Community-based Knowledge Networks: an Australian case study (커뮤니티 기반 지식 네트워크: 호주 사례 연구)

  • This paper reports on a structural view of a knowledge network comprised of clubs and organisationsexpressly concerned with cultural activities in a regional Australian city. Social network analysis showed an uneven distribution of power, influence, and prominence in the network. The network structure consisted of two modules of vertices clustered around particular categories of creative arts and these modules were linked most frequently by several organisations acting as communication hubs and boundary spanners. The implications of the findings include 'network weaving' for improving the network structure and developing a systemic approach for exploring the structures of social action that form community-based knowledge networks.

Full mouth prosthetic rehabilitation based on systemic analysis, diagnosis and treatment plan: a case report (체계적 분석, 진단, 치료 계획을 통한 전악 보철 재건 증례)

  • Data collection including checking the problem list and analyzing the etiology cannot be overemphasized in full mouth rehabilitation cases. It is well reported that favorable prognosis of prosthodontic treatment depends on accurate diagnosis. Seventy seven year old female patient presented with chief complaints of chewing pain on maxillary and mandibular left premolars and unesthetic existing prosthesis. Clinical evaluation, radiographic examination and cast analysis were performed to gather comprehensive data and problem lists. Treatment planning was based on collected data and related articles. Abutment evaluation, prognosis depending on various types of prosthesis, and maintenance were considered as well. Occlusal vertical dimension was increased to solve esthetic dissatisfaction and insufficient interocclusal space. Patient adaptation was verified with provisional restorations followed by diagnostic wax up. Function, esthetics and occlusal stability were verified during 8 week follow-up period. Prosthodontic reconstruction based on systemic analysis, diagnosis, and treatment plan led to satisfactory result after delivery of definitive prosthesis.

An Analysis of Students' Systemic Thinking and Teachers' Reflective Thinking after the Lesson of Digestion, Circulation, Respiration and Excretion of Middle School Science (중학교 과학 소화순환호흡배설 단원 수업 후 학생들의 시스템적 사고와 교사의 반성적 사고 분석)

  • This study aims to analyze middle school students' systemic thinking to express organic relations between different organ systems about the movement of food and air and teachers' reflective thinking about science teaching through their reflective journals after the lesson of digestion, circulation, respiration and excretion. Firstly, when investigating the moving route of hamburger eaten inside the body, students expressed the names, locations and forms of organs in the digestive system more than those in the circulatory system or the excretory system. When investigating the moving route of a painkiller taken inside the body, students seemed to have more difficulty in expressing the related organ systems than when investigating the moving route of other things, and they mostly drew pictures of organs in the digestive system as done for the moving route of hamburger. However, when investigating the moving route of water drunk inside the body, students mostly described organs in the digestive system but drew more pictures of organs in the excretory system, than when investigating the moving route of other things. When investigating the moving route of air inhaled inside the body, students mostly drew pictures of organs in the respiratory system, but the rate of their drawing pictures of circulatory organs was low. Secondly, this study analyzed one of the teachers' reflective journals, named Mr. Park. According to his journal, students showed different levels of understanding of organ names, depending on their degrees of familiarity with each organ, and in regard to the locations of organs, science teachers mostly aim to achieve learning objectives so much that they often forget to instruct the locations of organs in fact. As for the forms of organs, science teachers mostly spend so much time explaining the functions of organs that they often forget to describe the exact form of each organ.

A Study on Eco-systemic Factors that affect the job satisfaction of the care givers who involved in long-term care service center (요양보호사의 직무만족에 영향을 미치는 생태체계적 요인에 관한 연구)

  • The purpose of this study is to examine the eco-systemic factors that affect the care givers' job satisfaction, and to attain the real data that is useful for increasing the care givers' job satisfaction. The subjects were 195 care givers who involved in long-term care service center in D city. Collected date were analyzed through frequency, percentage, correlation and hierarchical regression analysis. The results are as follow. The factors that affect the care givers' job satisfaction are the age of care givers, the levels of self-esteem, the respect from client, and the levels of support received from friend. The implication of our findings is that for the puporse or increasing the care givers' job satisfaction, the education programs for the care givers and client are necessary.

