Anomalous Systemic Arterial Supply to Normal Basal Segments of Left Lower Lobe : A Report of Two Cases (폐격리 없이 좌하엽이 체순환 동맥으로부터 공급되는 기형적인 혈관 2예)

  • Two cases of an anomalous systemic arterial supply to the basal segments of the left lower lobe without pulmonary sequestration are presented. In the first case, a preoperative diagnosis was made by chest CT, and confirmed by angiography, in a 22-year old man who had a recurrent hemoptysis. There was systemic arterial supply that originated from the thoracic descending aorta and no pulmonary arterial supply to the basilar segments of the left lower lobe. However, the pulmonary parenchyma was normal without sequestration. Ligation of the abnormal artery and a left lower lobectomy were performed without complications. In the second case, there were characteristic features of this anomaly on chest CT and the angiogram in a 31-year-old man with symptoms of hemoptysis. The patient refused surgery.

A Study on the Development and Improvement of Simple Piped Water Supply System in Rural Area of Korea (농촌지역 간이상수도시설 개발 및 개선에 관한 연구)

  • It is very important to supply safe drinking water for rural area not only a prevention of entric diseases but also a promotion of health life. It is estimated that 6,981,000 rural inhabitants were covered by the simple piped water supply system at the end of 1987 in Korea. The programme for improvement of water supply system in rural villages was initiated by the government since 1967. But most of these systems have been operated carelessly by the hands of villagers who have no proper knowledge and experience. Since most of water sources were located nearby farmland, there might be a possibility that the sources could be contaminated by pesticides and fertilizers. For this reason, it is recommended to take underground water as a water source rather than surface water such as a pond or streamwater in rural areas. However, the system is supplied from the surface water, its water quality can be improved by using of simple sand filter and simple chlorinator inexpensively. On the basis of an on-site study, conducted during 1986-87, in San-Buk Village, Keum-Sa-Myon, Yeju-Gun, Kyong-Gi-Do, the new simple piped water supply system was designed by the Institute for Environmental Research, Yonsei University, and constructed by the villagers themselves in September 1987. This simple system which is protected by metal fences consists of three main parts, pump house, vertical sand filter and water tank. The pumped water from underground flows into the upper part of the sand filter, through the sand, and out the water tank which is connected to the bottom of vertical filter. And the simple plastic-bottle chlorinator was installed in the water tank for chlorination. The water quality was remarkably improved after completion of construction. The total bacterial count was not detected from the tap water in households distributed by this simple piped water supply system. The construction cost of this system which was connected 34 households in San-Buk Village, was 4,851,000 won (approximately 6,020 U.S. dollars : 1$=805.8 won) in 1987,77% of expenses was supported by the Community Development Foundation in Korea. This case study for simple piped water supply projects will be applicable to other programme for improvement of water supply system in rural areas of Korea, and other developing countries.

  • Jinyangho is a natural water supply source of tap water in west Gyeongnam area, but mass mortality of crusian carp occurred during the time of temperature rise in spring. Examinations on diseased fishes were able to isolate four bacteria isolates and then identified the bacteria as a member of Aeromonas sp. Challenge experiment with mirror carp (Gyrinus carpiospecilaris) was proved the virulence, the isolates were in turn believed as the causative agent of mass mortality in Jinyangho.

Determinant Factors for Expenditure of the Medical Insurance Program for Self-Employeds (지역의료보험(地域醫療保險) 재정지출(財政支出)의 결정요인(決定要因))

  • This study was conducted to examine the determinant factors for expenditure of the medical insurance program for self-employeds based on the analysis of 1991 'The Medical Insurance Program for Self-Employeds Statistical Yearbook', and also similar yearbooks in the metropolitan and other provinces. The major findings are as follows : We have divided benefits into these four components such as the utilization rate for out-patients, expenses per claim for out-patients as paid by the insurer, utilization rate for in-patients, and the expenses per claim for in-patients as paid by the insurer, in order to examine the determinant factors for it. The results of the study revealed the following findings, in urban areas, the supply of medical care had more influence on the benefits than other demographic and economic variables, while, in county areas, both the supply of medical care and the rate of those aged over 65 affected the provision of benefits. The determinant factors for financial balance of the medical insurance program for self-employeds are, first, the determinant factor for administrative expenses was the number of households. The more the number of households, the less the administrative expenses per the insured. This shows that the economy of scale is being. And so, the administrative district must be taken into consideration in the incorporation of small regional medical societies and should be re-organized for more efficient management. Second, in urban areas, the supply of medical care had more influence on utilization rate and expenses per claim as paid by insurer, and therefore it is necessary to control it. In county areas, the supply of medical care and the rate of those aged over 65 raised the utilization rate and expenses per claim as paid by insurer. For the financial stability of county areas, a common fund for medical care for the aged and expansion of finance stabilization fund would be necessary. But, in county areas, it would be unnecessary to control the supply of medical care because it was much more insufficient than in urban areas. The vitalization of public health facilities must be carried out in county areas, for they reduced benefits. Sice the more insured in a single household, the less the utilization of the medical insurance program, benefits for habilitation at home should be given consideration. The law of majority and the economy of scale were applied here, and therefore the incorporation of regional medical societies must be taken into consideration. In integrating regional medical societies, it would be absolutely necessary to review the structural differences among all regional medical societies, the medical demand of each region, and also the local characteristics of each region.

