This study was undertaken to observe the histologic changes of surrounding maxillary sutures to the widening of midpalatal suture, using two adult dogs, weighing about 10 kg, for experimental, and one for control group. After widening of the midpalatal suture with expansion screw for ten days was performed, and followed by sacrifice of experimental animals. Tissues were excised from 5 surrounding maxillary suture portions such as, internasal, interfrontal, midsagittal, zygomatrco-temporal, and midpalatal sutures. After that, the specimens were fixed and decalcified in $10\%$ formalin sol. and $5\%$ nitric acid. Embedding in paraffin and serial sections at a thickness of 5 micron was done, After Hematoxylin-Eosin staining and light microscopic examination, Following results were obtained: 1. In midpalatal suture area, which showed most prominent histologic change, High degree of fibronlastic and osteoblastic proliferation lining the bony trabeculae with osteoid tissue projection into the fibrous connective tissue were observed. 2. In interfrontal suture area, moderate degree of fibroblastic and osteoblastic proliferation was observed. 3. In internasal suture area, active osteoblastic and osteoclastic proliferation lining the bony trabeculae was observed, and separation of fibrous connective tissue was also observed. 4. In midsagittal and zygomatico-temporal suture areas, no histologic changes can be observed.
The purpose of this study is to know whether single layer continuous connell suture is an acceptable alternative to simple interrupted approximating suture for end-to-end intestinal anastomosis in dogs. Fourteen mixed-breed dogs weighing 2 to 5 kg were allotted to group treated with simple interrupted approximating suture (Group I) and group treated with single layer continuous Connell suture (Group II), each of 7 dogs. All dogs in each suture pattern were compared with time for total operation ad suture elapsed for intestines to anastomose, clinical signs, changing of pre-and postoperative luminal size, status of feces, adhesion at anastomotic site for 14 days after operation. Time for total operation and suture time for intestinal anastomosis were none significant between Group I and Group II, although those in Group II was about 3 minutes shorter than those in Group I, respectively. Group I spent average 47.08${\pm}$11.10 minutes on total operation, 20.97${\pm}$5.54 minutes on suture time for intestinal anastomosis and Group II spent average 44.74${\pm}$7.77 minutes, 17.73${\pm}$3.05 minutes, respectively. All dogs were no special differences in vitality, vomiting, appetite between Group I and Group II for 14 days after operation. All dogs, except one dog in Group I, had showed normal vitality and appetite since 6~8 days after operation. Initial return of fecal passage showed in all dogs before 6 days after operation and thereafter most dogs showed normal feces. According to results, it was thought that all dogs with normal vitality and appetite before 8 days had showed good prognosis. There were no changes of intestinal luminal size in 2 dogs performed Group In and one dog performed Group II between at operation and 14 days after operation. Narrowing rate of intestinal lumen in Group I was average 9.3% of the normal diameter, whereas in Group II, 9.5% of normal diameter. In complications after operation, only one dog in Group I showed intestinal intussusception but the others didn't. Length of adhesion was measured between intestinal anastomotic site and omental graft. Length of adhesion in dogs performed Group II was mostly shorter than that of Group I. Adhesion with proximate intestines occurred in five dogs, which consisted of 3 dogs performed Group I and 2 dogs performed Group II. Concurrently, they had a great length of adhesion between anastomotic site and omental graft. There were no great differences between Group I and Group II about speed of operation, clinical signs, complications such as leakage and stricture. And all dogs performed intestinal anastomosis showed good clinical condition and prognosis. In conclusion, Single layer continuous Connell suture can safely perform an intestinal anastomosis and be an alternative of simple interrupted approximating suture in aspect of speed clinically.
