Study of the Herbal Medicine for Stroke by Korean Standization of Pattern Identification for Stroke (한국형 중풍변증표준안-Ⅱ의 변증별 사용 처방에 대한 연구)

  • This study was performed to investigate using of herbal medicine based on pattern identification of symptoms, such as Qi deficiency pattern, Yin deficiency pattern, Phlegm -dampness pattern, Static blood pattern, Fire-heat pattern. These patterns settled by Korean Standard Differentiation of the Symptoms and Signs for Stroke in 2005. This study was done with 177 patients in Daejeon University Oriental Medical Hospital in the period of November 2006 to July 2007. Among the five types of pattern identification, Phlegm -dampness pattern showed significantly high frequency especially in 3 weeks after stroke. The sort of herbal medicine was 43 and Dodamhwalhyoel-tang(26.61%), Sunhwanki 1 hobang(14.52%), Banhabaekchulcheonma-tang(4.84%), Ansinchongnoi-tang(4.84%), Chongryuldodam-tang(4.03%) were most frequently used. Especially Dodamhwalhyoel-tang showed significantly high frequency in 2 weeks after stroke. Based on these results, it is suggested that more practical Korean Standard Differentiation of the Symptoms and Signs for Stroke would be established through continuous clinical studies by giving weight on relationship between herbal medicine and pattern identification.

  • Many patients have difficulties after stroke. Spasticity is an important one of that difficulties. So in these days, its management is becoming a major issue in rehabilitation. Gagamyounjo-tang was administered to a patient have had spasticity after stroke include Hyulhu(血虛) symptoms. And check the modified Ashworth Scale, Motor grade and VAS for being in the hospital. After treated with Gagamyounjo-tang, Modified Ashworth Scale, Motor grade and VAS have improved, and most of all symptoms about hyulhu(血虛) was disappeared. Based on this result, this study shows that Gagamyounjo-tang was effective in spasticity after stroke especially included Hyulhu(血虛) symptoms.

  • Objectives : This study is to report the effects of Bangkibokryeong-tang(Fanjifuling-tang(BBT)) on stroke patient with Urinary tract infection.(UTI) Methods : A stroke patient diagnosed as UTI was treated with BBT (防己茯笭湯) at one-month intervals. We observed body temperature, amount of urine and recorded photographs of any changes during the treatment period. Treatment efficacy was assessed using urinary analysis. Results : After the treatment, there was improvement in urine analysis. And the symptoms of UTI and her conditions got better. Conclusion : The report suggests that BBT is effective in controlling various symptoms related to UTI with antibiotic resistance on stroke patient.

  • Patients with neurogenic bladder undergo morphometric and functional changes of their bladder and urethra. As a result, voiding symptoms such as frequency, nocturia, urgency and incontinence appear. We experienced two cases of the urinary incontinence treated with Samboo-tang(Sanfu-tang). In the point of Differentiation of Syndrome, two cases were diagnosed as Deficiency Syndrome of Yang of the kidneys. After Samboo-tang(Sanfu-tang) administration, the frequency of the urinary incontinence were decreased and the accompanied symptoms also improved. So, we report two cases with a brief view of related literatures.

  • Stroke is the second leading cause of death in Korea and the leading cause of long-term disability. To develop the Korean Standard Differentiation of the Symptoms and signs, we investigate symptoms frequency and professional's symptoms weight in 248 stroke patients within 1 month of onset. We take professional advice by e-mail.

A Quantitative Analysis on Clinical Course of Acute Ischemic Stroke with National Institute of Health Stroke Scale (National Institutes of Health Stroke Scale을 이용한 급성기 허혈성 뇌졸중의 증상 경과에 대한 정량적 고찰)

  • Purpose : Studies on stroke were conducted by using the scaling system to estimate, compare severity and to predict prognosis. National Institute of Health Stroke Scale is widely used for this purpose due to its accuracy in predicting clinical symptoms. According to previous studies, neurologic symptoms in acute stage of ischemic stroke did not disappear, and approximately 27% to 43% of them were aggravated, We conducted this study to define clinical aggravation and to improve ischemic stroke in its acute stage by using NIHSS. Methods and Subjects : Due to their acute ischemic stroke, twenty five patients visited Wonkwang Oriental Medicine in Cheonju within forty eight hours from its beginning stage. Two oriental medicine doctors checked NIHSS every six hours during the first ninety six hours and later every twenty four hours for twelve days from the beginning of the disease. Results : The Aggravated group consisted of nineteen out of twenty five cases, which is seventy six % and NIHSS on admissions were $5.88{\pm}0.63$ and it increased linearly over time by the equation of NIHSS on admissions was $8.50{\pm}2.02$ and it decreased linearly over time by the equation of NIHSS=-0.01 one hour+7.91. Conclusion : During the study, we defined the clinical courses of acute ischemic stroke. There have been no reports on this so far. Therefore, we hope that this study will be beneficial to expand the scope of oriental medicine in dealing with aggravating ischemic stroke in the acute stage.