  • Personality characteristics of orofacial pain patients was analyzed psychologically by means of the SCL-90-R. 36 TMD patients, 20 burning mouth syndrome(BMS) patients, 31 trigeminal neuralgia(TN) patients, 20 control I and 28 control II were subjected at Orofacial pain clinic, Department of Oral Medicine and Health Promotion Center, Pusan National University Hospital during the period from 1998 to 1999. The obtained results were as follows: 1. Mean values of T-scores on 9 basic scales in all the groups were within normal range. 2. The T-scores of SOM, O-C, ANX and HOS in TMD patient group were significantly higher than those in control I group, but there was no significant difference in all scales between TMD patient group and control II group. 3. The T-scores of SOM in BMS patient group was significantly higher than those in control I group, but there was no significant difference in all scales between BMS patient group and control II group. 4. The T-scores of SOM in TN patient group was significantly higher than those in control II group. 5. As compared with present and absent of the history of systemic diseases, there was no significant difference of the scales in TMD, BMS and TN patient groups but the T-scores of the patient groups with the history of systemic diseases tended to higher than those of the patient group without the history of systemic diseases. 6. As compared with acute and chronic groups, the T-scores of O-C, I-S, PAR, PSY in chronic BMS patient group were significantly higher than those in acute BMS patient.

Incidence and Risk Factors for Leptomeningeal Carcinomatosis in Breast Cancer Patients with Parenchymal Brain Metastases

  • Objective : The objective of study is to evaluate the incidence of leptomeningeal carcinomatosis (LMC) in breast cancer patients with parenchymal brain metastases (PBM) and clinical risk factors for the development of LMC. Methods : We retrospectively analyzed 27 patients who had undergone surgical resection (SR) and 156 patients with whole brain radiation therapy (WBRT) as an initial treatment for their PBM from breast cancer in our institution and compared the difference of incidence of LMC according to clinical factors. The diagnosis of LMC was made by cerebrospinal fluid cytology and/or magnetic resonance imaging. Results : A total of 27 patients (14%) in the study population developed LMC at a median of 6.0 months (range, 1.0-50). Ten of 27 patients (37%) developed LMC after SR, whereas 17 of 156 (11%) patients who received WBRT were diagnosed with LMC after the index procedure. The incidence of LMC was significantly higher in the SR group compared with the WBRT group and the hazard ratio was 2.95 (95% confidence interval; 1.33-6.54, p<0.01). Three additional factors were identified in the multivariable analysis : the younger age group (<40 years old), the progressing systemic disease showed significantly increased incidence of LMC, whereas the adjuvant chemotherapy reduce the incidence. Conclusion : There is an increased risk of LMC after SR for PBM from breast cancer compared with WBRT. The young age (<40) and systemic burden of cancer in terms of progressing systemic disease without adjuvant chemotherapy could be additional risk factors for the development of LMC.

Systemic Inflammation Response Syndrome Score Predicts the Mortality in Multiple Trauma Patients

  • Background: Numerous statistical models have been developed to accurately predict outcomes in multiple trauma patients. However, such trauma scoring systems reflect the patient's physiological condition, which can only be determined to a limited extent, and are difficult to use when performing a rapid initial assessment. We studied the predictive ability of the systemic inflammatory response syndrome (SIRS) score compared to other scoring systems. Methods: We retrospectively reviewed 229 patients with multiple trauma combined with chest injury from January 2006 to June 2011. A SIRS score was calculated for patients based on their presentation to the emergency room. The patients were divided into two groups: those with an SIRS score of two points or above and those with an SIRS score of one or zero. Then, the outcomes between the two groups were compared. Furthermore, the ability of the SIRS score and other injury severity scoring systems to predict mortality was compared. Results: Hospital death occurred in 12 patients (5.2%). There were no significant differences in the general characteristics of patients, but the trauma severity scores were significantly different between the two groups. The SIRS scores, number of complications, and mortality rate were significantly higher in those with a SIRS score of two or above (p<0.001). In the multivariant analysis, the SIRS score was the only independent factor related to mortality. Conclusion: The SIRS score is easily calculated on admission and may accurately predict mortality in patients with multiple traumas.

