신생아 황달에서 광선치료 동안에 경피적 빌리루빈 측정기의 유용성 (Usefulness of the transcutaneous bilirubinometer during phototherapy in neonatal jaundice)

  • 목 적 : 신생아 황달은 신생아기에 가장 흔한 질환으로 핵황달 및 사망까지의 심각한 후유증을 유발할 수 있기 때문에 신속하고 정확한 빌리루빈 농도의 측정과 치료가 중요하다. 따라서 신생아 황달의 진단에서 빌리루빈 측정법 중 경피적 빌리루빈 측정기의 개발로 신생아 황달의 선별법으로 사용되고 있다. 그러나 광선치료 동안에 경피적 빌리루빈 측정기의 유용성이 연구되지 않아 이에 저자들은 광선치료 동안에 측정하는 신체의 부위별 TcB와 PB의 상관관계를 조사해서 경피적 빌리루빈 측정기의 유용성을 알아보고자 본 연구를 시행하였다. 방 법 : 2005년 6월부터 8월까지 제일병원 신생아실에서 광선치료를 받는 황달의 임상적 위험성(미숙아, Rh및 ABO 부적합증, 간손상 등 다른 합병증)이 없는 신생아 90명을 대상으로 경피적 빌리루빈 측정기인 JM-103을 사용하여 광선치료 시작전 이마와 흉골 중앙부에 opaque patch을 붙이고 광선치료 동안에 opaque patch 부위와 바로 인접한 부위에서 TcB를 측정하였다. TcB 측정시 각각 3회씩 측정하여 평균치를 TcB로 하였으며, 경피적 빌리루빈 측정기로 측정한 시각으로부터 30분 이내에 발뒤꿈치를 천자하여 PB를 측정하여 측정 부위간 TcB와 PB의 상관관계를 비교 관찰하였다. 결 과 : 대상에 포함된 신생아는 황달의 임상적 위험성(미숙아, Rh 및 ABO 부적합증, 간손상 등 다른 합병증)이 없는 신생아를 대상으로 하였고 신생아의 평균 재태주령은 $38.6{\pm}1.3$주였고 평균 출생 체중은 $3,207.0{\pm}472.1g$이었고 광선치료 시작 시기는 평균 $4.9{\pm}0.9$일이고 평균 광선치료 기간은 $1.3{\pm}0.6$일이었다. 대상 신생아의 광선치료 시작전 PB와 patch 붙인 흉골 부위의 TcB, patch 붙인 이마 부위의 TcB, 흉골 부위의 TcB, 이마 부위의 TcB 결과는 각 부위간에 유의한 상관관계를 보였다. 광선치료 동안에 각 측정부위별(TcB-UF, TcB-UC, TcB-PF, TcB-PC)와 PB는 의미있는 상관관계를 나타내었고 opaque patch를 붙인 부위가 인접한 부위보다 PB와 의미 있는 상관관계를 나타내었으며 TcB-PC가 TcB-PF보다 더욱 더 PB와 의미 있는 상관관계를 나타내었으며 일치도가 높았다. 결 론 : 본 연구의 결과로 경피적 빌리루빈 측정기는 신생아 황달의 선별검사 방법뿐 아니라 광선치료 동안에도 추적검사 측정법으로 사용하기에 적절하다고 생각되며 사용하는 기관에서 자기고유의 회귀직선을 작성하여 사용한다면, 신생아의 몸에 침해를 가하지 않고 편리하고 신속하게 신생아 과빌리루빈혈증 진단과 추적검사에 유용하게 사용될 수 있으리라 생각된다.

신생아에 있어 TcB[transcutaneous bilirubinometry]를 이용한 고빌리루빈혈증의 사정 (Assessment of Neonatal Hyperbilirubinemia Using a Transcutas Bilirubinometry)

