A Review of the Latest Research Trends in Rosacea and Recommendations for More Effective Oriental Medicine Treatments - Focusing on Autonomic Nervous System Regulation - (주사피부염의 최신 연구 동향 및 더욱 효과적인 한방치료를 위한 제언 - 자율신경 기능조절을 중심으로 -)

  • Objectives : The aim of this study is to explore the potential of oriental medicine in managing rosacea through the regulation of the autonomic nervous system. Methods : We reviewed studies on the pathophysiology and medical treatment (both western and oriental medicine) of rosacea, as well as the relationship between rosacea and the autonomic nervous system, using four databases: PubMed, OASIS, RISS, and NDSL. Results : Rosacea is a chronic recurrent inflammatory disease characterized by symptoms such as facial flushing, inflammatory papules, and pustules. In Western medicine, symptomatic treatments like vasoconstrictors, doxycycline, and anti-inflammatory drugs are primarily used. According to the pathophysiological mechanisms of rosacea, the autonomic nervous system is closely related, particularly with sympathetic overactivity causing vasodilation and local inflammation in rosacea patients. Additionally, recent studies report that rosacea patients frequently exhibit neuropsychiatric symptoms such as anxiety, depression, and insomnia, which are closely linked to autonomic dysfunction and contribute to the worsening of skin symptoms. However, current studies on the use of oriental medicine for rosacea focus mainly on anti-inflammatory effects at the local level, similar to conventional treatments. Conclusions : Based on the close involvement of the autonomic nervous system in the pathophysiological mechanisms of rosacea and numerous studies showing that oriental medicine can effectively regulate autonomic function, applying such treatments to rosacea patients may improve not only skin symptoms but also the frequently associated neuropsychiatric symptoms like anxiety, depression and insomnia.

Influences of Long Hour Guarding in Bodyguards on the Heart Rate Variation and Autonomic Nervous System (경호원들의 장시간 경계근무가 심박변이도와 자율신경에 미치는 영향)

  • In this study a practical guarding work is carried out for 180 minutes as the same as the actual guarding work that is done by nine students of the department of security services in K University in order to investigate changes in circadian rhythms during long hour guarding in bodyguards at practical sites. In the results of the tests of the heart rate variation and autonomic nervous system with the interval of 30 minutes using HRV (Heart rate variability), there are no significant differences in HRV and SDNN and that leads to maintain it stably during the guarding work for 180 minutes. In the case of TP which reflects the overall activity level in the autonomic nervous system, it shows a high significant difference (p<.05) at 90 and 120 minutes compared to that of normal states. Also, it shows a significant decrease in the level after a lapse of 120 minutes and that shows a decrease in the activity of the autonomic nervous system for the guarding work more than 120 minutes. Although differences in VLF, LF, and HF are not significant levels, these are influenced on the change in TP. The LH/HF ratio that represents the balance between the sympathetic nerve and the parasympathetic nerve shows a significant high level (p<.05) after a lapse of 30 minutes. Thus, it is considered that the concentration of the guarding work after a lapse of 120 minutes is decreased and there are some tensions and excitations after a lapse of 30 minutes since the beginning of the guarding work.

The Effects of Cervi Pantotrichum Cornu Pharmacopuncture and Bovis calculus.Fel Ursi Pharmacopuncture on the Heart Rate Variability (녹용약침과 웅담.우황(BU)약침이 정상인의 심박변이도(HRV)에 미치는 영향)

