  • Malnutrition is common and the major risk factor of mortality of end stage renal disease (ESRD) patients. The aim of this study is to assess nutritional status of malnutrition patients on dialysis by various methods and compare nutritional parameters of continuous ambulatory peritoneal dialysis (CAPD) patients with hemodialysis patients. 137 patients on dialysis from April 2009 to July 2013 were enrolled. Nutritional parameters of 66 CAPD and 71 hemodialysis patients were investigated by anthropometry, biochemical study, diet analysis and questionnaires. Malnutrition patients were selected by body mass index (BMI), serum albumin and pre-albumin based on International Society of Renal Nutrition and Metabolism (ISRNM) diagnostic criteria for protein-energy wasting and compared with non-malnutrition patients. In comparison of CAPD and hemodialysis patients, most anthropometric values showed no significant difference except total body water (TBW). TBW was lower in CAPD patients (P=0.024). Although serum albumin was slightly higher in hemodialysis patients (P=0.047), pre-albumin were significantly higher in CAPD patients (P=0.000). Serum blood urea nitrogen (BUN) was higher in hemodialysis patients (P=0.000). In diet analysis, Total calorie (P=0.000) and total cholesterol (P=0.012) intakes were higher in CAPD patients. Mean subjective global assessment (SGA) grade was higher in CAPD patients (P=0.003). Several nutritional parameters of CAPD patients were better than hemodialysis patients implying more intensive therapeutic approach may be needed for hemodialysis patients. We have to understand multiple factors contributing malnutrition of ESRD patients and individualized therapeutic approach is needed.

  • The study was carried out to evaluate the effect of exogenous bovine somatotropin on water metabolism in relation to mammary function in early lactation of crossbred Holstein cattle. Ten, 87.5% crossbred Holstein cattle were divided into two groups of 5 animals each. At day 60 of lactation, the control group was given placebo while animals in the experimental group were given recombinant bovine somatotropin (rbST) by subcutaneous injection with 500 mg of rbST (14-days prolonged-release rbST). In rbSTtreated animals, milk yield increased 19.8%, which coincided with a significant increase in water intake (p<0.01), while DM daily intake was not different when compared to the control animals. Water turnover rate as absolute values significantly increased (p<0.05), while the biological half-life of water did not change in rbST-treated animals. Total body water (TBW) and total body water space (TOH) as absolute values significantly increased (p<0.01) in rbST-treated animals, while it was decreased in the control animals. Absolute values of empty body water (EBW) markedly increased (p<0.05), which was associated with an increase in the extracellular fluid (ECF) volume. Absolute values of plasma volume and blood volume were also significantly increased (p<0.05) in rbST-treated animals. The increase in mammary blood flow in rbST-treated animals was proportionally higher than an increase in milk production. The plasma IGF-1 concentration was significantly increased (p<0.01) in rbST-treated animals when compared with those of control animals during the treatment period. Milk fat concentration increased during rbST treatment, while the concentrations of both protein and lactose in milk were not affected. The present results indicate that rbST exerts its effect on an increase in both TBW and EBW. An increased ECF compartment in rbST-treated animals might partly result from the decrease in fat mass during early lactation. The action of rbST on mammary blood flow might not be mediated solely by the action of IGF-1 for increase in blood flow to mammary gland. An elevation of body fluid during rbST treatment in early lactation may be partly a result of an increase in mammary blood flow in distribution of milk precursors to the gland.

  • The purpose of this study was to investigate the relationship among nutritional status, serum lipids and leptin of 134 elementary school students with different obesity index residing in Kyunggi-do, Korea. Subjects were assigned to under weight, normal weight or over weight group according to the their obesity index. Their nutritional status, serum lipid and leptin levels were evaluated based on 24-hr dietary recalls, anthropometric measurements and blood analysis. The mean weight, height and obesity index of total subjects was 150.8cm, 45.6kg and 5.4%. The mean % of body fat was higher while LBM and TBW (total body water) were lower in over weight subjects than those of other two groups. Total food, plant protein, carbohydrate, and vitamin C intakes of underweight male students were lower than those of overweight male students. The consumption of total fat was the highest in the female normal weight subjects and the lowest in female overweight subjects. Male students consumed more plant protein and vitamin C than female students. The mean daily intake of vitamin B$_2$, calcium and zinc of all subjects did not meet the Korean RDA. Also, iron intake of female students was lower than the Korean RDA. The mean level of serum leptin was the highest in male and female overweight subjects (p<0.001). The HDL-cholesterol level was the highest in the underweight group of male subjects (p<0.05) and in the normal weight group of female subjects (p<0.001) The serum level of leptin and atherogenic index (AI) of females were higher than those of males. Serum leptin of total subjects had significantly positive correlation with obesity index, weight, SBP, body fat, cholesterol intake, serum triglyceride, and AI. However, serum leptin was negatively correlated with LBM, TBW, intake of plant protein, vitamin B$_1$, vitamin C and serum HDL-cholesterol. Obesity index was also positively correlated with weight, SBP, body fat, serum triglyceride and AI. To summarize the results, the vitamin B$_2$, calcium, zinc, iron and fiber intakes should be supplied to elementary school students, especially those who were overweight. The average serum leptin level of students showed remarkably positive correlation with obesity index, body fat, serum lipids and AI. Also, it was suggested that the serum leptin might be related to the dietary factors like cholesterol, plant protein, vitamin B$_1$ and vitamin C of elementary school students.