  • Purpose: The purpose of the study is to investigate the relationship between total serum bilirubin(TSB) and transcutaneous bilirubinometry(TcB) in neonates with jaundice. Method: TcB from various sites(forehead, sternum, abdomen, buttock, hand, dorsalis-pedia) was measured using a JM-102 in a total of 102 neonate, 42 female and 60 male, with the mean 37.5 gestational week and the mean 2,903 gram of birth weight, as well as TSB from capillary punctures. Result: The mean bilirubin was 11.73 in serum, 20.55 on the forehead, 17.23 on the sternum, 16.19 on the abdomen, 18.22 on the buttock, 15.83 on the hand and 15.49 on the dorsalis-pedia. The relationship between TSB and TcBs were formulated by simple regression with 0.406 < r < 0.668(p < .000). A higher relationship was revealed between TSB and TCB at the forehead in infants of full-term, ABO incompatibility, and Hb greater than 16 mg/dl(r =0.725, 0.790, and 0.717, retrospectively). Phototherapy altered the measurement of TcB per site. Conclusion: TcB on the forehead is a reliable, noninvasive and convenient measurement of TSB in normal infants(Institutions need to establish quantitative equations representing the specific relationship between TSB and TCB according to the hemodynamic problems of infants such as ABO incompatibility, or low Hb).

The Effect of Vitamin {TEX}$B_{6}${/TEX} Deficiency and Age on Plasma Cholesterol Profile in Intensely Exercised Rats

  • The purpose of this study was to determine whether vitamin {TEX}$B_{6}${/TEX} deficiency and age affect the blood cho-lesterol profile in exercising rats. Fifty four rats were fed either a viramin {TEX}$B_{6}${/TEX} deficient dief(-{TEX}$B_{6}${/TEX}) of a control diet(+{TEX}$B_{6}${/TEX}) for 6 weeks, then subdivided into 3 groups:non-exercise group(NE), exercise and sacrifice group(ES), exercise and recuperation group(ER). ES group was exercised in treadmill({TEX}$10^{o}${/TEX}, 0.5~0.8km/h) for 2 hours and sacrifice. ER group was recuperated three days with respective diet after exercise. At week 3 and 6, and level of plasma total cholesterol(TC), high density lipoprotein cholesterol(HDL_C) and low density lipoprotein cholesterol(LDL_C) were compared. In NE group, there was no difference in the levels of TC, HDL_C and LDL_C between +{TEX}$B_{6}${/TEX} rats and -{TEX}$B_{6}${/TEX} rats. The plasma levels of TC and LDL_C of 6 weeks were higher than those of 3 weeks and on difference in HDL_V between 3 weeks rats and 6 weeks rats. In ES group, there was also no difference in the levels of TC, HDL_C and LDL_C between +{TEX}$B_{6}${/TEX} rats and -{TEX}$B_{6}${/TEX} rats and there was no difference in TC, LDL-cholesterol between 3 weeks rats and 6 weeks rats. The level of HDL_C pf 6 weeks was lower than that of 3 weeks rats. In ER group, there was no difference in the levels of TC and LDL_C not only between +{TEX}$B_{6}${/TEX} rats and-{TEX}$B_{6}${/TEX} rats but also between 3 weeks rats and 6 weeks rats. The level of HDL_C was lower in -{TEX}$B_{6}${/TEX} rats than in +{TEX}$B_{6}${/TEX} rats and higher in 6 weeks rats than in 3 weeks rats. These results suggest that vitamin {TEX}$B_{6}${/TEX} deficiency may affect the HDL_C during exercise and after recuperation. The desirable effect of exercise on plasma Cholesterol profile is strengthened in adult age than young age.

비타민 B6 부족이 정기적인 운동 훈련시 연료의 이용과 혈액 콜레스테롤 성상에 미치는 영향 (The Effect of Vitamin B6 Deficiency on the Utilization of Fuel and Blood Cholesterol Profile with Regular Exercise-Training in Rats)

  • The purpose of this study was to determine whether vitamin B6(B6) deficiency affects fuel utilization and blood cholesterol profile with exercise-training. Twenty-four rats were fed a B6 deficient(-B6) diet or a control (+B6) diet for 5 weeks and either exercised(EX) or nonexercised (NE). EX rats were exercised on treadmill(10$^{\circ}$, 0.5-0.8km/h) for 20 minutes everyday. Glucose(GLU), glycogen (GLY), protein(PRO), trglyceride(TG), free fatty acid(FFA), total cholesterl(TC), HDL-cholesterol(HDL-C) and LDL-choleterol(LDL-C) were compared in plasma(P), liver(L) and skeletal muscle(M) of rats. There was a vitamin effect on the level of P-GLU, P-TG, M-TG, L-GLY, L-PRO and an exercise effect on the level of P-PRO, P-FFA, M-PRO, L-GLY, L-TG, P-TC, P-HDL-C, P-LDL-C. Compared to +B6 rats were lower and there were no differences in P-GLU, P-FFA, P-TG. M-GLY, L-TG, P-TC and P-HDL-C. In EX group, the level of P-TG was higher and M-PRO was lower in -B6 rats. There were no differences in M-GLY, L-TG, P-TC and P-HDL-C. These results suggest that a lowered intake of vitamin B6 may impair the adaptation of animals to fuel metabolism related to a decrease of fatty acid oxidation and attenuates the exercise-traning effect on blood lipid profile.

