18F-Florbetaben PET/CT 검사에서 영상분석에 대한 고찰 (A Discussion on Image Analysis in 18F-Florbetaben PET/CT)

  • 18F-FBB 판독은 회백질과 백질의 신호강도를 육안으로 비교하여 이루어진다. 정량화된 영상분석을 판독과 비교하여 영상분석의 유용성을 평가하고자 한다. 환자는 판독결과를 기준으로 음성과 양성을 100명씩 나누었고 FBB 300 MBq 주입하고 90분 뒤 20분간 촬영했다. 장비는 Discovery 600 (GE Healthcare, MI, USA)을 사용하였다. 제조사에서 제공하는 아밀로이드 판독 기준을 근거하여 4개의 관심영역을 설정하였다. 영상분석은 각 SUVmean을 소뇌로 나누어 SUVr를 산출하고 전체 영역에서의 평균 SUVr로 진행하였다. 통계분석은 ROC Curve를 통한 Cutoff 도출과 독립표본 t-test의 그룹간 차이, 그리고 Kappa test를 통한 판독결과와 일치도를 분석하였다. 전체 영역에서의 평균 SUVr의 Cutoff는 1.23으로 나왔다. Cutoff를 사용한 판독결과와 일치도는 음성에서 95/100 (95 %), 양성에서 92/100 (92 %)로 나왔다. t-test 결과 그룹 간에 통계적으로 유의한 차이가 있었고(P < 0.05) Kappa 통계 결과 0.867로 높은 일치도를 나타냈다(P < 0.05). 영상분석의 결과가 통계적으로 유의하며 판독결과에도 높은 일치도를 보여 주었다. 추가적으로 FBB 영상분석은 아밀로이드가 축적된 부위를 3D 매핑하여 볼 수 있고 위치추정이 가능하며 정량분석 결과를 세분화하여 볼 수 있다. 정량화된 FBB 영상분석을 보조지표로 활용한다면 판독에 도움이 될 것으로 사료된다.

프랙탈 이미지를 이용하여 본 미적 경험의 뇌 활성화: 기능적 자기공명영상 연구 (Neural correlates of the aesthetic experience using the fractal images : an fMRI study)

  • 본 연구에서는 객관적인 아름다움에 대한 주관적인 미적 경험으로서의 인지적, 감정적 경험에 관여하는 뇌 활성화 과정을 기능적 자기공명영상을 이용하여 검토해 보았다. 우선, 프랙탈 이미지에 대한 미적평가 행동과 제로 보편적인 아름다움의 준거를 확인하였다. 평정 결과에 기초하여, 전체 270개 이미지 중 가장 점수가 높은 50개를 아름다운 이미지로, 점수가 낮은 50개를 아름답지 않은 이미지로 선정하였다. 두 가지 조건을 블록으로 제시한 신경영상 연구 결과, 아름답다고 평가한 이미지에 대해서는 미적 경험에 관여하는 인지적, 정서적 처리에 관여된 부위인 전두엽과 대상회와 뇌섬엽이 활성화되었으며, 아름답지 않다고 평가한 이미지에 대해서는 부정적인 정서와 관련된 중후두회와 전설소엽의 활성화가 관찰되었다. 아름다움 평가에 대한 조건별 접속분석의 결과, 미적평가가 긍정적인 이미지에 대해서는 측두엽의 활성화가, 부정적인 이미지에 대해서는 두정엽의 활성화가 특징적으로 나타났다. 이에 대해 의미론적 해석과 추상화 과정과 연결하여 논의하였다.

핵의학(SPECT)을 이용한 뇌혈류변화에 대한 침구효과 검증방법의 새로운 모델에 관한 연구 (New Model of Verifiation for Demonstration of Neuronal Basis of Acupuncture by Comparison of Two Different Methods of Acupuncture which Increase Regional Cerebral Blood Flow ( rCBF ) on SPECT)

  • Objective : The mechanism of acupuncture to increase cerebral blood flow is still uncertain. The purpose of this study was to evaluate the neural basis of acupuncture by comparing the cerebral regions activated by the stimulation of two different methods of acupunctures at the same acupoint which was suggested by oriental medicine to increase rCBF. Materials and Methods : Thirty-nine healthy volunteers(26 males, 13 females, age $31{\pm}11$ years) were studies by rest/acupuncture Tc-99m ECD brain SPECT using a subtraction method. SPECTs using two methods(needle retention and heated needle with 90% alcohol) at two acupoints (right LI. 4 and ST. 36) were peformed at an interval of three days. For the needle retention method, acupuncture needle was inserted to a depth of about 2 cm into each acupoint 8 minutes after the lst acquisition and continued to retain, and the second injection of Tc-99m ECD was done 15 minutes after the insertion of needle. For the heated acupuncture method, heated needle was inserted in a twinkle within several msec 20 second after the second injection of Tc-99m ECD. The differences of between rest and acupuncture activation state were statistically analyzed using a statistical parametric mapping software. Result : Acupunctures of both methods reveal similar patterns of increase in rCBF. Acupuncture at ST.36 increase rGBF in left anterior temporal, right inferior frontal lobes, and left cerebellum. Acupuncture at LI. 4 increase rCBF in the left frontal cortex, right temporal pole, both inferior frontal cortices and right cerebellum. Conclusion : The effects of two different acupunctures to the same acupoints on rCBF were similar. Therefore, this result suggests Chat the mechanism of acupuncture in the increase of cerebral blood flow have a neural basis.

