참깨발병 품종저항성 검정방법 연구 (An Evaluation Method for Sesame (Sesamum indicum L.) Resistance to Phytophthora nicotianae var. parasitica)

  • 본 시험은 참깨 발병(병원균 : Phytophthora nicotianae var. parasitica)에 대한 품종저항성 검정방법 확립을 위해 실시하였다. 공식작물의 묘령, 접종방법, 토양수분상태 및 접종원의 농도는 공시한 중도저항성계통 B-67, IS 103과 나병성계통 수원9호, 수원 2006 모두의 병진전속도와 발병심도에 영향을 주는 것으로 나타났다. 그러나, 파종후 20일된 유묘에 1 $m\ell$당 200개 포자상의 접종원을 폿트(8$\times$16.7$\times$7.2cm)당 10$m\ell$씩 토양표면에 접종한 후 저면관수로 토양수분을 포화상태로 유지하였을 때 계통간 저항성의 차이를 갈 볼 수 있었다. 묘령 70일의 성묘를 사용하였을 때는 토양접종을 하였을 때나 분무접종을 하였을 때 모두 계통간 저항성 차이를 잘 볼 수 있었다. 전술한 유묘검정방법에 의해 이병성계통인 수원2006는 이병주율로는 100%, 발병심도로는 최대치인 9.0을 보였다. 반면에 중도저항성계통인 B- 67은 이병수율로는 20%, 발병심도로는 1.7을 보였다. 발병심도는 0에서 9까지 지수화하여 나타내었다. 예를 들면, 발병심도 0은 병징이 없는 상태, 5는 지면부 줄기가 변색된 경우, 9는 잎마름증상과 함께 줄기변색이 지면부로 부터 10cm 이상인 경우이다. 품종간 저항성의 차이는 병총면적률로도 조사를 하였으며, 이 결과는 발병심도로 조사한 검정결과와 잘 일치하였다.

간경변 환자의 증상경험에 영향을 미치는 요인 (Factors influencing Symptom Experience in Patients with Liver Cirrhosis)

  • Purpose: The purpose of this study is to examine the factors influencing symptom experience in patients with liver cirrhosis(LC). Method: A descriptive correlational study design was used. A convenience sample of 129 subjects was recruited from the gastroenterology department at two university hospitals in Seoul, Korea. Symptom experience in LC was measured with the instrument developed by the researcher based on Theory of Unpleasant Symptoms(Lenz et al, 1995) and the Child-Pugh Score, the Korean version of Profile of Mood States, and the Family Support Questionnaire were used to identify the factors influencing symptom experience. Results: The mean score of symptom experience was relatively low(M=41.67, SD= 24.71). Among individual symptoms, fatigue had the highest score in all dimensions. Fatigue, abdominal distension and/or peripheral edema, muscle cramps, dry mouth, and change in appearance were explored as symptoms needing management. In the regression analysis, symptom experience was found to be influenced significantly by anxiety/depression($R^2=.418$, p=.000) and the severity of LC(Child-Pugh Score)($R^2=.125$, p=.000). These variables explained 54.3% of the variance in symptom experience(F=63.607. p=.000). Conclusion: It suggests that nurses need to take into consideration psychological factor as well as physiological factor in symptom management for patients with LC.

부인암 여성의 증상 클러스터(Symptom Cluster) (Symptom Clusters in Women with Gynecologic Cancer)

  • Purpose: Women with gynecologic cancer often experience various physical and psychological symptoms relating to the cancer and its treatment. The purpose of this study was to identify symptom clusters. Method: A survey was conducted on 184 women with diagnoses of cervical, ovarian or endometrial cancer. Fifty symptoms were assessed for prevalence, severity and interference, and symptom clusters were identified. Cluster analysis was done using SPSS version 12.0. Results: Fatigue was identified as the most prevalent symptom (81.52%), lack of vaginal lubrication (2.26) as the most severe symptom, and lack of vaginal lubrication as the most interfering one (2.15). Identified six clusters were: Anorexia-pain cluster (loss of appetite, taste change, weight loss, appearance change, alopecia, weakness, pain), Fatigue cluster (lack of concentration, lack of memory, fatigue, dry mouth), Urinary-bowel distress cluster (urinary difficulty, constipation), Abdominal discomfort cluster (lower abdominal pain, abdominal pain, bloating), Emotional distress (sadness, anxiety-worry, nervousness, restlessness), and Menopausal cluster (sweating, hot flush, fever). Conclusion: The result of this study provides fundamental data to health care professionals in developing interventions for effective symptom management for women with gynecologic cancer by understanding identified 6 symptom clusters.

