Purpose: Supraventricular arrhythmia is a well-known complication of cardiothoracic surgery, and is common in patients wirth underlying cardiovascular disease. Also, it's treatment and prognosis are well known. However the incidence, the contributing factors, and the prognosis for supraventricular arrhythmias in noncardiothoracic surgical patients are less well known. This study was undertaken to investigate the incidence, the clinical presentation, the prognosis, and the factors comtributing to the prognosis for supraventricular arrhythmia in the surgical intensive care unit. Methods: We performed a retrospective study of 34 patients with newly developed or aggravated supraventricular arrhythmias in the surgical intensive care unit between March 2004 and February 2005. The incidence, the risk factors, and the prognosis of supraventricular arrhythmias were analyzed. Results: During a 12month period, the incidence of supraventricular arrhythmia was 1.79% (34/1896). Most patients had pre-existing cardiovascular disease and sepsis. The mortality rate was 29.4%, and the most common cause of death was multiple organ failure due to septic shock. The mean value of the APACHE II score was 20.9, and the surgical intensive care unit and the hospital lengths of stay were 9.9 days and 25.8 days, respectively. The APACHE II score measured when the arrhythmia developed was a significant factor in predicting mortality, Conclusion: Supraventricular arrhythmias result in increased mortality and increased length of stay in both the surgical intensive care unit and the hospital. The arrhythmia itself did not cause death, but a high APACHE II score incicated a poor prognosis. This may reflect the severity of the illness rather than an independent contributor to mortality.
Arrhythmias in the neonatal period are not uncommon, and may occur in neonates with a normal heart or in those with structural heart disease. Neonatal arrhythmias are classified as either benign or nonbenign. Benign arrhythmias include sinus arrhythmia, premature atrial contraction, premature ventricular contraction, and junctional rhythm; these arrhythmias have no clinical significance and do not need therapy. Supraventricular tachycardia, ventricular tachycardia, atrioventricular conduction abnormalities, and genetic arrhythmia such as congenital long-QT syndrome are classified as nonbenign arrhythmias. Although most neonatal arrhythmias are asymptomatic and rarely life-threatening, the prognosis depends on the early recognition and proper management of the condition in some serious cases. Precise diagnosis with risk stratification of patients with nonbenign neonatal arrhythmia is needed to reduce morbidity and mortality. In this article, I review the current understanding of the common clinical presentation, etiology, natural history, and management of neonatal arrhythmias in the absence of an underlying congenital heart disease.

Conversely, ventricular tachyarrhythmias are frequently difficult to characterize and localize electrophysiologically and their basic mechanisms are poorly understood. The role of the surgeon in the treatment of cardiac arrhythmia has changed dramatically during the past decade. This report is a case of 26 years old male with supraventricular tachyarrhythmia. The result of endocardial electrophysiologic study demonstrated accessory pathway connecting left atrium to left ventricle which located at left atrial free wall about 4 cm apart from the coronary sinus orifice. The accessory bundle interruption has been successfully accomplished utilizing the internal open heart technique. The operation consisted of dissection of the atrioventricular fat pad and division of all the superficial fibers going from the ventricle to the annulus. Following this, cryoablation made with cryoprobe at - 60$^\circ{C}$ for 90 seconds. The accessory pathway was successfully ablated without specific problems.

The atrioventricular node reentrant tachycardia[AVNRT] is a common type of supraventricular tachycardias. Recently we experienced two cases of AVNRT. One is AVNRT with severe aortic regurgitation[grade IV] and the other is AVNRT with patent ductus arteriosus. Dissection of perinodal tissue has been successfully carried out in the beating heart under the normothermic cardiopulmonary bypass, and aortic valve replacement and ligation of patent ductus arteriosus were also performed, respectively. Postoperatively, permanent atrioventricular block was not occured and AVNRT was not developed during the follow up. We propose that the surgical dissection of perinodal tissue is a simple and effective treatment for the patient with refractory AVNRT.

In addition, the ability to map and ablate the sometimes fleeting automatic atrial tachycardia is greatly enhanced by computerized mapping systems. In this study, we have developed 64 channel computerized data analysis system using microcomputer (Macintosh ${II}_{x}$) for basic research of electrophysiology and electrical propagation. The bipolar electrogram information is acquired from 64 cardiac sites simultaneously at a sampling rate of 1 ksampls/sec with continuous and total data storage of up to 30 seconds. When the reference electrogram is selected and reference point is picked up, delay time from the reference point is displayed on two dimensional diagram of the heart. System design permits easy expansion to almost 256 simultaneous sites. this system is expected to enable us to study pathophysiology of cardiac arrhythmia and to improve the result of diagnosis and surgical treatment for cardiac arrhythmia.

