The Study on the Diagnostic Value of Salivary Gland Scintigraphy in Patients with Xerostomia (구강건조증 환자의 타액선 스캔의 진단학적 가치에 관한 연구)

  • The present study was performed to investigate the relationship between the salivary flow rate and the interpretation results of salivary scan in the patients with dry mouth. Twenty-five patients with dry mouth who visited the Dept. of Oral Medicine & Oral Diagnosis, Seoul National University Dental Hospital, were included. The unstimulated whole salivary flow rate was determined by the spitting method and the stimulated whole salivary flow rate was measured with gum-base chewing. Salivary scan was performed after the infusion of $^{99m}technetium$ pertechnetate(Tc) and interpreted. The obtained results were as follows: 1. The unstimulated and stimulated whole salivary flow rate were significantly decreased compared to normal value, which reflected the extensive destruction of salivary gland function in the patients with dry mouth. 2. The unstimulated and stimulated whole salivary flow rate were decreased in the group with decreased function in salivary scan compared with the group with normal function in salivary scan. However, there was no statistical significance between groups. 3. The difference between the stimulated and unstimulated whole salivary flow rates was greater in the group with normal function in salivary scan compared with the group with decreased function in salivary scan. 4. There was significant positive correlation between the stimulated and unstimulated whole salivary flow rates. The level of correlation was higher in the group with decreased function in salivary scan than the group with normal function in salivary scan. Collectively, these data suggested that salivary scan had the limited value. The comprehensive evaluation including history taking, clinical examination, clinical laboratory as well as the measurement of salivary flow rate are need for patients with dry mouth.

  • 99mTcO4- is used to measure thyroid uptake. When measuring background radioactivity of the thigh, the distance between the detector and the thigh was 20 cm at a position 10 cm above the knee from the knee to the pelvis. At 25 cm, many hospitals are measuring at different distances. The purpose of this study was to study the difference in effectiveness in obtaining the thyroid uptake rate by quantitatively analyzing the uptake rate measurement at two distances. As a result, the average thyroid uptake rate measured at 20 cm in the thigh of 50 patients was 3.532±4.312%, and the average thyroid uptake rate measured at 25 cm was 3.680±4.304%. As a result of the thyroid uptake rate test result, the femoral background radioactivity value measured at a distance of 20 cm was relatively higher than the background radioactivity value measured at a distance of 25 cm, and as a result, the thyroid uptake rate was 0.148% lower.

A Clinical Manifestation of Meckel's Diverticulum (Meckel 게실의 임상양상)

  • Purpose : The diagnosis of Meckel's diverticulum is difficult and delayed because it presents with various clinical symptoms. We evaluated clinical, imaging and pathologic findings of Meckel's diverticulum to facilitate detection of Meckel's diverticulum in children. Methods : Review of clinical, imaging, surgical and pathological findings in 10 children aged 7 days to 14 years with Meckel's diverticulum during an 8-year period, 1993-2001, at Ewha Womans University Hospital was undertaken. Results : The male to female ratio was 2.3 : 1. The chief complaint was painless lower gastrointestinal( GI) bleeding; others were abdominal pain, abdominal distention and vomiting, in order of frequency. The diagonsis before surgery were Meckel's diverticulum in 5 patients, non-reducible intussusception in 3 patients and intestinal obstruction in 2 patients. The diverticulum was located between 35 cm to 70 cm proximal to the ileocecal valve. The length of the diverticulum ranged from 4 cm to 12 cm and 80% of it was within 5 cm. A Meckel scan($^{99m}Tc-pertechnetate$ scintigraphy) after cimetidine administration was done in 6 cases. All 5 cases that presented with lower GI bleeding had ectopic gastric mucosa confirmed on pathology. Out of 5 cases of ectopic gastric mucosa, only 4 cases were positive on the Meckel's scan. Conclusion : In cases of unexplained GI bleeding, obstruction, or inflammation diagnostic workup should be carried out to rule out Meckel's diverticulum. Laparoscopy, high resolution ultrasonography and computed tomography of the abdomen may be indicated in the assessment of pediatric patient with lower GI bleeding, especially in patients with suspected bleeding from Meckel's diverticulum showing negative Meckel's scan.

