The author studied the histopathologic changes according to a single or a split dose and the time after irradiation on the acinar cells of rat parotid gland. 99 Sprague Dawley rats, weighing about l20gm, were divided into control and 3 experimental groups. In experimental groups, GroupⅠ and Ⅱ were delivered a single dose of l5Gy, 18Gy and Group Ⅲ and Ⅳ were delivered two equal split doses of 9Gy, 10.5Gy for a 4 hours interval, respectively. The experimental groups were delivered by a cobalt-60 teletherapy unit with a dose rate of 222cGy/min, source-skin distance of 50㎝, depth of l㎝ and a field size of l2×5㎝. The animals were sacrificed at 1, 2, 3, 6, 12 hours, 1, 3, 7 days after irradiation and examined by light and electron microscopy. The results were as follows: 1. As the radiation dose increased and the acinar cells delivered a single dose exposure were more damaged, and the change of acinar cells appeared faster than those of a split dose exposure. 2. The histopathologic change of acinar cells appeared at 1 hour after irradiation. The recovery from damaged acinar cells appeared at 1 day after irradiation and there was a tendency that the recovery from damage of a split dose exposure was somewhat later than that of a single dose exposure. 3. Light microscope showed atrophic change of acinar cells and nucleus, degeneration and vesicle formation of cytoplasm, widening of intercellular space and interlobular space. 4. Electron microscope showed loss of nuclear membrane, degeneration of nucleus and nucleoli, clumping of cytoplasm, widening and degeneration of rough endoplasmic reticulum, loss of cristae of mitochondria, lysosome, autophagosome and lipid droplet. 5. Electron microscopically, the change of rough endoplasmic reticulum was the most prominent and this appeared at 1 hour after irradiation as early changes of acinar cells. The nuclear change appeared at 2 hours after irradiation and the loss of cristae of mitochondria was observed at 2 hours after irradiation in all experimental groups.

This study was designed to investigate the effects of split irradiation on the salivary ductal cells, especially on the intercalated cells of the rat parotid glands. For this study, 24 Sprague-Dawley strain rats were irradiated on the head and neck region with two equal split doses of 9Gy for a 4 hours interval by Co-60 teletherapy unit, Picker's model 4M 60. The conditions of irradiation were that field size, dose rate, SSD and depth were 12×5㎝, 222 cGy/min, 50㎝ and 1㎝, respectively. The experimental animals were sacrificed 1. 2, 3, 6, 12, hours and 1, 3, 7, days after the irradiation and the changes of the irradiated intercalated cells of the parotid glands were examined under light and electron microscope. The results were as follows: 1. By the split irradiation, the degenerative changes of intercalated cells of the parotid glands appeared at 3 hours after irradiation and the most severe cellular degeneration observed at 6 hours after irradiation. The repair processes began from 12 hours after irradiation and have matured progressively. 2. Under electron microscope, loss of nuclear membrane, microvilli and secretory granules, derrangement of chromosomes, degeneration of cytoplasm, atrophy or reduction of intracytoplasmic organelles were observed in the intercalated ductal cells after split irradiation. 3. Under light microscope, derrangement of ductal cells, widening of cytoplasms and nuclei, hyperchromatism and proliferation of ductal cells were observed in intercalated ducts after split irradiation.