Associations between Brain Perfusion and Sleep Disturbance in Patients with Alzheimer's Disease

  • Background and Purpose Although sleep disturbances are common and considered a major burden for patients with Alzheimer's disease (AD), the fundamental mechanisms underlying the development and maintenance of sleep disturbance in AD patients have yet to be elucidated. The aim of this study was to examine the correlation between regional cerebral blood flow (rCBF) and sleep disturbance in AD patients using technetium-99m hexamethylpropylene amine oxime single-photon emission computed tomography (SPECT). Methods A total of 140 AD patients were included in this cross-sectional study. Seventy patients were assigned to the AD with sleep loss (SL) group and the rest were assigned to the AD without SL group. SL was measured using the sleep subscale of the Neuropsychiatric Inventory. A whole-brain voxel-wise analysis of brain SPECT data was conducted to compare the rCBF between the two groups. Results The two groups did not differ in demographic characteristics, severity of dementia, general cognitive function, and neuropsychiatric symptoms, with the exception of sleep disturbances. The SPECT imaging analysis displayed decreased perfusion in the bilateral inferior frontal gyrus, bilateral temporal pole, and right precentral gyrus in the AD patients with SL group compared with the AD patients without SL group. It also revealed increased perfusion in the right precuneus, right occipital pole, and left middle occipital gyrus in the AD with SL group compared with the AD without SL group. Conclusions The AD patients who experienced sleep disturbance had notably decreased perfusion in the frontal and temporal lobes and increased rCBF in the parietal and occipital regions. The findings of this study suggest that functional alterations in these brain areas may be the underlying neural correlates of sleep disturbance in AD patients.

공간능력, 시지각 회상 능력, 학습양식에 따른 지구와 달의 운동 개념 (Concepts on Motion of Earth and Moon to Spatial Ability, Visual-Perception-Recall Ability, Learning Styles)

  • The purpose of this study was to investigate the relationship among spatial ability, learning styles, visual-perception- recall abiltiy, and the conceptual construction of the earth and moon's motion. Four paper-and-pencil tests were used to measure students' cognitive variables. Spatial ability was measured by Spatial Visualization Test, visual-perception-recall ability was measured by Rey's Figure which also have used to test visual- perception-recall ability of right-temporal lobes, and VVT were used to investigate students' learning styles. further, the test of concept construction was consisted of 15 items about the earth and moon's motion developed by researcher One hundred and twenty-seven 6th-, one hundred and sixteen 7th-, eighty-seven 9th-grade, ninety-three college students were participated in the investigation of the effects of age and learning style on conceptual construction. In the analysis of students' performances, spatial ability, visual-perception-recall ability, and conceptual achievement showed an increasing pattern with grading. In addition, visual learner's conceptual achievement showed a significantly higher score on conceptual test than verbal learner's(p<0.05). The results of the present study supported tile hypothesis that learning styles would differently influence to learning atmospheric concepts by students'learning styles. This study also indicated to be considered the students' spatial ability in learning atmospheric concepts.

Clinical Neuropathological Analysis of 10 Cases of Cerebral Amyloid Angiopathy-Related Cerebral Lobar Hemorrhage

  • Objective : The clinical and pathological characteristics of 10 cases of cerebral amyloid angiopathy (CAA)-related cerebral lobar hemorrhage (CLH) that was diagnosed at autopsy were investigated to facilitate the diagnosis of this condition. Methods : The clinical characteristics of 10 cases of CAA-related CLH were retrospectively reviewed, and a neuropathological examination was performed on autopsy samples. Results : The 10 cases included two with a single lobar hemorrhage and eight with multifocal lobar hemorrhages. In all of the cases, the hemorrhage bled into the subarachnoid space. Pathological examinations of the 10 cases revealed microaneurysms in two, double barrel-like changes in four, multifocal arteriolar clusters in five, obliterative onion skin-like intimal changes in four, fibrinoid necrosis of the vessels in seven, neurofibrillary tangles in eight, and senile plaques in five cases. Conclusion : CAA-related CLHs were located primarily in the parietal, temporal, and occipital lobes. These hemorrhages normally consisted of multiple repeated CLHs that frequently bled into the subarachnoid space. CAA-associated microvascular lesions may be the pathological factor underlying CLH.