  • The purpose of this study is to examine the eco-systemic factors that affect the independent will of female single parent householder The subjects were 176 female single parent householder who reside in public facilities for single mother family. Collected date were analyzed through frequency, percentage, correlation and hierarchical regression analysis. The results are as follow. The factors that affect the female single parent householder's independent will are the age, duration of living in public facilities, levels of self-efficacy, the ability to cope with problem-solving, and the levels of support received from extended family. According to these findings, intervention strategies that focus on increasing the female single parent householder's independent will are suggested.

The Role of Primary Tumor Resection in Patients with Pleural Metastasis Encountered at the Time of Surgery

  • Background: Evidence is lacking on whether the resection of lung parenchymal cancer improves the survival of patients with unexpected pleural metastasis encountered during surgery. We conducted a single-center retrospective study to determine the role of lung resection in the long-term survival of these patients. Methods: Among 4683 patients who underwent lung surgery between 1995 and 2014, 132 (2.8%) had pleural metastasis. After excluding 2 patients who had incomplete medical records, 130 patients' data were collected. Only a diagnostic pleural and/or lung biopsy was performed in 90 patients, while the lung parenchymal mass was resected in 40 patients. Results: The mean follow-up duration was 29.8 months. The 5-year survival rate of the resection group (34.7%±9.4%) was superior to that of the biopsy group (15.9%±4.3%, p=0.016). Multivariate Cox regression analysis demonstrated that primary tumor resection (p=0.041), systemic treatment (p<0.001), lower clinical N stage (p=0.018), and adenocarcinoma histology (p=0.009) were significant predictors of a favorable outcome. Interestingly, primary tumor resection only played a significant prognostic role in patients who received systemic treatment. Conclusion: When pleural metastasis is unexpectedly encountered during surgical exploration, resection in conjunction with systemic treatment may improve long-term survival, especially in adenocarcinoma patients without lymph node metastasis.

A Study on Urinary Tract Infections in Intensive Care Unit Patients with an Indwelling Urinary Catheter

  • This study was performed in order to evaluate the risk factors for nosocomial urinary tract infection and the frequencies of organisms isolated, and to provide the epidemiologic and basic data of hospital acquired urinary tract infection in intensive care unit. A prospective analysis was performed with 1,235 urine samples following urinary bladder catheterization in 569 patients, who had no evidence of UTI at the time of catheter insertion, admitted to intensive care unit in Pusan P hospital between June 1997 and May 1998. To identify risk factors for UTI, clinical characteristics of infected patients were analyzed. We analyzed these data by percentage, chi-square and odd ratio. Obtained results were as follows: A total of 569 patients (male 341 and female 228) were an average age of 50.8 years and catheterization of 8.04 days. Incidence of UTI was 16.1% (199/1,235) and The risk factors of UTI were duration of catheterization over 7 days, no use of systemic antibiotics, summer and female, and During the first 7 days these risk factors were no use of systemic antibiotics, summer, place of first catheter insertion (ICU) and type of intensive care unit (NSICU). A total of 220 the isolated strains were Gram negative rod 83 (37.7%), yeast like fungi 74 (33.6%) and Gram positive cocci 63 (28.6%). The common organisms isolated were Enterococcus faecalis 23 (10.5%), Serratia marcescens 19 (8.6%), Pseudomonu spp.17 (7.7%), E. ooh 16 (7.3%), Staphylococcus epidemidis 11 (5.0%) mdklebsiellapneumoniae 8 (3.6%). Therefore, in these results 199 of 569 (35%) patients in ICU with indwelling urinary catheter developed UTI. The risk factors for UTI are prolonged duration of catheterization, no use of systemic antibiotics, summer, and female.
