만성 편마비 뇌졸중 환자의 인지기능이 신체기능 수준 및 기능적 활동에 미치는 영향 (Effects of Cognitive Function on Physical Performance and Functional Activities in Persons with Chronic Hemiparetic Stroke)

  • Purpose: Cognitive function is a main concern for rehabilitation progression in individuals who have sustained brain damage, even among those whose motor function has returned after brain damage. The purpose of this study was to investigate how cognitive impairment relates to functional independence in postural stability and gait performance in patients with chronic hemiparetic stroke. Methods: This was an observational design in an outpatient rehabilitation hospital. Twenty-eight adults with chronic hemiparetic stroke, receiving a course in an outpatient rehabilitation program, participated in this study. They were divided into two groups (i.e., non-cognitive impairment and cognitive impairment groups) via a cut-off score of 23 or less on a mini-mental state examination. Functional independence was assessed with the timed up-and-go test (TUG), 10-meter walk test (10mWT), five times sit-to-stand test (FTSST), Berg balance scale (BBS), and modified Barthel index (MBI). The independent t-test was used for statistical analysis when comparing the two groups. Results: The cognitive impairment group had less functional independence, balance, and gait performance than those of the non-cognitive impairment group had. The former also showed a statistically significant decrease in their TUG score, FTSST score, BBS score, and MBI score compared to the latter, but not in their 10mWT score (p<0.05). Although the non-cognitive impairment group walked faster than the cognitive impairment group did, that difference was not statistically significant (p>0.05). Conclusion: The results of this study suggest that cognitive impairment relates to functional independence in postural stability and the activities of daily living. In rehabilitation settings, cognitive impairment would be considered a major component in therapeutic rehabilitation to overcome the patients difficult physical problems and to treat for improving functional independence more after stroke.

슬관절 전치환술 후 만성기 여성 노인의 슬관절 신전근에 근에너지기법이 근력, 관절가동범위, 균형, 보행능력에 미치는 영향 (Effects of Muscle Energy Technique on Knee Extensor Muscle Strength, Knee Range of Motion, Balance, and Walking Ability in Elderly Women during the Chronic Phase after Total Knee Replacement)

  • Background: This study aimed to use the muscle energy technique (MET) with total knee replacement (TKR) during the chronic phase in a clinical setting and confirm its effects on the knee extensor strength and ROM, balance, and walking ability. Methods: A total of 20 female patients who underwent TKR 1~4 years ago were assigned to two groups (Control: Q setting exercise+general physical therapy, n=10; Exp: MET+general physical therapy, n=10). Interventions were performed three times a week for 4 weeks. The strength of the knee extensor was evaluated using an aneroid sphygmomanometer, and ROM was evaluated using degrees at the end range on active knee flexion. The main balance outcomes were evaluated using two standard scale (TSS) and timed up and go (TUG) test, whereas the walking ability was evaluated using the 10 meter walk test (10MWT). Results: Analysis showed that both groups had significant increases in strength, ROM, TSS, TUG, and 10MWT. Differences in all variables were significant between the control and Exp groups at the post-intervention evaluation (p<.05). However, no significant difference was observed in strength and TUG. Conclusion: Results of this study demonstrated that MET would help improve the strength, ROM, balance, and walking ability of patients with chronic TKR who want to enhance their abilities and performance in activities of daily living.

The Effects of Exercise Program on Pain and Balance ability in Patients with Total Knee Replacement : Meta-analysis

  • 본 연구는 무릎관절 전치환술 환자에게 시행된 운동 프로그램이 통증과 균형에 미치는 효과크기를 확인하고, 추후 이에 관한 운동 프로그램 개발에 대한 기초자료를 제공하고자 한다. 총 3개의 데이터베이스에서 문헌을 검색하여 본 연구에 적합한 연구 10편을 최종 선정하였다. R 프로그램을 이용하여 개별 연구의 효과크기를 확인한 결과, 전체 효과크기는 0.87로 큰 효과크기로 나타났다. 또한 통증에 관한 효과크기는 0.62로 중간 효과크기로 나타났고, 균형에 관한 효과크기는 1.00으로 큰 효과크기로 나타났다. 이처럼 각각의 운동 프로그램이 무릎관절 전치환술 환자의 통증을 감소시키고 균형 능력을 개선하는데 긍정적인 영향을 미친다.

