PTSD Symptoms in Elementary School Children After Typhoon Rusa

  • Purpose. A natural disaster negatively affects children's emotional and behavioral adjustment. The purpose of this paper was to examine the prevalence, symptoms, and correlates of PTSD after the occurrence of Typhoon Rusa. Method. 261 elementary school children living in Kimcheon, which was a devastated rural area in South Korea by Typhoon Rusa, were selected. Data were collected 4 months after the disaster using the PTSD Reaction Index categories recommended by Frederick, severity of PTSD. Results. 12.3% of the children had either moderate or severe PTSD symptoms; 22.7% reported mild symptoms; and the remaining 65% had sub-clinical symptoms of PTSD. The most frequent symptom was recurrenct fear(67.0%). 13% to 17.2% of children exhibited difficulty in concentration, sleep disturbance, and guilt feeling. The regression model of severity of PTSD was composed of the level of exposure to traumatic experiences, grade in school, gender, negative coping style, and social support, and explained 34.3% for PTSD symptoms. Exposure to traumatic experiences was the strongest factor of all predictors. Conclusion. Emotional support from friends and coping style were correlated with PTSD severity. School-based interventions that emphasizes coping with disaster related problems and problem-solving may prove to be useful, and may aid in building close and supportive ties with teachers, classmates, and friends.

The Pathways of Nurse Turnover in Long-term Care Hospitals

  • Background: This study explores the pathways of nurse turnover in long-term care hospitals (LTCHs) and its underlying conditions in Korea. Although the factors of staying or leaving of nurses in LTCHs have been reported, few studies have examined the trajectory and conditions of nurses staying in and leaving LTCHs. Methods: A qualitative study design with a grounded theory approach was conducted. Data were collected in one-to-one interviews. Purposive and theoretical sampling led to the inclusion of 20 registered nurses from 15 LTCHs in South Korea. Results: Seeking work-life balance was the core category of the nurses' turnover pathway. The consequences of the nurses' turnover pathway were categorized into three groups: thriving, surviving, and leaving. Thriving nurses found meaning in their work, fostered good relationships, and saw opportunities for growth. Surviving nurses were enduring their jobs in LTCHs, having a work-life balance, and supportive nursing leaders. Leaving group nurses wished to leave LTCHs due to a lack of professional growth, unappealing work, continued conflict, and social stigma. Conclusion: This study provided the trajectory and conditions for nurses to enter, stay, move, or leave. Understanding the pathways for staying or leaving can be used as a strategy for successful retention of registered nurses in LTCHs.

한국의 간호교육과정 경향과 전망 (The trend of Current Nursing Curriculum in Korea and it's perspectives for the future)

  • This review article provides the trend of current nursing curriculum of todays in Korea and its perspectives. This article reviewed 5 aspects of the current nursing curriculum for the undergraduate program, those are the (1)educational philosophy and believes (2)the nursing concepts revealed in the current nursing curriculum (3)the educational goals and objectives (4)the framework of nursing curriculum (5)the syllabus and credit hours. The common nursing concepts in the current nursing curriculum are Nursing, Human being, Environment/society, Health, Nursing science. The examplary vertical concepts composing of the current nursing curriculum are nursing process, life styles and the horizontal concepts are client, health promotion /recovery /maintenance, leadership, and research. The common subjects composing of the nursing syllabus are the introductory subjects(nursing history, nursing professionalism, foundamental nursing), and the supportive subjects (communication, human relationship, human growth and development, health education, etc) and nursing research, the intrductory basic sciences (anatomy, physiology etc.), and the major nursing subjects (adult nursing, child nursing, maternity nursing, psychiatric nursng, community nursing, nursing administration.) In order to have more advanced nursing education, the suggestions were provided as follows : (1) It is necessary to have revision and evaluation of the process and the structure of the current nursing curriculum periodically. (2) The concept of health promotion for all human being should be integrated into the nursing curriculum. (3) The nursing education program should be unified into baccalauliate level to get one type of nurse-registration system. (4) The nursing courses will need to provide the necessary contemporary Informations to allow the nurses to fuction efficiently in this rapidly changing era. (5) The use of new technology in nursing education is necessary to expand nursing education more.

