Takayasu씨 동맥염의 수술치험 1예 (Surgical treatment of Takayasu's arteritis : Report of one case)

  • Takayasu`s arteritis is an arteritis of unknown etiology involving larger elastic arteries such as aorta and its branches, pulmonary arteries, and rarely coronary arteries. The late pathologic feature is vascular obstructive change and the resulting clinical manifestations are local ischemic symptoms such as syncope, visual disturbance, claudication of extremities, hypertension, and angina. the disease occurs predominantly in females, with the age of onset between 10 and 30 years. Recently we have experienced one case of Takayasu`s arteritis involving aortic arch and all its major branches. The patient was 36 year-old female and she was admitted because of headache, blurred vision, and easy fatigability and motor weakness of upper extremities. Aortogram revealed total obstruction of both carotid arteries at the site of its origin and partial irregular obstructive change in the innominate artery and both subclavian arteries. Bypass graft surgery using Gore-Tex grafts was performed with successful result.

  • A 61-year-old woman who presented with claudication and dyspnea on exertion was found to have severe calcified narrowing of the descending aorta and severe insufficiency of the aortic valve. These findings were compatible with Takayasu arteritis. To treat these hemodynamic abnormalities, extra-aortic bypass surgery combined with replacement of the aortic valve and ascending aorta-to-hemiarch replacement was performed through a separated upper hemi-sternotomy and limited median laparotomy. We present our successful surgical experience with this case.

  • Isolated stenotic lesion located at the ostium of the left main coronary artery associated with Takayasu`s aortitis is quite rare. This report herein described a case of 25 year old woman with isolated ostial stenosis of the left main coronary artery underwent pericardial patch [fixed with 0.6 % glutaraldehyde] angioplasty. An anterior approach was used and postoperative coronary angiogram of the patient showed normal coronary ostial contour with normal runoff.

  • Takayasu씨 동맥염은 원인이 잘 모르는 만성 염증성 질환의 일종이다. 이 질병은 주로 대동맥궁과 대동맥의 주분지를 잘 침범하는 것으로 알려져 있으며, 관상동맥과 신동맥을 동시에 침범하는 경우는 극히 드물다. 최근 고려대학교 흉부외과학교실에서는 관상동맥과 신동맥을 동시 에 침범한 Takayasu씨 동맥 염 환자를 성공적으로 치료하였기에 보고하는 바이다. 환자는 23세 여자로, 운동시의 흉통을 주소로 내원하였다. 혈관조영술상 좌주관상동맥, 양측의 신동맥, 좌쇄골하동맥의 협착 소견을 보였다. 치료로는 양측의 신동맥에 스텐트를 삽입하는 혈관성형술을 시행하였고, 1주일 뒤 대복재정맥을 이용한 관상동맥우회술과 인조혈관(PIFE)을 이용한 대동맥-쇄골하동맥간 우회술을 동시에 시행하였다. 환자는 수술 후 13일째 특별한 합병증 없이 퇴원하였다.

  • 저자들은 타카야수동맥염에 전형적인 맥박 감소, 사지 혈압의 차이, 사지 파행 등의 증상 없이 기침, 흉통, 객혈과 체중 감소의 호흡기 증상과 전신증상을 주소로 내원한 17세 남자 환자에서 진찰소견과 폐혈관조영술을 통해 폐동맥염을 확인하고, 대동맥조영술을 통해 쇄골하동맥과 온목동맥의 협착을 관찰하여 타카야수동맥염으로 진단한 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • The Takayasus arteritis is the obstructive disease in the principal branches arising from the aortic arch, and it can cause circulatory disturbances in the head or arms. We have experienced a case of complete obstruction of the left subclavian artery, the patient was undergone surgical repair with Knitted-Dacron patch graft between the left common carotid artery and the left subclavian artery. After operation, there were no symptoms except hypertension, and also no postoperative complications were seen. In the postoperative aortogram, the patency of the graft was good. The postoperative course was eventful and he has been in good conditions up to now so far.

  • We report a case of successful endovascular treatment of a pseudoaneurysm and the obstruction of an aorto-aortic bypass graft, which had been performed to treat Takayasu's arteritis fifteen years prior, at the thoracic aorta. Along with the immediate relief of proximal hypertension that had caused severe heart failure, the successful exclusion of the pseudoaneurysm and the patency of the stem graft were maintained three years after the procedure.

  • There are two pathogenic models of renovascular hypertension (RVH) originating from renal artery stenosis. We noted a case of a boy who had severe hypertension with atrophic left kidney, hypertrophic right kidney, a segmental stricture of the abdominal aorta, and total occlusion of the right renal artery. Due to the atrophic change of the contralateral, unclipped left kidney, this patient presented with various clinical manifestations related to both models of pathogenesis of RVH occurring at the same time. We conclude that this patient presented with the middle stage of the two RVH pathogenetic models, so called the 'one-clip, one and half-kidney model.'
