한의사인력의 수급전망과 대책 (Demand and Supply of Physicians for Oriental Medicine : Review and Prospects)

  • This paper estimated the demand and supply of physicians for oriental medicine for the period of $2009{\sim}2019$. Two equation models were used in the estimation of manpower. In 2004, the total number of physicians of oriental medicine was amounted to 13,662 registered and 10,532 available in clinical practice, respectively. According to estimates in the study, overall excess supply of physician manpower in oriental medicine was expected in the period, such as $5,300{\sim}5,700$ persons in 2009 and $900{\sim}1,700$ persons in 2019. However, the excess supply would be mitigated after 2019 mainly due to an increase in demand for oriental medical services. Specially, opening medical service market to overseas could be an exogenous variable in physician supply. An alternative manpower policy for oriental medical doctors is needed in a way of controlling oversupply.

Assumption of estimation of demand and supply we will briefly assumptions used for presumption once more before comparing the result of estimation of demand and supply examined previously 1) supply - The average applying rate for state. examination of graduate: ${\alpha}$=1.03109 - The ratio of successful applicants of state examinations: ${\beta}$=0.97091 - Mortality classified by age : presumed data of the Bureau of statistics - Emigrating rate: 0 % - Time of retire: unconsidered - An army doctor number: unconsidered and regard number of employed oriental medicine doctor. - Standard of 1995 : The number of survival oriental medicine doctor is 8195. the number of employed oriental medicine doctor is 7419. 2) demand - derivated demand method Daily the average amount of medical treatment: according to medical insurance federation data. there is 16 or 6 non allowance patient, we consider amount of medical treatment as 22 persons in practical because 21.94 persons (founded practical examination) are converted to allowance in comming demand. Daily the proper amount of medical treatment: 7 hours form -35 persons 5 hours 30 minutes form -28 persons. Yearly medical treatment days: 229 days. 255 days. 269 days . Increasing rate of visiting hospital days: -1996 year. 1997 year. 1998 year- . Rate of applying insurance: yearly average 71.51% (among the investigated patient) B. Comparison of total sum result 1) supply (provision) Table Ⅳ-1 below shows the estimation of the oriental medicine doctor in the future.

  • 한국의료 시장의 변화와 한의학 (The Changes of Oriental Medicine and Medical Market in Korea)

    • We investigated the changes in oriental medical market based on supply and demand of market in Korea. It is shown that the supply of western medical doctors is 6.6 times as large as that of oriental medical doctors(醫師) in 1998. The supply of western medical doctors(韓醫師) showed the greater increasing rate than that of oriental medical doctors in 1975. However, the supply of western medical doctors was almost the same as that of oriental medical doctors in the increasing rate between 1985 and 1995. Similar trends was observed in the number of hospitals and clinics. From of viewpoint of demand, the use of oriental medicine was remarkably increasing from 1990 to 1997. Oriental medical institution showed a marked increase of 48.1 % in the total medical institution showed an increase of 21.6%. These results provided a strong evidence that oriental medicine had greater increasing rate than western medicine in the supply and demand of medical market and suggest that the use of oriental medicine may play a role in the specialization of oriental medicine.

    한의사인력의 중장기 수급 추계 연구 (Future Demand and Supply of Physicians for Korean Medicine)

    • Objectives : This study was conducted to estimate the future demand and supply of physicians for korean medicine from 2016 year to 2026 year in order to make an adequate manpower policy in a way of keeping a balance between demand and supply. Methods : Baseline projection method and trend analysis(a polynomial log power equation model) were used in the estimation of future supply and demand respectively. We used data about the amount of oriental doctors from Ministry of Health and Welfare Statistics Yearbook and the treatment days from HIRA Statistics Yearbook. Results : It was projected that the total number of physician of Korean medicine will be 25,178 registered and 18,967 available in clinical setting. According to polynomial equation model which explained the trend of demand and had the highest score of $R^2$ among the equation models, 3,800~5,600 physician in Korean medicine will be oversupplied in 2016 year, 9,000~10,700 physicians in 2021 year and 15,700~17,000 persons in 2026 year depends on annual working days which is 265days, 255days or 239days. Log equation model also showed that overall excess supply of physician manpower in Korean medicine. Conclusions : Alternative manpower policies for Korean medicine doctors should be implemented in a way of both dwindling supplies and growing demand in Korean medical service in terms of Korean medical services utilization and improving physician's productivity.

