A Case of Focal Reexpansion Pulmonary Edema after Chest Tube Insertion (흉관 삽관 후 발생한 국소성 재팽창성 폐부종 1예)

  • Reexpansion pulmonary edema is not a common phenomenon after chest tube insertion but some reports from 0% to 14%. There are various resulting complications, including acute respiratory distress syndrome. We report a case of focal reexpansion pulmonary edema after chest tube insertion. A 49-year-old male came to the hospital due to ongoing dyspnea and left chest pain for 3 days. On chest X-ray, the patient had a left pneumothrax. We planned to insert a chest tube for symptom relief. To determine whether or not the chest had expanded as a result of the chest tube insertion, the patient underwent repeated chest X-rays the following day. The patient experienced brief respiratory symptoms upon initial suction; a chest PA showed patchy consolidated infiltration at the inserted site. After 5 days of conservative management, the recovered completely.

Are Anxiety and Depression Distinct? : Exploratory Factor Analysisof Zung's Self-Rating Anxiety and Depression Scales (불안과 우울은 다른가? : Zung 자기보고식 불안 및 우울 척도의 탐색적 요인분석)

  • Objectives There is a controversy regarding the construct validity of anxiety and depression. Some believe that these two symptoms are basically the same construct, that is, both measure what is called 'general distress' or two phenomena are distinct experiences which often coexist. To further understand relationship between anxiety and depressive symptoms, we investigated the factor structure of a combined anxiety and depression scale among psychiatric outpatients. Methods Data of Zung's Self-Rating Depression and Anxiety Scales were gathered from 401 newly visiting psychiatric outpatients at a university-affiliated hospital. We performed a component analysis on the 40 items from two scales. Results Exploratory factor analysis revealed a seven factor structure explaining 56% of total variance. Overall finding indicated that depression and anxiety scales consisted of four symptom domains : mainly depressive symptoms, mainly anxiety symptoms, common somatic symptoms, and others. Conclusions Our results suggest that the construct of self-reported depressive and anxiety symptoms are more complex than previously thought, i.e., either one or two factor theories. These findings also support that anxiety and depression can be better modeled by dimensional approach. Clinicians may be alert for the fact that both depression and anxiety scales measure distinct and also common aspects. Further researches on other scales especially, interview based instruments are needed.

Influencing Factors of Depression in Women with Endometriosis (자궁내막증 여성의 우울에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to understand depression in women with endometriosis and to identify the factors influencing depression. Method: The instruments used were the Beck Depression Inventory for depression, Lemaire Scale for endometriosis syndrome, Mishel Uncertainty in Illness Scale-Community form (MUIS-C), the Rosenberg Self-esteem Scale, and Quality of Life (Endometriosis Health Profile Questionnaire; EHP-5) by Jones et al. The Subjects of Study were 118 outpatients diagnosed with endometriosis from 7 hospitals in Busan City and Kyungsangnam-do. Data was collected from Aug 1, 2004 to Jan 31, 2005. For statistical analysis of collected data, frequency analysis, analysis of variance, and stepwise multiple regression analysis were used with the SPSS statistical program. Result: The general characteristic showing a statistically significant difference in depression in the women with endometriosis was marital status. The score of the depression showed a significantly positive correlation with the score of endometriosis TSD(r= .464, p= .000), uncertainty(r= .393, p= .000), and quality of life(r= .543, p= .000). Depression showed a significantly negative correlation with the score of self esteem(r= - .557, p= .000). Stepwise multiple regression analysis revealed that the most powerful predictor of depression in the women with endometriosis was self-esteem ($R^2$=0.311). A combination of self esteem, quality of life, and total symptom distress accounted for $50.0\%$ of the variance in depression in women with endometriosis. Conclusion: The influencing factor on depression in women with endometriosis was self- esteem, quality of life, and endometriosis TSD. Further studies need to be done to identify methods of overcoming and the presentation of depression in endometriosis.

SURGICAL TREATMENT OF AIRWAY OBSTRUCTION IN INFANTS WITH PIERRE ROBIN SEQUENCE (Pierre Robin sequence 환자에서 기도 폐색의 외과적 치료)

  • The deformities of micrognathia and glossoptosis in the newborn are frequently associated with a cleft palate, which is known as Pierre Robin sequence. Upper airway obstruction is the most serious problem in these patients. Treatment of Pierre Robin sequence includes either positional or surgical intervention. Mild cases are often managed in the prone position. However, when the patient fails to thrive due to chronic upper airway obstruction, or severe respiratory distress ensures despite positional treatment, surgical intervention is mandatory to relieve the obstruction. We experienced three infants with Pierre Robin sequence who showed a symptom triad of micrognathia, glossoptosis, and cleft palate. Intermittent cyanosis, depression of the chest, respiratory difficulty and feeding problems were also observed. To relieve severe upper airway obstruction caused by micrognathia and glossoptosis, we simultaneously performed modified tongue lip adhesion (TLA) and a subperiosteal release of the floor of the mouth (SRFM). Respiratory and feeding difficulties were relieved, the tongue positioned anteriorly, body weight increased, and mandibular growth improved. Simultaneous TLA and SRFM may constitute a simple and reliable method for surgical treatment of airway obstruction in patients with Pierre Robin sequence.