With the advancement of shoulder arthroscopy, use of biodegradable suture anchors in the surgical repair of rotator cuff tears has increased. Because of the radiolucency of these anchors, radiography is not appropriate for early detection of anchor failure. Ultrasonography is an advantageous modality in visualizing biodegradable, radiolucent anchors on a real-time basis without risk of radiation exposure. We report on two cases of displacement of a biodegradable suture anchor diagnosed on ultrasonography during the postoperative follow- up, which has not been previously reported. Because this displacement could be missed in the postoperative follow up ultrasonography, we describe the ultrasonographic features of the displaced biodegradable anchors. Surgeons and radiologists should pay special attention to the possibility of displacement of the suture anchor in patients who underwent rotator cuff repairs using suture anchors.
For a partial tear of the subscapularis tendon, the presenting technique requires only the anterior portal for preparing the footprint and suture management, as well as the subclavian portal for placing the suture anchor and suture hook without inserting a cannula. It provides both a good angle for anchor placement and sufficient space for managing the upper portion of a subscapularis tendon tear. A spinal needle was inserted through the subclavian portal in order to identify the appropriate angle for placing the suture anchor. A 3-mm incision was made for the subclavian portal and a biosuture anchor was placed on the footprint portion of the subscapularis tendon. In order to avoid crowding, each limb of both strands of the biosuture anchor were passed through the tendon- posteromedial side first, and anterolateral side second, using a switching technique with suture hook embedded with no.1 PDS. A suture tie was applied in a reverse sequence (the lateral strand first and the medial strand second) through the anterior cannula using a sliding technique.
Surgical treatment was performed in 507 heads out of 1078 dairly cows which were diagnosed as abomasal displacement. Out of 507 heads. bar suture was performed in 280 heads. Twelve heads out of 280 heads(4.9%) recurred. Incorrective suture of the reticulum or pylorus instead of abomasum occurred in 3 cows. The cows in which abdominal wall was sutured but the suture was not inserted into the abomasum were 4 heads. The cows in which bar was broken are 4 heads. A cow suffered peritonitis after bar suture. Bar suture technique is thought to be a good method as a first choice for the correction of abomasal displacement.
We present an unusual case of bone metastases from renal cell carcinoma around orthopedic implants in a 78-year-old female with osteolytic, expansile, highly vascularized, malignant infiltration around suture anchors in the proximal humerus. The patient had undergone arthroscopic rotator cuff repair using suture anchor implants 6 years previously. After diagnosis of bone metastasis, she was successfully treated with metastasectomy and internal fixation using a plate and screws, with cement augmentation. This report is the first to document metastases around a suture anchor in a bone and suggests the vulnerability of suture anchor implants to tumor metastasis.

Background: The purpose of this study was to evaluate the postoperative magnetic resonance imaging (MRI) results of minimal-tying (one medial-row tie among 4 medial-row sutures) on the medial-row in double-row suture-bridge configuration ($2{\times}2$ anchor with $4{\times}4$ suture stands). Methods: From 2011 March to 2012 July, 79 patients underwent arthroscopic rotator cuff repair using $2{\times}2$ anchor double-row configuration. The mean age was 61.3 years (range, 31-81 years). Two double-loaded suture anchors were used for medial-row. Four medial-row stitches were made with only one medial-row knot-tying (the most anterior suture). Lateral-row was secured using the conventional suture-bridge anchor technique; all 4 strands were used for each anchor. Repair integrity was evaluated with MRI at mean 6.2 months postoperatively. Retear and the pattern of retear, change of fatty infiltration, and muscle atrophy of supraspinatus were evaluated using pre- and postoperative MRI. Results: Repaired tendon integrity was 38 for type I, 30 for type II, 6 for type III, 4 for type IV, and 1 for type V, according to Sugaya classification. Considering type IV/V as retear, the rate was 6.3% (5 out of 79 patients). Medial cuff failure was observed in 4 patients. Fatty atrophy of supraspinatus was significantly improved postoperatively according to Goutallier grading (p=0.01). The level of muscle atrophy of supraspinatus was not changed significantly after surgery. Conclusions: Minimal tying technique with suture configuration of four-by-four strand double-row suture-bridge yielded a lower retear rate (6.3%) in medium to large rotator cuff tears.
