An unusual complication during arthrocentesis: N. facialis paralysis, with N. lingualis and N. alveolaris inferior anesthesia

  • This case report aims to review complications that can occur during arthrocentesis and report an unusual complication observed in a 55-year-old man. The patient received arthrocentesis in an attempt to treat painful locking episodes of his right temporomandibular joint (TMJ). One hour after the operation, the patient experienced temporary facial paralysis in the area of the facial nerve and anesthesia of the lingual and alveolar inferior nerves. No persistent complications were detected during the postoperative follow-up. We suspected this complication occurred after anesthetic solution overflowed from a traumatic perforation in the joint capsule to the infratemporal area during the operation. To our knowledge, this complication has not been previously reported in the literature.

  • In the past, many dentist were interested mainly in the mechanical aspects such as tooth preparation and retainer types for making dental restoration. But, these days, the concept of restorative treatment emphasizes the importarce of gnatic system and masticatory muscles in addition to oral cavity. So, the current considerations for the fixed prosthodontic treatment include the stabilization of temporomandibular joint and neuromuscualr system and the relationship of periodontal ligament and occlusion. To achieve the above objectives, occlusal splint has been used as one of the mouth preparations for restorative treatment. The objectives of occlusal splint are as follows; 1. To use as preliminary application for periodontal-occlusal treatment 2. To provide proper vertical dimension 3. To control abnormal habits and parafunction 4. To treat the temporomandibular disease and myofascial pain 5. To establish the new therapeutic position In some cases, the patients had improper vertical dimension and occlusal interferences caused by prostheses reconstucted using centric relation recorded without considering the health of TMJ and manticatory muscle. And these prosthesis act as primary source that cause pathologic phenomenon in periodontal ligament, muscles and TMJ. Physiologically, in order to make the treatment occlusion guided by proper centric relation method, the method should be guided after the use of occlusal splint for some period. The main objective of prosthetic treatment is to maintain the function and health of stomatognathic system. So, one of the most important things that have to be performed by clinicians is a clinical ability to do the correct diagnosis and treatment planning based on the stomatognatic function and occlusion.

A Study of Comparative the Chewing Ability and Oral Health Status of some Elderly People (일부 노인의 구강상태와 저작능력 비교연구)

  • Purpose : The present study aimed to investigate the of the based on their oral health status and the study findings will provide the basic data for establishing future oral health related policies for the elderly. Methods : A total of 50 elderly participants were selected for the study. Oral examinations were performed by a single trained dental hygienist, the oral health status was classified according to the use of dentures, presence of temporo-mandibular joint (TMJ) noise, presence of TMJ pain, presence of trismus, presence of preferred chewing, and number of residual teeth. An oral health-related survey using the in-person interview method was conducted by two dental hygienists and 30 types of foods were selected for the assessment of chewing ability. For data analysis, the Mann-Whitney U test and Spearman's Rho analysis were performed to investigate the association between oral health status and chewing ability. Result : The results of the present study revealed that there was a statistically significant positive (+) correlation between the number of residual teeth and chewing abilities. In other words, as the number of residual teeth increased, chewing ability increased as well, with the differences being statistically significant. Conclusion : Improvement of oral health in the elderly is a fundamental aspect of healthy aging. Therefore, the implementation of more efficient oral health care policies for the elderly is required as We move toward a super-aged society.

  • Temporomandibular ankylosis is defined as a situation in which the condyle is fused to the fossa by bone or fibrous tissue. Conditons such as trauma, infection, or systemic disease may predispose to various types of ankylosis, bringing about different levels of limitation in mandibular movement. Most patients with temporomandibular ankylosis are associated with limitation of maximal mouth opening, deviation of the chin toward the affected side, impaired occlusion, chronic pain, compromised oral hygiene, severe facial asymmetry & impeded mandibular molar eruption occurring in childhood. Several techniques to release ankylosis have been described in the literature, showing variable and often unsatisfactory results. The most frequently used operations are gap arthroplasty, interpositional arthroplasty, and exicision and joint reconstruction with autogenous or alloplastic materials. We have managed the two patients of TMJ ankylosis. They had previously TMJ surgery and we treated with gap arthroplasty & active physial therapy. We have obtained favorable results and report these cases with literatures review.