The Effect of Sexual Abuse on Posttraumatic Psychiatric Symptoms in Children and Adolescents with Sexual Abuse (성학대를 경험한 소아, 청소년에서 성학대가 외상 후 정신 증상에 미치는 영향)

  • Objectives : The purpose of this study is to examine the effects of the characteristics of victim and sexual abuse on posttraumatic psychiatric symptoms in children and adolescents with a history of sexual abuse. Methods : A total of 137 children and adolescents were recruited from the Seoul Sunflower Children Center, a nation-funded sexual violence victim protection center, from January 2009 to December 2013. We collected the demographic data of the victims and the Trauma Symptom Checklist for Children (TSCC) from victims. We hypothesized victims' age, sex, and intelligence quotient, and the characteristics of sexual abuse as the affecting factors of posttraumatic psychiatric symptoms. Descriptive analysis and hierarchical regression analysis were performed for analysis of demographic data, TSCC scores, and psychiatric symptoms. Results : The victims' age and the characteristics of sexual abuse were significantly related to the traumatic distress of sexual abuse. R-square was 23% for anxiety, 39% for depression, 21% for posttraumatic stress, and 37% for dissociation on TSCC. Conclusion : This study suggests that victims' age, type, frequency and duration of exposure, and disclosure of sexual abuse are significant affecting factors on posttraumatic psychiatric symptoms in children and adolescents. Exploration of psychiatric symptoms other than posttraumatic symptoms, and relations between pretraumatic and posttraumatic psychiatric symptoms is needed through collection of larger samples.

A Meta-Analysis on Factors Related to Quality of Life in Heart Transplant Recipients (심장이식 수혜자의 삶의 질 관련 요인에 대한 메타분석)

  • Purpose: This study was a systematic review and meta-analysis to explore the factors related to quality of life in heart transplant recipients. Methods: To identify studies that suggested the factors related to the quality of life in heart transplant recipients, we searched the articles published from 1974 to November 2018 using Six databases, PubMed, CINAHL, EMBASE, Cochrane, KMBASE and RISS. A total of 22 studies were selected out of 5,234 for the systematic review and meta-analysis on the basis of the PRISMA flow. The quality of study was assessed by assessment tool form the NIH and meta-analysis was performed using the 'R 3.5.2' version to analyze the correlated effect sizes. Results: Factors related to quality of life in heart transplant recipients were categorized into six domains based on the health-related quality of life model introduced by Ferrans: individual, environmental, biological function, symptoms, functional status, and general health perception. In the meta-analysis, 34 factors were used and 17 factors having significant effect sizes were as follows: self-efficacy, demoralization, perceived control, current occupational status, age, marital status, health promotion life style in the individual characteristics; stress in environmental characteristics; physical function status, creatinine level, left ventricular ejection fraction (LVEF) in biological function; anxiety, depression, symptom frequency and distress in symptoms domain; coping, self-care compliance in functional status. Conclusion: The findings indicate that the multi-dimensional factors influencing the quality of life in heart transplant recipients and provide the evidence for developing effective interventions for improving the quality of life of recipients.

A Study on comparison of menstrual pattern according to Mibyeong Index of Korean women in their thirties and forties (30-40대 여성의 미병상태에 따른 월경양상 비교연구)

  • Introduction : The aim of this study was to investigate menstrual pattern according to Mibyeong Index(MBI) and compare Mibyeong symptom score according to severity of menstrual pain. Mibyeong Index was designed to measure inconvenience and resilience of subject's complains including four physical symptoms(fatigue, pain, low sleep quality, indigestion) and mental distress including anxiety, anger, depression. Method : We used the clinical data of Korean medicine Date Center(KDC) for subjects who participated in the study of 'Clinical research for collecting of clinical cases based on the personal type of Mibyeng' conducted in Seoul, from June 26, 2015 to June 26, 2017. A total of 566 fertile women aged 30 to 50 who completed the questionnaires were included in this study. In this study, we used items of Mibyeong Index and the menstrual pattern by self-report questionnaires. The date were analyzed through Kruskal Wallis test, Pearson's chi-square test, and one-way ANOVA using SPSS statistics 19.0. Results : Mibyeong status was significantly associated with severity of menstrual pain(p<0.001), worst day of menstrual pain(p<0.05), and type of menstrual pain(low back pain, nausea, depression, and none (respectively, p<0.05)). In addition, The 7-subtype score (fatigue (p<0.001), pain(p<0.001), low sleep quality(p<0.001), indigestion(p<0.001), anxiety(p<0.05), anger(p<0.05) and depression(p<0.05)) of Mibyeong index and total score of Mibyeong Index (p<0.001) showed significant difference for severity of menstrual pain. Conclusion : This result indicates that a close relationship between Mibyeong status and severity of menstrual pain. Further studies are needed but, we hope that this results will be used as the basic data to improve mibyeong status through health care to alleviate dysmenorrhea.