Objective: The purpose of this study was to determine whether predicting maturation of the midpalatal suture is possible by classifying its morphology on cone-beam computed tomography (CBCT) images and to investigate relationships with other developmental age indices. Methods: The morphology of the midpalatal suture was assessed by using CBCT images of 99 patients. Axial plane images of the midpalatal suture were classified into five stages according to the classification scheme. To make the assessment more accurate, the morphology and fusion of the midpalatal suture were additionally investigated on coronal cross-sectional planar images and volume-rendered images. Bone age was evaluated using the hand and wrist method (HWM) and cervical vertebrae method (CVM); dental age (Hellman's index), sex, and chronological age were also assessed. To evaluate relationships among variables, Spearman's rho rank test was performed along with crosstabs using contingency coefficients. Results: The HWM and CVM showed strong correlations with the maturation stage of the midpalatal suture, while other indices showed relatively weak correlations (p < 0.01). Through crosstabs, the HWM and CVM showed high association values with CBCT stage; the HWM demonstrated slightly higher values (p < 0.0001). Based on the HWM, the midpalatal suture was not fused until stage 6 in both sexes. Conclusions: Among developmental age indices, the HWM and CVM showed strong correlations and high associations, suggesting that they can be useful in assessing maturation of the midpalatal suture.
Background: The purpose of this study is to investigate clinical outcomes and radiological findings of cyst formation in the glenoid around suture anchors after arthroscopic Bankart repair with either biocomposite suture anchor or all-suture anchor in traumatic anterior shoulder instability. We hypothesized that there would be no significant difference in clinical and radiological outcomes between the two suture materials. Methods: This retrospective study reviewed 162 patients (69 in group A, biocomposite anchor; 93 in group B, all-suture anchor) who underwent arthroscopic Bankart repair of traumatic recurrent anterior shoulder instability with less than 20% glenoid defect on preoperative en-face view three-dimensional computed tomography. Patient assignment was not randomized. Results: At final follow-up, the mean subjective shoulder value, Rowe score, and University of California, Los Angeles shoulder score improved significantly in both groups. However, there were no significant differences in functional shoulder scores and recurrence rate (6%, 4/69 in group A; 5%, 5/93 in group B) between the two groups. On follow-up magnetic resonance arthrography/computed tomography arthrography, the incidence of peri-anchor cyst formation was 5.7% (4/69) in group A and 3.2% (3/93) in group B, which was not a significant difference. Conclusions: Considering the low incidence of peri-anchor cyst formation in the glenoid after Bankart repair with one of two anchor systems and the lack of association with recurrence instability, biocomposite and all-suture anchors in Bankart repair yield satisfactory outcomes with no significant difference.
Appropriate suture technique is crucial for successful tracheal anastomosis. However, standards for an ideal suture method have not yet been established. A previous study suggested tracheal anastomosis using barbed sutures that do not require knots; however, their use in small animals has not been reported. In this study, we aimed to compare knotless barbed sutures with conventional smooth sutures in terms of maximum tensile strength and suturing time in canine tracheal models to demonstrate the feasibility of using barbed sutures in tracheal anastomosis in dogs. Tracheal segments harvested from nine beagle dog cadavers were randomly assigned to three suture groups: barbed suture (B), smooth suture in simple interrupted pattern (SI), and smooth suture in simple continuous pattern (SC). The maximum tensile force and suturing time were compared according to the suturing method, and the mode of failure was evaluated. The average suturing time was 3.29 min in the B group; 4.41 min, SC group; and 8.99 min, SI group (p < 0.001). The average maximum tensile force in the SC group was 134.97 N, which was stronger than the SI (110.57 N) and B groups (103.10 N) (p < 0.05 and p < 0.01, respectively). The difference between the B and SI groups was not significant (p = 0.05). The B group demonstrated comparable mechanical strength and shorter suture time compared with the SI group. Therefore, tracheal anastomosis using barbed sutures could be an effective alternative to conventional smooth sutures in dogs.
