The Impact of Referral to Mental Health Services on Suicide Death Risk in Adolescent Suicide Survivors

  • Objectives: This study aims to examine the effect of adolescent suicide survivors' experience on suicide death risk, and the effect of referral to mental health services (hereafter referral) in this regard. Methods: This study used the data of 878 suicide-deceased and suicide-attempted adolescents aged 8-19 years, managed by the Suicide and School Mental Health Institute from 2016 to 2018. Results: Regression analysis for main effects showed that although suicide experience had no direct effect on suicide death, non-referral status was associated with a greater risk of death by suicide. While the "non-suicide survivor with non-referral" and "suicide survivor with non-referral" groups showed 1.87 [adjusted odds ratio=1.87, 95% confidence interval (CI)=1.21-2.89] and 4.59 (adjusted odds ratio=4.59, 95% CI=2.02-10.42) times higher odds of suicide death, respectively, the "suicide survivor with referral" group showed no difference compared to the "non-suicide survivor with referral" group. Conclusion: From these findings, there is a need to strengthen referral to mental health services and apply complicated grief treatment to improve the mental health of adolescent suicide survivors.

Risk Assessment for Metalworking Fluids and Respiratory Outcomes

  • Background: Metalworking fluids (MWFs) are mixtures with inhalation exposures as mists, dusts, and vapors, and dermal exposure in the dispersed and bulk liquid phase. A quantitative risk assessment was performed for exposure to MWF and respiratory disease. Methods: Risks associated with MWF were derived from published studies and NIOSH Health Hazard Evaluations, and lifetime risks were calculated. The outcomes analyzed included adult onset asthma, hypersensitivity pneumonitis, pulmonary function impairment, and reported symptoms. Incidence rates were compiled or estimated, and annual proportional loss of respiratory capacity was derived from cross-sectional assessments. Results: A strong healthy worker survivor effect was present. New-onset asthma and hypersensitivity pneumonitis, at 0.1 mg/㎥ MWF under continuous outbreak conditions, had a lifetime risk of 45%; if the associated microbiological conditions occur with only 5% prevalence, then the lifetime risk would be about 3%. At 0.1 mg/㎥, the estimate of excess lifetime risk of attributable pulmonary impairment was 0.25%, which may have been underestimated by a factor of 5 or more by a strong healthy worker survivor effect. The symptom prevalence associated with respiratory impairment at 0.1 mg/㎥ MWF was estimated to be 5% (published studies) and 21% (Health Hazard Evaluations). Conclusion: Significant risks of impairment and chronic disease occurred at 0.1 mg/㎥ for MWFs in use mostly before 2000. Evolving MWFs contain new ingredients with uncharacterized long-term hazards.

골반골 골절로 인한 동맥 파열로 동맥 색전술을 시행받은 환자에서의 생존 비교 (Comparison of Survival in Pelvic Bone Fractures with Arterial Embolization)

  • Purpose: This study was to evaluate the effect of arterial embolization on survival in patients with pelvic bone fractures and arterial bleeding. Methods: From January 2001 to December 2007, in all, 18 patients with pelvic bone fractures that had been treated with interventional arterial embolization were included in this retrospective study. The Injury Severity Score (ISS), the Revised Trauma Score (RTS), the initial hemodynamic status, the blood gas analysis, blood transfusion data, and mortality were the main outcome measurements. Results: Pelvic bone fractures were classified into lateral compression (LC), antero-posterior compression (APC), vertical shear (VS), and combined (CM) type according to the Young-Burgess classification. The Survivor group included 11 patients (61.1%), and the non-survivor group included 7 patients (38.9%). The mean ages for the survivor and the non-survivor groups were 40.0 and 45.6 years (p=0.517). The types of pelvic bone fractures were LC 11 (61.1%), APC 6 (33.3%), and VS 1 (5.6%): LC 7 (63.6%), and APC 4 (36.4%) in the survivor group and LC 4 (57.1%), APC 2 (28.6%), and VS 1 (14.3%) in the non-survivor group. The internal iliac artery was the predominant injured vessel among both the survivors (n = 5, 45.5%) and the non-survivors (n = 4, 57.1%). No differences in initial blood pressures, ISS, and RTS existed between the two groups, but the arterial pH was lower in the non-survivor group (pH 7.09 (${\pm}0.20$) vs 7.30 (${\pm}0.08$), p=0.018). The number of transfused 24-hour units of packed RBC was greater in the non-survivor group ($24.1{\pm}12.5$ vs $14.4{\pm}6.8$, p=0.046). Conclusion: No differences in initial blood pressure and trauma scores existed between survivors and non-survivors with pelvic bone fractures, who had been treated with arterial embolization, but arterial pH was lower the in non-survivors.

