Comparison of Usefulness of Laboratory Tests in Diagnosis of Myasthenia Gravis (중증 근무력증 진단에 있어서 제반 검사법들의 유용도 비교)

  • Purpose: This study was undertaken to evaluate the clinical usefulness of Tensilon test, repetitive nerve stimulation test(RNST), single fiber EMG(SFEMG) test and acetylcholine receptor antibody(AchR Ab) assay for making diagnosis of myasthenia gravis(MG). Method: These tests were performed in 21 MG patients which were classified into 11 ocular, 5 mild generalized, 4 moderate generalized, and 1 chronic severe MG. Result: The overall positivity of Tensilon test, SFEMG and AchR Ab was 95%, 87%, and 76% respectively. The overall positivity of RNST was 67%; 38% on flexor carpi ulnaris, 43% on adductor digiti quinti and 62% on orbicularis oculi muscles. The positivity of each test was higher in generalized MG group than in ocular MG group. But we could observe the statistically significant difference only in the RNST(p<0.05). Conclusion: Tensilon test showed the highest positivity in all MG groups. So we would like to recommend the, Tensilon test for the diagnosis of MG at first, followed by RNST and AchR Ab assay, and SFEMG would be indicated to MG group which showed relatively low postivity in other tests.

  • The effects of the stretching speed and the swelling degree of cocoon filaments on load-elongation curve were tested by tensilon with special attachments. The values of breaking strength, elongation at break and initial tensile strength obtained from load elongation curves are summarized as follows; 1. The breaking strength of the swollen cocoon filament appeared to increase at the high stretching speed, while elongation at break decreased. 2. Load-elongation curve became crinkled at the low speed of stretching. It suggests that both slip and break of the macrofibrils may occur in swelling of cocoon bave. 3. Breaking strength appeared to considerably decrease and elongation at break not to increase in swelling test at 9$0^{\circ}C$ for 60 min. 4. The initial tensile strength was influenced by the stretching speed and swelling degree of cocoon filaments.

Thymectomy in Myasthenia Gravis: 2 cases report (흉선절제에 의한 근무력증 치료 2례 보고)

  • Myasthenia gravis is a neuromuscular disorder manifested by fatigability and weakness of voluntary muscles. The basic defect in the myasthenia is reduction of available acetylcholine receptors at neuromuscuiar junctions by an autoimmune attack. Removal of the thymus gland now play an Important role in the management of this disease. We have two experiences of thymectomy for myasthenic patients. The 31-year-old housewife(J.H) was admitted to the Taegu Presbyterian Medical Center because of bilateral ptosis, mastication and swallowing difficulties. The presence of thymoma was strongly suggested by roentgenographic studies. A 99.8 gm thymoma was removed completely by total thymectomy. On the 2nd postoperative day respiratory failure developed. Tracheostomy was performed and ventilatory assist was given for 3 days. The patient was completely recovered with antichollestrase drug only. C.N. was 17-year-old girl who has the symptoms of bilateral ptosis and diplopia for 3 years. Tensilon test was positive and antichollnestrase was given for several months, but the patient showed no improvement. After thymectomy she was free from myasthenic symptoms with out anticholinestrase drug.

A Case of Myasthenia Gravis Combined with Mediastinal Leiomyosarcoma and Stevens-Johnson Syndrome (종격 평활근육종 및 Stevens-Johnson증후군과 동반된 중증 근무력증 1예)

  • We report a case of 36-year-old woman with myasthenia gravis (MG) combined with mediastinal leiomyosarcoma (LMS) and Stevens-Johnson syndrome (SJS). She was admitted to ICU with the symptoms of acute onset headache, diplopia, ptosis, dysphagia, general weakness, and respiratory difficulty for several days. Physical examination revealed tachypnea, decreased breath sounds and dullness to percussion in right lower chest. Neurologic examination showed ptosis, diplopia, decreased gag reflexes, and generalized proximal weakness. Laboratory studies revealed increased serum acetylcholine receptor antibodies and positive Tensilon test. Chest CT showed a huge mass in the right middle mediastium but no evidence of thymic enlargement. Mediastinal LMS was diagnosed by ultrasound-guided needle biopsy. The myasthenic symptoms were fluctuated in spite og intravenous immunoglobulin, plasmapheresis, and corticosteroid. During therapy, SJS developed. She died 4 months after the onset of the myasthenic symptoms despite the chemotherapy for LMS.