  • Objectives : This study was to investigate the effects of Cervi Pantotrichum Cornu Pharmacopuncture and Bovis calculus Fel Ursi Pharmacopuncture on autonomic nervous system with Heart Rate Variability(HRV). Purpose of the trial was to observe what influence Cervi Pantotrichum Cornu Pharmacopuncture and Bovis calculus Fel Ursi pharmacopuncture have on the autonomic nervous system. Methods : 40 healthy male volunteers were divided into two experimental groups, such as Cervi Pantotrichum Cornu Pharmacopuncture group and Bovis calculus Fel Ursi Pharmacopuncture group. Study design was a randomized, double-blind clinical trial. 20 subjects in one experimental group were injected Cervi Pantotrichum Cornu Pharmacopuncture, 20 subjects in other experimental group were injected hydrolyzed Bovis calculus Fel Ursi Pharmacopuncture at GB21(Kyonjong). We measured HRV 7 times : baseline measurement and every 5 minutes for 30 minutes after injection. The SPSS 15.0 for Windows was used to analyze the data by the paired t-test(in group) and independent sample t-test(between the groups). Results A. Time Domain Analysis 1. Analysis of Mean HRV, SDNN and Complexity After injection of Cervi Pantotrichum Cornu Pharmacopuncture, Mean HRV was significantly decreased all the times after injection. SDNN was significantly increased at immediate and 25 minute after injection. Complexity was significantly decreased only at 15 minute after injection. After injection of Bovis calculus Fel Ursi Pharmacopuncture, Mean HRV was significantly decreased all the times after injection. Complexity was significantly decreased at 15 minutes after injection. 2. Analysis of HRV index, pNN50 After injection of Cervi Pantotrichum Cornu Pharmacopuncture, HRV index was significantly increased immediately after injection. pNN50 was significantly decreased at immediately, 20 and 25 minutes after injection. After injection of Bovis calculus Fel Ursi Pharmacopuncture, HRV index was significantly increased immediately after injection. pNN50 was significantly decreased at immediate and 25 minutes after injection. B. Frequency Domain Analysis 1. Analysis of Ln(TP), Ln(HF), Ln(LF), Ln(VLF) After injection of Cervi Pantotrichum Cornu Pharmacopuncture, Ln(TP) was significantly increased at immediate and 25 after injection. Ln(HF) was significantly increased all the times after injection. Ln(LF) was significantly increased at 15, 20 and 25 minutes after injection. Ln(VLF) was significantly increased at immediate and 25 minutes after injection. After injection of Bovis calculus Fel Ursi Pharmacopuncture, Ln(TP) was significantly increased at immediately and 25 after injection. Ln(LF) was significantly increased at 20 and 25 minutes after injection. Ln(VLF) was significantly increased at immediate and 25 minutes after injection. Conclusions : We results suggest that Cervi Pantotrichum Cornu Pharmacopuncture and Bovis calculus Fel Ursi Pharmacopuncture in healthy adult men tend to activate the autonomic nervous system and sympathetic nervous system within normal range.

Effect of Aromatherapy on the Autonomic Nervous System (Aromatherapy가 자율신경계에 미치는 효과)

  • This study was conducted to investigate the effect of aromatherapy on the autonomic nervous system by the inhalation. The subjects of the study were 64 people aged 20 and under 59 years old, 32 in the experimental group and 32 in the control group. Blending oil with Six aroma(Lavender, Bergamot, Mandarin, Lemon, Cedarwood, Roman Chamomile) was applied to the experimental group, and Jojoba oil was applied to the control group for 30min. Before and after the experiment, HRV(TP, VLF, LF, HF, LF/HF) of the experimental group and the control group were compared and analyzed using pulse wave mesuring instrument(uBioClip v70). There was a significant difference between the experimental group and the control group in 3 out of 5 items analyzed TP(p<.05), LF(p<.05), HF(p<.01). In other words, aromatherapy increases the total activity of the autonomic nervous system by enhancing the TP, increases the activity of the sympathetic and parasympathetic nerves, promotes the activity of the heart, Facilitate the activity of the heart. Therefore, if aromatherapy is used steadily in everyday life, it will enable healthy life by positively affecting the autonomic nervous system response.

