중환자실 환자의 입실시 반코마이신 내성 장구균 집락의 위험요인과 임상적 결과 (Risk Factors and Clinical Outcomes for Vancomycin-Resistant Enterococcus Colonization on Intensive Care Unit Admission)

  • Purpose: The purpose of this study was to identify vancomycin-resistant enterococcus (VRE) colonization rate in patients admitted to the intensive care unit (ICU), associated risk factors and clinical outcomes for VRE colonization. Methods: Of the 7,703 patients admitted to the ICUs between January, 2008 and December, 2010, medical records of 554 VRE colonized and 503 uncolonized patients were reviewed retrospectively. To analyzed the impact of colonization on patients' clinical outcomes, 199 VRE colonized patients were matched with 199 uncolonized patients using a propensity score matching method. Results: During the study period, 567 (7.2%) of the 7,703 patients were colonized with VRE. Multivariate analysis identified the following independent risk factors for VRE colonization: use of antibiotics (odds ratio [OR]=3.33), having bedsores (OR=2.92), having invasive devices (OR=2.29), methicillin-resistant Staphylococcus aureus co-colonization (OR=1.84), and previous hospitalization (OR=1.74). VRE colonized patients were more likely to have infectious diseases than uncolonized patients. VRE colonization was associated with prolonged hospitalization and higher mortality. Conclusion: Strict infection control program including preemptive isolation for high-risk group may be helpful. Further research needs to be done to investigate the effects of active surveillance program on the incidence of colonization or infection with VRE in the ICU.

프레임 차이법과 데시벨 임계치를 이용한 무인 환자 감시 시스템 (Unmanned Patient Monitoring System Using Frame Difference Method and Decibel Threshold)

  • 본 논문에서는 24시간 감시가 필요한 코마 환자나 움직이기 힘든 중환자들에게 사용될 수 있는 움직임 검출과 음향 감지를 이용한 무인 환자감시시스템을 구현하였다. 이 시스템의 구성은 노트북, 아날로그 CCTV 카메라(또는 PC 카메라), A/D 컨버터, 마이크로폰 그리고 감시 프로그램으로 구성되었다. 감시 프로그램은 프레임 차이법과 음향 레벨측정을 기본으로 사용하며, 움직임이 검출될 때의 자료수집과 저장과 같은 기능을 가진다. 모든 시스템은 코마 상태의 환자를 가정한 실험을 통해 검증하였다. 실험 결과 본 논문에서 제안한 무인 환자감시 시스템은 응급 상황과 코마 환자의 관리에서 효율적으로 사용될 수 있을 것이라 예상된다.

Multidisciplinary Approach to Breast Cancer Care

  • Aim: The purpose of this paper is to present the importance of multidisciplinary strategies in cancer prevention and control, especially comprehensive breast cancer care. Background: Worldwide, breast cancer is the most common cancer diagnosed among women and is the leading cause of cancer deaths. Although the incidence of breast cancer in Asian countries is still lower than in Western countries, the rate of increase for the last two decades is striking. Methods: Data on cancer mortality, incidence, and risk factors were summarized by using the most recent data available from population-based cancer registries affiliated with the International Union Against Cancer, the National Cancer Institute's Surveillance, Epidemiology, and End Results (SEER) program and the CDC's National Program of Cancer Registries (NPCR). Results: Global differences in breast cancer incidence and fluctuations in rates within a country still exist. The incidence of breast cancer in Asian countries was lower than in Western countries. Breast cancer incidence in the United States decreased each year during 1999-2003. On the other hand, morbidity and mortality related to breast cancer in Asia has increased significantly. Conclusion: Multidisciplinary strategies to reduce breast cancer mortality and promote breast cancer awareness are addressed. Lessons learned from multidisciplinary approaches to cancer treatment and control will be valuable in implementing future breast cancer research in the fields of basic, clinical, and population research in Asia.

