The relationship between intrinsic motivation and learning outcomes in problem-based learning (문제중심학습에서 내재적 동기와 학습 성과의 관계)

  • Purpose: The purpose of this study was to identify the relationship between intrinsic motivation and learning outcomes of nursing college students who took the Fundamentals of nursing as a problem-based learning method. Methods: In this cross-sectional study, we identified the intrinsic motivations of 114 nursing students who completed problem-based learning using the Intrinsic Motivation Inventory. The t-test was conducted to identify differences according to intrinsic motivation, and correlation analysis was performed to confirm the relationship between intrinsic motivation and learning outcomes. Results: The group with higher intrinsic motivation showed higher scores in all domains of self-assessed learning outcomes than the lower group. It was the 'Relatedness with an instructor' that showed the highest correlation with the learning outcomes in the domains of intrinsic motivation. Conclusion: Problem-based learning is an effective learning method for cultivating the competencies needed for nurses. The intrinsic motivation of students is an important factor in the performance of problem-based learning. For the efficiency of problem-based learning, efforts should be made to develop and apply autonomy-supportive interventions that can enhance intrinsic motivation.

Hospital Nurses' Experience of Bullying in the Workplace and Burnout, Organizational Commitment, Turnover Intention and Nursing Productivity (간호사의 근무지 약자 괴롭힘 경험과 소진, 조직몰입, 이직의도 및 간호생산성)

  • Purpose: This study was performed to investigate nurses' experience of bullying in clinical fields. Methods: Participants were 309 nurses working in general hospitals. Data were collected using self- report questionnaires which were evaluated in terms of workplace bullying, burnout, organizational commitment, turnover intention and nursing productivity. Results: Of the participants 17.2% reported being victims of workplace bullying of nurses in general hospital, based on the operational definition of bullying. Coping response to workplace bullying was mainly passive such as enduring, ignoring, and evading. Experience of workplace bullying was positively correlated with burnout and turnover intention and negatively correlated with organizational commitment and productivity. Conclusion: The findings suggest that there is a need for assessment of the reality of workplace bullying of nurses in clinical areas as well carefully reviewing the problem and impact. A supportive system, education programs and improved workplace climate are recommended to reduce workplace bullying in clinical fields.

A Survey on the Characteristics of Vulnerable Families in a City (일개 도시 취약 가족의 특성에 관한 조사연구)

  • Purpose: This study aimed at describing the characteristics and nursing needs of vulnerable families in a City. Methods: A total of 427 vulnerable families enrolled in the Visiting Health Care Center in K-gu of S city were investigated in this study. Data were collected using questionnaires for one year from Jan to Dec, 2003, and analysed using mean, standard deviation and $x^2$. Results: Most of the vulnerable families investigated here showed many deficits, especially incomplete family structure (62.8%), financial problem (84.0%), lack of support (55.8%) were prevalent. The score of level of economic status in Family Capability for Self management ($1.95{\pm}0.65$) was lowest, and the score of perception of family problem and health of family members ($2.62{\pm}0.78$) was highest. The four family groups divided according to the total score of family management capability showed significant differences in family interaction, support, and coping domain. The total score of family management capability increased as family functioning-related problems decreased in the vulnerable families. Conclusion: In order to find vulnerable families, and improve their family function, it is required to develop the systematic assesment tools, community supportive systems and nursing interventions for family strength.

A Study on Self Management, Hemoglobin A1c (HbA1c), and Perceived Health Status for the Type II Diabetes Patients (제2형 당뇨병 환자의 자가관리, 당화혈색소 및 주관적 건강상태)

  • Purpose: This study is conducted to examine the relationship among self management, HbA1c, and perceived health status of type II diabetes patients in community health department. Methods: Data were collected from 93 type II diabetes patients who are registered at five community health center using comprehensive survey, modified self care behavior, blood glucose and HbA1c were assessed by nurses. The data was analyzed by the SPSS (14.0) computer program, and it included descriptive statistics, t-test, ANOVA, and pearson's correlation coefficient. Results: There was significant relationship between perceived health status and HbA1c (r=-.252, p=.015). There was significant relationship between self management and HbA1c (r=-.279, p=.007). Conclusion: In order to control blood glucose in the type II diabetes patients, it is necessary to develop supportive self management programs by considering perceived health status.