  • Objectives: This study aims to investigate the changes of women's postpartum symptoms, including their weights, edema index and quality of life (QOL) over the first six weeks after childbirth. Methods: The study participants were 31 postpartum women treated in the Dept. of Obstetrics & Gynecology, at Woo-Suk University Korean Medicine Hospital, from May 20th to August 5th, 2016. In the first 2 weeks of the postpartum period, the main data collection method was to interview each patient and write down all of their complaints, and additionally referring to each participant's PHR (Personal Health Records). At the 3rd, 4th, 5th and 6th week, the researcher interviewed participants by phone once a week. The participants' weight, edema index (ECW/TBW) and the quality of life (by EQ-VAS) were measured at 1st and 2nd week after childbirth. And, EQ-VAS was assessed once more at the 6th week after childbirth. As a last step, the participants responded to a survey on satisfaction regarding their postpartum care with Korean Medicine. Results: 1. In the 1st one week after childbirth, edema was the most frequent complaint. It was about joint pain of the upper limbs in the 2nd and 3rd weeks, sweating in the 4th week, and joint pain of the upper limbs in the 5th and the 6th weeks. 2. In the 1st two weeks of the postpartum period, the weight of the participants decreased from $66.33{\pm}9.30kg$ to $62.60{\pm}8.92kg$ (p<0.001) and the edema index ECW/TBW decreased from $0.399{\pm}0.010$ to $0.385{\pm}0.0 4$ (p<0.001). The EQ-VAS significantly improved from $61.77{\pm}17.72$ to $73.51{\pm}14.67$ (p<0.001). In the last 4-weeks of the postpartum period, the EQ-VAS decreased from $74.30{\pm}14.25$ to $73.63{\pm}13.35$, but this difference was not statistically significant (p=0.749). 3. Regarding the satisfaction with postpartum care with Korean Medicine, 60% of the participants responded that it was 'Excellent' and 40% said it was 'Good'. Conclusion: Over the entire postpartum period, the most frequent complaints were about musculoskeletal symptoms. Postpartum care with Korean Medicine treatments in the early postpartum period improves various postpartum symptoms and the quality of life for postpartum women.

  • The body fat distribution and nutritional state of the Korean and Japanese young female subjects were compared. Three-day individually weighed, dietary intakes and anthropometric measurements were determinded in 48 Korean and 60 Japanese female students. 19 to 23 yr of age, from the divisions of nutritions. The body composition estimates were measured by bioeletrical impedance-total body water(BI-TBW) method. There were no significant difference between the Korean and Japanese young females in body size and body shape. The Korean young females had lower estimated total body fat and internal fat that calculated by BI-TBW method, but there was no great difference between the Korean and the Japanese in subcutaneous fat. In contrast, the Korean young females are reported to have the same total energy expenditure per day as the Japanes young females and the total energy intakes and carbohydrate energy intake ratio per day were significantly higher than those of the Japanese young females. The difference in body fat distribution and energy intakes in Korean, and the role of capsaicin in red pepper are discussed as a possible determinant of the internal fat in the Korean vs the Japanese.

Methods: Twenty seven mothers who received Korean medical treatment in the outpatient department treatment (from September 27th, 2017 to January 5th, 2018) were evaluated for Verbal numerical rating scale (VNRS), edema index, EuroQol Visual Analogue Scale (EQ-VAS), and Edinburgh Postnatal Depression Scale (EPDS). Results: There were 17 high risk participants (63.0%) and 10 normal participants (37.0%). The VNRS of edema is the highest in the first week, and the VNRS of joint pain is the highest from the second week to the sixth week in all patients. The Extra Cellular Water/Total Body Water (ECW/TBW) of high risk group significantly decreased from $0.403{\pm}0.011$ to $0.387{\pm}0.006$(p<0.05) in the first 2 weeks. The ECW/TBW of normal group significantly decreased from $0.393{\pm}0.070$ to $0.383{\pm}0.011$ (p<0.05) in the first 2 weeks. The EQ-VAS of high risk group increased from $64.12{\pm}13.941$ to $69.35{\pm}18.155$ (p<0.05) in the first 2 weeks. But this difference was not significant statistically (p=0.234). The EQ-VAS of normal group significantly increased from $62.50{\pm}21.763$ to $74.00{\pm}9.661$ (p<0.05) in the first 2 weeks. The difference of EPDS was not statistically significant between the first week and the sixth week in every participants. Conclusions: VNRS was the highest in edema in the first week, joint pain was the highest from the second week to six week. The edema index of high risk groups was higher than that of the normal group in the first week (p<0.05). The EQ-VAS of normal group significantly increased (p<0.05) in the first 2 weeks but high risk group didn't. In the EPDS, the ratio of nine or more points of high risk group was more than twice than normal group in the first 2 weeks.