인셉션 모듈 기반 컨볼루션 신경망을 이용한 얼굴 연령 예측 (Facial Age Estimation Using Convolutional Neural Networks Based on Inception Modules)

  • Automatic age estimation has been used in many social network applications, practical commercial applications, and human-computer interaction visual-surveillance biometrics. However, it has rarely been explored. In this paper, we propose an automatic age estimation system, which includes face detection and convolutional deep learning based on an inception module. The latter is a 22-layer-deep network that serves as the particular category of the inception design. To evaluate the proposed approach, we use 4,000 images of eight different age groups from the Adience age dataset. k-fold cross-validation (k = 5) is applied. A comparison of the performance of the proposed work and recent related methods is presented. The results show that the proposed method significantly outperforms existing methods in terms of the exact accuracy and off-by-one accuracy. The off-by-one accuracy is when the result is off by one adjacent age label to the above or below. For the exact accuracy, the age label of "60+" is classified with the highest accuracy of 76%.

A Case-Study of Implementation of Improved Strategies for Prevention of Laboratory-acquired Brucellosis

  • Background: In 2012, the Alaska Section of Epidemiology investigated personnel potentially exposed to a Brucella suis isolate as it transited through three laboratories. Methods: We summarize the first implementation of the United States Centers for Disease Control and Prevention 2013 revised recommendations for monitoring such exposures: (1) risk classification; (2) antimicrobial postexposure prophylaxis; (3) serologic monitoring; and (4) symptom surveillance. Results: Over 30 people were assessed for exposure and subsequently monitored for development of illness. No cases of laboratory-associated brucellosis occurred. Changes were made to gaps in laboratory biosafety practices that had been identified in the investigation. Conclusion: Achieving full compliance for the precise schedule of serologic monitoring was challenging and resource intensive for the laboratory performing testing. More refined exposure assessments could inform decision making for follow-up to maximize likelihood of detecting persons at risk while not overtaxing resources.

  • The primary objective of this paper is to introduce the application of Human-Machine Interface (HMI) and Augmented Reality (AR) technologies in flight operations. These include: self-check-in, baggage handling, airport security and surveillance, airport operations monitoring, In-Flight Entertainment and Connectivity (IFEC), cockpit design, and cabin crew support. This paper investigates the application status and development trends of HMI and AR technologies for airports and aircraft. These technologies can provide more efficient in-flight passenger service and experience by using AR devices. This paper also discusses the developments such as; the Integrated Control Application (ICA) for the IFEC interface, AR flight simulation training program using the fixed-based simulator, and the AR aircraft cabin interior concept test program. These applications present how HMI and AR techniques can be utilized in actual flight operations. The developed programs in this paper can be applied to their purpose within aircraft interiors and services to enhance efficiency, comfort, and experience.

Reduction of Reinfection Rates with Opisthorchis viverrini through a Three-Year Management Program in Endemic Areas of Northeastern Thailand

  • To clarify the reinfection profile associated with risk factors of opisthorchiasis, we conducted an epidemiological study on the chemotherapeutic effects on reinfection with O. viverrini in the endemic areas of Northeastern Thailand for 3 years. A total of 3,674 fecal samples were collected from participants in villages of 5 provinces. They were examined microscopically using a modified technique of formalin ethyl-acetate concentration. Egg-positive residents were reexamined year (2018) by year (2019) after treatment with a single dose (40 mg/kg) of praziquantel. Health education was provided to the participants yearly. The egg-positive rate of O. viverrini was 14.3%, and was highest (22.2%) in the 20-30 year-old group in 2017. The egg positive rate was 15.3% in dogs and 11.4% cats. Human reinfection rate was 15.5% and 6.3% in next 2 years, and was highest (23.2%) among the fishermen. Relative risk factors of reinfection were significantly higher for males, over 40-year-old age, or working as fishermen or farmers, and eating uncooked fish within the preceding year. A significant difference resulting from a health education program was observed in the third year. Therefore, health education and sustainable surveillance for opisthorchiasis should be maintained to decrease the risk of reinfection.