The Effects of Cultural Capital and Social Welfare Expenditure on the Elder's Subjective Happiness

  • The purpose of this study is to introduce policy and theoretical implications by analyzing affecting factors for the elder's happiness. For this study, we analyzed data using HLM. Data include a world value survey(hereafter, WVS) as personal level analysis data and also OECD's Social Expenditure Database(hereafter, SOCX) and database from the World Bank as national level analysis data. The subjects of personal level analysis were the elder who are over 65-years od age, and they were total 3,297 people, and while the subjects of national level analysis were total 9 OECD countries. For the data analysis, hierarchial linear model(HLM) analysis was done by using HML 7.0 program. As a result of analysis, First, for the elderly's happiness, they should improve self-disposition, members of social groups, and social class. Second, the old-age pension and the survivor's pension had no meaningful effect on the happiness. but it was found that self - disposition, social class, gender, and health status showed meaningful interaction effect according to old - age pension, survivor pension, per capita GDP, income inequality. This suggests that efforts to improve the happiness of the elderly should be made at the individual level and the national level at the same time.

동료멘토(peer mentor) 프로그램이 뇌졸중 환자의 회복에 미치는 영향: 연구 프로토콜 (A Effect of Peer Mentor Program on Recovery after Stroke)

  • 본 연구는 해외 많은 분야의 재활에서 적용되고 있는 peer mentor 프로그램을 국내에 연구적, 임상적으로 도입하여 뇌졸중 환자의 정신적, 신체적 회복을 돕는 방법을 소개하고자 하였다. 두 집단 실험 설계로 뇌졸중이 발생한지 3-6개월인 환자들을 대상으로 설계하였다. 동료멘토는 뇌졸중이 발생한지 2년이 지난 외래 환자로 서로 다른 회복수준을 보이는 환자들로 구성하고, 프로그램을 운영하기에 앞서 참가자들에게 심리적, 평가적, 정보적인 도움을 주는 방법과 프로그램 운영 방법에 대해 교육한다. 한국판 노인 우울 척도 단축형 (Geriatric Depression Scale Short Form-Korea Version; GDSSF-K)을 사용하여 프로그램 참가자들의 심리적 요인을 측정하고, 뇌졸중 영향척도(Stroke Impact Scale; SIS)를 통해 회복수준을, 노인 상지 기능평가도구(Upper Extremity Function Test for the Elderly; TEMPA)을 사용하여 신체적 요인을 측정한다. 본 연구를 토대로 연구를 진행하고자 하는 연구자는 본 연구에서 제안한 방법과 더불어 뇌졸중 환자의 회복에 더 효과적인 프로그램 적용 방법에 대한 모색이 필요할 것이다.

Assessment of Respiratory Problems in Workers Associated with Intensive Poultry Facilities in Pakistan