Epidemiologic Investigation to Identify the Cause of an Infant Methemoglobinemia (서울 S지역에서 발생한 영아 메트헤모글로빈혈증 1례의 원인구명을 위한 역학조사)

  • Epidemiologic investigation was conducted on January, 1993 in Seoul to identify the cause of an infant methemoglobinemia. Field investigation of the area of outbreak, survey of household and family members, analysis of ground water, and blood tests of involved family members were performed. Following results were obtained On analyzing the quality of the ground water on patient's household high levels of nitrate was found indicating contamination of water as the cause of a methemoglobinemia outbreak. On analysing the quality of the ground waters on seven other places within the neighborhood five were contaminated by nitrate in concentration that exceeded the permissible limit implying presence nearby source of contamination. Sources of contamination were thought to be originating from human waste in conventional bathroom facilities, chicken manure used in nearby orchards and plant fields or fertilizers. But the results of water analysis with presence of bacteria or E.coli, concentration of potassium, phosphate and the past history of diarrhea among family members, chicken manure suggested the most possible source of contamination. To evaluate the health status of members in the neighborhood past history was reviewed revealing no prior existence of patient with cyanosis and 65 people in the neighborhood had normal levels of methemoglobin concentration in their blood. Conclusively, the ground water on patient's household was contaminated with nitrate and despite provision of adequate water supply, family members of the patient along with their distrust in the water supply system had used ground water as their source of drinking water resulting of methemoglobinemia. Many suburban area of Seoul and country side thought to be having similar problems concerning contaminated ground water supply and dormant outbreak of patients as a result of the drinking of the contaminated water. Epidemiologic investigation and water analysis of ground waters are advised.

Case Report on the Activities for Cost Reduction and Performance Improvement in the Central Supply Room in A University Hospital (종합병원의 비용절감과 업무개선을 위한 중앙공급실의 활동)

  • Background : Because hospitals had to faced with financial hardship, we have to have more effective hospital management. In this study, we tried to improve job performance and to reduce cost maintaining safety in fields of patients care. Methods : Central Supply Room(CSR) staffs taught strategy of material cost reduction to middle level managers and staffs 14 times. All staffs and voluntary service workers, working in 33 nursing units, participated in this activity. We developed questionnaire to check the degree of participate in cost reduction activity and job performance in CSR, and quality improvement in the wards. Two hundred questionnaires were distributed and 197 returned. Results : Because CSR staffs developed new items and every item was managed at each department, stocks of each department were reduced. Overall, by comparing before and after cost reduction activity, almost 1.2 billion won reduced for one year. Staffs' loyalty and spirits of cost reduction were improved by changing clothes and duty environment. Furthermore, these activity improved patients' satisfaction. Conclusion : We suggest that CSR activity in a university hospital contributes to improve job performance and to reduce cost without any problem in patient care.

  • Systemic arterial supply from the descending thoracic aorta to the basal segment of the left lower lobe without a pulmonary arterial supply is a rare congenital anomaly within the spectrum of sequestration lung disease. The most common pattern of anomalous systemic artery to the lung arises from the descending thoracic aorta and feeds the basal segments of the left lower lobe. We report an extremely rare case of a 29-year-old woman who underwent a successful left upper lobectomy for the treatment of recurrent massive hemoptysis from anomalous bronchial arterial supply to the lingular segment of left upper lobe.

  • Pulmonary systemic arterialization to normal basal lung without sequestration is a rare congenital anomaly. In this rare abnormality, arterialization of the left lower lobe is the most common type. In general, surgical treatments have been performed. Recently, for reducing the complications and risks of surgery, embolization is mainly attempted by using coils. We report a case of 22-year-old male patient with a 10 mm anomalous arterial supply to his normal lung, which is being successfully treated by transcatheter embolization when using the Amplatzer Vascular Plug that has been adapted for the treatment of high-flows and large artery occlusions.

Development of HPLC Method for Quality Assessment of Marker Components in Atractylodis Rhizoma Alba (백출(Atractylodis Rhizoma Alba)의 품질평가를 위한 지표성분 분석법 평가)

  • Homogenization of quality was important in order to use herbal medicines as pharmaceuticals. To solve this problem, it was important to establish quality standards. Atractylodis Rhizoma Alba has no quantitative method in the Korean Pharmacopoeia. Thus, we have researched to improve the quality evaluation method of Atractylodis Rhizoma Alba with an HPLC. Atractylenolide III and atracylodin were selected as potential marker compounds. This analytical procedure was subject to validation. According to validation guideline of South Korea's Ministry of Food and Drugs Safety, the specificity, linearity, precision, range, quantitative limits, detection limits and accuracy were measured. Because the specificity, linearity, precision, range, quantitative limits, detection limits and accuracy meet criteria of the guideline, the analytic method was validated. With this analysis, Atractylodis Rhizoma Alba and Atractylodis Rhizoma analyzed. As a result, both atractylenolide III and atracylodin appear to be suitable standard compounds. it confirmed that tractylodes Rhizome, similar to Atractylodes Rhizome Alba, could be distinguished.