A Study for Home-based Physical Therapy Service Introduction through a Group of Professionals In-depth Interview (전문가 그룹의 심층면접을 통한 가정방문 물리치료도입에 관한 연구)

  • PURPOSE: The present study was aimed at providing political basic data for introduction home-based physical therapy service through conducting In-depth interviews with professionals for complementing the survey of previous studies. METHODS: we conducted In-depth interviews to 10 professionals with their consent after describing the intent and purpose of the study. RESULT: Interviewees has significant recognition of home-based physical therapy service, and were sympathetic to the need for the introduction of the home-based physical therapy service. They suggest that it should be required to public relations, incorporation home-based physical therapy into the regular curriculum and research the foreign cases. Also home-based physical therapy system service should be include therapeutic exercises, education for families & caregivers and providing the rehabilitation of the team approach. Most professionals think the sooner Home-based physical therapy service introduction, the better for public. Also they propose that hospitals and clinics under the national and local government become a principal and operate in conjunction with the health center. CONCLUSION: Home-based physical therapy service should be conducted in early for relieving the physical, mental and social discomfort of patients, physical therapists and caregivers, also providing the quality of physical therapy to chronically ill patients. It is necessary to efforts of the physical therapy association and physical therapist and government`s political supports. In addition, we must be prepared on the basis of policy development and operation through continuously converging the opinions of experts who have been associated with home-based physical therapy service.

Effect of Saccharin Intake in Restraint-induced Stress Response Reduction in Rats (구속 스트레스 쥐 모델에서 스트레스 반응 감소에 대한 사카린 섭취의 효과)

  • Purpose: Stress activates the sympathetic nervous system and hypothalamic-pituitary-adrenal (HPA) axis and induces the release of glucocorticoids. Saccharin is 300 times sweeter than sucrose, but does not increase blood insulin levels. Thus, this study was designed to evaluate the effect of saccharin intake in restraint-induced stress response reduction in rats. Methods: Adult male Sprague-Dawley (SD) rats had stress induced by restraint for 2 hours/day for 1 week. Saccharin was provided in sufficient amounts to allow them to intake it voluntarily at 0.1% diluted in water. The Y-maze test and forced swim test (FST) were performed to evaluate cognitive function and the depressive behavior of the rats. The protein expression of the glucocorticoid receptor (GR) in hippocampal cornu ammonis (CA) 1 was investigated by using immunohistochemistry. Results: It was found that, the percentage of alternation in the Y-maze test was significantly (p<.01) higher in the Stress + saccharin group than in the Stress group. Immobility time in the FST was significantly (p<.01) lower in the Stress + saccharin group than in the Stress group. Also, the positive cells of GR in hippocampus CA1 were significantly (p<.05) lower in the Stress + saccharin group than in the Stress group. Conclusion: This study showed that there was an effect of saccharin intake in restraint-induced stress response reduction in rats.

Effect of immobilization stress on the expression of TH, BDH and CRH gene in rat brain (부동스트레스가 흰쥐 뇌 조직 내 TH, BDH와 CRH 유전자 발현에 미치는 영향)

  • Purpose : Catecholamines are the neuro-transmitters in the sympathetic nervous system (SNS) and are activated by stress stimulus. Tyrosine hydroxylase (TH) and Dopamine-${\beta}$-Hydroxylase (DBH) are very important enzymes in the catecholamine synthesis. Corticotropin releasing hormone (CRH) is released in the process of reacting to stresses. The aim of this study is to find out what effects immobilization stresses have on the expression of TH, BDH and CRH mRNA in a rat's brains. Methods : We compare expression levels in rat's brains of TH, DBH and CRH mRNA induced by immobilization stresses between the test group and controled group. The expression levels of TH, DBH and CRH mRNA are measured by RT-PCR and the Western Blotting Analysis (WBA). Results : In brains and adrenal glands of the immobilization stress group, the expression levels of TH and DBH mRNAs are significantly two to three times higher (P<0.01), and CRH mRNAs are approximately one and a half times higher (P<0.05) than those of controlled group. Conclusion : This study suggest that the expression levels of TH, DBH and CRH mRNAs are activated by stress stimulus in a rat's brains and adrenal glands.