신규 간호사의 임상 경험 (The Clinical Experiences of Newly-qualified nurses)

  • Purpose: This phenomenological study is aimed at illuminating the lived experiences of newly-qualified nurses who working in general hospitals in C province. Method: The data was collected through unstructured, in-depth interviews with 10 new nurses from April to June 2001, and analysed by Colazzi' methodology. Results: Thematic content analysis identified 6 central themes in this experience-difficulties m making interpersonal relationship, negative feelings, fatigue and unskilled work performance, self-reflection, a sense of accomplishment, identity formation as a nurse. Also, this themes were constructed into 6 constructs explaining new nurses' experience. The results may contribute to a better understanding of what new nurses experienced and how their negative conflicts could be managed. Conclusion: New nurses need to get more education and supportive empowerment regarding diverse conflicts during training period. Educational programs like a preceptorship or individual counselling should be developed and adopted to all clinical settings.

임상간호사의 리더십 경험: 포커스 그룹 인터뷰 적용 (Leadership Experience of Clinical Nurses: Applying Focus Group Interviews)

  • Purpose: The purpose of this study was to understand and describe the leadership experience of clinical nurses. Methods: During 2014, data were collected using focus group interviews. Three focus group interviews were held with a total of 20 clinical nurses participating. All interviews were recorded as they were spoken and transcribed and data were analyzed using qualitative content analysis. Results: Fifteen categories emerged from the five main themes. 1) Thoughts on the leadership category: to lead others, to cope with problem situations adequately and to serve as a shield against difficulties. 2) Situations requiring leadership: situation that requires correct judgement, coping and situations that need coordination and cooperation. 3-1) Leadership behaviors: other-oriented approach and self-oriented approach. 3-2) Leadership behavior consequences: relevant compensation and unfair termination. 4-1) Facilitators of leadership: confidence and passion for nursing and external support and resources. 4-2) Barriers to leadership: non-supportive organization culture and deficiency in own leadership competencies. 5) Strategies of leadership development: strengthen leadership through self-development and organizational leadership development. Conclusion: In conclusion, the results indicate that it is necessary to enhance clinical nurses' leadership role in healthcare. Enhancement can be achieved through leadership programs focused on enlarging leadership experience, constant self-development, leadership training, and development of leadership competencies suited to the nursing environment.

웹 기반 간호 교육을 위한 튜터의 운영 전략 개발 및 효과 검증 연구 (A Study of Development and Evaluation of Tutorial Management Strategy for Web-based Nursing Education)

  • Purpose: This study was attempted and completed in order to settle down and qualitatively improve web-based nursing education by evaluating effect and managing strategy of tutor. Method: The development of tutor's managing strategy was based on "The Self-regulated Learning" and "The supportive Learning", then it was applied to 79 learners in one of the cyber-learning centers. After applying the tutor's managing strategy, self-regulated learning scale, attitude for school, preference for computer and academic achievement were evaluated. The development of tutor's managing strategy for web-based nursing education are consisted of participation promotion, psychological support and motivation, recognition and promotion strategy of learning activity, management strategy of evaluating stage. Result: The levels of learner's self-regulated learning, recognition, behavior, attitude on the school and learning achievement were meaningfully increased in statistics after applying for the managing strategy of tutor. The motivation level and learning participation kept high scores from the beginning with no significant statistical changes. Conclusion: It is required to develop an educational program for cultivating well-educated tutors in order to help the effective learning process of nurses based on understanding characteristics of learners.

간질아동 부모를 위한 교육 프로그램 개발 (Development of an Educational Program for Parents of Children with Epilepsy)

  • Purpose: The purpose of this study was to develop an educational program for parents of children with epilepsy. Method: The first step was to survey the need for information and social support for these parents. The second step was to design the educational program based on the survey results, literature review, and the theory of chronic sorrow that provided a framework for understanding and working with people suffering a loss of health. Result: The information which these parents were most anxiously seeking was information on the causes of seizures, appropriate steps deal with seizures, and appropriate and supportive parental re]e for their children. The educational program for parents of children with epilepsy included understanding the responses to loss, increasing the parents' knowledge about how to care for their children, managing their stress, enhancing the psychosocial adaptation of children, and encouraging the participation of the family. Conclusion: The educational program developed by the researcher can used with parents of children with epilepsy, and is expected to enhance the psychosocial adaptation of these children.

