 left precuneus, right middle temporal gyrus, bilateral supplementary motor area, right anterior cingulate cortex, left Inferior temporal gyrus, left insula lobe, right superior parietal lobule, bilateral postcentral gyrus 그리고 left precentral gyrus 같은 영역들이 활성화 되었다. 개발된 시스템을 통해서 visual feedback이 차단되었을 경우 운동 표상에 관한 뇌 메커니즘을 알아 볼 수 있었다.

흰쥐에서 Capsaicin 대조(Cisterna Magna) 내 주입 후 삼차신경 유해자극수용전달로에서의 Fos 단백의 발현 (Fos Protein Expression in Trigeminal Nociceptive Central Pathway of the Rat Brain by Cisternal Capsaicin Injection)

  • Background: Trigeminovascular system is implicated in the pathophysiology of the headache in migraine. This study was designed to evaluate the pattern of Fos protein expression in trigeminal nociceptive central pathway after meningeal stimulation of rats by capsaicin. Methods: The expression of Fos protein was examined by immunohistochemistry in thalamus, brainstem and upper cervical cord (at three levels corresponding to obex, 0.8 mm and 2 mm below obex) 2 hours after intracisternal injection of either diluted capsaicin solution (0.1 ml, $61{\mu}g/ml$) or normal saline (0.1 ml) through a catheter placed in the cisterna magna, or following epidural instillation of diluted capsaicin solution in urethane-anesthetized Sprague-Dawley rats. Results: Fos immunoreactivity was strongly expressed within lamina I, II of bilateral trigeminal nucleus caudalis (TNC) after cisternal capsaicin injection and magnitude of expression was greatest at level 2.0 mm below obex. Epidural capsaicin caused much less labelling than cisternal capsaicin. Fos positive cells were also observed in area postrema, nucleus of the solitary tract, medullary reticular nucleus and midline nuclear groups of the thalamus with similar intensity between capsaicin and control group. Conclusions: These results indicate that the injection of capsaicin into the cisterna magna is an effective stimulus for the induction of Fos protein within TNC through activation of trigeminovascular afferents and this animal model can be useful for the evaluation of the pathophysiology and drug development in migraine and related headache.

서산 지역에서의 중풍에 대한 임상적 고찰 (A Clinical Study on Stroke patients(CVA) in Seosan province)

  • Objective : The purpose of this study is about stroke patients in Seosan. Method : The subjects of this study were 45 patients who were admitted to Hanseo University Oriental Hospital because of stroke. Each patients was diagnosed with Brain CT, sasang constitutional analysis. Results : 1. The rates in CVA was 58% on cerebral infarction, and 42% on cerebral hemorrhage. 2. The sites of cerebral infarction were Basal ganglia, MCA, Internal. External capsule. Thalamus, ect. Sites of cerebral hemorrhage were Thalamus, Basal ganglia, Cerebellum. 3. The ratio of left and right hemiplegia in cerebral infarction was 1:1.6, and in cerebral hemorrhage it was 1:2. 4. The most chief complaints in cerebral infarction, were hemiplegia, dysarthria, facial palsy, headache. In cerebral hemorrhage, their were hemiplegia, dysarthria, headache, dizziness, and facial palsy. 5. Classification of human corporal constitution in cerebral hemorrhage, the most was Taeumin, Soyangin, Soeumin. And in cerebral infarction, the most was Soyangin, Taeumin, Soyumin. 6. The ratio between male and female was 1.25:2 in cerebral hemorrhage, 2.5:5 in cerebral infarction. 7. The most prevalent age groups in cerebral hemorrhage was fifties to sixties. and in cerebral infarction was fifties to sixties. 8. The most common preceding disease in cerebral hemorrhage was HTN, DM. 9. The recurrence rate of cerebral hemorrhage was 16%, and cerebral infarction was 8%. 10. The ratio of recovery in cerebral hemorrhage was 84%, in cerebral infarction 58%. Conclusion : From this study, in cerebral hemorrhage most patients were Taeumin, in their fifties to sixties. And in cerebral infarction most patients were Soyangin, in their sixties to eighties. In both stroke patients, there were more female than male patients.

  • Spontaneous pain and painful overreaction to external stimuli resulting from lesion confined central nervous system (CNS) were named as thalamic syndrome. Thalamic lesion and decreased regional cortical perfusion thought to the pathogenesis of thalamic syndrome due to decreased function of thalamocortical tract. We performed $^{99m}Tc-HMPAO$ regional cerebral perfusion in 10 patients with clinical diagnosis of thalamic syndrome due to thalamic lesion or near the thalamic lesion at Yonsei University Hospital, from January 1989 to August 1991. In contrast to five patients with lesions near the thalamus who did not show secondarily decreased perfusion at cerebral cortex, four among the five patients with thalamic lesions revealed decreased cortical perfusion in the ipsilateral cerebral cortex on brain SPECT. These phenomena may suggest the loss of afferent activating stimuli from the thalamus led to decreased neuronal activity and the followitng hypoperfusion of cerebral cortex, and might be one of the indirect signs for suggesting presence of the thalamocortical tract. A causal relationship between cortical hypoperfusion and neuropsychological deficit is strongly suggested.
