High Resolution MR Images from 3T Active-Shield Whole-Body MRI System (3T 능동차페형 전신 자기공명영상 장비로부터 얻어진 고해상도 자기공명영상)

  • Purpose : Within a clinically acceptable time frame, we obtained the high resolution MR images of the human brain, knee, foot and wrist from 3T whole-body MRI system which was equipped with the world first 37 active shield magnet. Materials and Methods : Spin echo (SE) and Fast Spin Echo (FSE) images were obtained from the human brain, knee, foot and wrist of normal subjects using a homemade birdcage and transverse electromagnetic (TEM) resonators operating in quadrature and tuned to 128 MHz. For acquisition of MR images of knee, foot and wrist, we employed a homemade saddle shaped RF coil. Topical common acquisition parameters were as follows: matrix=$512{\times}512$, field of view (FOV) =20 cm, slice thickness = 3 mm, number of excitations (NEX)=1. For T1-weighted MR images, we used TR = 500 ms, TE = 10 or 17.4 ms. For T2-weighted MR images, we used TR=4000 ms, TE = 108 ms. Results : Signal to noise ratio (SNR) of 3T system was measured 2.7 times greater than that of prevalent 1.5T system. MR images obtained from 3T system revealed numerous small venous structures throughout the image plane and provided reasonable delineation between gray and white matter. Conclusion The present results demonstrate that the MR images from 3T system could provide better diagnostic quali\ulcorner of resolution and sensitivity than those of 1.5T system. The elevated SNR observed in the 3T high field magnetic resonance imaging can be utilized to acquire images with a level of resolution approaching the microscopic structural level under in vivo conditions. These images represent a significant advance in our ability to examine small anatomical features with noninvasive imaging methods.

In Vitro Imaging of MRI and Ultrasound for Gastric Carcinoma (위암 조직의 자기공명영상과 초음파 소견에 대한 비교 연구)

  • Purpose : To evaluate and compare the diagnostic accuracy of MRI and ultrasound(US) for estimation of invasion depth of gastric carcinoma by correlation with histopathologic findings in vitro and to find out the best MR pulse sequence for detection and accurate delineation of tumor. Materials and Methods : Resected specimen of total or subtotal gastrectomy from 53 patients with gastric carcinoma were done of imaging studies of MRI and US. And US was examined by using high frequency linear transducer for tumor invasion depth by a radiologist. In each case, both imaging findings of MRI and US were evaluated independently for tumor detection and invasion depth by consensus of two radiologists and were compared the diagnostic accuracy between two imaging modalities according to the histopathologic findings. MR imaging with five MR pulse sequences, spin echo T1 and in- and out-of phase gradient echo T1 weighted images, FSE and SSFSE T2 weighted images, were performed. Five MR pulse sequences were evaluated and compared on the point of detection and accurate distinction of tumor from surrounding normal tissue. Results : In EGC, diagnostic accuracy of US(77%) was superior than that of MRI(59%) but no statistically significant difference was noted between two imaging modalities(p=0.096). In AGC, both imaging modalities of MRI and US showed relatively high diagnostic accuracy as 97% and 84% respectively. Diagnostic accuracy of MRI was statistically better than that of US at the significant level(p<0.001). The best MR pulse sequence among five in each specimen was FSE T2WI(75.5%, 40/53) in both EGC and AGC. In AGC, FSE T2WI showed excellent imaging quality by showing very high ratio (93.5%, 29/31) of accurate delineation of tumor. Conclusion : MRI and US show relatively high diagnostic accuracy in the evaluation of tumor invasion depth of resected specimen in AGC. The most excellent pulse sequence of MRI for the evaluation of tumor invasion depth is FSE T2WI on the point of detection and accurate delineation of tumor in both EGC and AGC.

Effectiveness of a fast spin echo technique using the signal void in acquisition of black blood images (흑혈류영상 획득 시 신호소실을 활용한 고속스핀에코기법의 유용성)

  • The purpose of our study is to shorten the scanning time and minimize the inconveniences of the patients in acquisition of the black blood images using the signal void effect in the fast spin echo technique while keeping the diagnostic value of the test. Thirty-two consecutive patients who underwent black blood MR imaging were examed with additional double inversion recovery (DIR) sequence and the conventional fast spin echo (FSE) sequence. Signal to Noise Ratio (SNR) and Contrast to Noise Ratio (CNR) of the internal carotid arteries' lumen were compared in T1 and T2 weighted images to determine whether there are differences between the two techniques for depiction of the signal void effect inside the vessel wall. The FSE images showed lower SNR values than the DIR images in both of the T1 and T2 weighted images (11.49% and 13.66% respectively). While the CNR values were higher in the FSE images than in the DIR images in both of the T1 and T2 weighted images (8.69% and 7.55% respectively).There was no significant difference between the two techniques for either of the SNR or CNR (p>0.05, p>0.05 respectively). The DIR and the FSE images demonstrated almost identical imaging patterns. Therefore, it is anticipated that the use of FSE technique in acquisition of the black blood imaging could reduce the inconveniences of the patients during the scanning and minimize exam time while keeping the diagnostic value of the test.