    • 1. The cause of Pehuh is deficiency of Peki(肺氣). Symptoms are tachypnea, thirst and deafness of hemoptysis. In treatment, control it's Ki(氣). 2. The cause of Peson is in addition to Pehuh Pimo(皮毛) is demaged by heat and cold. Symptoms are alopecia, cough, sputum, fever and chill. In treatment, supply it's Ki. 3. The cause of Pero is it's Ki which is demaged by lie down for a long time or because of anxiety Sang Ryum Hueng Kum(上炎刑金). Symptoms are tachypnea, chest and back pain, cough and sputum. In treatment, supply Sinki(腎氣). 4. In correlation of Pehuh, Peson and Pero, Pehuh is progressed from Kihuh(氣虛), Peson is progressed from Pehuh and Pero is progressed from Peson. 5. The treatment of Lung disease showed by chapter Huhro(虛勞). In early stage, control it's Ki. In middle stage, supply it's Ki. The last stage, supply Sinki(腎氣).

    한의사인력 수급 추계 및 정책 활용방안 (The Supply and Demand Analysis of the Oriental Medical Doctor and Its Uses in Assisting Policy Making)

    • This study was performed to investigate the supply and demand of the oriental medical doctor(OMD) based on the supply and demand analysis of OMD up to the year 1997. The baseline projection and demographic methods were considered to examine the supply of OMD. On the contrary, for the demand analysis, two different approaches were conducted with the nonlinear regression model. The findings of this study indicate that the OMD will be oversupplied before the year 2012 with decreasing rate. However, when we consider the demand of OMD in the future. it is anticipated that the demand of oriental medicine will be increased rapidly with two major aspects. The first is the expansion of insurance benefits. The second is the increasing number of adult diseases because of the aging of the population structure. Therefore, the effective cooperation system and mutual exchange between western and oriental medicine is required for the future. Also. it is necessary to make the oriental medicine of the pharmaceutical services in more scientific way for the appropriate policy of the demand and supply of OMD. For the future study, the students who study abroad, especially China should be considered. These students will be the key element for the future supply of OMD.

    • Intralobar pulmonary sequestration is a rare congenital lung anomaly. It is defined as a portion of nonfunctioning lung parenchyma that receives its blood supply from an anomalous systemic artery. Patients often present with chronic or recurrent pneumonia. A chest radiograph may show a cystic lesion with air-fluid levels in the lung base. A high index of suspicion is needed for a diagnosis. Surgical removal of a symptomatic intralobar pulmonary sequestration is generally the treatment of choice. Identifying the aberrant artery is a difficult problem when resecting a pulmonary sequestration. The thoracic and abdominal aortas are the most common origins for the abnormal blood supply. However, arterial supply from the celiac artery is quite rare. We present a case of intralobar pulmonary sequestration with the blood supply originating from the celiac artery.

    • To strengthen the R&D capability and the competitiveness of the Korean oriental medicine industry, an adequate supply of qualified R&D personnel including medical doctors of Korean oriental medicine is an important precondition. This study analyze current and future R&D manpower requirements including medical doctors in the Korean oriental medicine research area. Our analyses can be utilized for developing the government R&D manpower planning including the adequate supply of medical doctors for the Korean oriental medicine research. For the study, we conducted and analyzed a delphi survey of the experts, the principal investigators, with expertise in Korean oriental medicine research areas. The results of this study can be summarized as follows; First, in 2007 the Korean oriental medicine R&D personnel is currently under-supplied as many as 302 people including 111 medical doctors of Korean oriental medicine. The rate of under-supplied is 28.2%. Second, in 2017 the forecast shows that the R&D personnel in this area will be more severely under-supplied as many as 539 people including 185 medical doctor of Korean oriental medicine. The rate of under-supplied will be 32.6%. As a result, the confrontation of demand and supply forecasts shows that, in general, severe shortages of R&D manpower in the areas of Korean oriental medicine will result if there are not adequate manpower policy adjustment.

    Bronchopulmonary Sequestration with Dual Arterial Supply from Celiac Artery and Thoracic Aorta

    • Bronchopulmonary sequestration (BPS) is a rare congenital malformation of the lower respiratory tract. Most intralobar BPSs are provided with an arterial blood via the thoracic or abdominal aorta but such a supply is rarely found in patients older than 50 years. We report a case of an intralobar BPS with a dual arterial supply from the celiac artery and thoracic aorta in a 50-year-old man presenting with a respiratory tract infection and haemoptysis. To our knowledge, this is the first case report of a BPS supplied by the celiac artery and thoracic aorta in a 50-year-old man.
