호스피스 자원봉사자를 위한 웹기반 학습프로그램 개발 및 효과분석 (Development and Evaluation of a Web-based Multimedia Content for the Education of Hospice Volunteers)

  • Objectives: This study was to develop and evaluate a web-based multimedia content for the education of hospice volunteers. Methods: The multimedia content was developed based on Baik's teaching and learning structure plan model. The developed program was evaluated by 24 hospice volunteer university students. Results: On main page, there were 4 menu bars that consisted of a lecture guide, cyber lecture, pause and quiz. In the operation of the web based multimedia content, HTML, Java Script, Photoshop and multimedia technology were utilized. There were significant differences in the test scores, before and after using this web-based learning program. After using a web-based learning program, scores of students were much higher. Conclusion: The developed web based program is based on systematic structure and it can provide hospice volunteers with the educational flexibilities and will be help for them to perform the more efficient hospice care.

일부지역 노인요양기관 종사자의 구강보건지식 및 교육 필요도 (Oral health knowledge among nursing home employees and needs to educate on elderly oral care)

  • Objectives: The purpose of this study is to analyze the oral health knowledge among workers at nursing homes, and needs to educate them on elderly oral care. Methods: A self-reported questionnaire was filled out by 241 workers at nursing homes in Gwangju and Jeonnam, Korea from May to June, 2016. The data were analyzed with the SPSS 19.0 program for independent t-test or one-way ANOVA analysis. Results: The data analysis showed that workers at nursing homes had lower knowledge about periodontal diseases (65.1%) than about denture management (78.5%). The knowledge of periodontal disease and denture management of workers with experience of oral care education for elderly was significantly higher than that of non-experienced ones (p<0.01). On the question whether those workers should get oral health care education is necessary or not, respondents answered positively (They scored 4.10 points on the 5-point Likert scale from 1 point - Not at all to 5 points - definitely yes). Conclusions: It is necessary to provide elderly oral care education to workers at nursing homes. Furthermore, oral health professionals should be deployed for systematic and practical oral care for elderly.

응급환자 간호를 위한 시뮬레이션 교육효과: 메타분석 (Effects of Simulation-Based Education for Emergency Patient Nursing Care in Korea: A MetaAnalysis)

  • Purpose: The purpose of this review was to evaluate the effects of emergency nursing simulation program on nursing students and nurses. Methods: This systematic review was performed as per the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and using the R program meta package (version 4.9-2). RISS, KISS, and DBpia Library databases were searched for studies published between June 2000 and August 2018 using the following key words: ($Emerge^*$ OR nursing) AND ($nurs^*$ OR simulation). Selected studies were assessed for methodological quality using Risk of Bias for Non randomized Studies. Results: 7 studies were identified and all of them met the inclusion criteria. The outcome variables were significant clinical performance, self-efficacy except knowledge, and problem-solving ability. Conclusion: This review provides updated evidence of the simulation-based education program in emergency nursing. Further studies are required to increase generalizability using randomized population, research design and controlled trials with sufficient sample size. Moreover, valid measurements are needed to assess the main outcomes.

A Study on the Effectiveness of Skin Care Solution System using Non-Invasive Air Technology

  • The effectiveness of an innovative skin treatment system that delivers an anti-aging solution deep into the skin without invasiveness and pain using a non-invasive air technology was investigated. In addition, an effective change using a non-invasive technique for delivering a solution for skin improvement was confirmed. The equipment named Cellre Jet is an effective skin care and drug delivery equipment that instantly opens the skin epidermis by using a maximum output pressure of 6 bars and high-pressure purified oxygen of 75-90% purity to deliver various nano-sized vital substances deep into the skin, and it uses the method of precisely controlling the equipment through an 8-inch digital touch display to accurately dispense the prescribed dosage. In this study, changes in skin condition were analyzed using this equipment and nano ampoules on subjects with actual skin problems through a related comparison and effectiveness judgment program. Through this study, skin care and drug delivery are possible, which will contribute to verifying the effectiveness of this non-invasive drug delivery equipment in the future, and is expected to establish the systematic effect in observing and studying changes in the skin.

