  • Cleidocranial dysplasia (CCD) is an autosomal dominant skeletal dysplasia and is caused by mutation in the CBAFA1 gene of 6p21 chromosome band. Patients with CCD express skeletal dysplasia such as hypoplastic/aplastic clavicle, brachycephalic skull, midface hypoplasia and moderate short stature. In addition to skeletal dysplasia, specific symptoms may appear in respiratory organs, auditory area, and the more distinguished, dentition. Dental findings include: delayed eruption of permanent tooth, multiple supernumerary tooth more than five, malocclusion, etc. In Patients presenting excessive SNT, complications of SNT could be prevented and will be managed through pertinently timed treatment such as tooth extraction, using space maintainer, and orthodontic management after early diagnosis. This case is about the treatment of eruption disorders in permanent teeth owing to SNT in CCD patients, who are three family members in the $3^{rd}$ generation inherited from maternal grandfather through atavism. We performed the extraction of numerous SNT and orthodontic treatment on them in this case. On evaluating panoramic and cephalometric views, some classical signs of skeletal dysplasia due to CCD were recognized in a pool of three patients, the clavicle was distinctively displayed in all patients.

  • Background & Purpose : Acupuncture with aromatherapy (AA) is a branch of complementary or alternative therapy recently growing in use. As the use of aromatherapy itself has grown so rapidly in recent years, studies about its safety have been accumulating. However, safety of AA has not been studied clinically and the treatment method which has the needles inserted after spreading essential oils exposes the human body more directly to components than aromatherapy. Therefore, we examined the safety of AA on patients treated thereby and physicians practising it. Methods : We observed all inpatients treated with AA from November 21, 2005 to March 31, 2006, in Kyung Hee Oriental Medical Center. During treatment, we monitored all of the newly developed signs from the patients to assess the adverse effects of AA. The patients' and physicians' general characteristics (gender, age, present illness, and treatment days) were obtained from medical records and analyzed to assess the clinical safety of AA in detail. Results : There were 440 patients treated with AA; clinical adverse effects appeared in 3 of them, which can be assessed as 0.7%. The major symptom was chest discomfort and choking originated from smell aversion condition (2 patients), and the other adverse effect was sensation of itching and reddening. However, none of them seemed to have direct relation with AA. A total of eight physicians applied AA and there were no symptoms related to it among them. Conclusions : We suggest that our results will contribute to confirming the safety of AA by offering clinical evidence.

  • Streptococcus spp. of bacterial pathogen of fish were isolated from the cultured flounder (Paralichthys olivaceus) in fish farm of Jeju Island. Clinical signs of the infected flounder which are the most commons symptoms are as follows: erratic swimming, darkening of the body colour, unilateral or bilateral exophthalmia, corneal opacity, hemorrhages in the opercular and the bases of the fins, and the ulceration of the body surface. Biochemical characteristics of pathogenic fish Streptococcus spp, were gram positive, spherical form, catalase negative, oxidase negative and $\beta$-haemolytic, respectively, The viable cells counted from the tissue of the diseased flounder were the largest in the order of the ulcer, the kidney, the blood and the brain, The drugs used were ampicillin, ciprofloxacin, doxycycline, gentamycin, tetracycline, erythromycin, streptomycin and oxytetracycline, Streptococcus spp. were found to be sensitive to ampicillin, ciprofloxacin, doxycycline and gentamycin, but were resistant to tetracycline, erythromycin, streptomycin and oxytetracycline. The pathogenicity of Streptococcus spp. on the cultured flounder with an abdominal cavity injection was high. The haemolytic activity of the toxin against the sheep red blood cells reached the maximum after 30 min incubation at $37^{\circ}C$ or $50^{\circ}C$. The toxin showed highest activity at pH $5.5\sim6.5$.

  • Anticonvulsants, calcium channel blockers and immunosuppressants are representative drugs related with gingival enlargement. Clinical signs and symptoms caused by drug-induced gingival enlargment frequently appear within 1 to 3 months after medication. At initial stage, it is limited to attached gingiva but may extend coronally and interfere with esthetics, mastication and speech. Interproximal spaces are common beginning area and pathologic teeth migration could be occurred by the lesion. Withdrawal or substitution of medication would be the most effective treatment of drug-induced gingival enlargement. However, periodontal treatment and further supportive periodontal therapy should be provided where change in medication is impossible. The present study reports the cases which show the resolution of inflammation with spontaneous teeth migration without change in medication. In all cases discussed in this report could be efficiently managed with proper periodontal treatment and further supportive periodontal therapy.

