중증근무력증의 수술적 치료 결과 (Surgical Results for Myasthenia Gravis)

  • 중증 근무력증은 자가면역 반응에 의해 유발되는 비교적 드문 질환으로, 여러 가지의 치료 방법이 시행되고 있으나 현재는 완전 관해와 증상의 호전이 탁월하다는 점에서 광범위 흉선 절제술을 주된 치료의 방법으로 선택하고 있다. 본원에서는 지난 8년간 16명의 중증 근무력증 환자에 대해 흉선 절제술을 시행하여 결과를 분석하였다. 5례(31%)에서 완전 관해가 보였으며, 7례(44%)에서는 약 투여량의 감소 또는 증상의 호전을 보여 총75%에서 수술적 호전을 보았다 환자의 나이, 성별, 술전 증상의 정도, 수술적 접근 방법 및 조직학적인 결과 등을 분석하였으나 수술 후 예후와는 통계적 상관관계는 보이지 않았다.

Surgical Treatment of Thoracic Menigocele Associated with Neurofibromatosis and Kyphoscoliosis

  • A 46-year-old man presented with a lateral thoracic meningocele associated with cutaneous neurofibromatosis type I and kyphoscoliosis of the thoracic spine upon medical examination. In the majority of such cases, these meningoceles remain asymptomatic, but surgery is indicated when giant or symptomatic cysts are present. The large thoracic meningocele was successfully extirpated through the transthoracic approach in combination with lumbar puncture and cerebrospinal drainage for decompression of the cyst.

Salvage Approaches for Maintaining the Eye Globe in a Glaucomatous Patient with Severe Unilateral Exophthalmos

  • An eight-year-old Shih-Tzu dog was referred to Kyungpook National University Veterinary Medical Teaching Hospital because of severe glaucoma and exophthalmos. The symptoms included severe buphthalmos, conjunctival hyperemia, focal dried cornea and discomfort in the affected eye. Although enucleation was considered as a potential intervention measure in light of the severity of symptoms, it was possible to save the globe and relieve symptoms through evisceration, intraocular prosthesis implantation, and lateral canthoplasty. Our experience with this case suggests that symptomatic salvage therapy, rather than enucleation, is an appropriate approach for resolution of exopthalmos and other complications associated with glaucoma surgery.

Simple bone cyst recurred in adjacent areas: A case report

  • Simple bone cyst is a pseudocyst that typically occurs in patients during their second and third decades of life. This benign entity is an empty or fluid-filled cavity that lacks a true epithelial lining. Simple bone cysts are often asymptomatic and are commonly found in mandibular body, predominantly in the posterior region. The treatment of simple bone cysts can be influenced by factors such as the patient's age, the size of the lesion, and the presence or absence of symptoms. In the case of a simple bone cyst in the mandible that is small and symptomless, a watchful waiting approach may be appropriate. However, if the cyst is large and symptomatic, surgical treatment is recommended. This report presents a radiological examination of a simple bone cyst that developed around the root of the mandibular first molar in a 36-year-old female patient. The cyst recurred in adjacent areas despite surgical treatment.

  • Diverticula of the choledochus, better known as Todani type II cysts, are very rare and represent a predominantly pediatric pathology. Their identification by radiological methods, even if occasional, requires clinical doctors to request a surgical consultation, even for asymptomatic subjects, to proceed with their removal, given the risk of associated neoplasms. The laparoscopic approach for surgical treatment of these cysts has been recently introduced with excellent results. Due to the poor clinical records, currently there are neither shared protocols about their management nor long-term follow-up of operated patients. We report a case of an adult female suffering for years from biliary colic due to the presence of a duodenal diverticulum associated with microlithiasis' cholecystitis, who was laparoscopically treated, with excellent results in terms of symptomatic regression, reduced hospitalization, and no surgery-related complications.

Individualized Therapy to Manage Quadriplegic Spastic Cerebral Palsy with Unani Treatment Modalities

  • Objective: Cerebral palsy (CP) is a neurodevelopment disorder attributed to an insult or injury to the developing brain with abnormalities in muscular tone, movement and motor skill. Improvement in quality of life and ameliorating symptoms can be achieved. Therefore, this case report details a distinctive approach to treating a 5-year-old male child with quadriplegic spastic cerebral palsy utilizing Unani treatment modalities. Methods: The treatment regimen commenced with 'Habb Ayarij for constipation followed by Sharbat Ustukhuddus administered orally. Notably, Sharbat Ustukhuddus was combined with Melia Azedarach L. leaves vapour bath. Subsequently, Roghan Babunna douche was performed followed by Dalk Layyin andcontinued until symptomatic improvement was observed. Majun Falasfa, Khamira Marwareed and Khameera Gauzaban were administered for 30 days. The therapeutic outcome included anthropometrical measurements, developmental milestones, spasm/reflex scale, and muscle power grading. Results and conclusion: Over the course of a 2-year follow-up, several clinical findings emerged. These included notable improvements in anthropometric measurements, developmental milestones such as improved head control and sitting ability, and a reduction in spasticity of the upper limbs, along with decreased muscle spasms. The therapeutic outcome of this individualized and holistic approach is potentially due to the multifaceted properties of medicinal plants (Musakkin wa Muharrik wa Muqawwi-i- A'sab wa Dimāgh, Munawwim, Dafi-i-Tashannuj, Muqawwi-i-Qalb-i-Ruh). Furthermore, the use of Dalk and Naṭūl was instrumental in providing nourishment to musculoskeletal cells and initiating intracellular signaling cascades. While these findings are encouraging, further research in the form of case series andrandomized controlled trials is warranted to validate the efficacy of this unique holistic approach.