The Age-related Microstructural Changes of the Cortical Gray and White Matter Ratios on T2-, FLAIR and T1- weighted MR Images (T2, FLAIR, T1 강조 MR영상에서 나이에 따른 뇌피질의 회질과 백질의 미세구조 변화)

  • Purpose : The purpose of this study was to investigate the microstructural changes according to aging on the thickness and signal intensity (SI) of the cortical gray matter (GM) and white matter (WM) on the T2-, fluid-attenuated inversion recovery (FLAIR) and T1-weighted MR images in normal subjects. Materials and Methods : The 10, 20, 30, 40, 50, 60, 70, 80 and 90 year age groups of men and women (each 10 individuals) who underwent routine brain MRI, including the T2-, FLAIR and T1-weighted images, were selected for this study. We measured the thickness and the SI of the cortical GM and WM at the postcentral gyrus, which has an even thickness at the level of centrum semiovale, on the axial scans and we calculated the mean values of the thickness ratio of the gray/white matter (TRGW) and the signal intensity ratio of the gray/white matter (SRGW), and we compared the ratios of each age group. Results : On the T2-weighted images, the TRGWs were 0.81 and 0.79 at the age of 10 and they were 0.73 and 0.71 at the age of 90 in the men and women, respectively. So, the GM thickness was decreased more than the WM thickness was with aging. On the FLAIR images, the TRGWs were 1.09 and 1.00 at the age of 10 and they were 1.11 and 0.95 at the age of 70 in the men and women, respectively. On the T1-weighted images, the TRGWs were 0.66 and 0.80 at the age of 10, and the ratio was changed to 0.90 and 0.78 at the age of 90 in the men and women, respectively. On the T2-weighted image, the SRGWs were 1.53 and 1.43 at the age of 10, and they were 1.23 and 1.27 at the age of 90 in the men and women, respectively. On the FLAIR images, the SRGWs were 1.23 and 1.25 at the age of 10 and they were 1.06 and 1.05 at the age of 90 in the men and women, respectively. On the T1-weighted images, the SRGWs were 0.86 and 0.85 at the age of 10, and they were 0.90 and 0.87 at the age of 90 in the men and women, respectively. Conclusion : We suggest that the age-related microstructural changes of the thickness and the SI of the cortical GM and WM on the T2-, FLAIR and T1-weighted images are unique, and so this knowledge will be helpful to differentiate neurodegenerative disease from normal aging of the brain.

Evaluation of Traumatic Intracranial Hemorrhage Using Computed Tomography and Magnetic Resonance Imaging in Four Dogs (개에서 컴퓨터단층촬영술과 자기공명영상을 이용한 창상성 두개관내 출혈의 평가)

  • Four dogs with neurologic dysfunction resulted from intracranial hemorrhage by head trauma were referred to Veterinary Medical Teaching Hospital, Chungnam National University. There were no remarkable findings in survey radiography in four cases. CT and MRI scans were diagnostic in these cases. Three dogs underwent CT scanning. On CT images, the lesion was hyperdense and was not enhanced after intravenous contrast administration in dog 1 and dog 2. On CT of dog 4, there was no significant finding. All of four dogs were verified by dorsal, sagittal, and transverse T1-weighted (T1W) and T2-weighted (T2W) images. Appearance of the lesions in dog 1 and dog 2 was isointense (dog 2) or isointense with hyperintense rim (dog 1) on T1W images and hyperintense on T2W images. In dog 3 and dog 4, there were hypointense and hyperintense lesions on T1Wand T2W images respectively. The lesions in four dogs were located in the left intracerebral region, intracerebral and cerebellovestibular region, subdural space, and right cerebral hemisphere, respectively.