미숙아의 생물학적 리듬에 관한 주기적 빛 조절의 효과 (Effects of Cycled Lighting on Circadian Rhythms of Premature Infants)

  • Purpose: Instead of a chaotic non circadian environmental approach, continuing regular day-night rhythm in neonatal nurseries may benefit the development of preterm infants. The purpose of this study was to define the effects of cycled lighting on circadian rhythms of premature infants. Methods: The experimental group included 15 preterm infants, and the control group, 15 premature infants in the NICU of a general hospital in Seoul. All infants were between 32 and 37 weeks' postconceptional age. The experimental group infants were provided with cycled lighting before discharge. The incubator or bassinet cover of the experimental group was off between 7 AM and 7 PM, and was covered between 7 PM and 7 AM. Results: There were significant differences in the NNNS score ($p$=.039), and some significant differences in the sleep-activity pattern between the experimental group and the control group, but distinct differences in sleep-activity patterns between the two groups could not be defined. Conclusion: The results of this study suggest that cycled lighting can be helpful in the neurobehavioral development of preterm infants. By modifying the NICU environment to provide a more developmentally supportive milieu, nursing professionals can better meet the infants' physiologic and neurobehavioral needs.

간질환아 부모들의 정보와 지지에 대한 요구조사 (Needs Assessment for Information and Support of Parents of Children with Epilepsy)

  • Purpose: This study investigated the need for information and social support in parents of children with epilepsy. Method: A total of 119 parents of children with epilepsy were recruited and asked to fill out questionnaires. Result: Of 119 parents, two-third reported that they received full and sufficient information about their child's disease and its management but one-third felt the information was insufficient and incomplete. Most parents (62.2%) felt at loss when their child had a seizure, either at home or at school. They wanted information on the causes of seizures, adequate steps deal with seizure and steps they should take to become adequate and supportive parents for their children. However, most parents were reluctant to disclose the disease or to receive support from outsiders. Conclusion: Regardless of the fact that most parents received sufficient information about the management of epilepsy, they felt at a loss when their child had a seizure attack. Therefore nurses should give specific instruction on seizure management and assess the needs of parents on a regular basis.

인지적 간호중재 (Cognitive Nursing Intervention)

  • Nursing as it is practiced and taught, historically has been viewed as natural science. There are new movements to create a paradigm of nursing in the human sciences. A natural science methodologies elicit quantitative data from observable phenomena and reveal causal relationships. Human science methodology is the study of Unitary Man's participative experience in a situation, the simultaneity paradigm. In a theory of nursing rooted in human science assumptions about man and health are synthesized, and the practice of nursing is continuously expanded through research. To find independent nursing interventions especially cognitive nursing intervention models it has been necessary to consult a multitude of journals and text sources for references, and no one nursing textbook can be used in help patients achieve nursing goals. The goals of nursing in the simultaneity paradigm focus on the quality of life from the person's perspective. Cognitive interventions based on the person's perspective were selected from those that colleagues deemed appropriate to nursing and those that were identified in the nursing literature. They were supportive nursing care, reminiscence, meditation, relaxation and imagery. Nurses have been reluctant to make diagnoses, implement actions, and assume responsibility for this actions and this is of concern but can be understood because nurses have had little exposure to action and lack an intervention armamentarium from with which to choose actions to achieve nursing goals. Efforts in nursing education and nursing service are required to remedy this problem. Nurses must be convinced of the challenge and excitement associated with autonomous functioning. It is a characteristic of the true professional. Traditionally, nursing has prescribed one method for handling a situation. Fundamental nursing texts usually only present one way to handle a situation, because alternative interventions to achieve a client goal may be available. Considerably more research is necessary before these can be prescribed. However, unless a first step is taken, progress will not be made. The quality of health care or nursing care is enhanced when nurses transform dilemas into commited action. This is apparent from widespread experiences of nurses.
