폐암의 방사선치료 결과에 대한 간이보고 (Preliminary Report of the Lung Cancer)

  • The preliminary analysis has been made of 63 Patients who were treated in the Department of Radiation Therapy at Korea University Hospital for the lung cancer from April 1981 to December 1983. The patients were treated via Co-60 teletherapy unit and the doses were 5,400 rad/ 6 week for the curative, $3,600\~4,500\;rad/2\;1/2\;\~3$ week for the Palliative treatment. Thirty two $(32/63=51\%)$ patients have been for curative, $25/63 (40\%)$ were for Palliative, and $6/63 (9\%)$ were for post-operative radiation. A post-radiation treatment, $28/63 (53\%)$ were able to follow from 2 months to 3 yrs. During the follow up, chest X-rays and/or CT scans were taken and $22/27 (81.4\%)$ of patients were responded more than $50\%$ of tumor regression one month following completion of radiation.

  • 고 에너지 방사선으로 용적이 적은 표재성 종양 및 부정형의 체표면과 경사진 조사야에 대한 방사선 치료시 종양 용적내에 균등한 선량분포를 얻기 위해서 Wedge filter 기법이 많이 사용되고 있다. 본 연구의 목적은 코발트 $60{\Gamma}$선 치료를 위한 Semi-Universal Wedge filter의 제작과 임의의 조사야에 대한 최적한 출력 및 정확한 조작방법을 기술하고 방사선 치료계획의 최적화를 기하기 위해서 Wedge filter의 임상응용 방법을 제시하고자 한다.

  • This study was designed to investigate the effects of irradiation on the salivary ductal cells, especially on the intercalated ductal cells of the rat parotid glands. For this study, 36 Sprague-Dawley strain rats were irradiated on the head and neck region with absorbed dose of 15Gy by Co-60 teletherapy unit, Picker's model 4M60. The conditions irradiated were that field size, SSD, dose rate and depth were 12×5㎝m, 50㎝, 222 Gy/min. and 1㎝. respectively. The experimental animals were sacrificed 1, 2, 3, 6, 12 hours and 1, 3, 7 days after the irradiation and the changes of the irradiated intercalated duct cells of the parotid glands were examined under the light and electron microscope. The results were as follows: 1. Under the light and electron microscope, the nucleus, mitochondria and secretory granules showed severe changes in the early stage after irradiation and the most severe cellular de- generations were observed 2 hours after irradiation, but the repair processes began from 6 hours after irradiation. 2. Under the electron microscope, loss of the nuclear membranes, derrangement of the chromosomes, swelling and destruction of the secretory granules, and widening of the intercellular spaces were observed after irradiation. 3. Under the light microscope, atrophy and irregular proliferation of the ductal cells, cuboidal metaplasia, hyperchromatism, and the construction or obstruction of the lumen were observed after irradiation.

Co-60 방사선 조사가 백서 치아에 미치는 영향에 관한 실험적 연구 (AN EXPERMENTAL STUDY ON THE EFFECTS OF COBALT-60 RADIATION OF THE TEETH OF THE RAT)

  • This study was undertaken to compare the histopathological effects of the fractionated doses of 2,400, 4,800 and 7,200 rads of Cobalt-60 irradiation on the teeth of the sixty eight rats (Wester strain), weighing approximately 130gm. They were devided into four groups; no irradiation as a control (Group I) and others (Group II, III, and IV) which received different doses of radiation using 1.25 Mev RAC-120 Cobalt-60 Teletherapy Unit with exposure dose rate 69 rads/min, TSD 80cm. Experimental animals received 600 rads every three days until the total dosage of their groups were reached. Experimental animals were sacrified at the 1st day, 1st, 2nd, 3rd, 4th, 5th and 6th week irradiation. The following histopathologic findings were obtained: 1. The Group II, received 2,400 rads, showed no histopamologic changes essentially same as those of the control Group. 2. In the Groups III and IV, received 4,800 rads and 7,200 rads, disarray of the normal palasading arrangement of the odontoblastic cells, vacuolar degeneration in the odontoblastic layer, osteodentin and dentinal niches in the anterior teeth were observed. 3. There were no distinct changes in the posterior teeth in the Groups III and IV. 4. The ameloblast cells seemed to be less sensitive to radiation damage than the odontoblast cells.