  • Intravascular large B-cell lymphoma (IVLBCL) is a subtype of diffuse large cell lymphoma, characterized by proliferation of lymphoid cells in the intravascular space of various organs without causing a mass effect. Although $^{18}$F-FDG PET is a powerful imaging tool in lymphoma, the usefulness of $^{18}$F-FDG PET in the assessment of IVLBCL is still controversial. $^{99m}$Tc-MIBI, a tumor imaging radiopharmaceutical with a different mechanism from that of $^{18}$F-FDG, has been reported to be also effective in lymphoma. However, there is nearly no report on the efficacy of $^{99m}$Tc-MIBI in the assessment of IVLBCL. We present one case of IVLBCL that showed $^{99m}$Tc-MIBI accumulation in the involved bone marrow as an incidental finding, which was discrepant from that of $^{18}$F-FDG PET.

체외배양액과 첨가물질이 소 체외수정란의 체외발육에 미치는 효과 (Effect of Culture Medium and Additive on the Development of Bovine IVM/IVF Embryos)

  • The objective of this study were to investigate the effects of culture media and additives on the development of bovine in vitro matured(IVM) and in vitro fertilized(IVF) oocytes. In experiment 1, bovine oocytes were cultured in droplets of TC 199 supplemented with 10% fetal bovine serum(FBS) with or without hormones (5$\mu\textrm{g}$/ml FSH, 5$\mu\textrm{g}$/ml LH, 1$\mu\textrm{g}$/ml E2). Cleavage rates of embryos cultured for 40~44hrs after IVF were higher when embryos were cultured in TC 199 supplemented hormones (68.1%, 921/35) than without hormones (52.7%, 77/146), but the percentages of development beyond morulae stage were not difference (20.7%, 19.4%). In experiment 2, the effects of various media such as TC 199, synthetic oviduct fluid(SOF), CR1aa with different energy source (fatal bovine serum, FBS; bovine serum albumin, BSA) on developmental capacity of IVM/IVF bovine embryos were investigated. The developmental rates into morulae and blastocysts were 27.1, 10.7, 6.3 and 0%, respecitvely, in CR1aa plus 3mg/ml BSA, SOF plus 10% FBS, TC 199 plus 10% FBS, SOF plus 3mg/ml BSA. In experiment 3, the comparisons of bovine embryos developed to morulae and blastocysts in different culture media (TC 199, SOF, CR1aa, Menezo's B2) were investigated. The developmental capacity beyond morulae stage were 32.9, 26.6, 11.1 and 7.1%, respectively, in Menezo's B2 plus BSA, CR1aa plus BSA, SOF plus BSA, TC 199 plus FBS medium. The cell numbers of the blastocyst were not different in different cultrue media. In experiment 4, bovine embryos were co-cultured with vobine oviduct epithelial cells(BOEC) in TC 199 plus FBS, SOF plus BSA, CR1aa plus BSA, Menezo's B2 plus BSA. The morula and blastocyst rates were 44.7, 32.9, 26.0 and 23.3%, respectively, in CR1aa TC 199, SOF, and Menezo's B2 medium. The cell numbers of the blastocyst were similar to those of blastocyst developed in different culture media.

Pd(001) 위에 올려진 Tc 0.5 웃층의 자성과 전자구조 (Magnetism and Electronic Structure of Tc Half-monolayer on Pd(oo1) Surface)

  • Pd(001) 위에 올려진 Tc 0.5(Half-monolayer) 웃층의 자성상태와 전자구조를 FLAPW(Full-Potential Linearized Augmented Plane Wave) 에너지 띠 방법을 이용하여 이론적으로 연구하였다. Tc 웃층의 안정된 자성상태를 알아보기 위해 상자성 상태와 강자성 상태의 총에너지를 계산하였다. 계산결과 Tc 0.5 웃층은 강자성 상태가 상자성 상태보다 에너지적으로 안정하였고 그 자기 모멘트는 2.09 $\mu$B이었다. 0.5층 Tc/Pd(001)에 대한 머핀-핀(MT) 구 내의 전자수와 상태밀도 등의 전자구조 계산결과를 제시하고 논의 검토하였다.

