Clinical Results of Tarsal Tunnel Decompression in Case of Known Etiology (원인이 밝혀진 족근관 증후군의 수술적 치료의 결과)

  • Purpose: The purpose of this study is to present our clinical results after surgical treatment in tarsal tunnel syndrome due to space occupying lesions. Material and Methods: We performed surgical decompression for tarsal tunnel syndrome in 20 patients from July 2004 to February 2007. Out of them, thirteen cases were due to space occupying lesions around the tarsal tunnel. The average age at operation was 51.3 years old and the duration from symptom onset to surgery was 16.5 months. The operation included removal of space occupying lesions and tarsal tunnel decompression. The clinical parameters were pain visual analogue scale (VAS), AOFAS scale, and subjective satisfaction. Results: The ganglion cysts were the most frequent causes (ten cases) and synovial chondromatosis in 1 case, neurofibroma in 1 case, talocalcaneal coalition in 1 case. The average follow-up duration was 14.5 months. The AOFAS scale showed significant improvement from 77.8 to 92.7. The average VAS decreased from 6.4 to 2.2. Seven out of thirteen patients were satisfied with the results. The excellent results were shown in six patients, the good results in one patient, the fair result in three patients and the unsatisfactory results in three patients. Conclusion: Favorable results could be obtained in patients with known etiology. But not all cases with surgical decompression of space occupying lesions showed satisfactory results. We assume that the clinical results were related to the multiple factors, not only well performed surgery but also age, size lesions and duration of symptoms, ect.

The Results of Treatment for Tarsal Tunnel Syndrome Associated with Calcaneus Fracture (종골 골절과 연관된 족근관 증후군의 치료 결과)

  • Purpose: To review the results of surgical decompression for tarsal tunnel syndrome associated with calcaneus fractures. Materials and Methods: Six tarsal tunnel syndromes in five patients were surgically decompressed at our hospital with followed up of average 26.0 months (range, $12{\sim}36$ months). All patients were male and average age at surgery was 50.0 years (range, $33{\sim}69$years). All five cases developed after calcaneus fractures including one bilateral case. Clinical results were assessed according to the criteria of Pfeiffer and Cracchiolo. Results: The result was good in three cases, fair in one case and poor in two cases. Four cases in four patients were satisfied with the result of treatment. Conclusion: Clinical results of surgical treatment for tarsal tunnel syndrome associated with calcaneus fracture of the foot or ankle was improved and maintained in four of six cases.

Tarsal Tunnel Syndrome due to Varicose Veins Misdiagnosed as Ganglion Cyst: A Case Report (결절종으로 오인된 하지정맥류로 인한 족근관 증후군: 증례 보고)

  • Tarsal tunnel syndrome is a nerve entrapment syndrome of posterior tibial nerve as it passes through the tarsal tunnel. The patient presented posteromedial malleolar swelling, palpable mass and pain, and was misdiagnosed as ganglion cyst in the other clinic. Aspiration and steroid injection were done for years, which had no significant effect on symptom relief. As skin lesion and pigmentation abruptly appeared around the palpable mass, MRI and physical exam were done in our hospital. The patient was diagnosed as varicose veininduced tarsal tunnel syndrome and therefore tarsal tunnel decompression following dilatated varicose vein resection was done. The patient's symptom was improved after the surgery. Therefore, herein we report this case.

Diabetic Neuropathy (당뇨 신경병증)

  • Neuropathy is a common complication of diabetes. It is characterized by a progressive loss of peripheral nerve fibers. The development of the neuropathy is linked to poor glycemic control, age, and the duration of diabetes. Peripheral sensory polyneuropathy is the most common type in neuropathy. Diabetic neuropathy is the most significant etiologic factor of the foot ulcer that may leads to amputation. Current treatments in diabetic neuropathy have no definitive effects on repair or reverse the damaged nerve but only to relieve of symptoms, especially on pain. When the focal compressive neuropathy is combined with diabetic neuropathy, the nerve would be more vulnerable and symptoms might get worse. Surgery is indicated for decompression of an entrapped nerve, like posterior tibial nerve in tarsal tunnel, after failure of the initial conservative treatments.
