Anti-N-methyl-D-aspartate receptor encephalitis associated with ovarian teratoma in Korea: three case reports

  • Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a severe autoimmune paraneoplastic syndrome associated with ovarian teratomas. Most patients develop neurologic symptoms, including psychosis, memory deficits, seizures, or abnormal movements, and experience abdominal pain related to ovarian neoplasm. We present a case report of three patients diagnosed with anti-NMDAR encephalitis accompanied by ovarian teratomas at Ajou University Hospital in Korea. The patients demonstrated a different clinical course of the disease. However, upon diagnosis, all patients underwent surgical removal of the ovarian teratoma followed by intensive immunotherapy. The symptoms progressively improved following treatment. This is a case report of a rare autoimmune anti-NMDAR encephalitis associated with ovarian neoplasms, including immature teratoma.

폐실질내 기형종 -1례 보고- (Intrapulmonary teratoma -Report of a case-)

  • This paper reports a case in a 16-year-old female of intrapulmonary teratoma located in the right upper and middle lobes of the lung. The initial symptoms were high spiking fever, cough and chest pain. Initial chest X-ray revealed large homogenous mass in the right upper and middle lung fields. So intercostal tube drainage was done under the impression of the lung abscess. But mass density was remained on the follow-up chest X-ray, the patient had resection of the right upper and middle lobes of the lung. The gross and microscopic findings of the resected specimen revealed characteristic findings of the intrapulmonary teratoma. The patient is asymtomatic at present and living a normal life.

후부종격동에서 발생한 기형종 2례 보 (Posterior mediastinal teratoma: a report of 2 cases)

  • Tumors of the mediastinum are usually classified according to their location in the anterior, middle, or posterior mediastinum. Mediastinal teratomas are characteristically located at anterior mediastinum with only rare incidence in the posterior mediastinum. Two cases of posterior mediastinal teratoma were experienced recently, in the department of thoracic surgery, Seoul National University Hospital. The incidence of posterior mediastinal teratoma was 4.2% among 48 mediastinal teratomas in our experience. Two cases are all benign teratoma located at right posterior costophrenic angle, and histologically showed tissues from three germ layers including bone and fat.

  • Mediastinal tumor are frequently encountered in clinical practice. Hanten, in 1955, reported-2 adult patients with spontaneous rupture of mediastinal dermoid cysts into the pleural cavity and also, Thompson, in 1963, reported 2 child patients with spontaneous rupture of mediastinal teratoma into the pleural cavity. Mediastinal teratomas have also been reported rupture into other contiguous structures, such as the bronchus, aorta, pericardium, SVC and esophagus. This report presents an instance of spontaneous rupture of an anterior mediastinal teratoma into the right pleural cavity of a 43 year old female. Despite variable diagnostic procedures, the true nature of the lesion was not determined until a thoracotomy and window formation was performed for adequate drainage of empyema thoracis. Removal of the teratoma and mediastinal window formation resulted in complete cure.

  • Mature cystic teratoma, commonly called dermoid cyst, is the most common benign germ cell tumor of the ovary in children. Malignant transformation is rare, approximately 2 %. As laparoscopic procedures are applied widely in pediatric surgery, a female chlid with a mature cystic teratoma may be an ideal candidate for laparoscopic surgery. Two children received laparoscopic operations successfuly for lower abdominal crises, twisted adnexa. There was no operative complication. Laparoscopic approach for ovarian lesions in infancy and childhood appears to be an effective and safe method for diagnosis as well as definitive therapy.

  • 위기형종은 모든 기형종의 1% 미만을 차지하며, 대부분 1세 미만의 남아에서 발생하고, 상복부 종괴나 복부 팽만을 주소로 내원하여 방사선학적으로 US나 CT 소견에서 위 주위에 낭성 그리고 고형성분이 섞여 있는 종괴가 지방이나 석회화를 함유한 경우 진단이 가능하며, 보고된 대부분의 경우가 양성으로 완전한 수술적 절제 후에는 추가적인 치료가 필요하지 않고 예후는 매우 양호하다. 알파 태아 단백(AFP, ${\alpha}$-fetoprotein)이 종양 표지자로 사용되며, 추적 검사에서 점차적으로 증가하거나 또는 감소하지 않는 경우 재발이나 불완전한 절제를 생각할 수 있다.

  • Although intraperitoneal teratomas are rare, teratomas of the greater omentuma are seldom reported. A single case of omental teratoma with torsion in a 26-month-old girl is reported. The patient presented with abdominal pain and a palpable mass. Ultrasonography and CT of the abdomen showed a large cystic mass with a non homgenious solid component composed of fat and calcification. The lesion was surgically excised. Pathology revealed a mature omental teratoma. The hospital course was uneventful.

  • A 11-year, 9-month-old female Shih-tzu was referred for evaluation of the body conditions. According to the physical examination and ultrasonography, the left ovarian mass was detected. For the removal of ovarian mass, an ovariohysterectomy was performed. Grossly, the ovarian mass was full of hairmatrix on the cut surface of the mass. Histopathological findings revealed a characteristic of teratoma, such as laminated keratines, hair, sebaceous glands, neuron tissue, differentiated bone and cartilage. The ovarian mass was well-differentiated components of 2 germ layers, and diagnosed as a mature teratoma.

종격동에 발생한 거대 미성숙기형종 -1례 보고- (Huge Immature Teratoma of Mediastinum - A case report -)

  • Primary mediastinal immature teratoma is a rare germinal tumor which includes various elements of mature teratoma, choriocarcinoma, yolk sac carcinoma, embryonal carcinoma, and seminoma in some proportions. The tumor is virtually restricted to young man and the response to surgery and radiotherapy are poor. Recently, we experienced a case of primary mediastinal immature teratoma with elevated serum [-HCG and [-fetoprotein in 18 years old man. The well-encapsulated mass, weighing 4.5 kg, was completely resected and then adjuvant combination chemotherapy was tried with Vincristine, Bleomycin, and Cisplatin. Radical excision of tumor and adjuvant chemotherapy would appear to produce better result than have been reported in other cases. The postoperative course was uneventful and the tumor markers were returned to normal range.

  • This article reports a spontaneous thoracic teratoma in a laying hen (Deklb brown warren). In this case, a 40 days old hen was submitted for necropsy as part of an investigation into a flock problem suspected Marek's disease to Chonbuk Veterinary Service Laboratory in 1983. On the gross finding, mass was In the cranial subpulmonary cavity and attached to the vertebral column. It contained fully developed contour feathers. Histologically, feathers were arised from feather follicles complete with arrector plumi muscle, nerve, vessel and mucous gland composed with simple tall columnar epithelium. The outer surface of mass was lined by keratinized or nonkeratinized stratified squamous epithelium. It converted to pseudostratified ciliated columnar epithelium in some area. There were lymphocyte infiltration around gland tissue and eosinophilic intranuclear inclusion bodies in nonkeratinized epithelium. This thoracic teratoma was composed of ectodermal origin, squamous epithelium and nerve, and endodermal origin, mucous gland. This case in laying hen has never been reported in the literature in the world.
