Platysma myocutaneous flap - its current role in reconstructive surgery of oral soft tissue defects

  • Reconstruction of oral soft-tissue defects following resection of oral carcinomas can be achieved using various techniques including microsurgical tissue transfer. However, there seems to be a role for regional or local flaps. Small to medium-size defects can be functionally reconstructed with the platysma myocutaneous flap as an excellent choice particularly in medically compromised patients not being eligible for free tissue transfer. The present paper reviews the indication, surgical technique, and complications following reconstruction of defects of the oral cavity with the platysma myocutaneous flap.

  • This article has reviewed the alignment of the ankle and hindfoot in varus ankle osteoarthritis. The indication and surgical technique of the supramalleolar osteotomy has been reviewed. Alignment of tibial plafond and hindfoot is variable in early stages of ankle osteoarthritis. Supramalleolar osteotomy is indicated for the treatment of ankle osteoarthritis in patients with minimal talar tilt and neutral or varus heel alignment.

A clinical evaluation of surgical treatment of esophageal diverticulum (식도게실의 외과적 치료에 대한 임상적 고찰)

  • This is a report of 12 cases of esophageal diverticulum treated surgically at the Department of Thoracic and Cardiovascular Surgery, Hanyang University Hospital during the 16 years. 10 patients were female and 2 were male and the age distribution was between 31 and 61 years. Various subjective symptoms were noticed preoperatively. Diagnosis was confirmed by esophagography. The type of esophageal diverticulum were pulsion type in 11 cases and traction type in one case, and they were all located on the mid-thoracic esophagus, but one case was associated with epiphrenic pulsion diverticulum After diverticulectomy with or without myotomy, subjective symptoms disappeared and there were no operative complications and mortality. Indication of myotomy evaluated with motilities status by fluoroscopic finding of X-ray barium study. Recurrence of symptoms was not noticed during follow-up.

Clinical Review of Metastatic Lung Tumor (전이성폐종양에 대한 고찰)

  • 52 cases of metastatic lung tumor, including 22 [42.3%] choriocarcinoma., 13[25%] liver cancer, 6 [7.5%] stomach cancer, 2 bone tumor, etc, were reviewed at Busan National University Hospital, during a 5 year period ending with 1974. The age distribution of metastatic lung tumor were same in second decade to 5th decade. The most common appearances of roentgenograms of metastatic lung tumor were that of multinodular [42.3%], diffuse acinar [21.1%],infiltrative or pneumonic[21.1%], solitary lesion [9.6%] and diffuse micronodular [5.7%]. The metastatic lesions originated from choriocarcinoma revealed multinodular and acinar in roentgenogram, and the lesions originated from liver and stomach cancer revealed infiltrative and acinar. In our series, the positive hilar lymph node enlargement was encountered in 12 cases, of which 4 were in liver cancer and 3 were in stomach cancer. Pleural effusion was also encountered in 5 cases, of which 3 were in liver cancer. The treatment of this series was almost conservative except of 2 cases of pulmonary resection, and the reason of this limited cases of surgical treatment was seemed due to the delayed direction of metastasis to lung beyond the proper indication of surgery.

Surgical Experience of Infective Endocarditis (심내막염 환자의 외과적 치험)

  • From May 1984 through December 1991, twelve patients underwent valve replacement for infective endocarditis at National Medical Center. There were 7 male and 5 female, ranged in age 16 to 61[mean 34.1] years. Four had native valve endocarditis, six had prosthetic valve endocarditis and two were associated with congenital heart disease. The indication of surgery was medically intractable congestive heart failure in all patients. 5 patients revealed systemic embolization and 4 patients had uncontrollable sepsis. The causative organism was Streptococcus in 4 patients, Staphylococcus in 1 patient and Pseudomonas in 2 patients. Hospital mortality was 33.3%[4/12]. The main cause of death was low cardiac output due to perioperative myocardial damage and cerebral vascular accident. There were 2 late mortality because of recurrent endocarditis. This review showed much higher mortality in prosthetic valve endocarditis[66.7%] than native valve endocarditis[33.3%].