Anti-hyperlipidemic Effects of Bacillus strain-fermented Cheonggukjang Products in Mice

  • To evaluate the hypolipidemic effects of Cheonggukjang (CGJ), which is frequently used in Korea similar to Natto in Japan and Douchi in China like a dairy product, boiled soybeans were fermented with two Bacillus strains, B. subtilis and B. licheniforms, isolated from rice straw and their antihyperlipidemic effects of their products were investigated. Treatment with the CGJs significantly reduced blood triglyceride (TG) and total cholesterol (TC) levels and increased HDL cholesterol levels in Triton WR-1339-induced hyperlipidemic mice. The treatment of non-fermented soybeans alone also reduced blood TG and TC levels, but not significantly. Feeding the CGJs significantly lowered high blood TG and TC levels as well as body and epididymal mass weights in hyperlipidemic mice induced by the long-term feeding of a high-fat diet that increased blood HDL cholesterol levels. The B. subtilis-fermented CGJ products more potently reduced TG and TC levels, although the differences between the starters were not significant. These finding suggest that CGJ products may be effective as hypolipidemic foods by the synergistic interaction of soy and Bacillus strains.

Protective Effect of Panax ginseng Against Tetracycline Toxicity in Rats

  • Tetracycline hydrochloride (TC) caused 100%, 50% and 20% mortality rates among rats injected with 40 mg, 30 mg and 20 mg/100g. b.w. respectively; while the morta]ity rates were decreased to 50%, 20% and 10% when Panax ginseng (2 mg/100g. b.w.) injected with TC during 72 hrs. post-injection. Subacute-toxicity study demonstrated that TC caused severe hepato-nephrotoxicity (demonstrated by biochemical analysis of serum including: transferases , alkaline phosphatase, total protein, glucose, cholesterol urea and creatinine) in rats injected i.p. with 10 mg and 5 mg/100g. b.w. for 7 days of daily injection . These signes of toxicity were greatly diminished by P. ginseng addition to TC doses.

시판 육류중의 Tetracycline계 항생물질 잔류량과 가열분해에 관한 연구 (Studies on the Content and Heat Decomposition of Residual Tetracycline in Meats on the Market)

  • 육류중의 Tetracycline계 항상물질의 잔류량을 조사하기 위하여 시판 중인 쇠고기, 돼지고기. 닭고기 3종을 각각 3개시료씩 구입하여 Tetracycline(TC), Oxyteracycline(OTC) , Chlortetracycline(CTC) 의 잔류량을 분석하였으며, 아울러 잔류 항생물질의 조리가열에 의한 분해감소량을 조사한 결과는 다음과 같다. 1. 시판 육류중의 OTC 및 CTC 잔류량: OTC 잔류량은 닭고기의 경우 A 시료에서 흔적정도, B시료에서 불검출 , C시료에서는 0.09mg/kg이 검출되었다. 쇠고기 3개 시료에서는 C 시료에서만 돼지고기 A 및 B에서는 흔적정도로 검출되었다. CTC은 닭고기 A,C시료에서 0.14, 0.02mg/kg으로 각각 검출되었으며, 쇠고기와 돼지고기의 경우에는 쇠고기 B에서만 0.01mg/kg의 잔류량이 확인되었다. 2. TC 잔류량 : 쇠고기와 돼지고기의 Tc 분석에는 육류 자체에서 유래된 성분의 peak와 TC의 peak가 중복되어 잔류량의 판단을 방해하였으나, 닭고기의 경우에는 이러한 방해 성분이 없었으며, 3개중 A 시료에서만 0.01mg/kg의 잔류량이 검출되었다. 3. 가열방법별 항생물질의 분해정도 : 가장 고온가열인 굽기에서, 특히 TC 및 OTC의 분해가 많았다.. 4. 가열시간에 따른 분해정도 : 자불에서는 상당시간이 경과하여도 미량의 분해가 있었으나, 굽기에서는 시간경과에 따라 분해량의 현저한 증가를 보였다. 굽기와 같은 조리 효과를 가진 전자오븐 가열에서는 덜 분해되었다. 가열에 대한 각 항생물질의 내열성은 CTC>OTC>TC 의 순으로 나타났다.