  • Background: The poultry industry in Pakistan has flourished since the 1960s; however, there are scarce data regarding the impact of occupational exposure on the pulmonary health of farm workers in terms of years working in the industry. The objective of the present study was to assess the effect of poultry environment on the health of occupationally exposed poultry farmers in countries of warm climatic regions, such as Pakistan. This study will also show the effect of exposure to poultry facilities on the health of poultry farmers in the context of low-income countries with a relatively inadequate occupational exposure risk management. Materials and methods: The lung function capacity of 79 poultry workers was measured using a spirometer. Along with spirometry, a structured questionnaire was also administrated to obtain information about age, height, weight, smokers/nonsmokers, years of working experience, and pulmonary health of farm workers. The workers who were directly involved in the care and handling of birds in these intensive facilities were considered and divided into four groups based on their years of working experience: Group I (3-10 months), Group II (1-5 years), Group III (6-10 years), and Group IV (more than 11 years). The forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and the FEV1/FVC ratio were considered to identify lung function abnormalities. Statistical analysis was carried out using independent sample t test, Chi-square test, Pearson's correlation, and linear regression. Results: Based on the performed spirometry, 68 (86 %) of workers were found normal and healthy, whereas 11 (14 %) had a mild obstruction. Of the 11 workers with mild obstruction, the highest number with respect to the total was in Group IV (more than 11 years of working experience) followed by Group III and Group II. Most of the workers were found healthy, which seems to be because of the healthy survivor effect. For the independent sample t test, a significant difference was noticed between healthy and nonhealthy farmers, whereas Chi-square test showed a significant association with height, drugs, and working experience. Linear regression that was stratified by respiratory symptoms showed for workers with symptoms, regression models for all spirometric parameters (FVC, FEV1, and FEV1/FVC) have better predictive power or R square value than those of workers without symptoms. Conclusion: These findings suggest that lung function capacity was directly related to years of working experience. With increasing number of working years, symptoms of various respiratory problems enhanced in the poultry workers. It should be noted that most of the poultry workers were healthy and young, the rationale being that there is a high turnover rate in this profession. The mobility in this job and our finding of 86% of the healthy workers in the present study also proposed healthy worker survivor effect.

근로자의 화학적 노출과 주관적 호흡곤란 증상간의 연관성: 3차 근로환경조사 자료 분석 (Association of Exposure to Chemicals with Dyspnea among Employed Workers: Analysis of the 3rd Korean Working Conditions Survey)

  • Objectives: The purpose of this study was to investigate whether chemical exposure among workers has relevance to dyspnea using data from the $3^{rd}$ Korea Working Conditions Survey. Methods: The research subjects were 29,711 wage workers. Chemical exposures consist of four factors, (1) breathing in vapors, fumes, dust and dirt, (2) breathing in organic solvent vapors, (3) handling or touching chemicals, and (4) secondhand smoke. Multiple logistic regression analysis was performed to examine the association between chemical exposure and dyspnea in the last 12months using proc surveylogistic in SAS 9.3 statistical software excluding people who had received a diagnosis of hypertension or obesity that can affect the respiratory distress symptoms(n=27,842). Results: Chemical exposure among workers was associated with dyspnea after adjustment for demographics and job characteristics. Prevalence of dyspnea was 4.9 per 1,000 among men and 5.8 per 1,000. Compared to a total score of 0 points of chemical exposure among workers, a total score of 1, 2-3, and 4 points tended to have increased risk of dyspnea in a dose-response manner for both men (OR=1.43, 1.93, and 4.26; P-for trend=0.002) and women(OR=1.10, 2.81, and 7.70; P-for trend=0.002). Stratified analysis by duration of current job showed that the association between chemical exposure and dyspnea tended to get stronger until 15 years and then disappeared afterwards, which reflects healthy worker survivor effect. Conclusions: We observed significant association between chemical exposure and dyspnea using the data of the $3^{rd}$ Korea Working Conditions Survey. Our results warrants providing knowledge related to chemical exposure, performing prevention activities, and creating various health policies to protect workers.

  • Objectives: This study aimed to prepare basic data to improve the health-related quality of life of cancer survivors by confirming their oral and mental health statuses and identifying factors affecting their health-related quality of life. Methods: Of all participants in the 8th KNHANES (2019-2020), adults aged 19 years or older who responded 'yes' to the diagnosis of cancer and 404 cancer survivors who responded 'none' to the current cancer prevalence item were selected as the final research participants. Multiple regression analysis was conducted to confirm the effect of cancer survivors' oral and mental health on health-related quality of life. Results: Subjective oral health (p<0.01), chewing problems (p<0.05), subjective health (p<0.001), and depression (p<0.01) had an effect on health-related quality of life from multiple regression analyses. Conclusions: Therefore, oral and mental health promotion may improve health-related quality of life. Thus, it is necessary to recognize the importance of oral and mental health and implement preventive education and programs.

