  • Robotic thymectomy has been adopted recently and has been shown to be safe and feasible in treating thymic tumors and myasthenia gravis. The surgical indications of robotic technology are expanding, with advantages including an excellent surgical view and sophisticated manipulation. Herein, we describe technical aspects, considerations, and outcomes of robotic thymectomy.

  • Geometry calculation Using Abdominal internal organ image from traditional laparoscopy or robotic surgery system we can make depth informations through measured 3D structure informations is very helpful to doctors, depth information is mare useful then others that use traditional laparoscopy or robotic surgery system to many doctors. however, traditional method are incomplete. less experienced doctors make much mare prohability of mistake. Hence, 3D information of organ is very helpful to the less experienced doctors. it will be greate role of reducing medical accidents and surgical time. We can get 3D informations using geometrical calculation method in robotic surgical system. also suggested method is needed in traditional surgical method without the need to create a new system, finally, We can get 3D information from traditional system without any new system, it take advantage in cost and create high efficiency. mare information will provided to many doctors.

  • Robotic surgery have been increasingly applied in the past several years to assist in a variety of operations. Some applications, such as in prostatectomy, have been widely adopted and have largely supplanted the alternative minimally invasive or open techniques. However, the use of surgical robots in esophagectomy has not been as common. Although a limited number of case reports demonstrated the feasibility of robotic esophagectomy, no large series of patients has been reported to date. The data on robotic-assisted esophagectomy suggest that the procedure is safe, feasible, and associated with perioperative outcomes similar to open and minimally invasive esophagectomy. Benefits of robotic system could be maximized when we do a extensive total mediastinal lymphadenectomy. For this procedure to gain widespread acceptance, identifiable benefit relative should be demonstrated in near future.

Intracorporeal Esophagojejunostomy during Reduced-port Totally Robotic Gastrectomy for Proximal Gastric Cancer: a Novel Application of the Single-Site® Plus 2-port System

  • Purpose: Intracorporeal esophagojejunostomy during reduced-port gastrectomy for proximal gastric cancer is a technically challenging technique. No study has yet reported a robotic technique for anastomosis. Therefore, to address this gap, we describe our reduced-port technique and the short-term outcomes of intracorporeal esophagojejunostomy. Materials and Methods: We conducted a retrospective review of patients who underwent a totally robotic reduced-port total or proximal gastrectomy between August 2016 and March 2020. We used an infra-umbilical Single-Site® port with two additional ports on both sides of the abdomen. To transect the esophagus, a 45-mm endolinear stapler was inserted via the right abdominal port. The common channel of the esophagojejunostomy was created between the apertures in the esophagus and proximal jejunum using a 45-mm linear stapler. The entry hole was closed with a 45-mm linear stapler or robot-sewn continuous suture. All anastomoses were performed without the aid of an assistant or placement of stay sutures. Results: Among the 40 patients, there were no conversions to open, laparoscopic, or conventional 5-port robotic surgery. The median operation time and blood loss were 254 min and 50 mL, respectively. The median number of retrieved lymph nodes was 40.5. The median time to first flatus, soft diet intake, and length of hospital stay were 3, 5, and 7 days, respectively. Three (7.5%) major complications, including two anastomosis-related complications and a case of small bowel obstruction, were treated with an endoscopic procedure and re-operation, respectively. No mortality occurred during the study period. Conclusions: Intracorporeal esophagojejunostomy during reduced-port gastrectomy can be safely performed and is feasible with acceptable surgical outcomes.

Robotic Intraoperative Tracheobronchial Repair during Minimally Invasive 3-Stage Esophagectomy

  • Tracheobronchial injury (TBI) is an uncommon but potentially fatal event. Iatrogenic lesions during bronchoscopy, endotracheal intubation, or thoracic surgery are considered the most common causes of TBI. When TBI is detected during surgery, concomitant surgical treatment is recommended. Herein we present a case of successful robotic primary repair of iatrogenic tracheal and left bronchial branch tears during a robot-assisted hybrid 3-stage esophagectomy after neoadjuvant chemoradiotherapy. A robotic approach can facilitate the repair of this injury while reducing both the potential risk of conversion to open surgery and the associated increased risk of postoperative respiratory complications.

로봇 테크놀로지 기반의 노인간호 활용전망 (Prospects of Geriatric Nursing Application Based on Robot Technology)

  • Purpose: The purpose of this study was to investigate the possibility of using intelligent robot based nursing practice as discussed in previous research and also, to propose directions for robot care for elders. Methods: For this study 28 previous researches using robots in health care field were reviewed and related research trends introduced. Results: Robot applications in the healthcare field were mainly for rehabilitation, surgery, interaction, and nursing assistance through robotics. Especially types of robot include pet type robots, humanoid robots, surgical robots, rehabilitation robots, robot suit and entertainment robots with monitors. The research participants were patients with dementia and institutionalized elderly people. It was found that a human-robot interaction was effective from physical, mental, emotional and social aspects. Conclusion: Robots can be used for various purposes such as nursing assistance, patient health promotion and education. It is necessary to reduce the human burden of care work using robots and to introduce robot care programs which can meet the needs of elders. Therefore, korean nurses should make efforts to change their practice to new geriatric nursing through repeated research based on the scientific data.