FES와 과제지향 훈련이 뇌졸중 환자의 복부근 두께와 균형에 미치는 영향 (The Effect of Task Oriented Exercise and Abdominal Muscle Contraction using FES on Abdominal Muscle Thickness and Balance of Stroke Patients)

  • Purpose: The purpose of this study was to evaluate the effect of task-oriented exercise and abdominal muscle contraction using functional electrical stimulation (FES) on abdominal muscle thickness and balance of stroke patients. Methods: Ten stroke patients who met the selection criteria were assigned randomly into two groups of five. One group received FES therapy before task-oriented training (experimental group), while the other group received a FES placebo before task-oriented training (control group). The Mann-Whitney U test was used to compare the groups, and the Wilcoxon Signed-ranks test was used to compare differences between the groups before and after intervention. The Mann-Whitney U test was used to compare the rate changes of each item before and after intervention, between the two groups. Results: In the rectus abdominalis and external oblique muscle thickness tests that used ultrasound, there was a statistically significant difference in the experimental group (p<.05),but no significant difference in the control group (p>.05). There was also a significant difference between the groups (p<.05). In the Balance test that used the Berg Balance Scale (BBS) and timed up and go test (TUG), there was a statistically significant difference in the experimental and control groups (p<.05), but there was no significant difference between the groups (p>.05). Conclusions: FES therapy before task-oriented training increases the thickness of abdominal muscles and improves balance abilities.

집중재활프로그램이 하지 절단 환자의 신체활동 및 의지 착용감에 미치는 영향 -후향적 연구- (Effect of an Intensive Rehabilitation Program on Physical Activity and Wearing Satisfaction in Traumatic Lower Limb Amputees -A Retrospective Study-)

  • Purpose: This study aimed to investigate the effect of an intensively rehabilitation program on the gait, balance, functional performance, and wearing satisfaction of patients with traumatic lower extremity amputations caused by industrial accidents. Methods: In this study, the anonymized electronic medical records of individuals who participated in the intensive rehabilitation program (among those who were admitted to the hospital belonging to the Labor Welfare Corporation) due to an industrial accident from August 2018 to September 2019 were collected. As a result, the records of 12 subjects meeting the screening criteria were analyzed. Results: According to the time of application to the intensive rehabilitation program, the 10 Meter Walk Test (10MWT) (p < 0.01), Berg Balance Scale (BBS) (p < 0.01), Timed Up and Go Test (TUG) (p = 0.01), the L Test of Functional Mobility (L-test) (p < 0.01), Prosthetic Limb Users Survey of Mobility (Plus-M) (p < 0.01), and Houghton Score Question (HSQ) (p < 0.01) values significantly improved with time. Conclusion: This study confirmed the positive effect of an intensive rehabilitation program on the gait, balance, functional performance, and wearing satisfaction of patients with traumatic lower extremity amputations caused by industrial accidents. However, this study was limited by the absence of a control group, and, thus, it highlights the need for more extensive research with a large sample.

모래사장 위 맨발걷기와 운동화걷기가 허리통증이 있는 노인의 통증, 장애, 운동기능, 수면만족도, 삶의 질에 미치는 효과 비교 (A Comparison of the Effects of Barefoot Walking and Sneakers Walking on a Sandy Beach on Pain, Disability, Motor Function, Sleep Satisfaction, and Quality of Life in the Elderly with Low Back Pain)

  • Purpose : The purpose of this research was to investigate the effects of barefoot walking on the sandy beach on pain intensity, disability, motor function, sleep satisfaction, and quality of life in the elderly with low back pain (LBP). Methods : A single-blinded randomized controlled trial was conducted involving 32 elderly with LBP who underwent sand walking barefoot (intervention, n=16) or with sneakers (control, n = 16). Both walking methods were carried out for 30 minutes per day, three times a week for four weeks. Pain intensity and disability were assessed using the visual analog scale and Oswestry disability index, respectively. Balance and gait were evaluated using the Berg balance scale and timed up and go test, respectively. Sleep satisfaction was quantified with the Verran and Snyder-Halpern sleep scale. Finally, quality of life was assessed through the WHO quality of life scale's abbreviated Korean version. Results : Compared with the control group, the intervention group showed significant differences in pain intensity (p=.005), disability due to LBP (p=.002), static balance ability (p=.003), dynamic balance ability (p=.002), and sleep satisfaction (p=.017). There was no significant difference in the quality of life between the two groups. Conclusion : Barefoot walking on a sandy beach is significantly effective in improving pain, disability due to LBP, balance ability, and sleep satisfaction in the elderly with LBP. Further studies with larger sample sizes and longer intervention periods must to be conducted to generalize using barefoot walking in LBP management.

  • Purpose: This study aims to report on the effect of providing customized visiting exercise, a musculoskeletal intervention method, to the elderly in the community. Methods: In this study, subjective pain was measured to evaluate pain and physical ability of the elderly, and timed up & go (TUG) test and psychological level test were performed for myofascial pain perception symptoms and gait function. Measurements were performed twice before and 4 weeks after treatment. Participants in the exercise group (n=108) performed the spine exercise, whereas those in the spine exercise group (n=108) performed the spine exercise using complex exercise program (3 times/week for 4 weeks). Results: The significant test of visual analogu scale (VAS), pain pressure threshold (PPT), TUG, psychosocial factor according to applying the exercise within groups used T-test. In the result following analysis, there was significance on VAS (p=.000), PPT (p=.000), TUG (p=.000), Psychosocial factor (Depresion; p=.000, Anxiety; p=.000) within group. Conclusion: Then, exercise has benefit on the VAS, PPT, TUG, psychosocial factor in patients with myofacial pain syndrome. So, it may suggest that exercise will be helpful of the pain, gait and psychosocial factor improvement the patients with spine pain.

