Interrelationship research between difference of VAS and lesion area in Sono-Graphic after physical therapy of tendinitis. - Supraspinatus tendinitis, Lateral epicondylitis, Achilles tendinitis - (건염의 물리치료 후 VAS의 변화와 초음파 영상소견상의 면적 변화에 대한 상관성 연구 - 극상근 건염, 외측상과염, 아킬레스건염을 중심으로 -)

  • Purpose : The patients who have Supraspinatus tendinitis, Lateral epicondylitis, Achilles tendinitis are our targets, so we want to know their opinions in order to take Sono-graphic tests and to study utility value of Sono-graphic. Object and Methods : The case of 30 patients with Tendinitis results, we classified with three categories, Supraspinatus tendinitis, Lateral epicondylitis and Achilles tendinitis. Case by case, before medical treatment, we executed the inspections which are Sound side and Affected side. After 4 weeks of medical treatment, we executed the inspection which is Sono-graphic. Objectively comparing with Sono-graphic, we measured VAS(Visual Analog Scale) before and after physical therapy. Results : We could learn that VAS (Visual Analog Scale) and Sono-graphic wore decreased about changing the lesion area of each disease. For example, Supraspinatus tendinitis is from case 1 and case 2, Lateral epicondylitis is from case 8, case 9 and case 10, Achilles tendinitis is from case 16, case 17 and case 18. Conclusion : About organization of complicated anatomy, we could learn general ideas about Supraspinatus tendinitis, Lateral epicondylitis, Achilles tendinitis. And finally the results which appeared between the decrements of VAS and Sono-graphic are similar.

  • Calcific tendinitis of the shoulder joint is common disease causing acute pain, mainly involving the supraspinatus or infraspinatus muscle, and less frequently the teres minor or subscapularis muscle. This study reports on the satisfactory arthroscopic removal of calcium deposits as well as infraspinatus and supraspinatus muscle repair without relapse via minimal incision using suture anchors. This was a case of atypically extensive calcific tendinitis involving the infraspinatus muscle, with a bursal side partial rupture of the supraspinatus muscle in a 61-year-old female whose chief complaint was chronic pain of the right shoulder exacerbated by limited movement.

  • Objective : The purpose of this study is to evaluate the conservative treatment for supraspinatus tendinitis with kyphotic cervical curvature patient. Methods : We used chuna, Herbal medication, Acupuncture, Bee Venom Acupuncture, Physical Therapy for this patient. And we measured of VAS score, ROM and Extent of the subjective symptoms of shoulder pain to evaluate treatment effects. Result : Patient's shoulder pain and cervical curvature has improved. VAS score and Extent of the subjective symptoms of shoulder pain score were also decreased.

A Case Report of Patients with Calcific Tendinitis Located at Supraspinatus Treated by Oriental Therapy Including Sweet Bee Venom (봉약침을 포함한 한방치료로 호전된 극상근 석회화 건염 3례)

  • Objectives : The purpose of this report is to show that oriental medical treatments, chiefly using Sweet bee venom (SBV) are effective on symptom improvements and calcium removal in the patients diagnosed with calcific tendinitis in supraspinatus. Methods : We applied 0.3~0.6cc of SBV on $LI_{15}$ ($Gyeonu$) at a time and 0.1~0.2cc on each surrounding pressure point. In addition, we treated $Hwangryeonhaedok-tang$ pharmacopuncture on both acupoints of $GB_{21}$ ($Gyeonjeong$) with large intestine tonification of $Sa-Am$. Results : The symptoms improved within 3 weeks in case 1, 4 days in case 2, 5 weeks in case 3. The removal of calcium was detected by radiology at all cases. Conclusions : From the above results, it is considered that the oriental medical therapy including SBV is effective on the removal of calcium.

  • Calcific tendinitis usually occurs in the supraspinatus tendon of the shoulder. Calcific tendinitis of the peroneus longus tendon has not been reported on the Korean journals. Differential diagnosis includes bipartitis os peroneum, os peroneum fracture and peroneal tendinitis. We report a rare case of calcific tendinitis of the peroneus longus tendon which responded well with the conservative treatment.