An Unusual Case of Cerebral Penetrating Injury by a Driven Bone Fragment Secondary to Blunt Head Trauma

  • Temple trauma that appears initially localized to the skin might possess intracranial complications. Early diagnosis and management of such complications are important, to avoid neurologic sequelae. Non-penetrating head injuries with intracranial hemorrhage caused by a driven bone fragment are extremely rare. A 53-year-old male was referred to our hospital because of intracerebral hemorrhage. He was a mechanic and one day before admission to a local clinic, tip of metallic rod hit his right temple while cutting the rod. Initial brain computed tomography (CT) and magnetic resonance imaging demonstrated scanty subdural hematoma at right temporal lobe and left falx and intracerebral hematoma at both frontal lobes. Facial CT with 3-D reconstruction images showed a small bony defect at the right sphenoid bone's greater wing and a small bone fragment at the left frontal lobe, crossing the falx. We present the unusual case of a temple trauma patient in whom a sphenoid bone fragment migrated from its origin upward, to the contralateral frontal lobe, producing hematoma along its trajectory.

정보전달형 TV광고와 감성유발형 TV광고의 뇌파반응 비교 (A Comparison of EEG Response between TV Advertisements focused on Information Transfer and Emotional Reaction)

  • This study has investigated psychological responses to various TV advertisements by using EEG(electroencephalogram). In particular, it was assumed that the brain wave would show different patterns depending upon the style of the advertisement that could be categorized into two such as 'information transfer' type and 'emotional reaction' type. Ten healthy males participated in the experiment. EEG signal was extracted from six lobes such as right and left frontal, right and left occipital, right and left temporal while the subjects were watching TV advertisements. Alpha and beta relative power spectrum, and beta/alpha parameter were calculated to compare two kinds of advertisement each other. Additionally, subjective questionnaire was used to examine subject's response by using adjective words and preference test. In result, significant differences were found in left frontal and right occipital lobe in terms of beta/alpha between two different advertisements. And, subjects showed different preference between two advertisements. It was shown that the current method could analyze the brain reaction to advertisement quantitatively, that presented the possibility of using it to marketing research.

외상 후 후각이상에 대한 방사선학적 진단 (Radiological Diagnosis for Posttraumatic Olfactory Dysfunction)

  • Objective : To evaluate objectively the sites of injury in patients with posttraumatic olfactory deficits and to suggest the diagnostic procedure for evaluation of posttraumatic anosmia. Methods : Ten patients with posttraumatic olfactory dysfunction were examined by means of olfactory testing, sinoscopy, contrast filled paranasal sinus computed tomography(contrast filled PNS CT) and magnetic resonance imaging(MRI). Five normal persons without olfactory dysfunction were also evauluated. The aerodynamic patency of olfactory cleft was examined by contrast filled PNS CT. The olfactory system(oflactory bulbs, olfactory tracts, inferior frontal region, hippocampi, or temporal lobes) was investigated in detail with MRI. The difference in the size of the olfactory bulb between normal volunteers and anosmic patients was evaluated by Student's t test. Results : Contrast filled dynamic CT scan was useful method for the evaluation of dynamic patency of the olfactory cleft. Paranasal CT scan of the all anosmic patients showed dynamic reflux of contrast media in olfactory cleft on valsalva maneuver. For the largest cross-sectional area and great height, the difference in olfactory bulb size between normal volunteers and patients was statistically significant(p<0.001) in MRI study. Conclusion : Posttraumatic anosmia was completely evaluated by olfactory testing, sinoscopy, and contrast filled CT scan for differentiation between conductive type and neurogenic type. Neurogenic anosmia was confirmed by perfect localization with MRI study.

후대뇌동맥 경색으로 발생한 시각 및 인지장애 환자 치험 1례 (Case Report on a Posterior Cerebral Artery Infarction Patient with Visual and Cognitive Abnormalities)

  • The posterior cerebral arteries supply the temporal and occipital lobes of the left cerebral hemisphere and the right hemisphere. Clinical symptoms associated with occlusion of the posterior cerebral artery are visual abnormalities including opposite visual field defects, hallucination, visual amnesia and a variety of other symptoms, including confusion, cognitive disorders, thalamic syndrome, Weber's syndrome, contralateral hemplegia. This case report is about a patient with visual and cognitive abnormalities caused by posterior cerebral artery infarction. He was regarded as Soyangin(少陽人) in constitution and was treated with Yangkyuksanhoa-tang(凉膈散火湯) and other treatments. Improvement in his general symptoms was observed.