Recent Advances in Cancer Cachexia (암성 악액질의 최신 지견)

  • Purpose: The study was aimed to review and understand the meaning of cancer cachexia. Methods: Using the keywords "cachexia" and "cancer cachexia" 30 oncology research published from 1974 to 2009 were selected for the review. Results: The mechanism of cancer cachexia has not been fully understood, but various pathogenesis appears to be involved in the development cachexia including altered metabolism of carbohydrate, lipid, and protein associated with cytokines and hormone. As a result, muscle strength, food intake and resting energy expenditure (REE) are reduced. Most medications for the treatment of cachexia show debating results except some drugs such as megace. Supportive care including nutritional education, nursing care, and social support are found another effective treatment options. Conclusion: The results of this study would help oncology nurses to understand the mechanism of cancer cachexia and its management.

  • 목적: 종양환자 및 가족을 위한 환자교육의 필요성은 건강 돌봄의 중심이 완치의 개념에서 삶의 질 개념으로 바뀌어 감에 따라 더욱 대두되고 있다. 이들을 위한 환자교육이 성공적으로 이루어지기 위해서는 환자와 가족들의 교육선호 양상과 일치하는 교육적 전략이 필요하다. 본 연구는 종양환자와 가족들의 교육선호와 일치하는 효과적인 교육적 전략의 기초를 제공하기 위해 시도되었다. 방법: 본 문헌고찰은 컴퓨터 데이터베이스 에 수록된 1990년에서 2002년에 출간된 선행연구 결과를 토대로 분석하였다. 결과: 종양환자와 그 가족은 의료전문인과의 대화, 책자, 시청각 및 인터넷 매체 등의 다양한 방법을 통해 정보를 구하였으며, 이러한 교육 요구 및 선호 양상은 다양한 요인에 따라 매우 개별적이었다. 또한 이들의 교육 준비상태는 질병 경험의 정신 사회적 수용 상태와 밀접하게 연관되어 있었다. 결론: 다양한 교육 방법을 활용한 개별적 접근이 종양 환자와 가족을 위한 환자교육에 적절할 것이며, 이들의 교육 준비상태와 일치하는 환자교육을 제공하기 위해 질병에 대한 정신적 적응과 수용상태를 지속적으로 사정하는 것이 필요하다.

A Phenomenologic Study on the Children's Living under the Institutional Care (시설아동의 삶(현상학적 접근))

  • The purpose of this study was to discover knowledge about the sources and meanings of the children's living who is under the care of welfare institution. Participants were high school girls in Kwangju who offered unstructured description of their experience through interviews. The research was performed from March 2001 to September 2001. The results, analyzed and interpreted according to Gorgi's method of phenomenology, describe the structure of the phenomenon "living experience of the children under the institution care" with a relational perspective. The analysis revealed seven core themes : (1) anguish of heart against custom of institution, (2) resistance against their livelihood, (3) mortification on the distorted prejudice, (4) desire to escape, (5) fearfulness for the unreliable future, (6) self-pity, (7) challenge to the future. The foregoing argument suggest that children under the institutional care be supported by more mental health intervention and nurses be disciplined by supportive conversation technique.