A study of subjective oral health actual condition in elementary school teachers, Daegu area (대구광역시 일부 초등학교 교사들의 주관적인 구강보건실태)

  • Objective : This study was performed in order to figure out Oral Health Actual Condition in Elementary School Teachers in Deagu area. This study was conducted from March through May 2008. Method : A total of three hundred and ten Elementary School Teachers were surveyed. The collected data were analyzed by Oral Health Actual Condition and cognition, Diet habit or living and one's own intellect health state, Oral disease sign symptoms of percent and 2-test and One-way ANOVA test by using SPSS12.0 Program. Results : 1. The most of result Frequencies of tooth brushing per one day were 3 over 91.0% and Oral Examination, Oral Health Education need. 2. The result of oral disease sign and symptom were hypersensitivity due to cold food(39.0%), halitosis(21.6%), gingival bleeding tendency(21.3), clicking sound on TMJ(18.7%), hypersensitivity due to tooth burshing(17.1%), easy crown fracture and to be fine(10.0%), pain on TMJ or limitation of mouth opening(7.1%). 3. The most of result age a group oral hygiene assistance article age 20 for interdental tooth brushing(46.4%), age 50 over not used interdental tooth brushing 38.5%. 4. The result of sign and symptom and snack following was statistically significant(P<0.05), health of own cognition and Oral health of own cognition was statistically significant(P<0.001). Conclusion : The study of understanded the Subjective Elementary School Teachers Oral Health Actual Condition and Promotion of Oral Health follow up Oral examination and Oral Health Education have to system groping.

Interpretation and Generalization by Neuroscience and Material Mechanics on Deviation in Temporomandibular Joint Balancing Medicine (턱관절균형의학에서 편차발생현상의 신경과학 및 재료역학적 해석과 일반화)

  • Objectives: For the deviation phenomenon occurring during the treatment process in temporo-mandibular balancing medicine (TBM), hypotheses were established regarding the cause and mechanism of formation from the perspective of neuro-science and material mechanics, and a verification method was proposed. Methods: The deviation phenomenon was theoretically analyzed based on the structure theories of material mechanics of the joint and the neurological pain mechanism. Results: Deviation occurs due to temporary yield by the accumulation of heterogeneous stress in the temporo-mandibular joint and the affected joint. Because the joint structures are corresponding with material mechanics showing compressive and tensile properties. The size of the deviation is expressed in terms of strain. The occlusal surface of the teeth is level with the axial joint. Since the magnitude of the deviation has a proportional relationship with the degree of abnormality of the temporo-mandibular joint, the magnitude of the deviation calculated by the balance measurement can be replaced by the strain. The major variables involved in the occurrence of deviations are the strength of joint structures and neurological conditions. Therefore plastic deformation and adaptation occur as a long-term depression of neural circuits is strengthened in different ways at different locations each time in various clinical situations. This is the reason why the sequence of the restoration process while correcting deviations is following reverse order of the accumulation in many layers in the muscular nervous system. Conclusions: From the above results, it can be inferred that the occurrence and correction of the deviations are corresponding with the plastic deformation and neuro-plasticity.

A Case Report of Anxiety Disorder Patient with Dyspnea (호흡곤란 있는 불안장애 환자 치험 1례 )

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment with TBT (Temporomandibular Joint Balancing Therapy) on Anxiety disorder patient with dyspnea. Methods: In this study, an Anxiety disorder patient with dyspnea was admitted to △△ Korean medicine Hospital from Nov 1st, 2022 to Nov 23rd, 2022. During the admission period, the patient was treated by Korean medical treatment(acupuncture, pharmacopuncture, herb medicine, etc.), especially using TBT and Western medical treatment (Pharmacotherapy). Beck Anxiety Inventory (BAI) and VAS were used for measuring the Anxiety and Dyspnea. Results: After treatment for 30days, the BAI showed a decrease from 33 to 19, The VAS associated with dyspnea also decreased from 6 to 1 and VAS associated with Neck Pain decreased from 5 to 3. Conclusions: These results showed that Korean medical treatment especially using TBT may have an effect on reducing symptoms of dyspnea due to Panic Attack. But the further researches are needed.

Cephalometric Characteristics of TMD Patients based on RDC/TMD Axis I Diagnosis (RDC/TMD Axis I 진단에 따른 측두하악장애 환자의 측두 두부방사선적 특징에 관한 연구)