방사선 조사가 백서 악하선조직에 미치는 영향에 관한 실험적 연구 (AN EXPERIMENTAL STUDY OF THE IRRADIATION EFFECTS ON RAT SUBMANDIBULAR GLAND)

  • The purpose of this study was to investigate the effects of irradiation on the rat submandibular gland which composed of the epithelial growth factors. The experimental animals were the Sprague Dawley strain rats, which were the rats as the non-irradiated control group and the rats as the experimental group which were divided into groups as the experimental duration of 1 hour, 3 hours, 6 hours, 12 hours, 1 day, 3 days, 1 week, 2 weeks, 4 weeks. The experimental animals were singly irradiated at a dose of 8 Gray gamma ray to their head and neck region by the Co-60 teletherapy unit and sacrificed after each experimental duration. The specimens were stained with H-E and Azan stain and examined light microscopically. The results of this study were obtained as follows. 1. The all of mucous and serous acini in submandibular gland showed similar pattern of changes in structure according to the lapse of time. 2. The acinic cells started to change after 1 hour, and repaired after 12hours with mitosis and proliferation of the cells between acini. The changes were marked after 1day, and repaired gradually in course of time. 3. The duct were dilatated irregularly, and the outline of the eosinophilic stained ductal cells changed indistinctly.

  • The radical treatment of uterine cervical cancer by interacavitary radium or cesium, in combination with teletherapy are well known. Although the result of such treatment should not give rise to complacency, problem of radiation exposure to medical staff had not been resolved. Fortunately, many attempts have been made to reduce this hazard, most of which take the form of afterloading applicators with a suitably shielded radioisotope. In order to avoid hazardous radiation exposure to staffs concerned with brachytherapy, RALS using high intensity source of Co-60, have been employed at Yonsei Cancer Center since May, 1979. It allows rectal and bladder doses to be kept low, while maintaining a satifactory usual dose distribution of the other type of applicators, and the short treatment time allow four or five patients to be treated per hour. It also removes much patient's discomfort and the difficulties of nursing these patients. Since the first introduction in Korea, over seven hundred cases with various stage of uterine cervical cancer have been treated on a radical basis at this center last 4 years. These authors have strongly attracted attention to the results in terms of local control rate, survival s and morbidity compared with those of conventional low dose rate radiotherapy. Retrospective interim analysis of data was preliminarily accomplished through the labored follow-up study of 340 cases treated during initial 2 years and the radiobiologic standpoint of high dose rate intracavitary irradiation will be discussed.

방사선조사가 백서 협점막에 미치는 영향에 관한 전자현미경적 연구 (AN ELECTRON MICROSCOPIC STUDY ON THE EFFECTS OF IRRADIATION ON THE BUCCAL MUCOSA OF RAT)

  • The author studied the acute reaction of cobalt-60 irradiation to buccal mucosa in rats and difference of the effects of single versus fractionated exposure. 195 Sprague Dowley strain rats, weighing about 120gm, were used in this experiment. 3 rats served as controls and the remaining 192 rats were divided into six groups of 32 rats each. Experimental group Ⅰ, Ⅱ, Ⅲ were received a single dose of 15Gy, 16.5Gy, 18Gy and group Ⅳ, Ⅴ, Ⅵ were received two equal sized fractionated dose of 9Gy, 9.75Gy, 10.5Gy at 4 hour intervals, respectively. The experimental groups were irradiated with cobalt-60 teletherapy unit, Picker model 4M 60 (Field size, 12x5 cm, SSD, 50㎝, Dose rate, 222cGy/min, Depth, 1㎝). The animals were sacrificed at 1, 2, 3, 6, 12 hours, 1, 3, 7 days after irradiation and the changes of the irradiated buccal mucosa were observed by electron and light microscopy. The results were as follows: 1. A single exposure was more damaging than fractionated exposure, and as the radiation dose increased, the changes of cell organelles became faster, but the healing of radiation-induced damage in fractionated exposure was faster than in single exposure. 2. The radiation-induced changes of the basal cells were the most prominent in 18Gy-single exposure group, and the least in 18Gy-fractionated exposure group. 3. Electron-microscopically, there appeared nuclear changes, swelling of mitochondria and rough endoplasmic reticulum, decrease of free ribosome, presence of vesicles, widening of intercellular space, and loss of basal lamina. The early remarkable changes were partly loss of nuclear membrane and swelling of mitochondria. 4. Light-microscopically, derangement and pyknosis of basal cells, hydropic changes of spinous cells, enlargement of granular cells, indistinctness of basement membrane, and proliferation of epithelium were observed.