Technology-based self-management interventions for women with breast cancer: a systematic review

  • Purpose: Since technology-based interventions can facilitate convenient access to healthcare for women with breast cancer, it is crucial to understand innovative approaches to maintaining the effectiveness of these interventions. Therefore, we conducted a systematic review of technology-based self-management interventions for women with breast cancer in six countries. We analyzed the characteristics of these interventions and examined their diverse health outcomes. Methods: Six databases were systematically searched to extract research articles using the keywords "breast cancer," "technology," and "self-management." The search was carried out up until June 12, 2023. From the 1,288 studies retrieved from the database search, 10 eligible papers were identified based on inclusion/exclusion criteria. Two authors independently extracted and compared the data from these articles, resolving any discrepancies through discussion. Results: Most of the 10 studies utilized web- or mobile-based technology, and one used artificial intelligence-based technology. Among the 12 health-related outcome variables, quality of life and symptom distress were the most frequently mentioned, appearing in six articles. Furthermore, an analysis of the intervention programs revealed a variety of common constructs and the involvement of managers in the self-management intervention. Conclusion: Incorporating key components such as self-management planning, diary keeping, and communication support in technology-based interventions could significantly improve the self-management process for breast cancer survivors. The practical application of technology has the potential to empower women diagnosed with breast cancer and improve their overall quality of life, by providing timely and sustainable interventions, and by leveraging available resources and tools.

Clinical Evaluation of Traumatic Diappragmatic Injuries (외상성 횡격막 손상에 대한 임상적 고찰)

  • Diaphragm injuries are very important because, if both thoracic and abdominal viscera are damaged, a combination of shock and acute respiratory distress may develop. It can be highly lethal. This evaluation was based on the reviews of 17 cases of traumatic diaphragm injuries treated at the Department of Cardiovascular Surgery, Seoul Adventist Hospital during 5 years from March 1993 to February 1997. The mean age of the patients was 37.2 years and sex ratio was 3.2:1 with male dominance. Blunt trauma(N=5, Rt.=4, Lt.= 1) was 29.5%, penetrating trauma(N= 12, Rt.=5, Lt.=7) was 70.5%. Dyspnea(76%) was the most common symptom. Blunt trauma(9.8$\pm$3.7 Cm) was larger than the penetrating trauma(3.2$\pm$ 1.3 Cm)(P<0.05) in the size(mean$\pm$SD) of the injury. All of the patients had associated injuries and repaired immediatley with thoracic approach 11 cases(64%), abdominal approaih 3 cases(18%) and thoracoabdominal approach 3 cases(18%). f cases of penetrating diaphragmatic t auma was diagnosed on the operation of other organ injury Now we suggest that diaphragmatic injury should be suspected in all patients with penetrating as well as blunt injury of the chest and abdomen to protect the patient from its late complications.

  • Objective : There were few reports on the treatment of Adult-onset Still's disease and Diffuse cerebral dysfunction. This oriental medical treatment shows the possibility of healing Adult-onset Still's disease and Diffuse cerebral dysfunction, therefore we announce this. Methods : The acupuncture, herbal medicine, herbal acupucture(Jahageo, Nockyong) therapies were applied for treating this patient's chief symptom(chest discomfort, palpitation, rigidity, mentality change, global aphasia, dysphagia, coughing and sputum) Results : 1. Adult-onset Still's disease is a rare systemic inflammatory disorder of unknown etiology, characterised by salmon-colored rash, intermittent spiking high fever, arthralgia and variety of systemic features. This is one type of Juvenile arthritis. 2. Diffuse cerebral dysfunction is an anoxic-ischemic encephalopathy, and most cause of this is the hypoxia caused by hypotention or respiratory distress. 3. After oriental medical treatments, his chief symptoms were improved. Conclusion : The more study about oriental medical treatment on Adult-onset Still's disease and Diffuse cerebral dysfunction is needed.