골관절염 대상자에게 간호중재로 적용한 보완대체요법의 효과: 체계적 문헌고찰 (Effects of Complementary and Alternative Therapies Applied as Nursing Interventions in Patients with Osteoarthritis: A Systematic Review)

  • Purpose: The purpose of this review was to analyze the effectiveness of complementary and alternative therapies (CAT) as nursing interventions for patients with osteoarthritis (OA). Methods: A systematic literature review was conducted using the PRISMA statement guidelines. To search for studies related to the effects of CAT applied as nursing care in OA patients, a combination of the keywords 'osteoarthritis,' 'complementary and alternative therapy,' and 'nursing care' were used. Finally, 12 articles retrieved from five electronic databases were included for the analysis. Results: Twelve studies were classified into seven interventions: Tai chi exercise, yoga, qigong, massage. flax seed compress, music, and meditation relaxation, administered by a nurse, and were RCTs. Outcome variables used for pain evaluation were WOMAC and VAS. The duration of interventions varied from 2 to 12 weeks. All twelve interventions had a positive effect on the outcome variables. Conclusion: CAT applied in nursing care for patients with OA significantly reduced pain and improved mobility. When planning a nursing intervention program, it is recommended to combine multiple therapies, considering the duration of effects. In addition, it is recommended to design the study as an RCT to secure the evidence for practical application.

민간 의료기관 결핵관리의 오늘과 내일 (Tuberculosis Management of Private Health Care Institution : Current Situation and Task)

  • Since the inception of the National Tuberculosis Control Program in 1962, the incidence of tuberculosis and its associated mortality has declined dramatically due to effective anti-tuberculosis drugs and a systematic control program. The prevalence of radiographically active tuberculosis has fallen from 5.1% in 1965 to 1.0% in 1995. However, tuberculosis is still a major problem, as the mortality rate is still higher compared to other developed countries. Furthermore, tuberculosis is currently re-emerging in HIV/AIDS epidemic countries. In order to lower the tuberculosis death rate to the levels of developed countries, the tuberculosis control efforts in private healthcare institutions and the national tuberculosis control program in the public sector, need to work together more effectively and efficiently. In this paper, the quthor reviewed the current situation regarding tuberculosis management in private healthcare institutions of Korea based on the literature and the National Health Insurance Claim data, and the future tasks of tuberculosis management are suggested.

한국농촌노인의 건강증진관리요구에 관한 연구 (A Study on Health Promotion Needs Assessment of the Rural Elderly in Korea)

  • This study was purposed to find health promotion and care needs of the elderly in rural area of Korea. As the rural elderly are limited in accessibility to health care resources and could not immediately solve their health care needs when they need. health promotion and care services are expected to bring better and more practical solutions of their health care needs. Thus, the type of health care services to be developed in Korea rural area is discussed to have emphasis on health care service component in addition to health promoting components. Methods of this study was based on survey data analysis : total 322 persons aged older than 55 living at one 'Kun' in Korea administrative unit were interviewed by health workers working at the region and also get trained for this study data collection. The data collection interview was continued from February till May in 1996. The interview questions were modified with adjustment to Korea situation. with basis of the WHO's health promotion program components. The collected data were analyzed using SAS program for frequency, correlation, regressions. The major findings were as follows : (1) $74.8\%$ of the surveyed were sick at the survey time point. and $95.9\%$ known the diagnosis name of the disease. The most frequently complained diseases were Muscular-Skeletal diseases $(43.7\%)$. $34\%$ of those sick had never treated or discontinued therapeutic procedures. so that shown the necessity of systematic and usual health care services with health promotion program development for the elderly. (2) The percent of those who make social participation was $95.3\%$. and the activities were visiting neighbors $(70.4\%)$ and lack of qualified social activity programs. (3) $78.1\%$ of the surveyed had health counseling and education from professional health workers. Those ceased smoking and drinking were $59.6\%,\; 60.3\%$. respectively. Those had no application of therapeutic drugs or nutrion supplements was $40.7\%\;and\;94.1\%$ had regular meals. Those practiced exercises was low remarking $17.7\%$. (4) Positive health behaviors were better carried out by sick groups than by the healthier. except smoking. regular meals. and exercise. $17.5\%$ of sick group smoke more than one case of cigarettes. in contrast to $9.5\%$ of the healthier. (5) Mental health status was heathier among positive health behavior earners. Health counseling and education shown better score of mental health than those never counseled. (6) Positive health behavior practice frequency did not show significant differences when crossed by social activity participation status. (7) Health behaviors of the rural elderly people were carried out better when they had positive 'continuency in therapeutic procedure' 'health status'. 'familial relationship'. 'Health Status' of the rural olderly were explained by 'exercise'. 'drinking'. 'familial relationship'. 'activities of daily living'. Thus, health behaviors practice mutually interact with health status. In conclusion. the health promotion and care program component are recommended to include ation on the necessity of positive health promotion active social acitivities. pleasant life style, adaption into changes on the elderly, safety in residential area. community acitivity and resource utilization. etc .. in addition to the elderly's disability and sickness caring services.