  • The aims of this study were to investigate whether the facial skeletal patterns previously reported to be related to temporomandibular disorder (TMD) in other studies could be consistently observed in the TMD patients diagnosed according to Research Diagnostic Criteria for Temporomandibular Disorder (RDC/TMD) Axis I and evaluate its usability in the orthodontic clinics to examine the patients with TMD related symptoms. The clinical records and radiographs of female patients who visited the TMD and Orofacial Pain Clinic of Seoul National University Dental Hospital and were diagnosed as TMD were consecutively filed for this study. Patients were clinically examined and diagnosed according to the revised diagnostic algorithms of RDC/TMD Axis I and the lateral cephalogram, panoramic orthopantomogram, temporomandibular joint (TMJ) orthopantomogram, and transcranial radiograph of each patient were taken and digitalized. The data of patients who were under 18 years of age or had any systemic disease, trauma history involving the TMJ, or skeletal deformity at the time of the first examination were excluded. The remaining data of 96 female patients were finally analyzed. The obtained results were as follows: 1. There are no significant differences of cephalometric measurements between RDC I (muscle disorders) diagnostic groups. 2. Only the articular angle of the RDC group IIc (disk displacement without reduction without limited opening) patients was larger than patients of the no diagnosis of RDC II group (disk displacement). 3. Larger articular angle and smaller facial height ratio were observed in RDC IIIc group (osteoarthrosis) compared to IIIa group (arthralgia). Larger articular angle, larger Bjork sum, smaller posterior facial height, and smaller facial height ratio were observed in RDC group IIIc compared to no diagnosis of RDC III group (arthralgia, arthritis, and arthrosis). 4. According to the results of cephalometric analysis in simplified RDC groups, smaller overjet was observed in muscle disorders (MD) group. Facial height ratio and IMPA were smaller and articular angle was larger in disk displacements (DD) group than in no diagnosis of DD group. In arthrosis (AR) group, posterior facial height, and facial height ratio were smaller, and articular angle, gonial angle, facial convexity, FMA, Bjork sum, and ANB were larger than in no diagnosis of AR group. In joint pain (JP) group, only posterior facial height was smaller than no diagnosis of JP group. In conclusion, Facial morphologic patterns showing posterior-rotated mandible and lower posterior facial height is related to RDC group II and III diagnosis of the TMJ in female TMD patients. RDC/TMD Axis I diagnosis can provide a good clinical diagnostic tool for the standardized examination of the TMJ in orthodontic clinics.

A study on TMD and stress of first children (첫째 아이의 스트레스와 측두하악장애에 관한 연구 - 고등학생을 중심으로 -)

  • Objectives : The purpose of this study was to examine the relation between stress and temporomandibular disorder(TMD) of first the child. Methods : 500(the first child -250, non first child -250) high school students living in Jecheon city, form November 4th to 5th 2009, were the subjects of this questionnaire. The questionnaire was made up of three contests: sociodemographic characteristic, symptom & dysfunction of TMD and stress of high school students. The data were analyzed by chi-square test, two sample t-test and Pearson correlation. Results : Only 443(the first child -209, non first child - 234) students were evaluated due to inadequate responses. The obtain results was as follow. 1. Subjective symptom of TMD reported by 443 students were joint sound(63.5%), pain on chewing(55.1%), pain on opening the mouth(55.1%). 2. Analysis of subjective symptom of TMD showed the first child group was statistical significantly higher on joint sounds and TMJ pain than non first child group(<0.05). 3. Analysis of bad habit and dysfunction showed the first child group was statistical significantly higher on clenching of the teeth, difficulties of bite, headache than non first child group(<0.05). 4. Analysis of stress showed the first child group was statistical significantly higher on stress with parents than non first child. especially on marks and disagreement(<0.05). 5. Stress were partly related to subject symptom of TMD(<0.05, <0.01). 6. Analysis of relation stress and bad habit & dysfunction showed stress were partly related to bad habit & dysfunction(<0.05, <0.01). 7. bad habit were partly related to TMD(<0.05, <0.01). Conclusions : The first child stress and symptom of TMD were higher than non-first child, also stress and symptom of TMD was positively associated. therefore management of the first child' stress is necessary and related study is necessary in the future.

Clinical Observations of Temporomandibular Disorder Patients used Dong-Qi Acunpuncture Treatment (동기침법(動氣鍼法)을 이용한 악관절(顎關節) 장애(障碍) 환자(患者)의 임상적(臨床的) 관찰(觀察))

  • Objective : The purpose of This study is to analyze influence of functional and clinical improvement effect on temporomandibular disorders of Dong-Qi acupuncture treatment that often be used to temporomandibular disorder patients. Methods : We investigated to focus temporomandibular disorders in Jaseng oriental medicine hospital from September 8, 2000 to March 22, 2001. We compared to before and after treatment situation using temporomandibular disease & facial ache dolor pain and malignant function disorders & activation inhibition analysis from one cured Dong-Qi acupuncture treatment of outpatients-40 examples. Results : These results appeared that Sex distinction occurrence ratio was much more female than male of 1: 7. Age distinction occupied the most ratio in 20-30 generation. Jobs was mainly ranked students and companion. In cases treated before visiting our hospital, was mainly ranked our hospital first dignosis. the distribution interval of solider was mainly followed above 3 years. the distribution interval of treatment was mainly gone within 1 month and above 3 month, frequency of treatment was examined below 5 times, into 6 to 10 times, more than 11 times, respectively. Everything showed a very valuable effect except chewing and dentalgia in decrease of ache dolor pain and linkage pain related to mal-function. There are meaningful effect in Every part on Mal-function improvement. Every contents showed a very excellent effect except activation such as motivation ingestion and recreation in influence of activation. Conclusion : The above results showed a very valuable treatment effect of temporomandibular disorders on Dong-Qi acupuncture treatment. Therefore we are considered the necessity of continuous study and observation applying to other disease as well as temporomandibular disorders.