성폭력 피해에 대한 조직배반과 우울의 관계: 외상 후 인지의 매개효과 (Institutional Betrayal to Sexual Victimization and Depression: The Mediating Role of Posttraumatic Cognitions)

  • 자신이 신뢰하는 대상에게 성폭력 피해 경험을 이야기했을 때 받게 되는 부정적인 반응은 상대방에 대한 믿음이 깨지는 배반외상 경험이 될 수 있으며, 이는 자신과 세상에 대한 부정적인 인지를 형성하고 우울로 이어질 수 있다. 기존의 연구들에서는 가족, 친구, 지인 등의 비공식적 주체가 보이는 사회적 반응에 대한 연구가 주로 이루어졌다. 그러나 개인은 생존, 안전, 생계, 교육 등의 다양한 이유로 자신이 속해있는 조직을 신뢰하고 의지하게 되며, 조직이 개인의 성폭력 피해에 대해 보이는 부정적인 반응인 조직배반(institutional betrayal) 역시 조직이 자신을 보호해줄 것이라는 믿음이 배반되는 외상 경험이 되어 외상 후 인지 및 우울과 연관될 수 있다. 그러나 조직배반이 우울로 이어지는 구체적인 기제를 밝히는 연구는 아직 이루어지지 않았다. 이에 본 연구에서는 외상 후 증상을 이해하고 치료하는 대표적인 접근 중 하나인 인지행동이론에 기반하여, 조직배반이 우울로 이어지는 경로를 외상 후 인지가 매개하는지를 살펴보고자 하였다. 이를 위해 성폭력 피해에 대해 조직배반을 경험한 여성 462명을 대상으로 설문조사를 실시하고 구조 모형을 검증하였다. 그 결과 조직배반은 우울에 직접적으로는 영향을 미치지 못하였으며, 자신에 대한 부정적 인지와 세상에 대한 부정적 인지를 통해서 간접적으로 우울에 간접적으로 영향을 미치는 것으로 확인되었다. 이는 성폭력 피해에 대해 조직배반을 경험한 사람들이 자신과 세상에 대해 가지고 있는 부정적인 인지를 확인하고 다루는 것이 이들의 우울을 예방하거나 치료하는 데 있어서 중요함을 시사한다. 본 연구의 결과를 바탕으로 연구의 의의와 제한점 그리고 후속 연구를 위한 제언을 논의하였다.

글라이포세이트 중독 환자에서 포함된 염의 종류에 따른 예후의 차이 (The Prognosis of Glyphosate herbicide intoxicated patients according to their salt types)

  • Purpose: Glyphosate herbicide (GH) is a widely used herbicide and has been associated with significant mortality as poisoned cases increases. One of the reasons for high toxicity is thought to be toxic effect of its ingredient with glyphosate. This study was designed to determine differences in the clinical course with the salt-type contained in GH. Methods: This was a retrospective study conducted at a single hospital between January 2013 and December 2017. We enrolled GH-poisoned patients visited the emergency department. According to salt-type, patients were divided into 4 groups: isopropylamine (IPA), ammonium (Am), potassium (Po), and mixed salts (Mi) groups. The demographics, laboratory variables, complications, and their mortality were analyzed to determine clinical differences associated with each salt-type. Addtionally, we subdivided patients into survivor and non-survivor groups for investigating predictive factors for the mortality. Results: Total of 348 GH-poisoned patients were divided as follows: IPA 248, Am 41, Po 10, and Mi 49 patients. There was no difference in demographic or underlying disease history, but systolic blood pressure (SBP) was low in Po group. The ratio of intentional ingestion was higher in Po and Mi groups. Metabolic acidosis and relatively high lactate level were presented in Po group. As the primary outcome, the mortality rates were as follows: IPA, 26 (10.5%); Am, 2 (4.9%); Po, 1 (10%); and Mi, 1 (2%). There was no statistically significant difference in the mortality and the incidence of complications. Additionally, age, low SBP, low pH, corrected QT (QTc) prolongation, and respiratory failure requiring mechanical ventilation were analyzed as independent predictors for mortality in a regression analysis. Conclusion: There was no statistical difference in their complications and the mortality across the GH-salt groups in this study.