In this paper, we proposed an algorithm for arrhythmia classification, which is associated with the reduction of feature dimensions by linear discriminant analysis (LDA) and a support vector machine (SVM) based classifier. Seventeen original input features were extracted from preprocessed signals by wavelet transform, and attempts were then made to reduce these to 4 features, the linear combination of original features, by LDA. The performance of the SVM classifier with reduced features by LDA showed higher than with that by principal component analysis (PCA) and even with original features. For a cross-validation procedure, this SVM classifier was compared with Multilayer Perceptrons (MLP) and Fuzzy Inference System (FIS) classifiers. When all classifiers used the same reduced features, the overall performance of the SVM classifier was comprehensively superior to all others. Especially, the accuracy of discrimination of normal sinus rhythm (NSR), arterial premature contraction (APC), supraventricular tachycardia (SVT), premature ventricular contraction (PVC), ventricular tachycardia (VT) and ventricular fibrillation (VF) were $99.307\%,\;99.274\%,\;99.854\%,\;98.344\%,\;99.441\%\;and\;99.883\%$, respectively. And, even with smaller learning data, the SVM classifier offered better performance than the MLP classifier.

Purpose: To analyze electrocardiograms (ECGs) of patients with a salt-losing tubulopathy (SLT) and to determine the frequency and risk factors for long QT and arrhythmia. Methods: A total of 203 patients aged <19 years with SLT, specifically Bartter syndrome and Gitelman syndrome, who had a 12-lead ECG were included in this retrospective study. We analyzed the presence of an arrhythmia or prolonged corrected QT (QTc) on ECGs obtained for these patients. Demographic and laboratory data were compared between patients with abnormal and normal ECG findings. Results: Out of the 203 SLT patients, 38 (18.7%) underwent electrocardiography and 10 (40.0%) of 25 patients with inherited SLT had abnormal ECG findings, including prolonged QTc and arrhythmias. The abnormal ECG group had significantly lower serum potassium levels than the normal group (median [interquartile range]: 2.50 mmol/L [2.20-2.83] vs. 2.90 mmol/L [2.70-3.30], P=0.036), whereas other serum chemistry values did not show significant differences. The cutoff level for a significant difference in QTc interval was serum potassium level <2.50 mmol/L. One cardiac event occurred in a 13-year-old boy, who developed paroxysmal supraventricular tachycardia and underwent cardiac ablation. No sudden cardiac deaths occurred in this cohort. Conclusions: The incidence of ECG abnormalities in patients with inherited SLT was 40.0%, whereas the ECG screening rate was relatively low (18.7%). Therefore, we recommend ECG screening in patients with inherited SLT, especially in those with serum potassium level <2.50 mmol/L.
심장의 원발성 종양은 소아에서는 매우 드문 질환이다. 횡문근종은 태아, 영아 및 소아 연령에서 가장 흔한 원발성 심장 종양으로 결절성 경화증이 동반되는 것으로 보고되고 있다. 결절성 경화증은 뇌, 심장, 피부 및 기타 장기들을 침범하는 신경피부 증후군으로, 결절성 경화증을 가진 영아의 50-64%에서 동반된다. 심장 횡문근종은 종괴는 심방을 비롯하여 심실 중격과 심실벽에 다발성으로 존재하며 종양의 위치와 크기, 침범 정도에 따라 심장 유출로 협착 및 심부전과 같은 여러 가지 임상 양상을 보일 수 있다. 드물지만 부정맥을 주증상으로 발현하는 경우도 보고되고 있다. 하지만 대부분 심장의 횡문근종이 특별한 문제를 일으키지 않고 성장과 함께 쇠퇴되는 경향을 보이므로 극히 일부분에서만 장기적인 항부정맥제 사용이 요구된다. 환자의 최종적인 예후는 동반되는 뇌병변에 의해 결정되는 경향이 있다. 저자들은 상심실성 빈맥으로 입원치료 중 진단된 심장 횡문근종을 가진 결절성 경화증 1예를 경험하였기에 보고하는 바이다.