병기 T1 성문암의 방사선치료 (Radiation Therapy in T1 Glottic Cancer)

  • Radiation therapy in T1 glottic cancer offers an excellent cure rate with preservation of voice. From 1983 to 1992 eighty nine patients with TNM staged T1N0M0 invasive squamous cell carcinoma of the glottis were treated at the Dept. of Radiation Oncology, Yonsei Cancer Center, Yonsei University. There were 84 men and 5 women with median age of 59 years. All patients were treated either with Co-60 teletherapy unit or 4MV linear accelerator with an median dose of 6400 cGy(6000-7000 cGy), 200 cGy per day, 5 days in a week. Fourteen local failures have been observed and the median time to local recurrence was 17 months. There were no nodal failure without local recurrence or distant metastases. The 5 year local control rate was 84.3%. The 5 year actuarial surivival rate and the 5 year disease free survival rate were 89.2%, 87.5%, respectively. The 5 year actuarial survival rate and the 5 year disease free survival rate of the nineteen patients with anterior commissure involvement were 77.8% and 74.5% which were lower than those of seventy patients without anterior commissure involvement(91.6%, 90.6%)(p < 0.05). Among the several influencing factors, anterior commissure involvement was the significant prognostic foctor. Final local control rate, taking into account the salvage surgery, was 89.9% at 5 years.

뇌수막종 환자에서 수술후 방사선 치료의 역할 (The Role of Postoperative Radiotherapy in the Management of Intracranial Meningiomas)

  • Purpose : To evaluate the role of postoperative radiotherapy in the management of primary or recurrent intracranial meningiomas. Methods and Materials : A retrospective review of 34 intracranial meningioma patients referred to the Yonsei Cancer Center for postoperative radiotherapy between 1981 and 1990 was undertaken. Of the 34 patients, 24 patients received elective postoperative radiotherapy after total or subtotal resection(Group 1), and 10 patients received postoperative radiotherapy as a salvage treatment for recurrent tumors(Group 2). Ten patients received postoperative radiotherapy after total resection, and twenty-four after subtotal resection. Ten patients who had total tumor resection were referred for radiotherapy either because of angioblastic or malignant histologic type(4 patients in Group 1) or because of recurrent disease after initial surgery(6 patients in Group 2). Radiation dose of 50-56 Gy was delivered over a period of 5-5.5 weeks using 4MV LINAC or Co-60 teletherapy unit. Results : Overall actuarial progression free survival(PFS) at 5 years was $80\%$. Survival was most likely affected by histologic subtypes. Five year PFS rate was $52\%$ for benign angioblastic histology as compared with $100\%$ for classic benign histology. For malignant meningiomas, 5 year PFS rate was $44\%$. The recurrence rates of classic, angioblastic, and malignant type were $5\%(1/21),\;80\%(4/5)$, and $50\%(4/8)$, respectively. The duration between salvage post-operative radiotherapy and recurrence was longer than the duration between initial surgery and recurrence in the patients of group 2 with angioblastic or malignant histology. Conclusion . Postoperative radiotherapy of primary or recurrent intracranial meningiomas appears to be effective modality, especially in the patients with classic meningiomas. In angioblastic or malignant histologies, a more effective approach seems to be needed for decreasing recurrence rate.