Changes in age-specific seroprevalence of Japanese encephalitis virus and impact of Japanese encephalitis vaccine in Korea

  • The Japanese encephalitis (JE) virus is the leading cause of vaccine-preventable encephalitis in Asia. Since the introduction of a universal JE vaccination program and urbanization of Korea, the incidence of JE has dramatically decreased in Korea. However, recent JE cases have occurred, predominantly among unvaccinated adults and with a shift in age distribution. Here we aimed to review the changes in age-specific JE seroprevalence over time and discuss the implications of JE vaccination programs in Korea. Following the last epidemic in 1982-1983, mandatory vaccination for all children aged 3-15 years was conducted annually until 1994. However, JE has reemerged, predominantly affecting unvaccinated adults aged 40 years or older and demonstrating a shift in age distribution toward older populations. The age-specific seroprevalence of the JE virus in Korea has changed noticeably over time. Seropositivity in children and adolescents increased from 10%-59% in the 1970s to 90%-92% in the 1980s after the implementation of the JE vaccination program and increased further to 98% in 2012. No age-specific difference in the seroprevalence of JE was found, and appropriate levels of immunity to JE were maintained for all age groups. Continuous surveillance of the seroprevalence of JE is essential to establish a proper immunization policy in Korea.

Qualification Test of ROCSAT -2 Image Processing System

  • ROCSAT-2 mission is to daily image over Taiwan and the surrounding area for disaster monitoring, land use, and ocean surveillance during the 5-year mission lifetime. The satellite will be launched in December 2003 into its mission orbit, which is selected as a 14 rev/day repetitive Sun-synchronous orbit descending over (120 deg E, 24 deg N) and 9:45 a.m. over the equator with the minimum eccentricity. National Space Program Office (NSPO) is developing a ROCSAT-2 Image Processing System (IPS), which aims to provide real-time high quality image data for ROCSAT-2 mission. A simulated ROCSAT-2 image, based on Level 1B QuickBird Data, is generated for IPS verification. The test image is comprised of one panchromatic data and four multispectral data. The qualification process consists of four procedures: (a) QuickBird image processing, (b) generation of simulated ROCSAT-2 image in Generic Raw Level Data (GERALD) format, (c) ROCSAT-2 image processing, and (d) geometric error analysis. QuickBird standard photogrammetric parameters of a camera that models the imaging and optical system is used to calculate the latitude and longitude of each line and sample. The backward (inverse model) approach is applied to find the relationship between geodetic coordinate system (latitude, longitude) and image coordinate system (line, sample). The bilinear resampling method is used to generate the test image. Ground control points are used to evaluate the error for data processing. The data processing contains various coordinate system transformations using attitude quaternion and orbit elements. Through the qualification test process, it is verified that the IPS is capable of handling high-resolution image data with the accuracy of Level 2 processing within 500 m.

여대생의 미디어 노출과 관여도가 객체화된 신체의식 및 외모관리행동에 미치는 영향 (The Effects of Media Exposure and Involvement of Female University Students on Objectified Body Consciousness and Appearance Management Behaviors)

  • This study examined the effects of media exposure and involvement on objectified body consciousness and appearance management behaviors. The subjects were 325 female university students in Daejeon and Chungnam Province. The research method was a survey, and the questionnaire was composed of media exposure and involvement, objectified body consciousness, appearance management behaviors, and subjects' demographic characteristics. The data were analyzed by descriptive statistics, Cronbach's ${\alpha}$, factor analysis, and regression analysis using SPSS program. The results were as follows. First, female university students used mass media for about 3.76 hours a day and showed a high level of media involvement. Second, three factors (body surveillance, body shame, and appearance control belief) emerged regarding objectified body consciousness with female university students showing a high level of objectified body consciousness. Third, six factors (make-up, plastic surgery, weight control, fashion, hair, and skin care) emerged regarding appearance management behaviors with female university students showing high intention to perform various appearance management behaviors. Fourth, media exposure and involvement had important effects on objectified body consciousness and appearance management behaviors, and media involvement was a more important variable than media exposure. Fifth, objectified body consciousness had important effects on appearance management behaviors. Body shame had more important effects on skin care, weight control, and plastic surgery behaviors, while body surveillance had more important effects on fashion, make-up, and hair management behaviors than other objectified consciousness factors. The implication of this study was that media involvement is a more important variable affecting objectified body consciousness and appearance management behaviors than media exposure, and among objectified body consciousness dimensions, body shame has important effects on more active appearance management behaviors like weight control and plastic surgery behaviors.
