후두암과 식도암의 이증원발성 종양에서의 합이식술을 이용한 식도 재건술 (Composite Graft Reconstruction of Esophagus for Double Primary Cancer of Larynx & Esophagus)

  • 식도절제술 후 재건술에서 위장은 가장 흔한 식도 대체물로 사용된다 그러나, 인두 이상의 위치까지 위장을 끌어 올려 연결하는 경우 위장만으로는 길이가 모자라는 경우가 많이 있다. 후두암과 식도암을 동반한 환자에서 후두전절제술과 식도전절제술을 시행하였고 유리공장이식편을 이용하여 경부에서 인두공장위장문합을 시행하였다. 유리공장을 이용하여 문합에 충분한 길이를 확보함으로써 문합부 긴장을 줄이고, 혈액공급을 확보할 수 있었다.

  • Simultaneous second primary tumors are not uncommon in patients with oral cavity squamous cell carcinoma. Many studies have previously shown that oral cavity squamous cell carcinoma patients with simultaneous second primary tumor generally have a poor prognosis. Additionally, the choice of the optimal therapeutic modality for oral cavity squamous cell carcinoma patients who present with simultaneous second primary tumor remains problematic. We reported a case of simultaneous squamous cell carcinomas in mouth floor and esophagus, that multidisciplinary team performed resection and reconstruction simultaneously.

식도 천공;6례 보고 (Esophageal Perforation; 6 cases report)

  • We have experienced 6 cases of esophageal perforation from September, 1988 to June, 1993, in the department of Thoracic and Cardiovascular Surgery, Chungang Univesity Hospital and obtained the following results. The number of male patients was 5, and female 1.The causes of esophageal perforations were spontaneous, post-emetic in 2 cases, spur of cervical spine in 1 case, foreign body in 1 case, surgical trauma in 1 case and blunt trauma in 1 case. Perforation developed in cervical esophagus in I case,and others in distal third of the esophagus. One case needed only conservative treatment, and others needed surgical intervention minor or major. There were 2 mortality cases, and 2 cases healed satisfactorily without complication, 2 cases had complications that needed reoperations.

생선뼈'에 의한 식도천공의 외과적 치료: 4례 보고 (Surgical Management of Esophageal Perforation due to Fish Bone: A Report of Four Cases)

  • This is a report on a total of four cases of esophageal perforation due to fish bone in the Department of Thoracic Surgery, Hanyang University Hospital. The perforated portions of esophagus were upper third of esophagus, that is, cervical esophalgus principally. The complications after esophageal perforation were acute mediastinitis with mediastinal emphysema in 2 cases, acute mediastinitis with both pyothorax in one case and cervical subcutaneous abscess alone in one case. Collar mediastinostomy was required to control disturbance of cardiopulmonary function as emergency procedure. Gastrostomy was of worthy for the various purposes, that` is, for feeding, absolute rest of the esophagus, and for prevention against continuous infection from esophageal leakage. After the gastrostomy. 3 cases were healed by spontaneous closure of esophageal perforation between one to four weeks. One case expired from severe septic shock due to acute diffuse mediastinitis and both pyothorax.

  • Postemetic rupture of the esophagus is an intrathoracic catastrophe which is universally fatal if untreated immediately. We have experienced one case of Boerhaave`s syndrome at the Thoracic & Cardiovascular Surgery Yonsei University College of Medicine. He was 56 years old male who complained severe chest-pain, dyspnea with sudden vomiting after alcohol. Esophagogram showed barium leakage through rupture site on lower esophagus into left thoracic cavity. He had taken repair of the rupture site of lower esophagus and wrapping it with the body of the stomach wall because of relatively clean rupture margin even though 48 hours following the rupture. He has been uneventful in post-operative courses & he discharged with good conditions.

식도암의 임상적 고찰 (A clinical Evaluation of Esophageal Cancer)

  • The records of 67 patients who had been operated as an esophageal cancer during the period from 1973 to 1989 at the Department of Thoracic and Cardiovascular Surgery, Hanyang University Hospital were reviewed retrospectively. The results were summarized as follows ; The age ranged from 28 years old to 80 years old. The highest incidence was 5th decades, then 6th decades, and the incidence of male was 10 times as the incidence of female[M:F= 10.16: 1]. The locations were lower esophagus 44%, middle esophagus 38.8%, upper esophagus 11.9% and cervical esophagus 4.4% The frequent symptoms were dysphagia [88%], epigastric or substernal pain and discomfort [29.8%], weight loss [20.8%], and laryngeal dryness [1.4%]. The most common interval between the onset of dysphagia and admission was 2-3 months; 82% of patients was within 6 months, The cancer consisted of stage I [3%], stage II [11.9%], stage III[47.6%], and stage IV [33.7%] The resectability of cancer was 67%. The organs of substitute were stomach in 21 cases, right colon 6 cases, and jejunum in 8 CRSCS. The relation between invasion of tumor and lymph node metastasis was analyzed: mucosal involvement: 1 case/2case, muscle invasion; 0/2 full thickness; 4/6, adjacent structure 7/12. Postoperative complications were pneumonia, pleural effusion, hoarseness, mediastinitis, anastomosis site leakage, reoperation due to stenosis, chylothorax, empyema, mechanical ileus, wound infection, meat impaction at anastomosis site, and repair of gastrostomy site leakage. Adjuvant therapies were irradiation [15cases], chemotherapy [14cases], and Bougie dilatation [4 cases],

