A Case Report of Patients with Labrum Tear in Shoulder Treated by Oriental Therapy, Including Sinbaro3 Pharmacopuncture (신바로3 약침과 한방치료를 병행한 견관절 관절와순 파열 환자 치험 3례)

  • Objectives : The purpose of this report is to show that oriental therapy, using Sinbaro3 pharmacopuncture in particular, is effective for symptom improvements and ROM in patients diagnosed with labrum tear in shoulder. Methods : 3 patients were admitted to Jaseng Oriental Medicine Hospital, were diagnosed with a tear of the labrum, and were treated with oriental medicine including Sinbaro3 pharmacopuncture. 3 case were measured and assessed by SPADI(Shoulder Pain and Disability Index), shoulder range of motion (ROM) and a shoulder physical examination. Results : After treatment, patients' SPADI decreased and they showed improvement of ROM(range of movement). In patients with labrum tear, oriental medicine treatment and Sinbaro3 pharmacopuncture was effective for relieving pain and facilitating better movement. Conclusion : In line with the above results, it can considered that oriental medical therapy including Sinbaro3 pharmacopuncture is effective for the relief of pain and for recovering function in the shoulder joint.

Associated Changes During Arthroscopic Evaluation of the Glenohumeral Joint in Rotator Cuff Tear - Comparison According to Tear Size - (회전근 개 파열의 관절경적 치료 시 관절된 관절와 상완관절의 동반 변화 -파열의 정도에 따른 차이-)

  • Purpose: To identify associated findings in glenohumeral joint in rotator cuff tear and evaluate its clinical significance, we examined minor and major changes during arthroscopic or mini open repair. Materials & Methods: We reviewed 66 patients of rotator cuff tear treated from March, 2001 to January, 2004. Of 38 cases of small to medium tear, average age was 53 years old and involved in dominant arm in 27 cases. Of 28 cases of large to massive tear, average age was 58 years old and involved in dominant arm in 26 cases. Minor and major associated changes of the glenohumeral joint were evaluated in the tendon of biceps long head, biceps pulley, cartilage of the glenoid and humeral head, labrum and synovium. Results: Minor changes in biceps tendon were in 35% of cases, biceps pulley in 18%, cartilage of humeral head in 27%, cartilage of glenoid in 18%, labrum in 38%, and synovium in 42%. Major changes in biceps tendon were in 6% of cases, biceps pulley in 35%, arthritis of humeral head in 3%, arthritis of glenoid in 2%, labrum in 6%, and synovium in 21 %. Major changes in biceps tendon were 5% in Group I and 7% in Group Ⅱ(p>0.05) and in biceps pulley, 18% and 57% in each (P<0.05). Minor changes of arthritis were prevalent in glenoid cartilage and major changes were more prevalent in humeral head. There were no differences in minor changes of labrum and synovium, but major changes were more prevalent in Group Ⅱ. Conclusion: The prevalence of intraarticular associated changes of rotator cuff tear were 63% in synovium, 54% in labrum, 53% in biceps pulley, 41% in biceps tendon, 30% in humeral head and 20% in glenoid cartilage in order. Major changes of biceps pulley, humeral head, labrum and synovium were more prevalent in Group Ⅱ.

  • Objectives : The purpose on this study is to show the clinical effects of oriental medical treatments for labral tear. Methods : The patients were treated using acupuncture, pharmacopuncture, herbal medication, moxibustion, physical treatment and cupping treatment. And the effects for labrum tear have measured in VAS, ROM of shoulder joint. Results : VAS of shoulder pain went down to 2 in case 1, 4 in case 2. ROM of shoulder joint was improved. Conclusions : From the above results, It is demonstrated that oriental medical treatments is effective on labral tear.

Optimal Shoulder Position for Visualization of SLAP Ⅱ lesion on MR-Arthrography (SLAP Ⅱ 병변의 진단을 위한 관절 조영 자기 공명 영상에서 견관절 위치에 따른 비교)

  • Purpose : The purpose of this study was to evaluate the efficacy of oblique coronal MR images, oblique axial images of neutral, internal rotation and external rotation positions in the diagnosis of SLAP Ⅱ lesion. Materials and Methods: MR arthrography of the glenohumeral joint was evaluated retrospectively in 16 patients(16 shoulders) who underwent arthroscopic surgery(mean age; 38 years old, Male; 13, Female; 3). Oblique coronal fat-suppressed Tl-weighted spin echo images were performed with each shoulder in the neutral position of the arm and oblique axial images were performed in neutral, internal and external rotations of the arm respectively. The preoperative findings of MR were classified as definite tear, possible tear and no tear. Arthroscopic findings were correlated with MR findings of several different position of the arm. Results: Arthroscopic surgery revealed 8 SLAP Ⅱ lesion, 2 SLAP I lesion, and 6 normal superior labrum respectively. The accuracy of diagnosis in the 8 SLAP Ⅱ lesion were high on oblique axial image in external rotation which were interpreted as 8 definite tear, to compare with oblique axial images in neutral position which were interpreted as 4 definite tear, 3 possible tear, 1 no tear. The 6 normal superior labrum lesion were interpreted as no tear in all three position. The 2 SLAP I lesion were interpreted as 1 definite tear, 1 no tear on oblique axial image in neutral position and 1 definite tear, 1 possible tear on oblique axial image in external rotation. Conclusion: This study showed that axial MR images in external rotation of the arm combined with oblique coronal images have proved to be effective to detect SLAP Ⅱ lesion, and should be considered in imaging protocol for MR arthrography of the SLAP Ⅱ lesion.