Laryngeal Paralysis in a Cat with Malignant Thymoma

  • A 10-year-old castrated male Domestic Shorthair cat visited a veterinary medical teaching hospital for emergency dyspnea. The cat was hypoxic and hypotensive, and stridorous respiration was remarkable. Visual inspection confirmed laryngeal paralysis and a lack of mobility of the left larynx. Megaesophagus, aspiration pneumonia, cranial mediastinal mass, and positive Tensilon test results using neostigmine were observed, indicating acquired myasthenia gravis secondary to thymic neoplasia. After 10 minutes of neostigmine 0.02 mg/kg IV administration, laryngeal paralysis and dyspnea resolved. Histopathlogical examination for the cranial mediastinal mass after surgical resection confirmed malignant thymoma. Here, we report a case of acquired myasthenia gravis in a cat with a malignant thymoma that presented with life-threatening dyspnea due to laryngeal paralysis. Feline laryngeal paralysis is uncommon, and myasthenia gravis, a cause of laryngeal paralysis in cats, has not yet been reported. Myasthenia gravis should be considered in cats with laryngeal paralysis.

An Experimental Study on the Lightfast of Silk Fabrics (견직물의 내광성에 관한 연구)

  • This experiment was made on the basis of the general observation that silk fabrics is flaccid in sun light. The purpose of this test is to examine efficiency between the processed thread and the unprocessed one after putting the later under the xenon-lit system, which produces continuous spectrum of bright white beam similar to sun light. (i) Color division of silk thread : white, yellow blue (ii) light division and irradiation time : sun lightlongrightarrow210 hrs, Xenonlongrightarrow50,100, 150 hrs. Under the above conditions each object was be tested with and extensometer, TENSILON Type III for the results of (a) the weight-expansibility curve of the silk thread (b) the relationship between cutting in tensity and maximum, weight (c) maximum expansion rate (d) the expansion energy of the tested silk thread (e) the beginning pull-stretch resistance rate of the tested silk thread (f) color difference after processing The results are illustrated in the appropriate tables and figures. Consequently the generalobservation that silk fabrics is flaccid is relevant only in terms of color. So for as Cutting-intensaty is concerned, it turned out to be less flaccid than in color. On the contrary when the untested silk thread was conpared with the xenon-irradiated ones, the latter proved itself to be more flaccid in cutting expansibility than the former. We have learned through this experiment that, after the all rounds of this test, the color tone and chroma of the colored materials remained without significant change, where as their color value changed to a large extent.

TENSILE STRENGTH OF DIRECT-BONDING BRACKETS (Direct-bonding bracket 의 인장강도(引張?度))

  • As adhesive systems were improved, a great variety of bracket bases were available. The purpose of this study was to evaluate the adhesive porperties of 3 types of direct-bonding brackets by testing the tensile stremgth. 60 noncarious premolars extracted for orthodontic treatment were used. The tensile strength was tested by Tensilon/UTM-1-10000C after 24 hours from bonding. Following results were obtained; There was no difference between the tensile strength of foil-mesh base bracket and photoetched base bracket statisstically. However, the accurate test result of plastic bracket could not be obtained due to the distortion of bracket wing during testing. Of the failure in fail-mesh base bracket and photo-etched base bracket, the combination type of failure, where part of the adhesive remained on the tooth and part on the bracket was the most common type (50%, 50%), The second type of failure occured at the bracket-adhesive interface (30%, 35%) and the last type of failure occured at the adhesive-tooth interface (20%, 15%).