보건의료정보관리 실습교육을 위한 실습모델 연구 (A Study on the Practice Model for Practical Education for Health and Medical Information Management)

  • 본 연구에서는 보건의료정보관리사 양성을 위한 대학에서 EMR 교육시스템을 활용하여 교육할 수 있는 실습모델을 연구하였다. 현재 보건의료정보관리사의 직무역량 강화를 위해 도입된 보건의료정보관리교육 평가·인증 기준에 보건의료정보관리를 위한 실습과정이 제시되어 있지 않다. 이에 따라서 보건의료 정보관리교육 평가·인증 편람에서 교육환경으로 제시한 실습프로그램을 EMR 교육시스템에서 실습할 수 있도록 프로그램을 구성였다. 또한 프로그램별로 보건의료정보관리 현장실습지침서에 따라 수행할 수 있는 실습모델을 연구하였다. 보건의료정보관리 교육용 EMR시스템을 이용하여 마스터데이터관리, 환자 등록, 의사처방, 진료비 수납, 건강보험청구관리, 서식관리, 퇴원등록, 암등록, 미비기록관리, 보건의료데이터관리, 보건의료통계, 정보보호/보안관리에 대한 실습을 수행할 수 있도록 실습모델을 연구하였다. 대학에서 체계적이고 표준화된 보건의료정보관리 실습과정을 수행하여 보건의료정보관리 교육의 질적 수준을 높임으로써 보건의료정보관리 전문가로써 역할을 수행할 수 있을 것이다. 이에 따라서 보건의료정보관리사의 실습교육을 통하여 의료데이터 분석을 기반으로 의료서비스를 개발하고 관리하는 보건의료 정보관리 전문가를 양성할 수 있도록 해야 할 것이다.

방문교육지도사가 인지하는 여성 결혼이민자의 교육요구 (A Study on Educational Needs in Married Immigrant Women Perceived by Their Home Visiting Educator)

  • Purpose: This study was conducted to identify the educational needs of married immigrant women perceived by their home visiting educator. Methods: Data were collected using in-depth interviews and structured questionnaires during July 1st to 28th, 2009. 5 HVEs participated in focus group interview, and 27 HVEs in the questionnaire survey. Data were analyzed by content analysis and descriptive statistics using SPSS/WIN program. Results: Educational needs of Married Immigrant Women were conjugal and family relationship, child rearing and child health care, health management, sex education, and self-identification. There is no proper teaching materials about child rearing for HVEs. The educational needs of HVEs were physical and cognitive characteristics of child development, developmental screening skill and play according to the each developmental stages of the multi-cultural families. Conclusion: Family relationship, child rearing and child health care, must be stressed in educational program for Married Immigrant Women. To maximize the effectiveness of support for multi-cultural families, it must be developed a systematic educational program and teaching materials according to family developmental stages.

가정간호 사업에 대한 의사, 간호사, 진료관련부서 직원 및 환자의 인식 비교 (A Study on Differences of Opinions on Home Health Care Program among Physicians, Nurses, Non-medical personnel, and Patients.)