  • Calcific tendinitis of rotator cuff is a common disease which could be the cause of shoulder pain, and frequently occurs in supraspinatus, infraspinatus and teres minor in descending order. Calcific tendinitis of subscapularis is rare and arthroscopic treatment of that has also been rarely reported. So, we report a case of arthroscopic treatment of calcific tendinitis of subscapularis with excellent result.

  • Objectives The objective of this study is to report the improved case of Korean medicine treatment with Shinbaro3 pharmacopuncture for supraspinatus tendinitis and subdeltoid bursitis. Methods We used Shinbaro3 pharmacopuncture with other Korean medicine treatment (acupuncture, herbal medicine, cupping and chuna) for this patient. This case is measured and assessed by Numerical Rating Scale (NRS), shoulder range of motion (ROM) and shoulder physical examination. Results The NRS decreased from 10 to 2 and the patient showed nearly full shoulder range of motion. Also physical examination of shoulder was improved. Conclusions Korean medicine treatment with Shinbaro3 pharmacopuncture is proved to be helpful to relieve pain and recover function on shoulder joint.

  • Objective : The purpose of this study is to report the patient diagnosed as calcific tendinitis located at supraspinatus, who improved by Oriental medical treatment, through analysis of radiograph. Methods : The patient was managed by acupuncture, moxibustion and cupping without medication and physical therapy. We measured the shoulder state by radiograph and evaluated with UCLA shoulder rating scale. Results : After 5 week treatment, the pain of patient and The UCLA functional average score was improved. Also we found that the deposits of calcific tendinitis was almost disappeared in radiograph. Conclusions : The results suggest that acupuncture, moxibustion and cupping can be effective in reducing the pain and the deposits of calcific tendinitis. We think that it need the further study and clinical trial for calcific tendinitis.

Arthroscopic Treatment of Calcific Tendinitis of the Rotator Cuff (회전근 개 석회화 건염의 관절경적 치료)

  • Purpose : The purpose of this study was to evaluate results of the arthroscopic treatment of calcific tendinitis of the shoulder resistant to conservative treatment. Materials and Methods : From March, 1996 to June, 1998, fourteen patients underwent shoulder arthroscopy to treat resistant calcific tendinitis of the rotator cuff despite conservative treatment for more than 6 months. Calcium deposits were localized to the supraspinatus tendon only in eleven patients, the supraspinatus and infraspinatus tendon in two patients, and to the supraspinatus and subscapularis tendon in one patient. Each shoulder was evaluated with UCLA shoulder rating scale and Constant-Murley score Results : The Constant-Murley pain score improved from average score 3.2 before surgery to average score 8.3 after surgery, and the UCLA functional average score improved from 4.5 preoperatively to 8.3 postoperatively. Preoperative ROM averaged $110^{\circ}$ of flexion, $45^{\circ}$ of external rotation, $90^{\circ}$ of abduction, and internal rotation with the thumb reaching to the spinous process of the third lumbar vertebra, but postoperative range of motion averages improved as follows: $170^{\circ}$ or flexion, $50^{\circ}$ of external rotation, $140^{\circ}$ abduction, and internal rotation with the spinous process of the twelveth thoracic vertebra. Overall 3 patients were rated excellentm 9 were good, 2 were fair. Conclusion : Shoulder arthroscopy is an effective treatment in patients with refractory calcific tendinitis.

  • Acute calcific tendinitis (ACT) is a benign painful inflammatory disorder characterized by resorptive process of calcific deposits following the formation of calcium hydroxyapatite crystals in the tendons. It can occur at various sites, especially in the shoulder or hip joint. ACT involving the lateral epicondyle of the humerus and the cervical spine is very rare. Few reports have demonstrated successive ACT at different sites. We report three cases of successive ACT in women, occurring at the subscapularis followed by the lateral epicondyle, flexor carpi ulnaris followed by the supraspinatus, and longus colli followed by the iliopsoas, respectively.