MR Imaging Findings of Recurred Dermatofibrosarcoma Protuberans of the Scalp: A Case Report (두피에서 재발한 융기성 피부섬유육종의 MR영상: 증례 보고)

  • A 48-year-old man presented with a dermatofibrosarcoma protuberans (DFSP) of the scalp associated with local recurrence. Axial T1- and T2-weighted images demonstrated a well-circumscribed hypointense and intermediate hyperintense mass in the skin and subcutaneous layer of the scalp, respectively. Contrast-enhanced T1-weighted images showed the strongly enhanced mass invasion to the skin, subcutaneous layer and adjacent galeal layer. Scalp DFSP is very uncommon but is an aggressive tumor, so MR imaging diagnosis of the extent of the lesion to underlying structures, and initial wide local resection is important to prevent recurrence.

  • 목적 현재 3.0T MRI system은 세계적으로 개발이 진행되고 있는 가운데, 3.0T에서 사용 할 수 있는 RF coil의 개발이 시급한 상황이다. 1.0T 및 1.5T MRI 와는 달리 3.0T에서 사용할 수 있는 Body coil 및 그에 따른 High power RF amplifier 제작에 많은 제약이 있다. 작은 용량의 RF amplifier를 이용하여 신체의 부분을 촬영 하고자 한다면, Tx/Rx 가능한 coil을 이용하면 가능할 것이다. 이러한 이유로 본 연구에서는 Tx/Rx 가능한 Quadrature type Th-spine RF coil을 설계, 제작하여 3.0T 고자장 자기공명 영상장치에서의 임상진단 활용범위를 확대하였다. 3.0 Tesla 자기공명 영상장치에 사용을 위한 Quadrature type의 L-spine TX/RX RF 코일을 개발하여 고자장 자기공명 영상장치에서의 임상진단 활용범위의 확대를 목적으로 한다.

Quantitative evaluation of MRI distortion using orthopedic prosthetic metal (정형보철용 금속을 이용한 자기공명영상왜곡의 정량적 평가)

  • Despite the many advantages of magnetic resonance imaging in orthopedic prosthetic body image distortion to the differences in the magnetic susceptibility occurs. Attached to the phantom and pork produced by the same $65{\times}15{\times}2mm$ stainless steel and titanium specimen examined the relationship between magnetic resonance imaging and phantom images, the signal intensity changes of the subcutaneous tissue, fat-suppressed quantitative assessment of the degree through the length of image distortion and pig bones. Stainless steel to titanium to 2.8 times 4.4 times in the longitudinal direction than in the direction of the height of large image distortion, signal strength is relatively low 58.5%. Normal 56.2% compared to the subcutaneous tissue, fat-suppressed, were stainless steel 16.04%, 54.53% titanium. Experimental results than the diagnostic value of magnetic resonance imaging (MRI) images of stainless steel with a titanium metal if better could see.

A Study on Indirect Attachment Method of Compensation Materials to Increase Signal Intensity in Magnetic Resonance Imaging (자기공명영상검사 시 신호강도를 높이기 위한 보상물질의 간접부착 방식에 관한 연구)

  • Previously, studies on compensation material to increase the signal intensity have been conducted which does not affect the reading of images. However, the compensation material has a concern on patient infection as it is attached directly on the skin. Therefore, in this study, we tested an indirect attachment of the compensation material as an alternative method of the direct attachment. The silicon compensation material was fabricated in the form of a cylindrical bar and attached to each element of the 8 channel head coil. Then the signal intensities of the water phantom pre and post application of the silicon were measured. T1 and T2-weighted images were acquired using an 8-channel head coil and a 3.0T superconducting MRI. Signal intensities were measured by using an image measuring program. Paired t-test was used to verify if there were significant differences. The signal intensity before application of the silicon was significantly increased by 3.39% and 2.62% in T1 and T2 weighted images, respectively. Although the indirect attachment method had a limitation to completely replace the existing method, it was considered to be useful in patients with infectious diseases such as diabetic complications since it had a meaningful improvement in signal intensity based on the filling factor increase.

Intradural Extramedullary Ependymoma with Hemorrhage: A Case Report (출혈을 동반한 경막내 수외 뇌실막세포종: 증례 보고)

  • In adults, spinal ependymomas are usually found in intramedullary locations. However, intradural extramedullary spinal ependymomas are rare. Additionally, spinal ependymomas usually show iso to hypointensity on T1-weighted images without hemorrhage. Herein, we present a rare case of a 43-year-old female with a pathologically confirmed intradural extramedullary ependymoma that showed hyperintensity on T1-weighted imaging accompanied by hemorrhage.