한방치료로 호전된 임신성 아토피 발진환자의 치험례 (A Case Report of Atopic Eruption of Pregnancy by Traditional Korean Medicine)

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment on a patient with atopic eruption of pregnancyMethods: Patient who diagnosed by atopic eruption of pregnancy was enrolled in this study. She received Korean medicine therapies such as herbal medicine, acupuncture and external application. Sopungchungyoung-tangs and Seungmahwangryeon-tangs were prescribed according to the patient’s constitution which was pungyeol. The severity of symptom was evaluated by the severity SCORing for Atopic Dermatitis index (SCORAD index).Results: Subjective symptom such as Systemic pruritus, sleep loss, and Dermatitis lesions was reduced by Korean medicine therapies.Conclusions: The present study suggests that Korean medicine therapies have a effect on atopic eruption of pregnancy.

  • Objective:The differences of various neurocognitive functions, including attention, memory, motor function, and higher cognitive function were compared between PTSD patients and normal control subjects. Also, correlation with PTSD symptom severity and neurocognitive functions were evaluated between PTSD patients and normal control subjects. Method:We assessed the neurocognitive functions by computerized neurocognitive test(CNT) batteries. The visual continuous performance test(CPT) and digit span test, finger tapping test and Wisconsin card sorting test(WCST) were executed. The Impact of Event Scale-Revised(IES-R) was used in the evaluation of the severity of PTSD. Result:The PTSD patients showed significantly impaired neurocognitive performance in all of the items, compared with normal control subjects. The relation between impairment in neurocognitive functions and symptom severity showed significant correlations. Conclusion:These results imply that PTSD patients have impaired neurocognitive functions concerning with specific brain areas, especially the frontal area. For the thorough evaluation of further neurocognitive functions, more detailed evaluation items of neurocognitive functions and brain imaging studies are necessary in the future study.

근육이완요법이 항암화학요법으로 인한 오심구토, 불안, 불편감의 감소에 미치는 영향 (Efficacy of Progressive Muscle Relaxation Training in Reducing Nausea and Vomiting, Anxiety and Symptom Distress of Cancer Chemotherapy)

  • Twenty five cancer patients were assigned to two groups: The experimental group which received Progressive Muscle Relaxation (PMR) training and the control group without the training. Base line data for dependent variables -nausea and vomiting, state anxiety and symptom distress- were collected when the subjects of both groups received the first cycle chemotherapy. Then the experimental group was trained for PMR between the first and the second cycle chemotherapy. The same dependent variables were measured during the second cycle chemotherapy from the subjects. Results indicated that the PMR group showed no significant difference in severity of nausea and vomiting compared to the control group. Although the significance was not supported, the severity of nausea and vomiting in experimental group decreased while control group increased between the first and fifth day of the second cycle chemotherapy. The experimental group showed significantly less state anxiety during the second cycle than the control group (p<0.01). Significantly low symptom distress was also reported in the experimental group compared to the control group(p<0.01). The result suggests that PMR training may be an effective procedure for helping cancer patients cope with the adverse effects of their chemotherapy.

柴胡淸肝湯加減方으로 호전된 사타구니, 대퇴내측 습진 환자 4례 증례 보고 (Four Cases of the Patients with the Groin, Inner thigh Eczema Treated with Sihochunggan-tang gagambang)

  • Objective: The purpose of this study is to report the effectiveness of Sihochunggan-tang gagambang in the four patients with eczema ongroin, inner thigh.Methods: This case study was conducted for four groin, inner thigh eczema patients who have visited Korean Medicine Clinic. Sihochunggan-tang gagambang was prescribed to all four patients and evaluated the symptom change through photographs and two assessment methods. The assessment methods are Atopic Dermatitis Severity Index(ADSI) and Visual Analogue Scale(VAS). ADSI was used to evaluate the objective symptom change and VAS was used to evaluate the subjective symptom change.Results & Conclusion: The ADSI and VAS were decreased compared to the first in all four patients in our study. And most symptoms of four patients were improved. As a result of examining ADSI, VAS scores and photographs, Sihochunggan-tang gagambang can be effective on the groin, inner thigh eczema in four cases in our study.