In standard teletherapy, a treatment plan is generated with the aid of a treatment planning system, but it is common to perform an independent monitor unit verification calculation (MUVC). In exact analogy, we propose and demonstrate that a simple and accurate MUVC in Intensity Modulated Radiotherapy (IMRT) is possible. We introduce a concept of Modified Clarkson Integration (MCI). In MCI, we exploit the rotational symmetry of scattering to simplify the dose calculation. For dose calculation along a central axis (CAX), we first replace the incident IMRT fluence by an azimuthally averaged fluence. Second, the Clarkson Integration is carried over annular sectors instead of over pie sectors. We wrote a computer code, implementing the MCI technique, in order to perform a MUVC for IMRT purposes. We applied the code to IMRT plans generated by CORVUS. The input to the code consists of CORVUS plan data (e.g., DMLC files, jaw settings, MU for each IMRT field, depth to isocenter for each IMRT field), and the output is dose contribution by individual IMRT field to the isocenter. The code uses measured beam data for Sc, Sp, TPR, (D/Mu)$\_$ref/ and includes effects from MLC transmission, and radiation field offset. On a 266 MHZ desktop computer, the code takes less than 15 sec to calculate a dose. The doses calculated with MCI algorithm agreed within +/- 3% with the doses calculated by CORVUS, which uses a 1cm x 1cm pencil beam in dose calculation. In the present version of MCI, skin contour variations and inhomogeneities were neglected.
To assess the result of radiation therapy for fifteen fears experience, a total of 125 cases of pathologically proven supraglottic laryngeal cancer had been analyzed according to patient survival retrospectively. All the patients had been treated with radiation therapy in curative aim using Co-60 teletherapy machine. The results are as follows ; 1. According to AJCC staging, five year survival rate was $58.3\%$ in stage I, $44.4\%$ in II, $31.8\%$ in II, and $28.6\%$ in IV. 2. According to T-staging, five year survival rate was $57.1\%$ in T1, $40.5\%$ in T2, $34.0\%$ in T3, and $19.0\%$ in T4. 3. According to N staging, five year survival rate was $43.5\%$ in negative node group and $26.8\%$ in positive node group. 4. According to the histologic grade, the better in differentiation, the more in number of cases and the better in prognosis. 5. In summary, five year actuarial survival rate was $37.3\%$ and ten year surrival rate was $34.2\%$, and ten year survivors totalled 16 cases.
To evaluate the result of radiation therapy for twenty years experience, a total of 115 cases of pathologically proven glottic carcinoma had been analyzed according tot survival respectively. All the patients had been treated with radiation therapy in curative intent using Co-60 teletherapy machine. The results are as follows: 1) Accoridng to sex, 5YSR & 10YSR were 58.7% and 50.4% in male patients, and 80.0% & 72.0% in female. 2) According to T staging, 5YSR & 10YSR were 83.4% & 83.4% in T1, 69.2% & 60.0% in T2, 34.3% & 21.8% in T3, 32.0% & 0% in T4. 3) According to N staging, 5YSR & 10YSR were 63.8% & 59.2% in node-negative groups, and 30.7% & 15.4% in node-positive groups. 4) According to the histologic grade, 5YSR & 10YSR were 66.8% & 57.6% in G1, 61.3% 54.3% in G2, and 35.0% 35.0% in G3. 5) According to AJC staging, 5YSR & 10YSR were 83.4%% 83.4% in stage I, 72.0% & 62.7% in stage II, 36.8% & 28.3% in stage III, and 14.3% & 7.1% in stage IV. 6) In summary, 5YSR & 10YSR wre 60.4% & 52.8% in glottic carcinoma.
Scatter-air ratios are used for the purpose of calculating scattered dose in the medium. The computation of the primary and the scattered dose separately is particularly useful in the dosimetry of irregular fields with shielding block in radiation field, dose distribution of scattered radiation using 18MeV Linear accelerator and Co-50 teletherapy measured. The effect of scattered radiation dose by protecting block was been ignored in radiation therapy, 2-3% of scattered radiation may be 90-200 cGy which could be influence vitial complications such as cataract, oligospermia or sterility. So that exect calculation of such scattered radiation especially for large field $\bar{c}$ small protection of vitial organ is very important. The purpose of this article is to calculate scattered radiation by protecting block exactly for irregular field $\bar{c}$ Linac or Co-60 irradiation and to applicate these data in clinical radiation field. Authors could obtain following results. 1. The lesser angle between shielding block showed more scattered radiation. 2. With decreasing distance between shielding blocks, the dependent of scattered radiation were increased. 3. Output of 18MeV Linear accelerator and Co-60 was related linear proportion on field size, but independent according to the size of shielding block in 18MeV Linear accelerator.

This study was undertaken to observe the histopathologic changes in salivary gland of the white rats when exposed to megavoltage fractionated dose of cobalt-60 irradiation and 78 female white rats, weighing approximately 180gm, were divided into control and 3 experimental groups. Irradiation on experimental groups was delivered by using 6000 curies MeV ALCYON cobalt-60 teletherapy unit with exposure rate 183 rads per minute, in source skin distance 80cm, 600 rads every 3 days. In experimental groups, Group Ⅰwas irradiated of total dose 1200 rads for a period of 6 days, Group Ⅱ was irradiated of total dose 2400 rads for a period of 12 days and Group Ⅲ was irradiated of total dose of 4800 rads for a period of 24 days. The animals were sacrificed serially at 3 hours, 6 hours, 10 hours, 1st day, 4th day, 7th day after each completion of irradiation exposure. At sacrifice, salivary glands were excised and examined microscopically and electromicroscopically. The results were as follows: 1. The acinar cells of parotid and submaxillary gland showed damage varied with dose, 1200 rads resulted in very mild injury while 4800 rads caused most extensive injury. 2. The acinar cells of parotid and submandibular gland showed similar ultrastructural alterations, appeared as pleomorphic nucleus, decreased numbers and pleomorphism of secretory granules, distention of rough endplasmic reticulum, expansion and pallor appearance of mitochondria, and hypertrophy of Golgi complex. 3. Parotid serous cells were the most sensitive components, displaying morphological alterations of radiation damage as early as 3 hours, followed by submandibular seromucinous cells and secretory tubular cells. 4. The mucous cells of sublingual gland, as well as the whole ductal lining cells of each salivary gland, displayed no significant alterations. No evidence of microvascular injury through whole experimental groups indicated that microvascular impairment does not contribute to early salivary gland injury.