  • Achalasia is a functional disorder of a short segment of the lower esophagus showing obstruction of the esophagogastric junction. Dysphagia. regurgitation and weight loss are outstanding features,however, complicated pulmonary troubles aspiration pneumonitis, bronchiectasis, lung abscess etc. --are sometimes more apparent than the disease entity in children though achalasia is rarely encountered in children. During the period of January, 1961, to May, 1968, the authors experienced 9 cases of achalasia of the esophagus in Chest Surgery Department,N.M.C. 1. Seven of nine were male and four were under 5 years of age. 2. So-called symptoms triad noted in almost every case, and 3 of 4 children showed recurrent attacks of pneumonitis. 3. Chest film showed widened superior mediastinum by dilated esophagus in 6 cases, and pulmonary infiltrations in 3 cases of children. 4. Preoperative diagnosis were achalasia,esophageal stricture by rodent-cidal ingestion and suggestive esophageal cancer in 7 cases,one case and remaining one case. respectively. 5. Modified Heller procedure was performed in all cases with definite diagnosis of achalasia at operation table in misdiagnosed 2 cases. 6. Immediate postoperative complications were 2 cases of wound infection and one case of atelectasis and no operative mortality encountered. 7. Seven of nine showed excellent result of operation, and good in one case. Remaining one case failed relieving obstruction and underwent interposition of ileocolonic segment with excellent result.

식도 해면상 혈관종 - 1례 보고 - (Cavernous Hemangioma of the Esophagus - One Case Report -)

  • 식도에서 발생한 혈관종은 전 세계적으로 30례 정도가 보고된 매우 드문 질환이다. 일반적으로 남자에 서 흔하고 증상이 없는 경우가 대부분이나, 연하 곤란 및 출혈 등이 있을 수 있다. 진단은 바륨 식도 조 영술과 내시경을 통해 이루어지고, 치료 방법은 내시경적 절제술과 개흉을 통한 절제술이 있다. 환자는 연하 곤란과 소화 불량을 주소로 내원한 46세 남자로 식도 조영술, 내시경 검사상 식도 중하부의 점막하 종양이 추정되어 수술을 시행하였다. 개흉수술을 통해 절제하였고 조직 검사상 해면상 혈관종으로 확진 되었다. 수술후 합병증이 없이 퇴원하였으며 수술후 관찰 추적중인데, 재발없이 양호한 상태를 보이고 있다.

식도의 기저세포양 펀평세포암 -1례 보고- (Basaloid Squamous Carcinoma of the Esophagus - Report of a Case -)

  • 기저세포양 편평세포암(basaloid squamous carcinoma)은 상부 기도소화기계에 발생하는 드문, 악성도가 높은 암종으로 침습성 편평세포암(invasive squamous carcinoma)이나 상피내암(carcinoma in situ) 형태의 편평상피암종성분과 밀접하게 혼재되어 있는 기저세포앞종성분(basaloid component)으로 특징 지워질 수 있다. 이런 조직학적 특성에도 불구하고 식도의 선양 낭성암(adenoid cystic carcinoma)이나 선양 낭성 분화를 보이는 암종(carcinoma with adenoid cystic differentiation)과 혼동되어 왔으나, 순수 선양 낭성암은 악성도가 기저세포양 편평세포암보다 낮아감별 진단이 중요하다. 기저세포양편평세포암의 임상적 경과는 식도의 편평세포암과 유사하다. 본 병원에서는 60세 남자의 식도 중간 113 부위에 발생한 기저세포양 편굉세포암을 수술치험하였다.

식도천공에 대한 외과적 치료 (11례) (Surgical Management for Esophageal Perforation: A Report of Eleven Cases)

  • This is a report on a total of 11 cases of esophageal perforation in the department of thoracic surgery, Chonnam University Hospital during the period of 8 years from 1962 to 1969. They occurred by the following agents, that is,lye solution[7 cases], fish bone[2 cases]. compress air [one case], strong acid [one case]. The perforated portions of esophagus were cervical esophagus in 2 cases, upper third of esophagus in 5 cases, middle third of esophagus in 3 cases and lower third of esophagus [abdominal esophagus] in one case. 4 cases out of cases of esophageal perforation after ingestion of Lye solution were due to Bougination to improve esophageal stenosis: 2 cases occurred 2 months after ingestion of Lye solution and the remaining 2 cases, 2 to 3 weeks after Lye solution ingestion. Therefore, It is realized that Bougination for esephageal stricture by Lye solution is particularly dangerous. The complication after esophageal perforation were mediastinitis,[10 cases], right pyothorax with mediastinitis [8 cases], peritonitis [4 cases], esophago-bronchial fistula[one case]. Owing to the various complications above mentioned, surgical approach to esophageal perforation is accordingly complicated and a combination of more than two of the following different procedures were properly used case by case, that is. gastrostomy or jejunostomy for feeding and esophageal rest,thoracotomy and chest drainage, lung decortication for pyothorax, primary closure of compress air perforation and esophago-bronchial fistula, mediastinostomy, retrosternal esophagoplasty using right colon to Lye stricture etc. 5 cases[45. 5%] of 11 cases were expired and the rest of 6 cases[54.5%]were survived with complete accomplishment of surgical procedures and satisfactory healing in 4 cases and interruption of follow up in 2 cases because of poor economical condition of the patients.