  • The government has adopted a policy to introduce Home Health Care Program, and has established a three stage plan to implement it. The three stage plan is : First, to amend Article 54 (Nurses for Different Types of Services) of the Regulations for Implementing the Law of Medical Services; Second, to tryout the new system through pilot projects established in public hospitals and clinics; and third, to implement at all hospitals and equivalent medical institutions. In accordance with the plan, the Regulation has been amend and it was promulgated on January 9,1990, thus establishing a legal ground for implementing the policy. Subsequently, however, the Medical Association raised its objection to the policy, causing a delay in moving into the second stage of the plan. Under these circumstances, a study was conducted by collecting and evaluating the opinions of physicians, nurses, non-medical personnel and patients on the need and expected result from the home health care for the purpose of help facilitating the implementation of the new system. As a result of this study, it was revealed that: 1. Except the physicians, absolute majority of all other three groups - nurses, non-medical personnel and patients -gave positive answers to all 11 items related to the need for establishing a program for Home Health Care. Among the physicians, the opinions on the need for the new services were different depending on their field of specialty, and those who have been treating long term patients were more positive in supporting the new system. 2. The respondents in all four groups held very positive view for the effectiveness and the expected result of the program. The composite total of scores for all of 17 items, however, re-veals that the physicians were least positive for the- effectiveness of the new system. The people in all four groups held high expectation on the system on the ground that: it will help continued medical care after the discharge from hospitals; that it will alleviate physical and economic burden of patient's family; that it will offer nursing services at home for the patients who are suffering from chronic disease, for those early discharge from hospital, or those who are without family members to look after the patients at home. 3. Opinions were different between patients( who will receive services) and nurses (who will provide services) on the types of services home visiting nurses should offer. The patients wanted "education on how to take care patients at home", "making arrangement to be admitted into hospital when need arises", "IV injection", "checking blood pressure", and "administering medications." On the other hand, nurses believed that they can offer all 16 types of services except "Controlling pain of patients", 4. For the question of "what types of patients are suitable for Home Health Care Program; " the physicians, the nurses and non-medical personnel all gave high score on the cases of "patients of chronic disease", "patients of old age", "terminal cases", and the "patients who require long-term stay in hospital". 5. On the question of who should control Home Health Care Program, only physicians proposed that it should be done through hospitals, while remaining three groups recommended that it should be done through public institutions such as public health center. 6. On the question of home health care fee, the respondents in all four groups believed that the most desireable way is to charge a fixed amount of visiting fee plus treatment service fee and cost of material. 7. In the case when the Home Health Care Program is to be operated through hospitals, it is recommended that a new section be created in the out-patient department for an exclusive handling of the services, instead of assigning it to an existing section. 8. For the qualification of the nurses for-home visiting, the majority of respondents recommended that they should be "registered nurses who have had clinical experiences and who have attended training courses for home health care". 9. On the question of if the program should be implemented; 74.0% of physicians, 87.5% of non-medical personnel, and 93.0% of nurses surveyed expressed positive support. 10. Among the respondents, 74.5% of -physicians, 81.3% of non-medical personnel and 90.9% of nurses said that they would refer patients' to home health care. 11. To the question addressed to patients if they would take advantage of home health care; 82.7% said they would if the fee is applicable to the Health Insurance, and 86.9% said they would follow advises of physicians in case they were decided for early discharge from hospitals. 12. While 93.5% of nurses surveyed had heard about the Home Health Care Program, only 38.6% of physicians surveyed, 50.9% of non-medical personnel, and 35.7% of patients surveyed had heard about the program. In view of above findings, the following measures are deemed prerequisite for an effective implementation of Home Health Care Program. 1. The fee for home health care to be included in the public health insurance. 2. Clearly define the types and scope of services to be offered in the Home Health Care Program. 3. Develop special programs for training nurses who will be assigned to the Home Health Care Program. 4. Train those nurses by consigning them at hospitals and educational institutions. 5. Government conducts publicity campaign toward the public and the hospitals so that the hospitals support the program and patients take advantage of them. 6. Systematic and effective publicity and educational programs for home heath care must be developed and exercises for the people of medical professions in hospitals as well as patients and their families. 7. Establish and operate pilot projects for home health care, to evaluate and refine their programs.