  • Cervical disc herniation is one of the most common causes of neck, shoulder and arm pain. There are many treatments for a cervical disc herniation, such as rest, physical therapy, medication, epidural steroid injection and surgery. However, conservative treatments sometimes have limited effectiveness, and a surgical discectomy is often associated with numerous complications. Nowadays, a percutaneous discectomy, using a $Dekompressor^{(R)}$, has been used in herniated disc patients, but a posterolateral extruded disc is not an indication. Herein, our experience using a 19 G $Dekompressor^{(R)}$, on a 52 year-old male patient with a left C6-⁣7 posterolateral extruded disc, is reported. Decompression was successfully performed, and the pain and range of motion was immediately improved.

  • Objective : The purpose of this study is evaluated the effects of Oriental treatments to symptoms and quality of life in the patients with herniated cervical disc being suggested operation. Methods : 20 patients diagnosed with herniated cervical disc by MRI in hospital were treated by oriental medical treatments. Symptoms of the patients were assessed by analogue scale(VAS) and SF-36. Results : After 12 weeks treatments, the VAS of symptoms of herniated cervical disc and the score of SF-36 were significantly improved. Conclusions : It could be suggested that oriental medical treatments were effective for the patients with herniated cervical disc being suggested operation. Further study of the effect of oriental medical treatments on herniated cervical disc is needed.

A Periosteal Fibrosarcoma in a Puppy Dog (강아지에서의 골막섬유육종 증례)

  • A 4 months old Golden retriever male dog was referred. By the history taking, it was kwon that the mass has been formed at the parietal region of the patient after fourth prevention inoculation. The appetite, urination and excretion were normal. The mass confirmation which was limited in the parietal region, did not seem to be enlargement of lymph node from physical examination palpation. Complete blood count and serum biochemical profiles were consistent with mild lymphocytosis, mild monocytosis, anisocytosis, regenerative anemia and elevated ALP, CPK. Survey radiographs indicated a mixed density in parietal region. We performed surgical resection and chemotherapy for cure and biopsy. Histological finding was periosteal fibrosarcoma. The application of cisplatin was topically used in operation site. Up to now the indication of recurrence is not visible.

Surgical Treatment of Pulmonary Aspergillosis (폐국균증의 외과적 치료)

  • From January, 1980, to August, 1989, 23 patients underwent thoracotomy for treatment of pulmonary aspergillosis on the Department of Thoracic and Cardiovascular Surgery, School of Medicine, Keimyung University. There were 12 male and 11 female patients ranging from 23 years to 61 years old[mean age, 40.7 years]. The main clinical symptoms were hemoptysis[91.3%], cough[65.2%], chest pain[34.5%]. Sixteen patients[69.6%] had simple aspergilloma and 7[30.4%] had complex aspergilloma. The most common indication for operation was a hemoptysis, indeterminate mass, chronic cough, or obstructed bronchus. Anatomical location of lesion was mainly located upper lobe [82.6%] and most of cases were managed by lobectomy. Postoperative pathologic findings showed that 13 case[56.5%] were combined with tuberculosis, two were combined with bronchiectasis and two were combined with lung tumor, but 6 cases were not combined with other disease. Early complications occurred in 33.5% of patients with simple aspergilloma and in 85.7% of patients with complex aspergilloma. But there was no hospital death.

Office Based Injection Laryngoplasty (성대 주입술)

  • Office based injection laryngoplasty for voice and swallow disorders is increasing steadily. In recent years, the application of injection laryngoplasty is extended to inject medicine, such as cidofovir or botox. The reason why office based injection laryngoplasty is increasing in popularity is several potential advantages when compared with other procedures. It can avoid surgical scar and is easily performed with local anesthesia. However, injection can be performed in a variety of settings, depending on several factors, including the goal of the procedure, patient comfort and anatomy, and physician skill. In this article, we describe indication of injection laryngoplasty and discuss about various techniques for procedure.