  • Purpose: Labral lesions and its anatomic variants have been studied by several authors in the last decade. Buford complex and sublabral recess are most common variants. Their recognition is important in order to distinguish them from superior and anterior labral tear. Materials and Methods: We report one case of a 19-year-old female who was mistaken SLAP lesion for normal variant and was treated with arthroscopic surgery. Results: The arthroscopic finding shows rare normal variant of biceps anchor and superior labrum. The biceps long head tendon was inserted at superior labrum and supraspinatus tendon area. This finding was mistaken to SLAP lesion. Conclusion: We report rare normal variant of biceps anchor and superior labrum that was observed during arthroscopic surgery for SLAP lesion.

Posterior Internal Impingement of the Shoulder in Baseball Players -Preliminary Report of 5 Cases Study- (야구 선수에서의 견관절 내부 후방 충돌 -5예에 대한 예비보고-)

  • Purpose: To investigate the arthroscopic findings, and to evaluate the clinical outcomes of the treatment of posterior internal impingement of the shoulder in baseball players. Materials and Methods: We followed up 5 cases who were diagnosed as posterior internal impingement for the mean 15 months. All of the cases complained of the pain in the posterior shoulder at late cocking, and were positive in the relocation test added by hyper-horizontal abduction at $120^{\circ}$ abduction of the arm. We sutured posterior labral tear and SLAP lesion arthroscopically, and conducted debridement for rotator cuff. Three cases were performed of anterior capsular plication and the other two were performed of thermal capsular shrinkage. Pain, range of motion, and level of return to sports activity were assessed for the results. Results: As to the arthroscopic findings, all the five cases showed the fraying in posterosuperior labrum, and two of them was accompanied with the flap tear in posterosuperior labrum and the other one was accompanied with type 2 SLAP lesion. All the cases showed the fraying in supraspinatus, and one case showed partial tear. Meanwhile, in all the cases, the rotator cuff was impinged to the labrum at $90^{\circ}{\sim}120^{\circ}$ of abduction and external rotation. As to the postoperative results, all the cases did not complain of pain or instability while pitching, and the competition was recovered to be the mean 88%($80{\sim}100%$) of that before the injury. Conclusion: Definite diagnosis for the posterior internal impingement would be possible through arthroscopic examination. Favorable outcomes could be obtained with capsular plication or shrinkage for anterior microinstability and stretching exercise for posterior capsule tightness inducing the internal impingement.

  • As the instruments as well as techniques for hip arthroscopy have developed, the interests and understandings of acetabular labral tear have also increased. As a consequence, the diagnosis itself was increased. However, it is still difficult to be diagnosed accurately in an ordinary clinic. In this brief review, the clinical and radiological characteristics and diagnostic implications of acetabular labral lesions were described to help the surgeon to make a right decision.

  • The purpose of this study is to report the improved case of superior labrum from anterior to posterior tear patient treated with Shinbaro pharmacopuncture and motion style acupuncture treatment. We used Shinbaro pharmacopucture, motion style acupuncture treatment and other korean medicine treatment for this patient. This case is measured and assessed by visual analogue scale (VAS), shoulder range of motion (ROM) and shoulder physical examination (Neer test, O'Brien test). After treatment, VAS decreased and the patient showed improvement of range of motion. Also physical examination of shoulder was improved. Shinbaro pharmacopuncture and motion style acupuncture treatment are thought to be helpful to relieve pain and recover function on shoulder joints, although further study is needed.

Direct MR Arthrography of the Hip: Diagnosis and Pitfalls of Acetabular Labral Lesions (고관절 직접 자기공명관절조영술: 비구순 병변의 진단 및 함정)

  • Tearing of the acetabular labrum is a common cause of chronic hip pain. MR arthrography (MRA) of the hip is the imaging procedure of choice for the evaluation of acetabular labrum. Familiarity with the various imaging findings of MRA of the hip allows recognition of normal variants and differentiation from true pathologic conditions. This article comprehensively reviews the technical aspects and interpretation of MRA of the hip. The appearances of normal and abnormal labra in MRA are discussed. Potential pitfalls in labral tear interpretation, such as sulci around the hip, normal variants of the labrum and plicae, and osseocartilaginous and soft tissue lesions identified on MRA of the hip are also described.