  • Noise is not only affecting the ear and the auditory cortex locally, but its influence is widely spread throughout the brain structures, e. g., the reticular formation, the brain stem nuclei or the subcortical forebrain area. Hence, any of the organism's activities can be hindered or stimulated by noise. High noise is a stressor and the catecholamine level can be used both as a stress marker and as an indicator of modified sympathetic nervous system activity. Several recent studies have found that the urinary excretion of catecholamines is increased due to high noise intensity, especially unexpectedly high and long lasting noise. The present study was conducted in order to examine the effects of noise stress on urinary excretion of ctecholamines in rats and humans. Rats were exposed to 90 dB noise for 10, 30, and 60 minutes, 3 and 12 hours. 24 hour . urinary samples were collected and the catecholamones were extracted by alumina and analyzed by HPLC-ECD. Catecholamine levels increased with time of exposure up to 60 minutes : norepinephrine concentration at 60 min of noise=1.038 ng/ml, epinephrine=0.636 ng/ml. Urine catecholamines of blue collar workers exposed to 90 dB of noise at the work place were collected between 2 and 4 p.m. and compared to that of white collar workers exposed to 70 dB. Mean norepinephrine level of the blue collar workers was 0.89 ng/ml (${\pm}0.25$), epinephrine 0.24ng/m1 (${\pm}0.09$), and that of the white collar workers 0.48 ng/ml (${\pm}0.12$), epinephrine 0.19 ng/ml(${\pm}0.05$). It was concluded that noise acts as a stressor and increases the catecholamine levels in both rats and humans.

Effects of Fixed-intensity and Varied-intensity Electroacupuncture on Heart Rate Variability in Healthy People with Stress Task (고정자극 전침과 변동자극 전침의 정상인 스트레스에 대한 심박변이도 변화 연구)

  • Objectives : This study was performed to investigate the effects of fixed-intensity and varied-intensity electroacupuncture on heart rate variability in healthy people with stress task. Methods : Forty healthy subjects were randomly assigned to either group A or group B or C. Group A received fixed-intensity electroacupuncture(F-EA) at $ST_{36}$ and $ST_{37}$. After 1 week wash out period to prevent overlapping residual effects, they received varied-intensity electroacupuncture(V-EA) in which the intensity was changed every two minutes based on individual heart rate variability at the same points. Group B received the treatments in reverse order. Group C received no intervention as a control group. Results : Control group showed a significant increase of LF and LF/HF during the 2nd working memory task, while both F-EA and V-EA did not. In addition, V-EA showed a significant increase of HF during 2nd rest while F-EA did not. Conclusions : These results suggest that F-EA and V-EA can inhibit sympathetic activation more than control group and that V-EA can enhance parasympathetic activation more than F-EA.

The Clinical Effect of Manipulation of Acupuncture to Shen-Men and Nei-Kuan on Blood Pressure of Normal Male Subjects

  • Objectives : With an assumption of traditional oriental medical theory, to evaluate the effect of tonification/sedation manipulative acupuncture on blood pressure in male normotensives. Methods : This study was a randomized cross-over trial. We enrolled healthy male normotensive subjects, and assigned them randomly to group A or group B. Group A received acupuncture therapy on Shen-Men (He-7) and Nei-Kuan (EH-6) with tonification-manipulation, while group B received it with sedation-manipulation. Blood pressure was assessed every ten minutes for 2 hours. After 3 days of washout period, the subjects were crossed over to the other manipulation, and the same procedures were performed. Results : We observed that blood pressure stimulated by the sedation-manipulation had a tendency to decrease compared to that by the tonification-manipulation. Conclusions : We suggest the sedation manipulation has more synergic effect with the inhibitory effects of Shen-Men and Nei-Kuan on the sympathetic nervous system than the tonification-manipulation.