Factors Associated with Unawareness of Early Symptoms of Stroke and Myocardial Infarction in Adults with Hypertension and Diabetes: Focused on Management related to Disease (고혈압, 당뇨병 성인의 뇌졸중 및 심근경색증 조기 증상 비인지 관련 요인: 질병 관리 관련 특성을 중심으로)

  • The purpose of this research was to determine the degree of awareness of early symptoms of stroke and myocardial infarction in adults diagnosed with hypertension and diabetes, and to analyze factors related to unawareness. Raw data from the 2017 Community Health Survey were utilized. A total 12,277 adults older than 40 years were included in analysis finally. The early symptom awareness rates of stroke and myocardial infarction were 53.6% and 46.8%, respectively. Logistic analysis was performed to assess factors associated with unawareness of early symptom of stroke and myocardial infarction. After correcting for socio-demographic variables, education for the management of hypertension, recognition of blood pressure level and HbA1c level were significantly related to stroke early symptom unawareness on multivariate analysis. (Education for the management of hypertension, recognition of blood pressure level and HbA1c level were significantly related to the unawareness of early stroke symptoms even after socio-demographic variables were corrected in multivariate analysis.) Recognition of blood pressure level and HbA1c level were also significantly associated with unawareness of early symptoms of myocardial infarction. Therefore, it is required to interventions and strategies to improve the level of awareness of early symptoms of cardio-cerebrovascular disease, such as accelerating promotion and strengthening education for high-risk groups who must manage both hypertension and diabetes.

A Study of Correlation Between Change in Pattern Identification and Scandinavian Stroke Scale in Ischemic Stroke Patient Who had Receive Traditional Korean Medical Treatment and Conservative Treatment (한·양방 병행치료를 시행한 뇌경색 환자의 변증지표 변화와 Scandinavian Stroke Scale의 상관관계에 대한 연구)

  • Object : The purpose of this study is to observe the correlation between change in pattern identification symptoms and scandinavian stroke scale in ischemic stroke patient who had receive traditional Korean medical treatment and conservative treatment. Methods : 43 subjects were recruited from patients with ischemic stroke within 30 days of onset. We chose the subjects who had at least one follow up session and had checked the score between last follow up session and first session in pattern identification and scandinavian stroke scale. We also assessed the correlation between pattern identification and scandinavian stroke scale. Results : There were significant negative correlation between pattern identification and scandinavian stroke scale in Fire-heat pattern and positive correlation in Dampness-phlegm pattern. Conclusions : This study provides evidence that collaborative treatment maybe effective in improving neurologic symptoms in ischemic stroke patients diagnosed as Fire-heat pattern. Further studies with larger scale and longer observation period, more neurologicscales scales, control group would be required.

Activities of Daily Living and Fatigue in the Stroke Patients (뇌졸중 환자의 일상생활동작 수행정도와 피로감에 관한 연구)

  • Purpose: The purpose of this study were to describe the level of activities of daily living (ADLs) and fatigue of stroke patients and to identify related factors of ADLs and fatigue. Method: A sample of 132 were used who were recovering from stroke. The face-to-face interviews were conducted to collect data. The levels of ADLs and fatigue were evaluated using the Modified Barthel Index and Kim's fatigue scale respectively. A series of t-test and analysis of variance analyses were conducted to examine study purposes using SPSS 15.0. Results: The levels of fatigue and ADLs were 65.6 (SD=16.52) and 74.6 (SD=22.32) respectively. Significantly poorer ADLs were found: women (t=2.05, p=.001), older people aged ${\geq}$ 70 years (F=2.74, p=.046), the duration of onset (F=4.34, p=.006), the use of assist devices (F=35.64, p<.001), the parts of paralysis (F=4.25, p=.007), the time to attack (F=3.34, p=.039), and accompanying symptoms (F=15.23, p<.001). There was a significant difference in fatigue with patients with lower accompanying symptoms having lower fatigue (F=11.08, p<.001). Conclusion: These findings suggest gender, the duration from onset, the use of assist device, the parts of paralysis, the time to attack and accompanying symptoms were significant factors of the ADLs and fatigue post stroke. These factors should be considered when developing and testing nursing intervention programs for stroke survivors.

A Case of Bradycardia with Atrioventricular Block Treated with Korean Medicine (방실차단에 의한 서맥을 보이는 환자 한방 치험 1례)

  • This is a case report of a 57-year-old patient diagnosed with bradycardia and atrioventricular (AV) block. The chief complaint of the patient was palpitation and dizziness. An electrocardiogram (ECG) showed a second degree AV block with 2:1 AV conduction. The patient was treated with Korean medicine, including herbal medicine, for 35 days. After 2 weeks of treatment, the heart rate increased from 33 to 56 and the follow-up electrocardiogram showed normal findings. The symptoms of palpitation and dizziness were also improved. The effect on heart rate, ECG, and symptoms lasted until the end of the treatment. This case report demonstrated that Korean medicine could be effective for treatment of symptomatic bradycardia with AV block.