  • Objectives: Strokes have diverse symptoms and signs. One of ten stroke patients has chronic pain after a stroke. Pain after a stroke interrupts rehabilitation and worsens quality of life, but there is no efficient treatment for this pain. This study surveyed and reports on the clinical studies of treatment for chronic pain after a stroke. Methods: We searched journals for reports on clinical studies of treatment for chronic pain after a stroke through the databases OASIS (http://oasis.kiom.re.kr), NDSL (ndsl.kr), Kmbase (http://kmbase.medric.or.kr/), and PubMed (http://www. pubmed.com). The search words were "stroke & pain", "jungpung中風 & pain", "pungbi風痺", "cerebral hemorrhage & pain", and "cerebral infarction & pain". Results: Twenty-nine studies of treatment for chronic pain after a stroke were found. Of these, 15 were randomized controlled trials, 10 were nonrandomized controlled trials, and 4 were "before and after" studies. Treatments were diverse, including acupuncture, electroacupuncture, herbal acupuncture, herbal medicine, and more. The treatment periods were longer than 3 weeks on average. Conclusions: These results show that good quality randomized controlled trials of treatment for chronic pain after a stroke are small in number. We need larger and more diverse studies of treatment for chronic pain after a stroke.

  • Bruxism can be generally regarded as a diurnal clenching or nocturnal teeth grinding or a combination of both. Clenching of the teeth is forceful closure of the opposing dentition in a static relationship of the mandible to the maxilla, whereas grinding of the dentition is forceful closure of the opposing dentition in a dynamic maxillo-mandibular relationship as the mandibular arch moves through various excursive positions. The causes of bruxism are not yet discovered clearly, but most consistently mentioned cause is psychological stress. Bruxism can be also associated with sleep disorders, medication, and disturbances of the central nervous system. There is no permanent treatment method of bruxism, so the objectives for management of bruxism are reduction of psychological stress and treatment of signs and symptoms of bruxism by occlusal adjustment, occlusal splint, systemic medication and physical therapy. These cases report present three cases of children with bruxism. The bruxism was reduced in these patients wearing occlusal splint.

  • Coleosporium neocacaliae, a needle rust fungus, was described and illustrated for the first time on the needles of Pinus koraiensis (Korean pine) in Korea. Detailed descriptions of the species were verified based on symptoms and signs, and light and scanning electron microscopic observations on aecia, aeciospores and peridial cells. The present needle rust fungus was apparently different from C. eupatorii, a solely recorded Coleosporium species on P. Koraiensis in Korea until this time, in having comparatively large sized aeciospores as $20-39{\times}14-27\;{\mu}m$ than those of latter species (size of aeciospores $15-27{\times}10-20\;{\mu}m$). Verrucose surface structure of aeciospores in light microscopy was revealed to be annulated with three to four rings and root-like base in scanning electron microscopy. Aecial stage of C. neocacaliae were formed on the middle days of April to the early days of June and collected around the central parts of Korean peninsula, including high mountains of about 1,000 m above sea level.

  • Purpose: The advantages of air reduction are the ease of performing the procedure, reduced radiation time and lower morbidity rate if perforation occurs. But, patients who fail air reduction undergo a laparotomy at which 10% have spontaneously reduced. The first enema decreases the edema and venous congestion of bowel wall, thus repeated reduction may succeed. The aim of this study is to evaluate the efficacy of delayed repeated pneumatic reduction of intussusception in patients with failure of an initial attempt. Methods: Between January 1998 and December 1999, 21 patients with proven intussusception received repeated delayed reduction 1 to 3 hours following the first failed attempt. These patients were in stable condition and did not have peritonitis, shock or toxic sign. Results: Before reduction, the patients had symptoms and signs for 6 to 48 hours (median 24 hr). The interval from the first reduction to the repeated was 1 to 3 hours (median 2.2 hr). The success rate of repeated reductions was 76.2% (16 of 21 patients), and 23.8% (5 of 21 patients) required surgery. Four of 5 patients requiring surgery were manually reduced and 1 spontaneously reduced in the operating room. Conclusion: We recommended a repeated reduction in patients with intussusception who are in stable clinical condition after an unsuccessful initial reduction attempt.

  • Transfusion related acute lung injury (TRALI) is a serious, potentially life-threatening complication of transfusion therapy that is sometimes under diagnosed and under reported. Patients with TRALI present with dyspnea/respiratory distress and fever. The symptoms, signs and chest radiological findings in TRALI are similar to transfusion associated circulatory overload, which makes it is difficult to distinguish it from circulatory overload. Although the mortality rate in cases of TRALI is relatively low, TRALI is the third most common cause of fatal transfusion reactions next to ABO blood type incompatibility and hepatitis. Mild-to-moderate cases of TRALI may be misdiagnosed as volume overload. Recently, we encountered two cases where the patients suffered from dyspnea and fever after a transfusion. and review of the relevant literature.