The Process of Overcoming the Professional Burnout of Clinical Nurses (임상간호사의 직무소진 극복과정)

  • Purpose: The purpose of this study was to identify the process of overcoming professional burnout among clinical nurses. Methods: Data were collected from 12 clinical nurses through in-depth interviews about their actual experiences. The main question was "How do you describe your experience of the process of overcoming professional burnout?" Qualitative data from the field notes and transcribed notes were analyzed using the grounded theory methodology developed by Strauss & Corbin. Results: The core category of experience of the process of professional burnout among clinical nurses was identified as "Keeping a balance by awakening-relaxing-coexisting". The participants used six interactional strategies such as 'Confronting problems', 'Exposing problems', 'Laying the mind's burdens down', 'Taking a breather', 'Seeing from a different perspective', 'Accepting realities'. The results of overcoming process of professional burnout were 'Coexisting' and 'Revitalizing'. Conclusion: The results of this study provide useful information for designing supportive programs and policies to solve professional burnout in clinical nurses.

Effects of LGBT Nursing Education Using Simulation (시뮬레이션을 활용한 LGBT 간호 교육의 효과)

  • Purpose: As health care needs for Lesbian, Gay, Bisexual and Transgender (LGBT) are becoming increasingly important, it has become imperative for the nurses to be attentive towards their health problems and provide nursing care with an open-minded attitude. Due to limited opportunity to provide direct nursing care to LGBT patients, it is hypothesized that simulation would provide good opportunity for students to experience LGBT nursing care in a safe environment. This study was conducted to develop and apply simulation of LGBT nursing care to ultimately provide unbiased nursing care for LGBT population and prepare basic data for LGBT nursing education. Methods: This study was a single-group pre-post experimental design study for 57 senior nursing students based on the comparison of existing LGBT knowledge, general attitude towards LGBT, and nursing attitude towards LGBT before and after simulation. The scenario content included discussion of coming out issue, providing sexual health information, and supportive nursing care for LGBT population. Data were analyzed with descriptive statistics and Wilcoxon signed rank. Results: Simulation education-led to a significant increase in LGBT knowledge and nursing attitude. However, there was no change in the general attitude towards LGBT. Conclusion: The results of this study suggest that LGBT education using simulation may be effective for nursing students and nurses from the point of concern for LGBT population. It is hypothesized that future LGBT educational programs might need more detailed information from both care recipients and nurses. Finally, LGBT education needs to be included in the nursing education curriculum.

A Comparative Study of Eastern and Western Nursing for Pyrexia Patient (발열환자의 동.서 간호 비교고찰)

  • This study was done for understanding the desirable direction of eastern and western nursing by comparing the nursing practice for the Pyrexia patient occuring most commonly. Body temperature usually maintains around $37^{\circ}C$ owing to the thermoregulatory center but pyrexia is caused by exogenous pyrogen like infection, cancer or disturbances in body's homeostatic heat balance. Pyrexia is defined that body temperature rise above $37.2^{\circ}C$. It has chill phase, course of the fever, termination accompanied various symptoms. Oriental medicine explains that pyrexia comes from Yang's(陽) abundance and Yin's(陰) lack. Pyrexia mainly happens when body constitution is in bad condition by Six Dirty's(六陰) affection to Wi Area (偉分). It also occurs because of unbalance between Qi (氣) and Yin(陰) caused by the lack in Seven Emotions, labor, food. The Sanghanron(傷寒論), explains that pyrexia is categorized exogenous fever like Poong Han(風寒), Poong Yul(風熱), Sup Yul(濕熱) and endogenous fever due to the Qi and blood deficiency. Explained above, even though pyrexia has different meaning in oriental medicine and western medicine, but this study have compared the oriental and western medicine assuming that pyrexia is rise of body temperature. From this point of view, oriental and western nursing shows the similarity in the field of 1) use of antipyretic to control the body temperature 2) rest and comfort 3) watering 4) nutrition and case study was executed in nursing practice. From the above study, western nursing has superiority in decreasing the patient's risen body temperature using antipyretic and ice bag. But in case of empty heat, oriental nursing which recommends the patient's body keep warm and prevent the use of ice bag as the first step and helps patient's sweating by drinking of hot water was comparatively effective. In conclusion, it is desirable that oriental nursing emphasizing the supportive nursing and western nursing should be harmonized according to the status of pyrexia patient and it is needed to study the nursing method appropriate in our culture.