관절경하 후격막 통과 도달법을 이용한 후방십자인대 결절종의 치료 (Resection of Ganglion Cyst of the Posterior Cruciate Ligament using Arthroscopic Posterior Trans-septal Approach)

  • 목적: 관절경하 후격막 통과 도달법을 이용한 후방십자인대 결절종의 제거술시 전방 도달법 보다 우수한 접근성 및 제거의 용이성과 그 치료 결과에 대하여 분석하였다. 대상 및 방법: 후방십자인대 결절종의 관절경하 후격막 통과 도달법을 이용하여 제거술을 시행한 7예에 대하여 조사하였다. 증상을 동반한 후방십자인대 결절종에서 동반손상 유무, 전방 접근법과 후격막 통과 도달법의 수술시야의 차이 등에 대하여 비교 분석하고, 결과의 평가는 Lysholm 슬관절 지수를 이용하였다. 결과: 증상이 동반된 7예의 후방십자인대 결절종 중 4예에서 동반 손상이 있었으며, 또한 외상과 연관된 경우도 3예이었다 임상평가 결과, Lvsholm 슬관절 지수는 술 전 평균 93.3점에서 술 후 평균 97.8점으로 향상을 보였다. 결론: 후방십자인대 결절종의 관절경적 제거술시 수술시야의 확보 및 후방구조물의 이상 유무를 확인하기 위해서는 후격막 통과 도달법이 효과적인 술식이라 생각된다.

이상경상돌기증의 치험례 (A CASE OF SURGICAL TREATMENT FOR SYMPTOMATIC ELONGATED STYLOID PROCESS)

  • Styloid process is a slender pointed bone of variable length, which project downward, forward, and slightly medialward from the posteroinferior portion of the tympanic part of the temporal bone. Embryologically, the styloid process is derived from Reichert's cartilage, a structure of second branchial arch origin. Most patients with elongated styloid process remain asymptomatic, but some patients complain pain and sensation of foreign body in the throat, dysphagia, dysphonia, referred otalgia. Recently, we have experienced a case of elongated styloid process in a fifty-nine year old man. He had been suffered from Rt. side sore throat, headache and pharyngeal discomfort during head movement. Through various X-ray examination, Xeroradiography, and palpation of tonsillar fossa, elongated styloid process on Rt. side was confirmed. Under the general anesthesia, it was successfully removed out via transoral approach technique, described by Eagle. The resected styloid process was 2.5cm in it's length and he was freed from the symptoms without further complication.

Pulmonary Nodular Lymphoid Hyperplasia in a 33-Year-Old Woman

  • Pulmonary nodular lymphoid hyperplasia is a reactive lymphoproliferative disease. It is very rare, which means that many aspects of the disease are unknown or have not been proven. Pulmonary nodular lymphoid hyperplasia can be symptomatic or asymptomatic, progressive or not, and solitary or multiple, and a surgical approach is the current treatment of choice. We present a case of pulmonary nodular lymphoid hyperplasia that was visualized as multiple ground glass opacities on a computed tomography (C T) scan, and observed for 1 year because the patient was pregnant. Over this period, the number and extent of the opacities progressed, but no symptoms were reported. A surgical biopsy was done and some remaining lesions regressed on follow-up CT scans, while others progressed, without any appearance of symptoms.

간질의 분류법에 대한 동서의학적 문헌고찰 및 새로운 제안 (A Critical Review on the Epilepsy-related Classification Systems Delineated in the Literatures both Western and East Asian Medicine : A Suggestion to Develope a New Classification)

  • The major purpose of this study is to evaluate the classification of epileptic seizure types and epilepsy described in the literatures of both Western and East Asian medicine, especially based on the two criteria- a theoretical and a practical aspect of the classification systems. Currently, the 1981 classification of epileptic seizure types, and the 1989 classification of epilepsy syndromes and epilepsies which were proposed and approved by the International League Against Epilepsy(ILAE) have been generally accepted worldwide, although a variety of modifications have been consistently suggested. A large proportion of epilepsy cases cannot be easily classified as either 'focal' or 'generalized' or as either 'symptomatic' or 'idiopathic', so they fail to be precisely fallen into any of the ILAE categories. Terms and concepts used in the East Asian medicine are also inadequate to identify epileptic seizure types and epilepsy syndromes as discrete diagnostic entities because of ambiguities in definition and use. Therefore, this article suggests an alternative approach not only more helpful in understanding mechanism of epilepsy but also more easily applicable and effective in clinical value.
