우측폐 전절제술후 발생한 식도늑막루의 수술 치험 : 1례 보고 (Postpneumonectomy Esophagopleural Fistula: Muscle Flap Transposition for Closure)

  • Esophagopleural fistula is a rare complication that should be suspected in all patients with recurrent empyema following pneumonectomy and in whom a bronchopleural fistula can be excluded. In late postpneumonectomy esophagopleural fistula, diagnosis is difficult due to its rarity and no specific symptom and sign, but we have experienced a man who had suffered dysphagia and odynophagia. In surgical treatment of late postpneumonectomy esophageal fistula, closure of empyema space is of prime importance. We have adopted a type of latissimus dorsi muscle and serratus anterior muscle flap transposition We present here this technique and result obtained in patient with late postpneumonectomy esophagopleural fistula.

기관협착증 치료에 대한 외과적 고찰 (Surgical Study in Treatment of the Tracheal Stenosis)

  • Between 1985 and 1990, 41 patients underwent treatment of the tracheal stenosis. Nineteen patients underwent resection and end-to-end anastomosis including three cases of the subglottic stenosis. Other patients had had treatment such as LASER therapy, bronchoscopic removal, insertion of the Montgomery silastic T-tube or stent insertion Nineteen patients which underwent resection and end-to-end anastomosis were excellent result from three years to sixth months. Other patient were followed at OPD for the other complication or restenosis. There were no hospital death but one patient was managed by bronchoscopic removal of the granulation tissue and other one patient had underwent reoperation for the dehiscence at the anastomotic site.

심막파열을 동반한 흉부대동맥 파열 치험 1례 보고 (Traumatic Rupture of Thoracic Aorta with Pericardial Rupture - Report of 1 Case -)

  • Still a lethal injury, traumatic rupture of thoracic aorta occurs more frequently than we expect and comprises significant part of causes of deaths by blunt trauma. We recently experienced a thoracic aortic rupture accompanied by multiple injuries including pericardial and interatrial septal rupture and myocardial contusion in a patient who had been injured in a fall accident. Literatures are reviewed with the concern of early diagnosis, surgical technique and the result of operation.

  • Forty patients with spontaneous pneumothorax underwent vertical axillarty thoracotomy for surgical bullectomy of spontaneous pneumothorax between June, 1991 and september, 1992. We evaluated the method in terms of postoperative pain, wound complication, days of hospital stay and cosmetic result. It`s concludid that verical axillary thoracotomy provides satisfactory exposure for limited procedures within the thoracic cavity and offers the specific advantages of minimal and cosmetically acceptable results when compared with the thoracotomy methods.

폐에 발생한 경화성 혈관종 -3예 보고- (Sclerosing hemangioma of the lung -3 cases report-)

  • Sclerosing hemangiomas of the lung are uncommon benign tumors. Although their histologic appearances are distinct and well-defined, their histogenesis is uncertain. Liebow and Hubbell reported seven cases of unusual pulmonary lesions which they named sclerosing hemangiomas in 1856. Similar cases had previously been reported under different names: Xanthoma, pulmonary histiocytoma, Xanthofibroma. We have experienced 3 cases of sclerosing hemangioma of lung at the Department of Thoracic and Cardiovascular Surgery, Yonsei University, college of Medicine, Seoul, Korea during a period of December 1985 through September 1988. This report is an early result of surgical treatment of three patients with pulmonary sclerosing hemangioma and their pathologic findings.

흉선종양과 Myasthenia Gravis가 합병한 1례 보고 (A Case Report of Myasthenia Gravis Combined with Thymoma)

  • The incidence of myasthenia gravis in patients with thymomas has been reported from 10 to 50 percent, and the controversy ranges from 8-15 percent. This is the report of myasthenia grayis combined with benign thymoma and obtained relatively good result through surgical treatment at the department of thoracic surgery, National Medical center. The thymoma was mixed type and benign nature. Post operatively this patient was developed myasthenia crisis, and treated with mechanical respirator, and recurred the symptoms about 2months later, but well controlled with medical treatment.

빠르게 성장한 거대 종격동 양성기형종 (Rapidly Grown Huge Mediastinal Benign Teratoma ; one case report)

  • The benign teratoma is usually slow growing tumor, but we expirienced a case of primary huge mediastinal benign teratoma that had grown very rapidly, maximally during 3 years. The 14-year-old female patient was admitted to our hospital because of abnormal chest X-ray that showed 10$\times$10cm sized well definded mass with multiple calcificactions. but the mass was not present in chest X-ray perfomed on 3 years prior to admission. Under the diagnosis of teratoma, complete surgical resection was done by the left thoracotomy. The result of pathology was benign teratoma.

누두흉의 수술 교정 -8예 보고- (Surgical correction of funnel chest (8 cases))

  • Funnel chest is the most common deformity of the sternum. It is characterized by a funnel shaped depression of the sternum with sharp angulation and lengthening of the costal cartilages, with the result that the cartilages point posteriorly. We have experienced with eight cases of funnel chest for 2 years recently, and they were corrected by the method of Ravitch operation with or without Kirschner`s wire. The postoperative course was uneventful. And now we report these with literature review.

식도 평활근육종의 외과적 치료 -1례 보고- (Surgical Treatment of Esophageal Leiomyosarcoma -1 Case Report-)

  • Esophageal leiomyosarcoma is a rare lesion. [0.5 % of all esophageal malignancy] The patient was 74 year old female and complained postprandial vomiting of 4 years, duration & hematemasis of 2 months` duration. On gastrofiberscopy, huge exophytic mass nearly occluding esophageal lumen located 30∼38 cm from upper incisor was found. Endoscopic biopsy was squamous cell carcinoma. Transthoracic esophagectomy and esophagogastrotomy was carried out. Result of biopsy was esophageal leiomyosarcoma.