Transcutaneous medial fixation sutures for free flap inset after robot-assisted nipple-sparing mastectomy

  • The application of minimal invasive mastectomy has allowed surgeons to perform nipples-paring mastectomy via a shorter, inconspicuous incision under clear vision and with more precise hemostasis. However, it poses new challenges in microsurgical breast reconstruction, such as vascular anastomosis and flap insetting, which are considerably more difficult to perform through the shorter incision on the lateral breast border. We propose an innovative technique of transcutaneous medial fixation sutures to help in flap insetting and creating and maintaining the medial breast border. The sutures are placed after mastectomy and before flap transfer. Three 4-0 nylon suture loops are placed transcutaneously and into the pocket at the markings of the preferred lower medial border of the reconstructed breast. After microvascular anastomosis and temporary shaping of the flap on top of the mastectomy skin, the three corresponding points for the sutures are identified. The three nylon loops are then sutured to the dermis of the corresponding medial point of the flap. The flap is placed into the pocket by a simultaneous gentle pull on the three sutures and a combined lateral push. The stitches are then tied and buried after completion of flap inset.

Review of Photoacoustic Imaging for Imaging-Guided Spinal Surgery

  • This review introduces the current technique of photoacoustic imaging as it is applied in imaging-guided surgery (IGS), which provides the surgeon with image visualization and analysis capabilities during surgery. Numerous imaging techniques have been developed to help surgeons perform complex operations more safely and quickly. Although surgeons typically use these kinds of images to visualize targets hidden by bone and other tissues, it is nonetheless more difficult to perform surgery with static reference images (e.g., computed tomography scans and magnetic resonance images) of internal structures. Photoacoustic imaging could enable real-time visualization of regions of interest during surgery. Several researchers have shown that photoacoustic imaging has potential for the noninvasive diagnosis of various types of tissues, including bone. Previous studies of the surgical application of photoacoustic imaging have focused on cancer surgery, but photoacoustic imaging has also recently attracted interest for spinal surgery, because it could be useful for avoiding pedicle breaches and for choosing an appropriate starting point before drilling or pedicle probe insertion. This review describes the current instruments and clinical applications of photoacoustic imaging. Its primary objective is to provide a comprehensive overview of photoacoustic IGS in spinal surgery.

수술로봇을 이용한 심장수술 첫 체험 (Initial Experience of Robotic Cardiac Surgery)

  • 배경: 일반적으로 심장수술은 정중흉골절개를 통해 행해져 오고 있으며, 과거 십 년간 내시경 장비와 수술 수기의 향상은 작은 절개를 이용한 최소 침습적 심장수술의 발전을 이끌었다. 술자의 음성 명령을 인식하여 내시경을 움직이는 로봇 팔(AESOP 3000, Automated Endoscope System for Optimal Positioning)의 등장으로 심장수술은 로봇 시대에 진입하였다. 대상 및 방법: 2004년 4월부터 12월까지 총 78명의 환자들에게 수술로봇을 이용한 심장수술을 시행하였고 그 중 64명의 환자들에게는 음성명령으로 조절되는 로봇 팔과 대퇴 동정맥관 삽관, 경피적 내경정맥관 삽관, 흉곽을 통한 대동맥 겸자를 사용하여 5cm 우외측 최소개흉으로 로봇을 이용한 최소 침습적 심장수술을 시행하였다. 다른 14명의 환자들에게는 AESOP을 이용한 내흉동맥 박리를 통해 최소 침습적 관상동맥 우회술(MIDCAB)을 시행하였다. 결과: 로봇을 이용한 심장수술은 승모판막 성형술이 37예, 승모판막 치환술이 10예, 대동맥판막 치환술이 1예, MIDCAB이 14예, 심방중격결손증 수술이 9예, Maze 수술만 시행한 경우가 1예였다. 승모판 수술의 경우 평균 체외순환시간은 $165.3\pm43.1$분이었고 평균대동맥 차단 시간은 $110.4\pm48.2$분이였다. 재원일수의 중간값은 승모판 수술인 경우 6일($3\~30$일), MIDCAB은 4일($2\~7$일), 심방중격결손증 수술은 4일($2\~6$일)이였다. 합병증으로는 술 후 출혈로 재수술한 경우가 3예이였고 사망환자는 없었다. 결론: 수술로봇을 이용한 심장수술을 시행한 우리의 경험으로 볼 때 많은 심장외과 의사들이 로봇을 이용하여 작은 창상을 통해 최소 침습적 심장수술이 가능하리라 본다. 수술로봇을 이용한 심장수술의 이점을 분석하기 위해서는 잘 계획된 연구와 긴밀한 장기간의 관찰이 필요할 것으로 판단된다.