According to T-staging, five year survival rate was $75.0\%$ in T1, $73.1\%$ in T2, $24.3\%$ in T3, and $25.0\%$ in T4. 5, According to nodal status, five year survival rate was $59.4\%$ in negative node group and $14.2\%$ in positive group. 4. According to the histologic grade, the better in differentiation, the more in number of cases and the better in prognosis. 5. In summary, five year actuarial survival rate was $55.5\%$ and ten year survival rate was $49.8\%$ and ten year survivors totalled 12 cases.

The purpose of this study was to investigate the changes of morphology and structure of bone tissue in the irradiated mandibular bone in rats which were fed a low calcium diet. In order to carry out this experimant, 64 seven-week old Sprague-Dawley strain rats weighing about 150gms were selected and equally divided into one experimental group of 32 rats and one control group with the remainder. The experimental group and the control group were then subdivided into two groups when the rats reached ten-week old, 16 were assigned rats for each subdivided group, exposed to irradiation. The two irradiation groups received a single dose of 20 Gy in the jaws area only and irradiated with a cobalt-60 teletherapy unit. The rats in the control and experimental groups were serially terminated by fours on the 3rd, the 7th, the 14th, and the 21st day after irradiation. Following termination, both sides of the dead rats mandibular bodies were removed and fixed with 10% neutral formalin. One side of the mandibular body was radiographed with a soft X-ray apparatus. Thereafter, the obtained microradiographs were observed by a light microscope. The remaining side of the mandibular bone was further decalcified and embedded in paraffin as using the general method. The specimen sectioned and stained with hematoxylin and eosin, and Rabit Anti-Human Tumor Necrosis Factor-a, observed by a light microscope. The obtained results were as follows: 1. Microradiogram revealed that thinning of the cortex and a decrease in the trabecula of the interradicular bone and mandibular body were observed and noted from the start to finish throughout the experiment in the non-irradiated rats on the low calcium diet rather than in the non-irradiated rats on the normal diet. In microscopic observations, there were marked osteolytic changes in the center of the bone marrow. 2. Microradiogram revealed that thinning of the cortex and a decrease in the trabecula of interradicular bone and mandibular body were more marked after 7 days in the irradiated rats on the low calcium diet rather than in the non-irradiated rats on the low clacium diet. In microscopic observations, osteoblasts were decreased markedly after 7 days, and a few osteoclasts were observed. 3. Microradiogram revealed that thinning of the cortex and a decrease in trabecula of interradicular bone and the mandibular body were more marked from the strart to finish troughout the experiment in the irradiated rats on the low calcium diet rather than in the irradiated rats on the normal diet. In microscopic observations, there were no osteolytic changes noted in the irradiated rats on the normal diet. 4. In immunocytochemical findings, a few bone marrow cells including Tumor Necrosis Factor were observed in the irradiated rats on the normal diet, but was not observed in the rats on the low calcium diet.

The purpose of this study was to investigate the changes of periodontal tissues in the irradiated mandibular bone in rats which were fed normal diet and low calcium diet In order to carry out this experiment, 64 seven-week old Sprague-Dawley strain rats weighing about 150gms were selected and equally divided into one experimental group of 32 rats and one control group with the remainder. The experimental group and the control group were then subdivided into two groups when the rats reached the age of 10 weeks, 16 rats were allotted for each subdivided group was composed of 16 rats and exposed to irradiation. The two groups were irradiated a single dose of 20Gy on the only jaw area and irradiated with a cobalt-60 teletherapy unit The rats in the control and experimental groups were serially dissected by fours on the 3rd, the 7th, the 14th, and the 21st day after irradiation. After each dissection, both sides of the dead rat mandibular bodies were removed and fixed with 10% neutral formalin. The specimens sectioned and observed in histopathological. histochemical. and immunocellular chemical methods. The obtained results were as follows: 1. In the mandibles of rats with low calcium diet the increased number of fibroblasts of periodontal ligaments. many small capillaries and irregular arrangement of loose collagen fibers were detected and the partial resorption of dentin and cementum could be found by the microscopic studies. 2. In the group of irradiated rats, degenerated periodontal tissues led to the condition of irregular arrangement of collagen fibers and the decreased number of fibroblasts. But this condition was somewhat restored after 21 days of experiment. 3. Periodontal tissues of the irradiated rat group with low calcium diet were destroyed earlier than those of the irradiated rat group with normal diet. Soon this condition was restored and then high cellularity and dense collagen fibers were observed. 4. Many periodontal cells bearing tumor necrosis factor could be clearly observed in the nonirradiated group of rats with normal diet, whereas could not be observed on the 7th day and reappeared on 14th day in the irradiated group of rats with normal diet. A few of them could be observed in the group of rats with low calcium diet, but they could be clearly observed in the both groups after 21 days of experiment.