The effects of virtual reality training on gait, balance, and upper extremity function in patients with stroke: A meta-analysis

  • Background: The purpose of this study is to investigate the effects of virtual reality on gait, balance, and upper extremity functions compared to other independent variables or no variables. Additionally, the possibility of virtual reality for stroke patients was discussed. Design: Meta-analysis. Methods: The search for this study was a search term that combined stroke, virtual reality, and training, and the electronic search was conducted through EMBASE, MEDLINE, and Cochrane Library. As a result of the search, 21 studies satisfying the selection criteria of the target study were confirmed as the final analysis target. This study consisted of 21 randomized experimental studies and 21 randomized controlled trials, and the total number of participants was 642. [Experimental group (n=314), control group (n=328); total 642]. As a result of the study, upper extremity function was assessed using a box and block test, a modified Ashworth scale, and a scale including range of motion. The balance was evaluated by the berg balance scale. Gait was a Timed Up and Go test (TUG), stride length, and gait function. Scales including a walking rate scale were evaluated. The effect size for the intervention of the analytical study was meta-analyzed with the RevMan 5.3.3 program of the Cochrane library. Results: The results of the study showed that the function of walking was statistically significant. Balance showed statistically significant results. The upper extremity function showed no statistically significant results. Conclusion: Through this rehabilitation treatment by applying virtual reality environment to the rehabilitation of stroke patients in the future can be proposed as an effective intervention method for the balance and gait function of stroke patients.

불안정 지면에서 복합 균형 운동이 무릎 전치환술 환자의 기능적 능력과 일상생활능력에 미치는 영향 (The effect of complex balance exercise on unstable surfaces on functional ability and daily living ability in patients with total knee arthroplasty)

  • Purpose: Patients with total knee arthroplasty (TKA) have impaired balance and movement control. Exercise interventions have not targeted these impairments in this population. This study aimed to investigate the effect of complex balance exercises on unstable ground, on the gait, balance, and daily living ability of patients with total knee arthroplasty. Design: Randomized controlled trial. Methods: The participants consisted of 30 patients placed into two groups of 15 each: a experimental group (complex balance exercise) and a control group (physical therapy exercise). Both group exercise was applied for 3 times a week for 30 minutes for four weeks. Force plate for balance ability and Timed up and go (TUG) test were the primary outcome measures. The secondary outcome measures included 10-m walk test (10MWT) and the daily living ability using the Knee Outcome Survey Activities of Daily Living scale (KOS-ADL). Results: The result of this study showed that the experimental group had a significant difference in TUG and 10MWT than the control group, and balance was significantly different in CEA, CPL, and CAV. There was a significant difference in daily living ability between the experimental and control groups. This study confirms that the physical therapy with complex balance exercise on unstable surfaces has positive effects on balance, gait and daily living ability in patients with total knee arthroplasty. Conclusion: As a result of this study, complex balance exercise on unstable surface was more effective in improvement gait, balance and daily living ability in total knee arthroplasty. From this study, physical therapy with complex balance exercises on unstable ground may be proposed as and effective intervention method for improving gait, balance, and daily living ability in patients with early total knee arthroplasty.

The effect of balance training using visual information on the trunk control, balance and gait ability in patients with subacute stroke: Randomized controlled trial

  • Background: This research was conducted to understand balance training in trunk control, balance, and walking in stroke patients. Design: Randomized controlled trial. Methods: The subjects included 40 stroke patients, of whom 20 undertook balance training using visual information and the other 20 undertook balance training using balance boards. Using visual feedback, the balance training group used a training program within the static balanced evaluation tool, while the balance training group trained using a balance board. All subjects underwent 20 mins of neurodevelopmental treatment, and both target groups underwent 10 mins each of balance training by using either visual feedback or a balance board. The treatment period lasted a total of 4 weeks, twice a day. Trunk control before and after training was evaluated with the Trunk Impairment Scale. Balance capability was assessed by the Berg Balance Scale, Functional Reach Test, Timed Up and Go test, and Static balance measurement tool. Walking capacity was measured using gait measuring equipment, and cadence and velocity were measured. Results: Both groups showed a significant improvement in their interstitial control, balance, and gait ability after the experiments compared to before the experiments (p<0.05). The difference between the two groups was not significant. The visual feedback balance training group showed a more substantial improvement than the balance board training group. Conclusion: In this study, we found that the balance training combined with visual feedback contributes to improving trunk control, balance, and gait in patients with hemiplegia due to stroke. In addition to this, I believe that balanced training combined with visual feedback can be used as a training method when considering patients who lack interstitial control, balance, and gait ability.