Usefulness of wearing pocket dosimeter in nuclear medicine (핵의학 영상검사에서 Pocket dosimeter 착용의 유용성 평가)

  • Purpose: The aim of study is to find accuracy of pocket dosimeter in measuring exposed dose in compared with survey meter and to compare exposed dose according as Nuclear medicine exams. Materials and Method: First, radiation dose to point source(185 MBq,370 MBq, ${\ldots}$, 1665 MBq, 1850 MBq) were measured in using a pocket dosimeter and a survey meter. Second, radiation dose to 12 patients injected $^{18}F$-FDG 370 MBq were measured in using a pocket dosimeter and a survey meter. Third, radiation dose to 10 patients injected $^{99m}Tc$-DPD 925 MBq were measured in using a pocket dosimeter and a surveymeter. Result: The average is $70.12{\pm}39.36{\mu}Sv/h$ in measurement of point source with Surveymeter and $5{\pm}3.06{\mu}Sv$ in measurement of point source with Pocket dosimeter. The average is $25.04{\pm}6.16{\mu}Sv/h$ in measurement of PET/CT patients with Surveymeter and $2.41{\pm}0.51{\mu}Sv$ in measurement of PET/CT with Pocket dosimeter. The average is $8.58{\pm}0.96{\mu}Sv/h$ in measurement of Bone Scan patients with Surveymeter and $1{\mu}Sv$ in measurement of Bone Scan patients with Pocket dosimeter. Significant difference found between Survey meter value and Pocket dosimeter value in all experimentation (p<0.001). Conclusion: Accoring to rusult Wearing Pocket dosimeter is usefulnee in manerage of exposed dose in nucler medicine exams.

Diagnosis of Functional Nasolacrimal Duct Obstruction Using Dacryoscintigraphy (누비공신티그라피를 이용한 기능적 누도폐쇄의 진단)

  • Purpose: To evaluate the value of dacryoscintigraphy in the assessment of patients with a clinical diagnosis of functional nasolacrimal duct obstruction. Materials and Methods: Dacryoscintigraphy was performed in symptomatic 35 lacrimal drainage systems in 18 patients (age range: $37{\sim}76$, 8 males, 10 females) that were patent on syringing. Results: Abnormalities were detected with dacryoscintigraphy in 75.8% of systems. The positive scintigrams were subdivided into those demonstrating prelacrimal sac delay (31.8%), delay at the lacrimal sac/junction (40.9%), or delay within the duct (21%). Conclusion: Dacryoscintigraphy is noninvasive useful technique in the assessment of the functional nasolacrimal duct obstruction and very sensitive at detecting abnormalities in patients with lid laxity caused by senile change and facial nerve palsy.

Role of Dacryoscintigraphy in the Diagnosis and Treatment of Pediatric Patients with Epiphora (유루를 호소하는 소아 환자의 진단 및 치료에서 누비공신티그라피의 역할)

  • Purpose: We conducted this study to evaluate the role of dacryoscintigraphy in the diagnosis and treatment of pediatric patients with epiphora. Materials and Methods: In 58 patients aged from 2 months to 15 years (mean age $2.8{\pm}2.3$ years), dacryoscintigraphy was performed using a gamma camera with 4 mm pinhole collimator. We correlated symptoms with dacryosicntigraphic findings in all patients. In 37 patients who underwent ophthalmologic procedures, we analyzed the agreements of dacryoscintigraphic findings with the operation. Results: High rates of agreements between epiphora and obstruction on dacryoscintigraphy (69/72, 95.8%), and between scintigraphic obstructive findings and operation sites (44/47, 93.6%) were noted. Nine of foully-four (20.5%) asymptomatic eyes showed obstructive findings on dacroscintigraphy. Conclusion: In pediatric patients with epiphora, dacryoscintigraphy is a useful tool not only in diagnosing nasolacrimal duct obstruction but also in making a decision for therapeutic procedures.