류마티스 관절염 환자가 지각하는 불확실성에 관한 모형 구축 (Model Construction of Perceived Uncertainty in Rheumatoid Arthritis Patients)

  • Rheumatoid arthritis, unlike other chronic diseases, causes the patients to experience uncertainty in their daily lives and thus to feel threat on their emotional comfort because of inconsistent and unpredictable symptoms such as pain. Therefore, a theoretical framework is needed for explanation of uncertainty in patients having rheumatoid arthritis. A hypothetical model was constructed on the basis of Mishel's Uncertainty Theory and other literature review. The model included 9 theoretical concepts and 19 paths. Subjects of the study constituted 330 partients who visited outpatient clinics of two university hospitals and one general hospital in Seoul. Self report questionnaires were used to measure the variables affecting uncertainty. Reliability coefficients of these instruments were found Cronbach's Alpha=$.70{\sim}.94$. In data analysis, SAS program and PC-LISREL 8.03 computer program were utilized for descriptive statistics and covariance structure analysis. The results of covariance structure analysis for model fitness were as follows : 1) Hypothetical model showed a good fit to the empirical data : Chi-square($X^2$)=41.81 (df=11, P=.000), Goodness of Fit Index=.974, Root Mean Square Residual=.049, Normed Fit Index=.928, Non Normed Fit Index=.814. 2) For the validity and the parcimony of model, a modified model was constructed by appending 2 paths and deleting 5 paths according to the criteria of statistical significance and meaningfulness. 3) The results of hypothesis testing were as follows : (1) Educational level, event familiarity and severity of illness had a direct effect on uncertainty : Event congruency had both direct and indirect effect on uncertainty : Credible authority and symptom consistency had a nonsignificant direct effect on uncertainty, (2) Illness duration, symptom consistency, and event congruency had a direct effect on severity of illness ; Credible authority had a both direct and indirect effect on severity of illness ; Event congruency had the greatest effect on severity of illness, and event familiarity had a nonsignificant direct effect on severity of illness.

두부외상 환자에서 전두엽 손상과 신경인지기능 변화에 따른 주관적인 증상 연구 (The Study of the Subjective Symptoms according to Frontal Lobe Damage and Change in Neurocognitive Function in Traumatic Head Injury Patients)

  • Objective : The purpose of this study was to analyze the correlation between symptom severity and neurocognitive factors in traumatic head injury patients. In addition, the effect of frontal lobe damage on these parameters was examined. Methods : We selected 18 patients who had brain damage for the moderate to severe traumatic brain injury (MSTBI) group, and 17 patients who met the diagnostic criteria for post-traumatic stress disorder (PTSD) without the finding of brain damage for the comparison group. For the evaluation of neurocognitive function, K-WAIS, Rey-Kim Memory Test, K-FENT, WCST, and MMPI-2 were used. Results : The results of the comparison (using the malingering scale) revealed that the values of PDS and PK, which express the severity of symptoms, and the values of the validity scale F, F (B), and F (P) were significantly higher in the overly-expressed group. F (B) in overly-expressed group and PK, Pt, and Sc in the properly-expressed group had significant correlation with the severity of symptoms. F (B), S, and Stroop error inhibition in PTSD, and PK, Pt, Sc, and MQ in MSTBI had significant correlation with the severity of symptoms. The results of the comparison based on the finding of frontal lobe damage revealed that PDS, EIQ, and MQ ware significantly higher in the group without brain damage. Conclusions : It was revealed that each neurocognitive factor was correlated with the severity of symptoms. There was a decrease in complaints or symptoms reported by the frontal lobe injury group, and this is believed to be due to degenerative change in the personality and emotional functioning of these patients following frontal lobe damage.

Reduced Heart Rate Variability in Somatic Symptom Disorder: Associations with Alexithymia

  • Objectives : We investigated heart rate variability (HRV) patterns in patients with somatic symptom disorder (SSD) and the relationships of these patterns with alexithymia. Methods : In total, 42 patients with SSD and 33 healthy controls were enrolled in this study. Demographic, psychological, and HRV data were assessed at baseline, and 24 patients with SSD were reassessed after 6 months of treatment. The psychological data included somatic symptoms and levels of depression, anxiety, and alexithymia as indicated by the somatic symptom subscale of the Symptom Checklist 90-Revision (SCL-12), Beck Depression Inventory-II (BDI-II), Beck Anxiety Inventory (BAI), and the Toronto Alexithymia Scale 20 (TAS-20), respectively. Results : Patients with SSD had a lower standard deviation of normal-to-normal R-R intervals (SDNN) and lower proportions of adjacent R-R intervals greater than 50 milliseconds (pNN50) compared with controls. These HRV parameters were negatively correlated with alexithymia severity. After treatment, patients exhibited significantly decreased levels of somatic symptoms and reduced anxiety and depression, but there were no significant differences in the HRV parameters. In patients with alexithymia, a high baseline SDNN and pNN50 were associated with a decrease in somatic symptoms. Conclusions : Patients with SSD have different HRV patterns, and several HRV parameters are associated with alexithymia severity. These findings suggest that ANS regulation is involved in the pathophysiology of SSD, mediated by alexithymia. Furthermore, these results suggest that certain HRV parameters may be associated with clinical outcomes of SSD.
